The programme is clinically-led and is being delivered in partnership with our staff and trades unions and our local health and social care partnerships. It is structured to respond to issues and make changes to services in the short to medium-term, as well as presenting an ambitious vision of transformation for Tayside in the longer term. It will provide clear and credible choices for transforming the future of local healthcare.

A new, innovative leadership model for clinical directorates is now in place to drive the changes necessary in health and social care across the whole system. Clinicians are taking the lead to design and provide evidence-based models of care which are safe, accessible, effective and person-centred in the Transforming Tayside programme.

Innovation and research are at the forefront of what NHS Tayside stands for and we are proud to have established the first academic health science network in Scotland, jointly supported by the Scottish Government and the University of . The Academic Health Science Partnership in Tayside brings together the delivery of healthcare services, education, quality improvement and research with the purpose of improving the health of patients. NHS Tayside, alongside the Academic Health Science Partnership, has welcomed funding announced in the Tay Cities Deal to grow the Tayside Biomedical Cluster. The project aims to make the region one of the most attractive and sought after biomedical locations in the UK. A £25million investment to grow the Tayside Biomedical Cluster was one of the projects announced to receive investment in the Tay Cities Deal Heads of Terms Agreement. The funding will stimulate a step change in innovation-led growth and create new jobs for the region.

The final thing I would like to say to any potential applicant is that Tayside occupies an enviable location on the east coast of Scotland with rivers, hills, mountains and rolling countryside right on our doorstep, as well as vibrant urban areas. The V&A Museum of Design in Dundee is an international centre of design and the centrepiece of Dundee’s innovative new waterfront development. This, along with Tayside’s great links to all other major cities in Scotland, means the area can offer employees plenty of choice when it comes to both accommodation and opportunities of things to do in the pursuit of the right work-life balance.

NHS Tayside is responsible for delivering healthcare to more than 415,000 people living in Tayside. We employ around 14,000 staff and provide a comprehensive range of primary, community-based and acute services for the populations of Dundee City, Angus and Perth & Kinross. Tayside area

Within NHS Tayside and based on the western edge of Dundee, is one of the largest teaching in Europe. Health and Social Care Partnerships

There are three Health and Social Care Partnerships in Tayside: Angus; Dundee; and Perth and Kinross.

NHS Tayside works very closely with colleagues in the partnerships as they provide a range of adult health and social care services to our local communities.

Academic Health Science Partnership in Tayside (AHSP)

NHS Tayside has embarked on a major collaborative initiative, Academic Health Science Partnership in Tayside (AHSP). It is the first Academic Health Science Network in Scotland with an ambition to transform healthcare locally and globally at the time when it faces many difficult challenges.

Building upon the success of previous University of Dundee and NHS Tayside collaborations, AHSP is bringing together the delivery of healthcare services, education, quality improvement and research with the purpose of improving the health of patients through more effective integration of these functions in routine practice.

Statement of Policy regarding fitness to practise proceedings by a licensing/regulatory body and relating to criminal investigations in the UK or overseas.

Registration with the General Medical Council or General Dental Council imposes on doctors and dentists the duty to provide a good standard of medical care for, and to behave appropriately, towards patients. NHS Employers also have a duty to ensure that patients receive a good standard of medical care and ensure as far as possible the safety of patients. We therefore need to establish if you have been found guilty of a criminal offence, been bound over or cautioned or are currently the subject of proceedings which might lead to a conviction, an order binding you over or a caution, in the UK or any other country.

This post is exempt from the 1974 Rehabilitation of Offenders Act (Exclusions & Exceptions) (Scotland) Order 2003. PERTH AND KINROSS HEALTH AND SOCIAL CARE PARTNERSHIP CONSULTANTS - PSYCHIATRY OF OLD AGE Two full time substantive posts Up to 10 Programmed Activities per post.

Perth and Kinross, known as ‘The Heart of Scotland’ is an area of central Scotland renowned for its natural beauty and excellent transport links with Edinburgh and Glasgow both just an hour away. It is an area of approximately 2000 square miles with Perth as the main centre and several market towns approximately 20 to 30 miles away at Kinross, Auchterarder, Crieff, Aberfeldy, Pitlochry and Blairgowrie. Perth and Kinross has one of the largest populations of elderly people in Scotland and there is a mixture of all levels of socio- economic class and deprivation.

We seek two enthusiastic and committed individuals to work within Perth and Kinross Health and Social Care Partnership’s Psychiatry of Old Age Service. Health and Social Care Integration has created much opportunity for innovative ways of working. The organisation values the knowledge, experience and leadership skills of Consultant Old Age Psychiatrists. One of the posts has arisen due to a retirement and the other is a new, additional post in recognition of the increased need for senior medical expertise.

In collaboration with social work and other partnership agencies we aim to deliver a person centred service with older people, their families and carers at the centre of all we do. We also recognise the importance of supporting and valuing our staff.

Both of the posts are based at , Perth. One post is predominantly community focussed covering North Perthshire, areas such as Blairgowrie and Pitlochry with an option to also lead on delivering the local ECT Service. The second post is a mix of community and in-patient commitments. Community work is in South-East Perthshire, areas such as Kinross and the Carse of Gowrie with in-patient commitments being functional and dementia assessment units at Murray Royal Hospital. Prospective candidates who feel their skills lie in other areas of the service can be reassured that the current consultant group are willing to be flexible within the job planning process. The Direct Clinical Care Sessions to Supporting Professional Activities Sessions split would be 8:2. POST ONE - CONSULTANT PSYCHIATRIST IN PSYCHIATRY OF OLD AGE BASED AT MURRAY ROYAL HOSPITAL, PERTH JOB DESCRIPTION

INTRODUCTION We seek an enthusiastic and committed individual to work within Perth and Kinross Health and Social Care Partnership’s Psychiatry of Old Age Service. Health and Social Care Integration has created much opportunity for innovative ways of working. The organisation values the knowledge, experience and leadership skills of Consultant Old Age Psychiatrists. This post has been developed in recognition of the increased need for senior medical expertise.

Perth and Kinross, known as ‘The Heart of Scotland’ is an area of central Scotland renowned for its natural beauty and excellent transport links with Edinburgh and Glasgow both just an hour away. It is an area of approximately 2000 square miles with Perth as the main centre and several market towns such as Kinross, Auchterarder, Crieff, Aberfeldy, Pitlochry and Blairgowrie all being 20-30 miles away. Perth and Kinross has approximately 147,780 residents of whom approximately 33,156 are over the age of 65, and just over 15,412 of whom are over the age of 75. Perth and Kinross has one of the largest populations of elderly people in Scotland and there is a mixture of all levels of socio-economic class and deprivation.

Integration of Health and Social Care has resulted in three localities; Perth City, North Perthshire and South Perthshire. The new build Murray Royal Hospital opened in 2012 and is the base for all of the Consultant Old Age Psychiatrists which enhances good communication, peer support and cross cover. It is also the base for the Perth City and South-East Perthshire’s Older Peoples Mental Health Teams and all of the Psychiatry of Old Age Acute Assessment In-Patient Wards. There are Older Peoples’ Mental Health Teams based in Community Hospitals within Crieff, Pitlochry and Blairgowrie. Out-patient clinics are held at Murray Royal Hospital, the Community Hospitals and in Health Centres across the area which facilitates communication and collaborative working. Within Perth and Kinross there are 41 Care Homes which are supported by locality based Care Home Liaison Nurses. is a district general hospital and we have excellent links with Medicine for the Elderly.

This post is based at Murray Royal Hospital, Perth comprised of community and in-patient commitments. Community work is in South-East Perthshire, areas such as Kinross and the Carse of Gowrie with in-patient commitments being functional and dementia assessment units at Murray Royal Hospital. The South-East sub-locality has 4300 residents over the age of 65. There are no community hospitals and the sub-locality only has 4 Care Homes. Prospective candidates who feel their skills lie in other areas of the service can be reassured that the current consultant group are willing to be flexible within the job planning process. The Direct Clinical Care Sessions to Supporting Professional Activities Sessions split would be 8:2. Applicants may apply for one or both posts and we welcome applications from individuals wishing to work less than full-time.

Referrals to Consultants in Old Age Psychiatry are accepted from all medical practitioners and hospitals in Perth and Kinross. New referrals for any patients aged over 65 years plus patients of any age with dementia are welcomed. Transitions from General Adult Psychiatry to Old Age Psychiatry are agreed in order to best serve the needs of the patient. Referrals

of older offenders are accepted with Old Age Psychiatry and Forensic Psychiatry working together to ensure an optimal service.

SERVICE ETHOS The Perth and Kinross Psychiatry of Old Age Service has a long history of excellent multi- disciplinary working with good relationships already established with Primary Care and Medicine for the Elderly. Health and Social Care Integration will allow us to extend this to our partners in Social Work and other Partnership organisations such as the voluntary sector. Our aim is to support Older People with Mental Health Conditions and their families and carers at or close to home for as long as is appropriate so that they remain active and valued members of their local community. The service follows a Locality Model with each area having a Locality Manager. The Psychiatry of Old Age Liaison Service This multi-disciplinary team is co-located within Perth Royal Infirmary, a district general hospital with 261 beds.

ECT Service Perth and Kinross ECT Service is based at Murray Royal Hospital. It is Scottish ECT Accreditation Network (SEAN) Accredited with Excellence. It is Consultant Old Age Psychiatrists who have the role of Lead and Deputy ECT Consultant.

Day Hospitals There are no NHS day hospital facilities within Perth and Kinross Health and Social Care Partnership. However, the Older Peoples’ Mental Health Teams have excellent links with Day Care facilities provided by other partners.

Senior Medical Staff In June 2019 NHS Tayside’s Associate Medical Director for Older People resigned after many years having successfully established Scotland’s first Older Peoples Clinical Board within NHS Tayside. At the point of writing clarification is awaited regarding the replacement process and the professional management structure for senior medical staff within the department is under review. The consultant group currently adopts a collegiate approach to Clinical Lead related decision making and task allocation. Dr. Hamish Dougall is Clinical Director for the Perth and Kinross Health and Social Care Partnership. The consultants within the department are: 1. Dr. Susan Angus (0.75 whole time equivalent WTE) Murray Royal Hospital in-patients, Perth Royal Infirmary Psychiatry of Old Age Liaison Service, ECT Deputy 2. Dr. Mhairi Hepburn (1 WTE) Perth City Locality Older People's Mental Health Team and Foundation Link for Psychiatry. 3. Dr Candida Graham (1 WTE) South Locality Older People's Mental Health Team and In-patients at Murray Royal Hospital. 4. Dr Gary Cousland (1 WTE) North West Sub-Locality Older Peoples’ Mental Health Team, Murray Royal Hospital Inpatients and ECT Lead. Due to retire in March 2020.

5. Locum (1 WTE) North East and South East Sub-Locality Older Peoples’ Mental Health Teams

The consultant group are friendly, supportive and get on well. They meet weekly to discuss clinical and management issues and have an equal say in issues that affect the group. The department has an excellent track record of supporting medical staff with health problems. Dr Mhairi Hepburn is one of the local contacts for both Doctors' Support Network and Tea and Empathy. Other Medical Staff The department is supported by a Core Psychiatry Trainee, a GP Speciality Trainee and a Foundation Year 1 Trainee. Higher Specialist Trainees rotate into the Perth and Kinross Service and are allocated a trainer dependant on their training needs. There are no Speciality Doctors or Associate Specialists within the department.

Medicine for the Elderly The Medicine for the Elderly Service is based at Perth Royal Infirmary with a growing emphasis on community-based working. The service is approachable and accessible with a designated consultant available for advice and a session per week for review of in-patients at Murray Royal Hospital.

