Y concur with Dr. Jolson, Deputy Director, as well as the NDA review team that this application is approvable. Several issues raised in the review process and described in the action package require clarification. Issues related to appropriate dosing, description of selectivity ) for specific alpha receptors, adequate information regarding the risk of orthostatic hypotension and elimination of references to statistical testing at time points other than those intended per protocol have been discussed with the review team and with the sponsor. Appropriate labeling, which addresses these concerns, is now included in the action package. In terms of Phase IV commitments recommended by the review team: I agree with the proposal to allow the submission of information related to in vitro dissolution and metabolic testing to be post-approval. The sponsor has performed this study and we anticipate submission of results within the next six months. The biopharmaceutics review also raises the issue of a possible drug interaction study between tamsulosin and other alpha-adrenergic blocking agents. On discussion with the biopharmaceutics review team it seems that this was a misunderstanding of the M~ reviewer’s concern for possible drug interactio-ns of tamsulosin with a 5-alpha reductase inhibitor. A drug interaction study between tamsulosin and other alpha-adrenergic blocking agents is not recommended as this product is specifically labeled (in the Drug-Drug Interactions. section of the prescribing information) NOT to be used in combination with alpha-adrenergic blocking agents. The possible requirement for—a drug interaction study of the combination of tamsulosin with a 5-alpha reductase inhibitor is discussed below. The MO review raises two issues for possible post-approval consideration: I agree with Dr. Jolson that the first issue--further quantifying the incidence of retrograde ejaculation--is not a required commitment for approval. The ‘“ - information regarding the observation of a dose- dependent effect on retrograde ejaculation is included in the current label. Unless the sponsor intends to make claims regarding the combined safety and effectiveness of tamsulosin in combination with a 5-alpha reductase inhibitor, I ) collude that this sponsor should not be required to undertake such a study. We look forward to the results of an NIH sponsored study which is evaluating the efficacy of other approved alpha adrenergic blocking agents in combination with finasteride (a 5-alpha reductase inhibitor) for this indication. The sponsor claims that this ~-antagonist is more selective for the la receptors of the prostatic smooth muscle than for receptors at other sites. In support of the request for approval, the .) sponsor has submitted the results of five controlled studies of which two studies (US92-03A and US93-O 1) provide the primary evidence of safety and efficacy. Together, these two studies randomized 1486 men with the signs and symptoms of BPH to receive either tarnsulosin 0.4 mg or 0.8 mg, or placebo for 13 weeks of treatment. Primary efficacy parameters included: 1) changes from baseline in total American Urological Association (AUA) Symptom Score, and 2) changes from baseline in peak urine flow rate. I concur with Dr. Fourcroy, the primary medical reviewer, that this application is approvable and that the efficacy of this product based on the previously noted efficacy parameters is comparable to other products in its class. Tolerability with this product during the conduct of the clinical trials was generally acceptable, however notable adverse experience with dosing. includes: orthostasis and associated symptoms (as with other products in this class) and retrograde ejaculation. Issues of note at the time of this regulatory action include: 1. Dosing The sponsor has requested that the label provide a recommendation for dose escalation to 0.8 mg for men who fail to respond to the 0.4 mg dose. Unfortunate y, in the clinical trials, men were randomized to the lower and higher dose, rather tha~ a~lowing for titration-to-effect.     SUMMARY OF PRODUCT CHARACTERISTICS 1. NAME OF THE MEDICINAL PRODUCT XATRAL 2.5 mg film-coated tablets. XATRAL SR 5 mg sustained-release tablets 2. QUALITATIVE AND QUANTITATIVE COMPOSITION Each Xatral 2.5 mg tablet contains 2.5mg alfuzosin hydrochloride. Each Xatral SR 5 mg tablet contains 5mg alfuzosin hydrochloride. 3. PHARMACEUTICAL FORM Xatral 2.5 mg is a white round film coated tablet for oral administration. Xatral SR 5 mg is a pale yellow biconvex film coated sustained release tablet for oral administration. 4. CLINICAL PARTICULARS 4.1 Therapeutic indications Xatral 2.5mg: Treatment of certain functional symptoms of benign prostatic hypertrophy, notably when surgery has to be delayed for whatever reason and during episodes of severe symptoms of adenoma, especially in elderly patients. Xatral SR 5 mg: Treatment of certain functional symptoms of benign prostatic hypertrophy, particularly if surgery has to be delayed for some reason. 4.2 Posology and method of administration Oral use Xatral SR 5mg tablet must be swallowed whole with a glass of water (see Section 4.4). The first dose of Xatral SR 5mg or Xatral 2.5 mg tablets should be given just before bedtime. Adults: Xatral 2.5mg: The recommended dosage is one tablet Xatral® 2.5mg three times daily. The dose may be increased to a maximum of 4 tablets (10mg) per day depending on the clinical response. ® Xatral SR 5mg: The usual dose is one Xatral SR 5 mg tablet morning and evening. Elderly patients (over 65 years) or patients treated for hypertension: Xatral 2.5mg: as a routine precaution, it is recommended that treatment be started with one Xatral 2.5 mg tablet morning and evening and that the dosage then be increased on the basis of the patient's individual response, without exceeding the maximum dosage of 4 Xatral 2.5 mg tablets daily.

