Many people with breast cancer face unexpected medical expenses and are not able to work during the course of treatment, adding a huge financial toll to their already difficult struggle. Bridging the Gap Emergency Breast Cancer Fund seeks to lessen the burden on people with breast cancer by providing emergency short-term financial assistance to those in need to help cover household bills and living expenses. Not worrying about unpaid bills will help recipients to better focus on treatment, healing, and recovery. Bridging the Gap recipients will be referred by a healthcare professional to the program to receive financial assistance with such things as rent, utilities, car expenses, insurance bills, etc. Potential recipients will be screened by the program manager to ensure they meet the program requirements. Eligibility criteria will include residency in one of the 17 counties served by the Komen St. Louis Affiliate, current income at or below 350% of the Federal Poverty Level, and being within a three-month window of receiving active breast cancer treatment (defined as surgery, radiation therapy and/or chemotherapy, excluding long-term hormone therapy). Early Detection of Breast Cancer in African American Women Community Health-In-Partnership Services (CHIPS) Judy Bentley, WHNPBC, MA, President and CEO, (314) 652-9231 ext. 19 http://www.chipsstl.org/ This is the 2nd year of Komen St. Louis Affiliate funding for this program. While African American women are less likely to develop breast cancer than Caucasian women, African American women are much more likely to die from the disease. CHIPS’ Early Detection of Breast Cancer in African American Women Program addresses this disparity in the St. Louis Metropolitan Area. CHIPS seeks to improve breast cancer outcomes for uninsured African American women between the ages of 40 and 64 by promoting breast health self-awareness and the importance of early detection. CHIPS will provide one on one instruction, educational materials and referrals to 1,300 women during the funding period and will arrange for women who have not had a mammogram in a year or more to be screened. A mammography van visits CHIPS six times a year to provide free screenings. CHIPS operates a free clinic in North St. Louis staffed by volunteer physicians who care for uninsured and underserved clients from all over the metropolitan area. CHIPS also takes health screenings to at-risk individuals in the community at over 100 outreach and health education events each year.

East St. Louis Breast Cancer Screening & Treatment Southern Illinois Healthcare Foundation Beth Zurliene, RN, Project Director, (618) 482-7126 http://www.khrh.org/ This is the 3rd year of Komen St. Louis Affiliate funding for this program. Southern Illinois Healthcare Foundation is a Federally Qualified Health Center that operates 32 primary health care clinics. Educating Women About Hereditary Risk for Developing Breast Cancer Saint Louis University, Department of Internal Medicine Suzanne Mahon, RN, DNSc, AOCN, APNG, (314) 577-8854 http://www.slu.edu/cancer-center.xml This is the 9th year of Komen St. Louis Affiliate funding for this program. An estimated 10% of women diagnosed with breast cancer have a hereditary predisposition and are at substantial risk for a second breast cancer; 50% of their first degree relatives also carry a very high risk (92%) for developing early onset breast cancer. The established Hereditary Cancer Program at Saint Louis University provides comprehensive education and counseling regarding the potential risks, benefits, and psychosocial ramifications of genetic testing as well as recommendations to detect cancer early, and prevent cancer by using appropriate strategies including prophylactic surgery. A trained genetics professional ascertains there is a comprehensive assessment of risk, the correct test is ordered, and provides coordination of follow-up care for other at-risk relatives. For families in which a specific genetic mutation cannot be identified the program assists in enrollment in clinical trials that will ultimately advance the science of genetics. This request is to continue to improve and expand the existing Hereditary Cancer Program by providing education and counseling to 90 new at-risk families and continuing to provide services to an estimated 75 established families. A series of community programs and personal meetings with health care providers will inform a wider sector about cancer genetic risk and the availability of services.

Empower and Engage SSM St. Mary’s Health Center Foundation Sandy Cobb, RN, Director, (314)768-8696 http://www.ssmhc.com/ This is the 9th year of Komen St. Louis Affiliate funding for this program. and coordination of services and treatment. SSM St. Mary’s Breast Center’s half-time RN Clinical Coordinator is dedicated to the recruitment of underserved women and to coordinating services for those women both in the Empower and Engage Program and Show Me Healthy Women (SMHW) Program. Empower and Engage will serve 200 women during the grant year by conducting 12 recruitment events, along with accepting walk-ins to the SSM Breast Center at St. Mary’s Health Center. The Breast Center will also enlist the services of collaborating programs and organizations, in order to provide exceptional healthcare with a full range of services.

FCHC Breast Health Screening Access Project Family Care Health Centers Kathleen Garst, Chief Operations Officer, (314) 481-1615 ext. 1314 http://www.fchcstl.org/ This is the 2nd year of Komen St. Louis Affiliate funding for this program. Upon initiation of a navigator program, FCHC has been able to implement evidence-based techniques that increase mammography opportunities for our patients as well as guide them through the screening process. With continued collaboration with Siteman Cancer Center, FCHC will continue to promote patient education and awareness of initial mammogram screenings and expedite follow-up appointments. FCHC plans to increase capacity to 900 mammograms.

Gateway to Hope Gateway to Hope Cynthia Frank, Executive Director, (314) 569-1113 http://www.gthstl.org/ This is the 6th year of Komen St. Louis Affiliate funding for this program. Gateway to Hope (GTH) arranges comprehensive medical and surgical treatment for uninsured and underinsured low income women and men diagnosed with breast cancer and those genetically at high risk for breast cancer. These individuals earn between 200-350% of federal poverty guidelines and are not eligible for publicly funded medical programs. GTH will arrange breast cancer treatment for a minimum of 30 eligible patients and will continue to arrange treatment for approximately 25 individuals already in the program. Additionally, a minimum of 80 referrals will be made to healthcare providers or social service agencies to help individuals not eligible for GTH services. This project supports the work of two nurse managers who arrange treatment for GTH patients and one outreach education nurse.

Iniciativa de Salud para Mujeres (Women’s Health Initiative) Puentes de Esperanza Nereida Avendano, Project Director, (618) 398-0557 http://www.hoyleton.org/ This is the 3rd year of Komen St. Louis Affiliate funding for this program. Puentes de Esperanza (Bridges of Hope) is a faith-based organization whose mission is to enhance the quality of life within a growing Metro East Hispanic/Latino community. Due to language, cultural, social, and economic barriers, Hispanic and Latino women are typically screened for breast cancer later than other women, resulting in lower survival rates. Lack of proper identification and/or legal documentation of immigration status, exacerbates the problem. The Breast HealthCare Center at Missouri Baptist Medical Center continues to be a strong leader with its commitment to reach out to uninsured and under-insured women. With Komen funding, they are able to provide increased access to breast screening services as well as education, prevention, and early detection of breast cancer by providing screening and diagnostic mammograms. These services are provided in the Breast HealthCare Center and on the digital mobile mammography van for at risk women ages 40 - 64. Follow up services are provided at the Breast HealthCare Center for women with mammography abnormalities. The educational component provides knowledge about the importance of early detection of breast cancer by screening and the dynamics of breast self-exam. These educational events are held in the service areas of St. Louis City and County along with eleven rural communities which include: Jefferson, St. Francois, Warren, Ste. Genevieve, Perry, Lincoln, Madison, Washington, Franklin, St. Charles, and Montgomery County. The proven success of this project has demonstrated that women in these service areas desire to utilize resources to gain information on breast health and undergo a screening procedure on our van. With Komen funding, Missouri Baptist plans to educate and screen over 600 women in the current grant year.

Nutritional Support for Low Income Breast Cancer Patients Food Outreach, Inc. Greg Lukeman, Executive Director, (314) 652-3663 ext. 115 http://www.foodoutreach.org/ This is the 4th year of Komen St. Louis Affiliate funding for this program. Food Outreach’s Breast Cancer Program provides comprehensive nutritional support to low-income breast cancer patients through a combination of scratch-made nutritious meals, groceries, and nutrition education. Many are ineligible for senior meals programs and due to the nature of the illness and treatments require a tailored nutrition program, not available through traditional food pantries. Our on-staff Chef and Registered Dietitian develop menus targeting our clients’ specific nutrition needs to help them maintain strength, cope with side effects and have better treatment outcomes. Each client meets with our Dietitian for an initial nutritional assessment. The Dietitian recommends an eating plan taking into consideration the assessment results, treatments and side effects, and any secondary illnesses. Our Dietitian provides follow-up nutrition status assessments, and with approval, forwards results to the client’s healthcare team for continuum of care. Every two weeks, clients order the equivalent of two meals per day from a changing menu (up to 28 meals per order). Choosing their own meals/groceries is significant, since taste changes are prevalent throughout treatment. Our scratch-made meals are frozen in microwaveable containers – ensuring convenient access to healthful meals when clients are too fatigued to fully prepare a meal. Food Outreach also provides a meal home delivery program.