Support staff All parts of the service adopt multi-disciplinary working and have access to Occupational Therapy, Physiotherapy, Dietetics, Pharmacy, Speech and Language Therapy, Clinical Psychology, Social Work, Mental Health Officers and Alzheimer Scotland Link Workers. The in-patient units have dedicated Social Work input which has markedly improved flow and capacity. There are 3 Clinical Psychologist posts for over 65s and trainee psychologists are regularly on placement within the department. The Psychology Service for Older Adults provides a variety of psychological therapies, neuropsychological testing and works in collaboration with the Psychiatry of Old Age Service. There is access to Psychotherapy Services and the department will assess and treat older people upon request.

Administration and Clerical Support All administrative and clerical staff who support the consultant group are based at Murray Royal Hospital with each whole-time equivalent consultant having access to 0.5 whole time equivalent secretarial support.

Voluntary agencies Voluntary agencies are active in Perth and Kinross; the Crossroads Organisation, Princes Royal Carers Trust and Alzheimer’s Scotland being amongst those most involved.

Social Care resources Residents in Perth and Kinross can be supported at home by local authority care providers, a number of private care organisations and increasingly older people and their families are choosing Self Directed Support to further improve continuity and person centred care. Sheltered Housing options are available throughout the region. Across Perth and Kinross there are 41 care homes offering residential, nursing and specialist dementia care and expertise. The Care Homes are supported by the Care Home Liaison Service which includes experienced nursing staff who are supported by their locality consultant psychiatrist. The Psychiatry of Old Age Service does not have any Continuing Care beds so it is important to maintain good working relationships with the care home sector. There is no Day Hospital provision within the Psychiatry of Old Age Service but day opportunities are available across Perth and Kinross, delivered by the local authority and voluntary organisations.

There is a drive to re-establish a service wide, multi-disciplinary forum to ensure that Psychiatry of Old Age Services are shaped centrally but delivered locally.

CLINICAL GOVERNANCE At NHS Tayside Health Board level there is the Older Peoples’ Clinical Board (OPCB). All Consultant Old Age Psychiatrists are invited to attend the Older Peoples’ Mental Health Sub- group of the OPCB. This is with a view to contributing to the regular Clinical and Care Governance Group meetings held regularly within each Locality and the In-patient Service. The governance groups must refer to NHS Tayside’s Older Peoples Clinical Board's Refreshed Clinical Services Strategy for Older People (2018-2020). Time is allocated within each consultant’s job plan for activity related to Appraisal and Revalidation which includes activities related to clinical governance. Audit is supported by the department and a Consultant Old Age Psychiatrist colleague in Angus co-ordinates audit activity in Psychiatry of Old Age across NHS Tayside.

TEACHING, TRAINING AND CONTINUING PROFESSIONAL DEVELOPMENT During the academic year there is at least one 4th year medical student from the University of Dundee on clinical attachment within the Psychiatry of Old Age Service. Final year medical students can also elect to undertake a Dementia Special Study Module over a 4-week period. The University of Dundee Medical School runs a 4-week psychiatry teaching block during 3rd year and medical staff are encouraged to volunteer to provide tutorials, practical sessions and/or lectures. In addition, there are many other medical education opportunities available at the University of Dundee such as student support, OSCE examinations and admissions interviews.

The department supports the training of a Foundation Year, GP Speciality Trainee and a Core Psychiatry Trainee. Responsibility for clinical and educational supervision of the trainees is shared by the consultant group. In accordance with the Royal College of Psychiatrists’ Guidelines one hour per week per trainee is dedicated for supervision. Trainers are encouraged to contribute towards mock CASCs and the weekly teaching programme based at Murray Royal Hospital, which provides Continuing Professional Development for Doctors in Training, Speciality Doctors and Consultants. There is also a monthly CPD afternoon for senior medical staff at the in Dundee. Murray Royal Hospital has excellent library facilities.

There are a number of peer groups available and the successful candidate would be encouraged to identify and engage with a mentor.

CLINICAL RESEARCH The Psychiatry of Old Age Service has a long history of promoting and engaging in research as well as publications. There is easy access to The Psychiatry of Old Age Research Unit within the University of Dundee. NHS Research Scotland’s Neuroprogressive and Dementia Network is based at Ninewells Hospital but maintains an excellent link with Murray Royal Hospital. JOB PLAN CONSULTANT OLD AGE PSYCHIATRIST

Full-time Consultant for NHS Tayside based at Murray Royal Hospital, Perth.

1 GENERAL PROVISIONS

Consultants will work with local Managers and professional colleagues in the efficient running of services. Subject to the provisions of their Terms and Conditions of Service, Consultants will observe the Trust’s agreed policy and procedures which on clinical matters will be drawn up after appropriate consultation with the profession. Subject to the provision of the Terms and Conditions of Service, Consultants will follow standing orders and financial instructions of NHS Tayside, and will comply with appropriate Health and Safety Policies.

The appointment is subject to the Terms and Conditions of Service of Hospital, Medical and Dental staff as applicable to Consultant grade and the National Health Service (Scotland) Superannuation regulations.

The appointment will be on a full-time 10 programmed activity basis.

The successful candidate will be expected to reside within an approved distance of Murray Royal Hospital.

Appointments will be subject to satisfactory Occupational Health and Disclosure (PVG) clearance.

Full registration with the General Medical Council and a License to Practise, the possession of the MRCPsych and CCST or equivalent are mandatory.

Employment is subject to a minimum of 3 month’s notice on either side but is subject to the provisions of paragraphs 190 -198 of the Terms and Conditions of Service of Hospital Medical and Dental Staff.

2 GENERAL DUTIES

Each Consultant will have overall clinical and organisational responsibilities for a specific part of the psychiatric service as detailed below. This will include responsibility for the diagnosis and treatment of illness and the proper functioning of their service.

