Minutes of the National Nutrition Cluster Meeting Tuesday, 7th May 2014, 10:00 am to 12:30 pm DoH Conference hall Afgooye Road, Wadajir Mogadishu, Somalia.
1. Welcome and Introductions: Dr Hufane and Abdullahi Nur chaired the meeting which was opened with Holly Quran prayer followed by introductions of the participants. The two appreciated the partners’ active participation. Abdullahi went through the agenda then called out for contributions to the Any Other Business agenda item, nonetheless no suggestions were made.
2. Review and endorsement of the last cluster meeting minutes and follow up on the action points: The review of last minutes - 7th of April 2014 meeting was done and approved as a correct record of issues discussed. Action points were also followed up accordingly; see the status on page 8 of the minutes.
3. Agency Profile presentation: GEWDO and AYUUB made a brief presentation of their agency profile.
Gedo Women Development Organization (GEWDO)
Gedo Women Development Organization is a non-governmental non-political, non-profit organization based in Garbaharey District, Gedo region, Somalia. The organization was founded by a group of Somali women intellectuals in 2006 for the welfare of the poor and vulnerable Somalia communities, especially women, children and marginalized people through relief, rehabilitation and development programs.
Vision
To create an empowered Somali society with good capacity and improved living standards that values justice and meets its own developmental needs.
Mission
To improve the quality of life of the poor and most vulnerable people living in marginalized areas of Somalia by promoting peace and justice, strengthening livelihoods to attain sustainable and self-reliance development.

AYUUB AYUUB: is a local NGO founded by educated and determined persons, to mitigate the plight of vulnerable people; orphans of war and famine being first priority by providing food, shelter, health and education. The organization is based in Merka district in Lower Shabelle region and has sub- offices in Mogadishu and Nairobi.
Vision: A new Somalia founded on morals, civil rights, democracy, ethnic and gender equality, solidarity.
Mission To strive to protect and advocate for better practice of the children’s rights and improve their lives by enabling them to become responsible members of Society.
Target Groups
AYUUB operations and target groups are as follows: Women, Children and old age People, Communities affected by disasters (Natural and man- made) Youth and the vulnerable Groups, the Internally Displaced People & Communities with special needs.
Comments: A partner observed that profile presentations made were not in line with the guidance note shared by the Nutrition Cluster for standardization and harmonization of partner profile presentations. Abdullahi Nur concurred on the comment raised and volunteered to recirculate the guidance note for the partner’s reference when developing profile presentations.

4. Key Nutrition services and Situation highlights: SRDA; WFP handed over the TSFP program the organization has been implementing in Gedo to World Vision which in turn partnered with SRDA to implement 12 nutrition sites.
GEWDO; Signed PCA with UNICEF to implement 1 fixed SC, 1 fixed OTP and 12 mobile OTP sites in Garbaharey.
TROCAIRE; running 4 OTPs integrated with TSFP since April 2014 in Gedo villages and planning to extend to 3 more OTPs integrated with TSFP.
Mercy USA; Agreed with UNICEF to release OTP supplies as long as the agency caters for the running cost. Working on ways to adjust beneficiaries’ increment for the TSFP program since WFP planned certain number of beneficiaries per district.
Muslim Aid; PCA expired since March 2013 so the organization has only been conducting screening, giving out micronutrients and referring malnourished cases where possible. No supplies have been received for the last six months. In Wanlaweyn malnourished beneficiaries are high due to recent influx of returnees from Saudi Arabia, Kenya and Yemen.
Comments:
A member wondered why UNICEF is scaling down the OTP programmes during this critical time where the partners are reporting increased admissions in their nutrition centres. No UNICEF representative was present in the meeting to respond however, the Nutrition Cluster Coordination Assistant enlightened members that PCAs were delayed due to funding shortfalls as Muhiadin-UNICEF Nutrition Specialist had earlier communicated to the partners in the last quarter of 2013, also the last Post Deyr 2013/2014 results by FSNAU indicates improved malnutrition rate across the country.
JCC; experiencing supplies shortage and difficulties in accessing some districts and villages in Lower Jubba.