Noradrenaline was about 3 and 10 times more potent than phenylephrine and methoxamine respectively. Phenylephrine and methoxamine were partial agonists. 3 Pretreatment with the alkylating agent, chlorethylclonidine (10-4 M) shifted the noradrenaline concentration-contraction curve about 3 fold to the right and depressed the maximum response by 31%. This shift is 100 fold less than that previously shown to be produced by chlorethylclonidine under the same conditions on OlB-adrenoceptor-mediated contractions. 4 Cumulative concentration-contraction curves for noradrenaline were competitively antagonized by WB 4101 (pA2 9.0), 5-methyl-urapidil (pA2 8.6), phentolamine (pA2 7.6), benoxathian (pA2 8.5), spiperone (pA2 7.3), indoramin (pA2 8.2) and BMY 7378 (pA2 6.6). These values correlated best with published pKi values for their displacement of [3H]-prazosin binding on membranes expressing cloned oczc-adrenoceptors and poorly with values from cloned lb- and cld-adrenoceptors.

Steroid 5α-reductase isozymes are expressed in key peripheral tissues and catalyze the conversion of testosterone into the more potent androgen 5α - dihydrotestosterone (DHT). Activation of maximal androgen signaling requires conversion of testosterone to DHT to bind to the androgen receptor (AR) and induce transactivation of downstream gene signaling. The evaluation of chemical-mediated disruption of androgen signaling is typically performed through a combination of in vitro and in vivo tests that interrogate both receptor and non-receptor events including accessory sex organ (ASO) development. Chemical-mediated disruption of ASO development may occur via inhibition of 5α-reductase, or direct disruption of AR signaling. The objective here was to establish a higher- tier human-based, in vitro assay of ASO growth using a multiplexed 3D prostate epithelial cell microtissue model. The androgen-sensitive LNCaP cell line was seeded in a 96-well hanging drop culture model to evaluate microtissue growth over a period of 11 days. Prostate Specific Antigen (PSA) secretion, a biomarker for AR signaling, yielded a wide dynamic range with a Positive/Negative control (P/N) ratio of 3,326 and Z’ of 0.78.

    YASAKLI MADDELER Ülkemizde yarış atlarında kullanılan/kullanılma ihtimali olan ilaçların/maddelerin belirlenmesinde; ilacın farmakolojisi (öncelikle etkisi, etki grubu), farmasötik şekli, kullanılma yolu, kullanılma alanı, yarış sonucunu etkileme durumu, ulusal mevzuata göre veteriner hekimlikte ve/veya yarış atlarında kullanmak için ruhsatlı/izinli olup-olmama durumu, etiket-dışı kullanılma gibi hususlar dikkate alınmıştır. Eşik değeri (Threshold level) olan maddelerin (vücutta şekillenirler, çevre ve yem kaynaklı olabilirler) miktarı, belirlenen miktarı aştığında yasaklı-madde kullanılması/doping ihlali olarak değerlendirilir. Tedavide kullanılan/tarama limiti (Screening level) olan maddelerle ilgili değerlendirmede; bu maddelerle ilgili olarak yapılan doping analizlerinde ölçülen miktar, idrarda/plazmada belirlenen tarama değerini aştığında yasaklı madde kullanılması/doping-ihlali olarakdeğerlendirilir.     Guideline: Preoperative Medication Management Guideline for Preoperative Medication Management Purpose of Guideline: To provide guidance to physicians, advanced practice providers (APPs), pharmacists, and nurses regarding medication management in the preoperative setting. Background: Appropriate perioperative medication management is essential to ensure positive surgical outcomes and prevent medication misadventures.1 Results from a prospective analysis of 1,025 patients admitted to a general surgical unit concluded that patients on at least one medication for a chronic disease are 2.7 times more likely to experience surgical complications compared with those not taking any medications. As the aging population requires more medication use and the availability of various nonprescription medications continues to increase, so does the risk of polypharmacy and the need for perioperative medication guidance.2 There are no well-designed trials to support evidence-based recommendations for perioperative medication management; however, general principles and best practice approaches are available. General considerations for perioperative medication management include a thorough medication history, understanding of the medication pharmacokinetics and potential for withdrawal symptoms, understanding the risks associated with the surgical procedure and the risks of medication discontinuation based on the intended indication. Clinical judgement must be exercised, especially if medication pharmacokinetics are not predictable or there are significant risks associated with inappropriate medication withdrawal (eg, tolerance) or continuation (eg, postsurgical infection).2 Clinical Assessment: Prior to instructing the patient on preoperative medication management, completion of a thorough medication history is recommended – including all information on prescription medications, over-the-counter medications, “as needed” medications, vitamins, supplements, and herbal medications. Allergies should also be verified and documented.