Consultants will ensure that they liaise adequately with other members of the Multi- disciplinary Team of which they may be a part, and with statutory or voluntary agencies as appropriate to their area of clinical responsibility and with other professionals as appropriate.

Each Consultant will have responsibility for a share of the in-patient and out-patient resources associated with the Mental Health Service and to undertake domiciliary consultations where appropriate requests are made. Where it is agreed between parties, Consultants will undertake work for other agencies.

Consultants will undertake duties as approved medical practitioners under the terms of the Mental Health (Care and Treatment) (Scotland) Act 2003.

Consultants may participate on committees as appropriate at a local and national level, and represent NHS Tayside or the Eastern Regional Post-Graduate Medical Education Committee at national level.

Consultants will cover for the annual and short-term sick leave of colleagues.

A Job Plan will be agreed with the person appointed.

The appointee will have secretarial and admin support and an individual office.

3 ON-CALL

The provision of out-of-hour mental health provision is currently under review. At present Consultant Psychiatrists participate in a third on-call rota covering all psychiatric specialties in Tayside at a frequency of about 1:12. The successful applicant would have the option to join this rota but at present it is not compulsory. However, it would be compulsory that the successful candidate actively participate in any discussions or planning around the provision of safe and effective out-of-hours care.

Higher Specialist Trainees provide a Tayside wide second on-call rota.

First contact is with the Crisis Team based at Carseview Centre, Dundee supported by Foundation Year Doctors, Core Psychiatric Trainees, and General Practice Trainees participating on a partial shift rota with Murray Royal Hospital having a separate first on-call rota.

Each Consultant will ensure that there are adequate arrangements for hospital staff involved in the care of their patients to be able to contact them when necessary throughout their working week and when they are on-call. Each Consultant is responsible for ensuring that there is adequate cover for their part of the service during periods of leave subject to the provisions of their Terms and Conditions of Service.

Your residence shall be maintained in contact with the Public Telephone Service.

4 EDUCATION

Consultants will be responsible for the supervision of Junior Medical Staff attached to their Unit. Clinical supervision of a trainee may be expected.

Consultants will also contribute to the Post-Graduate Education of Junior Doctors and the teaching of under-graduate medical students normally from the University of Dundee.

Consultant staff may be involved in educational activities, examination activities and accreditation duties as required. These activities and duties may be local or national.

Each Consultant has an entitlement to Study leave at nationally agreed levels. The Trust currently provides an annual budget of £1000 per Consultant for study leave.

There are various peer groups that include psychiatrists from multiple specialties, plus an active Tayside Old Age Psychiatry Peer Group which meets 3 monthly to cover peer support, education and governance issues amongst the consultant body.

5 AUDIT & RESEARCH

All Consultants will participate in some form of Clinical Audit and will ensure that Junior Staff undertaking audit on their behalf are adequately supervised.

Research interests may be pursued within the Mental Health Services following liaison with the Directorate and senior staff from the University Department as necessary.

6 MANAGEMENT

Each Consultant will be a member of the Psychiatry of Old Age Senior Management Team which holds responsibility for clinical management of the old age psychiatric services.

The Health & Social Care Partnership has a Clinical Director appointed by the NHS Tayside for a term of 3 years (renewable). The Clinical Director will be expected to liaise adequately with other Consultants on matters relating to the provision of clinical services and with other professionals where appropriate.

Consultants may be expected to represent the Psychiatry of Old Age Department in planning service developments and implementing changes, in association with other disciplines and agencies as appropriate, including users and carers.

Consultants are professionally and managerially responsible to the Medical Director. A detailed Organisational Structure will be available to all applicants prior to interview.

7 REMUNERATION

The appointment will be superannuable unless you opt out of the NHS Superannuation scheme.

The salary of appointment (exclusive of any distinction and service awards payable to you) will be that appropriate to a full-time Consultant with starting salary paid according to experience.

In addition to the duties outlined above you may occasionally be required to undertake other duties for a limited period.

8 SPECIALIST DUTIES

The Consultant will be responsible for the provision of inpatient and outpatient services for the population in a sectorised area of Perth and Kinross. A description of existing resources can be found in “Further particulars” below.

There are 4300 people over age 65 years within this sub-locality.

The team comprises the following: 1 WTE Band 6 nurse 1.8 WTE Band 5 nurses 1 WTE Band 4 nurse 1 WTE Health Care Assistant. Allied Health Professionals of all disciplines, Clinical Psychology, Alzheimer Scotland Link Worker for Post Diagnostic Support and Counselling and Social Work can all be accessed easily.

The team are located at Murray Royal Hospital.

Out-Patient Clinics are held at Murray Royal Hospital and there is plenty consultation space at Loch Leven Health Centre which facilitates good working relationships with the largest practice in this locality.

There are no Community Hospitals within the sub-locality.

There are 4 of Perth and Kinross’ 41 Care Homes in the sub-locality, the biggest of which only has 43 residents.

MURRAY ROYAL HOSPITAL - GENERAL DESCRIPTION

OLD AGE PSYCHIATRY A comprehensive service for over 65s is provided - as detailed above.

GENERAL PSYCHIATRY Five Consultants share the service. There are 2 x 18 bed acute admission wards and a Crisis Resolution and Home Treatment Team.

REHABILITATION Two Consultants in this service. This sub-speciality has in-patient beds, resettlement and day care components.The rehabilitation service has close working relationships to Perth & Kinross Social Work Department and access group homes supported accommodation based in Perth City.

FORENSIC PSYCHIATRY Five Consultants share the Forensic Service. They are based in the Rohallion Unit. There are low secure beds, a day hospital and a new medium secure inpatient facility for the North of Scotland opened in Summer 2012. All Consultants participate in the provision of psychiatric reports for the courts.

CHILD AND FAMILY PSYCHIATRY This is a Tayside-wide service but two Consultants are based in Perth, where they are supported by a multi-disciplinary team. Inpatient beds are based in Dundee.