Reaching In: A Comprehensive Program for Breast Screening Saint Louis University Mark Varvares, MD, Director, Saint Louis University Cancer Center, (314) 268-7053 http://www.slu.edu/cancer-center.xml This is the 2nd year of Komen St. Louis Affiliate funding for this program. Saint Louis University (SLU) Cancer Center is expanding its Breast Cancer Screening Program. Komen St. Louis Affiliate funding will provide a nurse navigator to assist underserved/uninsured or underinsured, low income women ages 40- 65 in St. Louis through a complex healthcare system to obtain necessary and regular breast cancer screening. The St. Louis area consists of a diverse population with significant psychosocial, logistical and physical issues impeding them from getting needed care. A nurse navigator acts as an accessible contact for each woman, offering continuity of care, coordination of services, education and psychosocial support throughout the care continuum. SLU is focusing in-reach efforts to provide breast health care to underserved, at-risk women who are currently receiving tertiary care at SLU, as well as family members, friends and support employees at the University that are uninsured or underinsured. SLU Cancer Center has expanded its community outreach efforts including participation at local health fairs, educational symposiums and community events to educate and provide referrals for services for at risk persons. SLU estimates that 600 women will be served through this Komen-funded partnership.

Rural Missouri Outreach Project East Missouri Action Agency Angel Prather, Program Director, (573) 431-5191 ext.1122 http://www.eastmoaa.org/ This is the 1st year of Komen St. Louis Affiliate funding for this program. EMAA will help uninsured and underserved women in Jefferson, Madison, Perry, St. Francois, St. Genevieve, and Washington counties in Missouri. The work will be provided in cooperation with Missouri Baptist Breast Healthcare Mammogram Van, Washington County Hospital, Madison County Regional Hospital, and Jefferson Regional Medical Center. EMAA has discovered that many women-particularly those in rural areas- do not have the transportation readily available for them to take advantage of the mammogram van when it visits larger towns in their counties. Having the van visit smaller, more out-of-the-way communities will give women better access to services. The program plans to provide screening mammograms to at least 185 women.

Solutions for Women: Breast Health Education and Outreach for Underserved Women Christian Hospital Northeast-Northwest Sandi Kenkel, Manager, Radiation Oncology, (314) 653-4498 http://www.christianhospital.org/ This is the 3rd year of Komen St. Louis Affiliate funding for this program. For women over 40 years of age and those at high risk for breast cancer and not eligible for this service through other means they will qualify for a screening mammogram. If necessary, additional diagnostic testing will be provided. For the small number of women who need additional services, they will be provided referrals to the Show Me Healthy Women Program which is also provided at Christian Hospital through a State of Missouri Grant. Through the outreach coordinator, women will be educated on the importance of good breast health. Through education, women who are at average risk can learn how to reduce their risk of developing the disease by changing certain lifestyle factors. The outreach coordinator is available to provide educational programs for organizations as well as participate in health fairs stressing the importance of self exams and the need for yearly mammograms.

A Phase I/II Trial of Letrozole in Combination with MK2206 in Clinical Stage II or III Estrogen Receptor Positive and HER2 Negative Breast Cancer Washington University School of Medicine Cynthia X. Ma, MD, PhD, Assistant Professor of Medicine Endocrine therapy, such as letrozole, is a standard adjuvant therapy following surgical removal of Estrogen Receptor positive (ER+) breast cancer to prevent recurrence. However, the occurrence of a complete pathologic response, a marker of complete eradication of the cancer, is rare by using endocrine therapy because of its inability to induce cell death. As a result, a prolonged course of therapy after surgery is necessary and thus, strategies to improve the effectiveness of endocrine therapy are needed. We have recently demonstrated, in cell culture studies, that drugs inhibiting the essential cell survival pathway are capable of killing the cancer cells when combined with endocrine therapy.