SHARDO; will cater for program running cost while UNICEF will continue releasing nutrition supplies till June 2014 to save lives of vulnerable people. The organization also signed FLA for TSFP program encompassing TSFP and IF.TSFP will be implemented in Warsheikh and Balad districts of middle Shabelle region and Abdiaziz, Shibis and Boondheere districts of Banadir region while the IF will be implemented in Dharkenley TB Center.
ACF; beneficiary trend in April 2014 increased up to 17% in April as compared to March especially K7 to K13 areas. The agency has surplus systematic treatment and willing to donate to agencies in need.
Intersos; opened 3 mobile OTPs that will be covering the floods affected villages in Jowhar and villages between Jowhar and Balacad.
SORRDO; signed FLA with WFP covering 6 districts in Banadir region targeting 2,973 moderately malnourished beneficiaries; under five children and pregnant and lactating women. The program started with previous supplies in stock.
Action Points:
UNICEF to be urged to attend the next-June national Nutrition Cluster Meeting in order update its implementing partners.
5. FSNAU post Gu’ 2014 Nutrition Assessment plan: Mohamed Moalim from FSNAU presented the Gu’ Nutrition Assessment plans to the cluster participants. FSNAU planned to conduct 52 surveys between 19th of May to 22nd of July 2014. He invited any partner/member willing to participate in the assessment. Partners can ccontribute in terms of; Human resource, transport, training Materials, Venues etc.
Action Point
Abdullahi to share FSNAU partner contribution form with the cluster members so that they include their contributions.
6. Access Aid KAP Survey finding: Project work • Formation of network of Care Groups at household level to enhance recommended feeding practices. • Cooking demonstrations. • Radio messages. • Home fortification for Pregnant/lactating mothers and children under 2 years. • Capacity building for partner organization on Infant and Young child feeding.

KAP Survey Objectives • To determine knowledge, attitude and practices on IYCF. • To identify knowledge gaps, cultural beliefs/practices and behaviour patterns that contribute to, and impede efforts to treat and prevent malnutrition among children aged 6 – 59 months and pregnant and lactating women. • To establish the community’s level of understanding on the importance of routine IMAM medications and micronutrient supplements e.g. vitamin A and vaccination. Methodology Sampling framework • The sampling framework for the survey was all villages and IDP camps in Abudwak district. • The target population were care givers of children 0-23 months. Study design and sampling strategies. • First stage: random selection of clusters using ENA. • Second stage used: EPI method to determine actual household to include. Recommendations • Traditional birth attendants and grandmothers should be targeted directly with IYCF programs. • Father support groups, grandmothers, religious leaders groups to be included in the care group model for a better behavior change. • Advocate for women’s literacy and girls education. • Integrate livelihood activities (Provide seedlings and encourage kitchen gardening). • Introduce cooking demonstrations. • Use media for health education. 7. BPSC Assessment on IYCF (Infant and young child feeding): BPSC carried out an assessment on Infant and young child feeding practices situation in 4 Districts/SC-Somalia including Baidoa district, Abudwak district, Beletweyne district and kismayu district. Random sampling was conducted in the 4 districts to find out the real situation on IYCF status.
Objectives of the Assessment: This assessment was designed to fill the information gaps left following the previous assessments. Specific objectives of the rapid assessment were: To learn more about IYCF status and Health Extension Workers’ and Voluntary Community Health Promoters’ knowledge and perceptions of IYCF components.

To identify Health Extension Workers’ and Voluntary Community Health Promoters’ training needs in order for BPSC to design appropriate training packages. To explore the feasibility of using various communication channels to deliver IYCF messages and educational programs. To test the practicability of “small doable actions” with mothers. To explore possible “emotional appeals” among Health Extension Workers and fathers.
Methodologies: The areas for the rapid assessment were selected in collaboration with local people and local authorities on the ground. Developed shorter questionnaire to assess IYCF practices and the complementary feeding based on UNICEF/WHO indicator. A group of Women promoters were recruited, Trained and standardized to randomly visit HHs and Health centres to collect data in each of the four districts. Simple random selection of households.
Key Messages Delivered Key infant and young child feeding messages are mostly delivered during three stages: pregnancy, postnatal period, and early childhood (first 2 years). Different messages are delivered during these different stages. Though HEWs use different ages for the cut-off points for the stages, the first is from pregnancy to 6 months old, the second from 6 months to 1 year, and the third from 1 to 2 years.
Challenges on IYCF Promoting Economic situation is the main challenge mentioned by HWS and Voluntary Community Health Worker (VCHPs) on IYCF promotion. However, lack of knowledge and awareness have also created significant problems, even when the resources are available. Mothers’ heavy workload poses another challenge in adopting IYCF practices. Due to household responsibilities and general workload, they lack time to attend to a specific needs of the child, as a lot is expected from the mother by the entire family as breadwinner.
Recommendations IYCF indicators could be included in the District Health Information System (DHIS). This would be a step closer to ensuring that nutrition issues receive the necessary attention from the national to the district level and that more meaningful information on IYCF is collected. More opportunities to include the indicators into community based research projects as well as larger scale population studies.

Comments: In order to avoid wastage of resource and time the partners were encouraged to carry out joint assessments instead of individual for better reliability and accuracy as per the nutrition cluster recommendation.

8. Nutrition Management Update: Abdullahi Nur made a brief presentation on the nutrition cluster information management. There was no much difference with IM presentation made in April and March except new admissions. See below charts on both MAM and SAM new admissions from January to March.

Update from Nutrition cluster coordinator Nutrition Cluster Visited Baidoa district: Lauryne and Abdullahi Nur paid one week visit to Baidoa town to provide support to the Bay and Bakool Sub-national Nutrition Cluster and hold discussions with individual partners on reporting issues as well as following up the missing reports. The same exercise was planned for the rest regions in Somalia.