OUTPATIENT SERVICES Clinics are held in Murray Royal Hospital and community team centres in Blairgowrie, Kinross, Auchterarder and Crieff. A Health Maintenance Clinic is held weekly in Murray Royal Hospital.

SUBSTANCE MISUSE A Consultant for Tayside Alcohol Problems Service offers sessional time to the Perth and Kinross area along with CPN colleagues. Inpatient facilities moved to Murray Royal Hospital along with the Drug Problem Service when the new building opened in the Summer 2012.

LIAISON PSYCHIATRY All senior medical staff in General Adult Psychiatry and suitably qualified junior medical staff provide a service to Perth Royal Infirmary for patients under the age of 65. There is a multidisciplinary Psychiatry of Old Age Liaison team based within Perth Royal Infirmary under the Consultant lead of a POA Consultant.

PSYCHOTHERAPY A developing Psychotherapy Service now provides a wide range of psychotherapeutic treatment concentrating on the brief psychotherapies. Trainees receive supervision and training at general professional levels as appropriate.

ECT Murray Royal Hospital has a dedicated ECT suite. The ECT department has SEAN Accreditation with Excellence.

CLINICAL PSYCHOLOGY The Clinical Psychology Service is based at Murray Royal although in excess of 95% of referrals are seen in Primary Care settings. A specialist psychology input is provided to the Eating Disorders Service.

OCCUPATIONAL THERAPY & PHYSIOTHERAPY There is a specific occupational therapy service provided to Old Age Psychiatry inpatient services and community teams providing a mix of trained OT and technical assistant input.

DIETETICS One Senior Dietician is based in the Mental Health Directorate.

SOCIAL WORK Considerable reorganisation in the Social Work Department consequent upon Local Government reorganisation has had a major impact on Social Work Services provided to the Directorate. Ongoing discussions between the Trust and the Social Work Department are addressing how this service will be provided in future though this will also be subject to the outcome of the General Adult Psychiatry and Old Age Psychiatry strategies.

TEACHING Murray Royal is part of the East Region’s Psychiatric Training Scheme at Core and Higher Specialist Training level.

There are close links with the Department of Psychiatry and the University of Dundee and the appointee may be awarded the post of Honorary Senior Clinical Teacher. Those with a significant research or teaching commitment may be awarded an Honorary Senior Lectureship.

There is an active multidisciplinary Continuing Education Programme for which responsibility is rotated amongst the consultant group.

There is an excellent Postgraduate Library at Murray Royal and also access to the main Medical Library in Ninewells Hospital in Dundee. The library facilities at Murray Royal have been upgraded to allow Internet access and access to medical databases.

AUDIT An active programme of Clinical Audit is pursued by medical staff throughout the hospital. There is a Medical Audit Sub-Committee consisting of the Clinical Director and the Clinical Tutor and Consultants, which co-ordinates audit activity throughout the hospital

RESEARCH Research interests are actively encouraged within the hospital. There are close links with the University Department of Psychiatry and Dundee Medical School, and Consultants as Honorary Senior Lecturers in Psychiatry have access to the University facilities and Medical Library in Ninewells Hospital.

Perth and Kinross has approximately 147,780 residents of whom approximately 33,156 are over the age of 65, and just over 15,412 of whom are over the age of 75. Perth and Kinross has one of the largest populations of elderly people in Scotland and there is a mixture of all levels of socio-economic class and deprivation.

Integration of Health and Social Care has resulted in three localities; Perth City, North Perthshire and South Perthshire. The new build Murray Royal Hospital opened in 2012 and is the base for all of the Consultant Old Age Psychiatrists which enhances good communication, peer support and cross cover. It is also the base for the Perth City and South-East Perthshire’s Older Peoples Mental Health Teams and all of the Psychiatry of Old Age Acute Assessment In-Patient Wards. There are Older Peoples’ Mental Health Teams based in Community Hospitals within Crieff, Pitlochry and Blairgowrie. Out-patient clinics are held at Murray Royal Hospital, the Community Hospitals and in Health Centres across the area which facilitates communication and collaborative working. Within Perth and Kinross there are 41 Care Homes which are supported by locality based Care Home Liaison Nurses. Perth Royal Infirmary is a district general hospital and we have excellent links with Medicine for the Elderly.

This post is based at Murray Royal Hospital, Perth. It is predominantly a community based post with the opportunity to either take on the role of ECT Lead within the Perth and Kinross ECT Service or other community based or inpatient commitments. Community work is in North Perthshire, areas such as Blairgowrie and Pitlochry with the ECT Service being based at Murray Royal Hospital. The North Locality has 10,357 residents over the age of 65. There are 2 community hospitals and 16 Care Homes. The post holder would be responsible for the two North Sub-Locality (North East and North West) Older Peoples’ Mental Health Teams, 1 of the community hospitals and 10 of the Care homes in the North-East Sub- Locality with alternative arrangements to be made for the North-West Sub-Locality’s 1 community hospital and 6 Care Homes. Prospective candidates who feel their skills lie in other areas of the service can be reassured that the current consultant group are willing to be flexible within the job planning process. The Direct Clinical Care Sessions to Supporting Professional Activities Sessions split would be 8:2. Referrals to Consultants in Old Age Psychiatry are accepted from all medical practitioners and hospitals in Perth and Kinross. New referrals for any patients aged over 65 years plus patients of any age with dementia are welcomed. Transitions from General Adult Psychiatry to Old Age Psychiatry are agreed in order to best serve the needs of the patient. Referrals of older offenders are accepted with Old Age Psychiatry and Forensic Psychiatry working together to ensure an optimal service.

