PQASSO for – Expression of Interest pack for organisations to take part in the PQASSO for Wales project

Introduction

Thank you very much for your interest in taking part in the PQASSO for Wales project. This Expression of Interest is for an organisation that wants to be one of the 125 organisations we are looking for to take part in the project to implement PQASSO in its organisation.

By submitting the Expression of Interest form we will be able to assess whether your organisation meets the project’s eligibility criteria, as well as making sure we have a good mixture of organisations taking part in the project in terms of geographical location, size and fields of work. Submitting the Expression of Interest form in itself does not guarantee that your organisation will take part in the PQASSO for Wales project.

The Expression of Interest pack includes Section 1 which explains what taking part in the project means. Section 2 is the Expression of Interest form itself. Please make sure you read both sections carefully before completing and submitting the form.

The deadline for submitting the Expression of Interest form is 5.00pm on Friday, 17th July 2015

We cannot guarantee that we will hold another Expression of Interest round.

If your Expression of Interest is accepted we will match your organisation to a PQASSO mentor, who will provide some support to implement PQASSO in your organisation. Please note that this process will take time. Our final course for training mentors will take place in the autumn and we will not be able to match some organisations to a mentor until later this year; this will mainly affect organisations based in West and Mid Wales. For organisations based in South and North Wales, if your Expression of Interest is accepted, we anticipate that you will begin taking part in the project from September.

For ease of identification, we are using current Local Authority boundary areas as a breakdown:
