We are happy to hear feedback on anything good or bad that can help us make future conferences better. Also we need to thank Rob Riley, our coach driver, who allowed us to get a good price for the coach as he donated his time. The coach was well used for the travellers to and from Wellington and the local use around Wanganui. Our host group SteamRail Wanganui did a great job of making us welcome, and putting together interesting and useful delegate packs. Thanks to the team led by Blair Jordon, particularly Ray Deerness, who at the last minute agreed to guide a local tour that included many locations of both railway and tramway heritage around the city. Our guest speaker Chris Northover gave a very interesting presentation on the early days at Taieri Gorge Railway as it was ushered from a small volunteer organisation into a major professional tourist operation. -

Report 01-107 passenger express Train 201 derailment Otaihanga (near Paraparaumu) 6 June 2001 Abstract On Wednesday, 6 June 2001, at about 1841, express passenger Train 201 Overlander derailed when a wheel on the leading axle of the trailing bogie of the power van fractured as the train approached Otaihanga Road level crossing near Paraparaumu on the North Island Main Trunk. There were no injuries to train crew or passengers and damage was limited to the trailing bogie of the power van. The safety issue identified was the suitability of the wheel set for use in passenger rolling stock. As a result of the actions taken by the operator following this incident, no safety recommendations are included in this report. Report 00-101 Train Control Incidents Hi-Rail Vehicles and Trains Occupying
Report 00-101 train control incidents hi-rail vehicles and trains occupying the same section of track and a collision various localities 17 December 1999 – 5 September 2000 Abstract On 17 December 1999, an incident occurred near Greymouth where a locomotive engineer was given permission by train control for his train to enter a section of track already occupied by a hi-rail vehicle. The driver of the hi-rail vehicle saw the train and was able to off-track in time to avert a collision. On 17 January 2000, train control gave permission for a group of hi-rail vehicles to on-track near Paerata in front of a passenger express train. The train collided with one of the hi-rail vehicles and pushed it some 300 m along the track. The hi-rail vehicle, which was unoccupied at the time, was destroyed in the collision. Another 2 incidents occurred where train control gave permission for trains to enter sections of track occupied by hi-rail vehicles: one on 14 February 2000 near St Andrews, and one on 5 September 2000 near Woodville. Neither of these 2 incidents resulted in collisions. Given the similar issues arising from each incident, all 4 were combined into this one report. Safety issues identified included: · the repeated non-adherence to basic train control techniques taught during training and covered by procedures in the operating code · inadequate auditing and assessment of train controller performance · train controllers not using, nor being required to use, signal “blocking commands” as a defence against them issuing conflicting instructions to track users · the potential for train controllers to report for duty when not fit to do so. -

Report 07-101 express freight Train 736, derailment, 5 January 2007 309.643 km, near Vernon The Transport Accident Investigation Commission is an independent Crown entity established to determine the circumstances and causes of accidents and incidents with a view to avoiding similar occurrences in the future. Accordingly it is inappropriate that reports should be used to assign fault or blame or determine liability, since neither the investigation nor the reporting process has been undertaken for that purpose. The Commission may make recommendations to improve transport safety. The cost of implementing any recommendation must always be balanced against its benefits. Such analysis is a matter for the regulator and the industry. These reports may be reprinted in whole or in part without charge, providing acknowledgement is made to the Transport Accident Investigation Commission. Report 07-101 express freight Train 736 derailment 309.643 km, near Vernon 5 January 2007 Abstract On Friday 5 January 2007, at about 2200, the leading bogie of UK6765, the rear wagon on Christchurch to Picton express freight Train 736, derailed at 309.643 kilometres (km) on the Main North Line. The derailed wagon was dragged a further 3.5 km until it struck the south end main line points at Vernon, derailing the wagon immediately in front. The derailed wagons were pulled another kilometre before the locomotive engineer became aware of the derailed wagons and brought the train to a stop. There were no injuries. A safety issue identified was the current track and mechanical tolerance standards. In view of the safety actions since taken by Toll NZ Consolidated Limited and Ontrack in response to safety recommendations made following similar investigations by the Commission, no further safety recommendations were made. -

Evaluation of occupational noise exposure levels on the Wellington Suburban Tranz Metro Rail Service 1 Ka’isa Beech and 2 Stuart J McLaren 1 Advanced student at Massey University and Wellington Suburban On-board Staff Member, KiwiRail, Wellington 2 School of Public Health, Massey University, Wellington 2 Email: [email protected] Abstract An occupational noise evaluation study was carried out by Massey University students on locomotive engineers and on-board staff on the two main train sets operating on the Wellington Rail Suburban Network. All measurement results conducted as part of the study show full compliance with the criteria for workplace noise exposure prescribed within the Health and Safety in Employment Regulations 1995. The health and safety noise criterion level permits a maximum dose of 100% which is equivalent to 85 dB LAeq for a normalised 8 hour working day. The highest measured sound exposure was 13% of the total permitted exposure. All occupational noise measurements were observed and written accounts were taken by the three person investigation team. Observations revealed atypical behaviour of one participant which likely compromised one set of readings. This atypical result was removed from the analysis and therefore did not alter the study conclusions. Regardless, such observed behaviour from the study team, reinforces the value of observed real time field monitoring during collection of data. Original peer-reviewed student paper 1.