SERVICE ETHOS The Perth and Kinross Psychiatry of Old Age Service has a long history of excellent multi- disciplinary working with good relationships already established with Primary Care and Medicine for the Elderly. Health and Social Care Integration will allow us to extend this to our partners in Social Work and other Partnership organisations such as the voluntary sector. Our aim is to support Older People with Mental Health Conditions and their families and carers at or close to home for as long as is appropriate so that they remain active and valued members of their local community. The service follows a Locality Model with each area having a Locality Manager. The post holder and the highly experienced North Perthshire Locality Older Peoples' Mental Health Teams will have the opportunity to develop services in their locality in keeping with NHS Tayside’s Older Peoples Clinical Board's Refreshed Clinical Services Strategy for Older People (2018-2020). The Psychiatry of Old Age Liaison Service This multi-disciplinary team is co-located within Perth Royal Infirmary, a district general hospital with 261 beds.

Day Hospitals There are no NHS day hospital facilities within Perth and Kinross Health and Social Care Partnership. However, the Older Peoples’ Mental Health Teams have excellent links with Day Care facilities provided by other partners.

Senior Medical Staff In June 2019 NHS Tayside’s Associate Medical Director for Older People resigned after many years having successfully established Scotland’s first Older Peoples Clinical Board within NHS Tayside. At the point of writing clarification is awaited regarding the replacement process and the professional management structure for senior medical staff within the department is under review. The consultant group currently adopts a collegiate approach to Clinical Lead related decision making and task allocation. Dr. Hamish Dougall is Clinical Director for the Perth and Kinross Health and Social Care Partnership. The consultants within the department are: 1. Dr. Susan Angus (0.75 whole time equivalent WTE) Murray Royal Hospital in-patients, Perth Royal Infirmary Psychiatry of Old Age Liaison Service, ECT Deputy 2. Dr. Mhairi Hepburn (1 WTE) Perth City Locality Older People's Mental Health Team and Foundation Link for Psychiatry. 3. Dr Candida Graham (1 WTE) South Locality Older People's Mental Health Team and In-patients at Murray Royal Hospital. 4. Dr Gary Cousland (1 WTE) North West Sub-Locality Older Peoples’ Mental Health Team, Murray Royal Hospital Inpatients and ECT Lead. Due to retire in March 2020. 5. Locum (1 WTE) North East and South East Sub-Locality Older Peoples’ Mental Health Teams

The consultant group are friendly, supportive and get on well. They meet weekly to discuss clinical and management issues and have an equal say in issues that affect the group. The department has an excellent track record of supporting medical staff with health problems. Dr Mhairi Hepburn is one of the local contacts for both Doctors' Support Network and Tea and Empathy. Other Medical Staff The department is supported by a Core Psychiatry Trainee, a GP Speciality Trainee and a Foundation Year 1 Trainee. Higher Specialist Trainees rotate into the Perth and Kinross Service and are allocated a trainer dependant on their training needs. There are no Speciality Doctors or Associate Specialists within the department.

Medicine for the Elderly The Medicine for the Elderly Service is based at Perth Royal Infirmary with a growing emphasis on community-based working. The service is approachable and accessible with a designated consultant available for advice and a session per week for review of in-patients at Murray Royal Hospital.

Support staff All parts of the service adopt multi-disciplinary working and have access to Occupational Therapy, Physiotherapy, Dietetics, Pharmacy, Speech and Language Therapy, Clinical Psychology, Social Work, Mental Health Officers and Alzheimer Scotland Link Workers. The in-patient units have dedicated Social Work input which has markedly improved flow and capacity. There are 3 Clinical Psychologist posts for over 65s and trainee psychologists are regularly on placement within the department. The Psychology Service for Older Adults provides a variety of psychological therapies, neuropsychological testing and works in collaboration with the Psychiatry of Old Age Service. There is access to Psychotherapy Services and the department will assess and treat older people upon request.

Administration and Clerical Support All administrative and clerical staff who support the consultant group are based at Murray Royal Hospital with each whole-time equivalent consultant having access to 0.5 whole time equivalent secretarial support.

Voluntary agencies Voluntary agencies are active in Perth and Kinross; the Crossroads Organisation, Princes Royal Carers Trust and Alzheimer’s Scotland being amongst those most involved.

Social Care resources Residents in Perth and Kinross can be supported at home by local authority care providers, a number of private care organisations and increasingly older people and their families are choosing Self Directed Support to further improve continuity and person centred care. Sheltered Housing options are available throughout the region. Across Perth and Kinross there are 41 care homes offering residential, nursing and specialist dementia care and expertise. The Care Homes are supported by the Care Home Liaison Service which includes experienced nursing staff who are supported by their locality consultant psychiatrist. The Psychiatry of Old Age Service does not have any Continuing Care beds so it is important to maintain good working relationships with the care home sector. There is no Day Hospital provision within the Psychiatry of Old Age Service but day opportunities are available across Perth and Kinross, delivered by the local authority and voluntary organisations.

There is a drive to re-establish a service wide, multi-disciplinary forum to ensure that Psychiatry of Old Age Services are shaped centrally but delivered locally.

CLINICAL GOVERNANCE At NHS Tayside Health Board level there is the Older Peoples’ Clinical Board (OPCB). All Consultant Old Age Psychiatrists are invited to attend the Older Peoples’ Mental Health Sub- group of the OPCB. This is with a view to contributing to the regular Clinical and Care Governance Group meetings held regularly within each Locality and the In-patient Service. The governance groups must refer to NHS Tayside’s Older Peoples Clinical Board's Refreshed Clinical Services Strategy for Older People (2018-2020). Time is allocated within each consultant’s job plan for activity related to Appraisal and Revalidation which includes activities related to clinical governance. Audit is supported by the department and a Consultant Old Age Psychiatrist colleague in Angus co-ordinates audit activity in Psychiatry of Old Age across NHS Tayside.

TEACHING, TRAINING AND CONTINUING PROFESSIONAL DEVELOPMENT During the academic year there is at least one 4th year medical student from the University of Dundee on clinical attachment within the Psychiatry of Old Age Service. Final year medical students can also elect to undertake a Dementia Special Study Module over a 4-week period. The University of Dundee Medical School runs a 4-week psychiatry teaching block during 3rd year and medical staff are encouraged to volunteer to provide tutorials, practical sessions and/or lectures. In addition, there are many other medical education opportunities available at the University of Dundee such as student support, OSCE examinations and admissions interviews.

The department supports the training of a Foundation Year, GP Speciality Trainee and a Core Psychiatry Trainee. Responsibility for clinical and educational supervision of the trainees is shared by the consultant group. In accordance with the Royal College of Psychiatrists’ Guidelines one hour per week per trainee is dedicated for supervision. Trainers are encouraged to contribute towards mock CASCs and the weekly teaching programme based at Murray Royal Hospital, which provides Continuing Professional Development for Doctors in Training, Speciality Doctors and Consultants. There is also a monthly CPD afternoon for senior medical staff at the Carseview Centre in Dundee. Murray Royal Hospital has excellent library facilities.

There are a number of peer groups available and the successful candidate would be encouraged to identify and engage with a mentor.

CLINICAL RESEARCH The Psychiatry of Old Age Service has a long history of promoting and engaging in research as well as publications. There is easy access to The Psychiatry of Old Age Research Unit within the University of Dundee. NHS Research Scotland’s Neuroprogressive and Dementia Network is based at Ninewells Hospital but maintains an excellent link with Murray Royal Hospital. JOB PLAN CONSULTANT OLD AGE PSYCHIATRIST

Full-time Consultant for NHS Tayside based at Murray Royal Hospital, Perth.

1 GENERAL PROVISIONS

Consultants will work with local Managers and professional colleagues in the efficient running of services. Subject to the provisions of their Terms and Conditions of Service, Consultants will observe the Trust’s agreed policy and procedures which on clinical matters will be drawn up after appropriate consultation with the profession. Subject to the provision of the Terms and Conditions of Service, Consultants will follow standing orders and financial instructions of NHS Tayside, and will comply with appropriate Health and Safety Policies.

The appointment is subject to the Terms and Conditions of Service of Hospital, Medical and Dental staff as applicable to Consultant grade and the National Health Service (Scotland) Superannuation regulations.

The appointment will be on a full-time 10 programmed activity basis.

The successful candidate will be expected to reside within an approved distance of Murray Royal Hospital.

Appointments will be subject to satisfactory Occupational Health and Disclosure (PVG) clearance.

Full registration with the General Medical Council and a License to Practise, the possession of the MRCPsych and CCST or equivalent are mandatory.

Employment is subject to a minimum of 3 month’s notice on either side but is subject to the provisions of paragraphs 190 -198 of the Terms and Conditions of Service of Hospital Medical and Dental Staff.

2 GENERAL DUTIES

Each Consultant will have overall clinical and organisational responsibilities for a specific part of the psychiatric service as detailed below. This will include responsibility for the diagnosis and treatment of illness and the proper functioning of their service.

Consultants will ensure that they liaise adequately with other members of the Multi- disciplinary Team of which they may be a part, and with statutory or voluntary agencies as appropriate to their area of clinical responsibility and with other professionals as appropriate.

Each Consultant will have responsibility for a share of the in-patient and out-patient resources associated with the Mental Health Service and to undertake domiciliary consultations where appropriate requests are made. Where it is agreed between parties, Consultants will undertake work for other agencies.

Consultants will undertake duties as approved medical practitioners under the terms of the Mental Health (Care and Treatment) (Scotland) Act 2003.

Consultants may participate on committees as appropriate at a local and national level, and represent NHS Tayside or the Eastern Regional Post-Graduate Medical Education Committee at national level.

Consultants will cover for the annual and short-term sick leave of colleagues.

A Job Plan will be agreed with the person appointed.

The appointee will have secretarial and admin support and an individual office.

3 ON-CALL

The provision of out-of-hour mental health provision is currently under review. At present Consultant Psychiatrists participate in a third on-call rota covering all psychiatric specialties in Tayside at a frequency of about 1:12. The successful applicant would have the option to join this rota but at present it is not compulsory. However, it would be compulsory that the successful candidate actively participate in any discussions or planning around the provision of safe and effective out-of-hours care.

Higher Specialist Trainees provide a Tayside wide second on-call rota.

First contact is with the Crisis Team based at Carseview Centre, Dundee supported by Foundation Year Doctors, Core Psychiatric Trainees, and General Practice Trainees participating on a partial shift rota with Murray Royal Hospital having a separate first on-call rota.

Each Consultant will ensure that there are adequate arrangements for hospital staff involved in the care of their patients to be able to contact them when necessary throughout their working week and when they are on-call. Each Consultant is responsible for ensuring that there is adequate cover for their part of the service during periods of leave subject to the provisions of their Terms and Conditions of Service.

Your residence shall be maintained in contact with the Public Telephone Service.

4 EDUCATION

Consultants will be responsible for the supervision of Junior Medical Staff attached to their Unit. Clinical supervision of a trainee may be expected.

Consultants will also contribute to the Post-Graduate Education of Junior Doctors and the teaching of under-graduate medical students normally from the University of Dundee.

Consultant staff may be involved in educational activities, examination activities and accreditation duties as required. These activities and duties may be local or national.

Each Consultant has an entitlement to Study leave at nationally agreed levels. The Trust currently provides an annual budget of £1000 per Consultant for study leave.

There are various peer groups that include psychiatrists from multiple specialties, plus an active Tayside Old Age Psychiatry Peer Group which meets 3 monthly to cover peer support, education and governance issues amongst the consultant body.

5 AUDIT & RESEARCH

All Consultants will participate in some form of Clinical Audit and will ensure that Junior Staff undertaking audit on their behalf are adequately supervised.

Research interests may be pursued within the Mental Health Services following liaison with the Directorate and senior staff from the University Department as necessary.

6 MANAGEMENT

Each Consultant will be a member of the Psychiatry of Old Age Senior Management Team which holds responsibility for clinical management of the old age psychiatric services.

The Health & Social Care Partnership has a Clinical Director appointed by the NHS Tayside for a term of 3 years (renewable). The Clinical Director will be expected to liaise adequately with other Consultants on matters relating to the provision of clinical services and with other professionals where appropriate.

Consultants may be expected to represent the Psychiatry of Old Age Department in planning service developments and implementing changes, in association with other disciplines and agencies as appropriate, including users and carers.

Consultants are professionally and managerially responsible to the Medical Director. A detailed Organisational Structure will be available to all applicants prior to interview.

7 REMUNERATION

The appointment will be superannuable unless you opt out of the NHS Superannuation scheme.

The salary of appointment (exclusive of any distinction and service awards payable to you) will be that appropriate to a full-time Consultant with starting salary paid according to experience.

In addition to the duties outlined above you may occasionally be required to undertake other duties for a limited period.

8 SPECIALIST DUTIES

The Consultant will be responsible for the provision of inpatient and outpatient services for the population in a sectorised area of Perth and Kinross. A description of existing resources can be found in “Further particulars” below.

9 RESPONSIBILITY FOR RECORDS MANAGEMENT

All records created in the course of the business of NHS Tayside are corporate records and are public records under the terms of the Public Records (Scotland) Act 1937. This includes email messages and other electronic records. Allied Health Professionals of all disciplines, Clinical Psychology and Social Work can all be accessed easily. There is also a Health Community Collaborative Project Worker.

Out-Patient Clinics are held within the community hospitals and some of the Health Centres within the locality which facilitates good working relationships.

1.5 DCCs – Options: a. ECT Clinical Lead This post would involve maintaining specialist knowledge and expertise to ensure the provision of a safe and effective ECT Service at Murray Royal Hospital. ECT is delivered every Tuesday and Friday morning. It is a fixed commitment which requires excellent organisation and communication with the team, including the ECT Deputy to ensure there are no service gaps. OR b. Provide Consultant Old Age Psychiatrist Input to North-West Sub-Locality’s Care Homes and Pitlochry Community Hospital. OR c. A commitment to a proportion of the dementia assessment or functional assessment unit beds at Murray Royal Hospital.

MURRAY ROYAL HOSPITAL - GENERAL DESCRIPTION

OLD AGE PSYCHIATRY A comprehensive service for over 65s is provided - as detailed above.

GENERAL PSYCHIATRY Five Consultants share the service. There are 2 x 18 bed acute admission wards and a Crisis Resolution and Home Treatment Team.

REHABILITATION Two Consultants in this service. This sub-speciality has in-patient beds, resettlement and day care components.The rehabilitation service has close working relationships to Perth & Kinross Social Work Department and access group homes supported accommodation based in Perth City.

FORENSIC PSYCHIATRY Five Consultants share the Forensic Service. They are based in the Rohallion Unit. There are low secure beds, a day hospital and a new medium secure inpatient facility for the North of Scotland opened in Summer 2012. All Consultants participate in the provision of psychiatric reports for the courts.

CHILD AND FAMILY PSYCHIATRY This is a Tayside-wide service but two Consultants are based in Perth, where they are supported by a multi-disciplinary team. Inpatient beds are based in Dundee.

OUTPATIENT SERVICES Clinics are held in Murray Royal Hospital and community team centres in Blairgowrie, Kinross, Auchterarder and Crieff. A Health Maintenance Clinic is held weekly in Murray Royal Hospital.

SUBSTANCE MISUSE A Consultant for Tayside Alcohol Problems Service offers sessional time to the Perth and Kinross area along with CPN colleagues. Inpatient facilities moved to Murray Royal Hospital along with the Drug Problem Service when the new building opened in the Summer 2012.

LIAISON PSYCHIATRY All senior medical staff in General Adult Psychiatry and suitably qualified junior medical staff provide a service to Perth Royal Infirmary for patients under the age of 65. There is a multidisciplinary Psychiatry of Old Age Liaison team based within Perth Royal Infirmary under the Consultant lead of a POA Consultant.

PSYCHOTHERAPY A developing Psychotherapy Service now provides a wide range of psychotherapeutic treatment concentrating on the brief psychotherapies. Trainees receive supervision and training at general professional levels as appropriate.

ECT Murray Royal Hospital has a dedicated ECT suite. The ECT department has SEAN Accreditation with Excellence.

CLINICAL PSYCHOLOGY The Clinical Psychology Service is based at Murray Royal although in excess of 95% of referrals are seen in Primary Care settings. A specialist psychology input is provided to the Eating Disorders Service.

OCCUPATIONAL THERAPY & PHYSIOTHERAPY There is a specific occupational therapy service provided to Old Age Psychiatry inpatient services and community teams providing a mix of trained OT and technical assistant input.

DIETETICS One Senior Dietician is based in the Mental Health Directorate.

SOCIAL WORK Considerable reorganisation in the Social Work Department consequent upon Local Government reorganisation has had a major impact on Social Work Services provided to the Directorate. Ongoing discussions between the Trust and the Social Work Department are addressing how this service will be provided in future though this will also be subject to the outcome of the General Adult Psychiatry and Old Age Psychiatry strategies.

TEACHING Murray Royal is part of the East Region’s Psychiatric Training Scheme at Core and Higher Specialist Training level.

There are close links with the Department of Psychiatry and the University of Dundee and the appointee may be awarded the post of Honorary Senior Clinical Teacher. Those with a significant research or teaching commitment may be awarded an Honorary Senior Lectureship.

There is an active multidisciplinary Continuing Education Programme for which responsibility is rotated amongst the consultant group.

There is an excellent Postgraduate Library at Murray Royal and also access to the main Medical Library in Ninewells Hospital in Dundee. The library facilities at Murray Royal have been upgraded to allow Internet access and access to medical databases.

AUDIT An active programme of Clinical Audit is pursued by medical staff throughout the hospital. There is a Medical Audit Sub-Committee consisting of the Clinical Director and the Clinical Tutor and Consultants, which co-ordinates audit activity throughout the hospital
