Subject: 2019 Social Service Funding

Issue: During the FY2019 Budget, the City Council approved $150,000 be allocated to Social Service Funding. These funds will be disbursed to social service agencies that provide services to Des Plaines residents that are in need. The Health and Human Services Division (HHS) has solicited and evaluated grant requests from the social service agencies that serve the Des Plaines community and seek City Council concurrence with the funding allocation as discussed and recommended.

Analysis: The Health and Human Services Division contacted all of the social service agencies that received funding in prior years. In addition, a press release was created and released through the Media Services Division. The press release and the application were both available on the City’s website.

HHS staff met with representatives of each agency that requested funding for 2019. The meeting included a brief agency presentation followed by questions. All of the agencies expressed their gratitude for the City’s program and stressed how valuable local support is for leveraging funding from other sources, government and/or private, which strengthens their service capabilities for residents. Of the 39 agencies requesting funding this year, there were 3 new applicants: “Clean-Up/Give Back” - “Community Backpack Project” - Northwest Compass “Helping Hands Program.”

Of the 20 agencies that receive State funding, 8 agencies expressed that their funding has been delayed between 1 to 4 months. The remaining 13 agencies shared that at this time the State is current with their funding.

HHS continues to utilize a point system to help assist with the allocation and distribution of the 2019 funding. The attached funding recommendations are based on the following criteria: • The number of Des Plaines clients served. • The percentage of Des Plaines clients served within the agency/program. • The program budget. • The abundance or scarcity of a service.

Page 1 of 13 • Staff feedback of an agency’s performance (resident feedback, staff experience, need of program, and application/presentation). • Distribution of funding each year for agencies is based on the total number of applications received. • First time applicants received $1,500.00. • In 2018, Health and Human Services staff received over 2,000 office visits and calls for information and/or referrals. The agencies, along with the services they provide, assist in meeting the vital needs of Des Plaines residents.

Recommendation: As a result of this process the 2019 recommendation is to fund the 39 agencies. A list of the 2019 recommendations and the point system spreadsheet are attached for review and approval of the City Council.

A RESOLUTION AUTHORIZING THE DISBURSEMENT OF SOCIAL SERVICES FUNDS TO THIRTY-NINE SOCIAL SERVICE AGENCIES.

WHEREAS, Article VII, Section 10 of the 1970 Constitution authorizes the City to contract with individuals, associations and corporations, in any manner not prohibited by law or ordinance; and

WHEREAS, the City has appropriated $150,000 in funds for disbursement during the 2019 fiscal year to social services agencies that provide services to Des Plaines residents ("Social Services Funding"); and

WHEREAS, the City Health and Human Services Division has received and reviewed applications for Social Services Funding from various social services agencies that serve the City; and

WHEREAS, the City Council has determined that it is in the best interest of the City to disburse the Social Services Funding in the manner set forth in this Resolution;

NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of Des

Plaines, Cook County, Illinois, in the exercise of its home rule powers, as follows:

SECTION 1: RECITALS. The foregoing recitals are incorporated into, and made a part

of, this Resolution as the findings of the City Council.

SECTION 2: APPROVAL AND AUTHORIZATION TO DISBURSE FUNDS. The

City Council hereby approves, and the City Manager is hereby authorized to make, the

The following is an alphabetical listing of the agencies that the Health and Human Services Division recommend for funding.

Advocate Lutheran General - Older Adult Services 2019 Recommendation: $ 4,500 Funding Request: To increase the frequency and duration of therapeutic activities provided to adults with memory loss, which has proven to decrease overall anxiety and wandering of individuals. In the last 12-months 1,800 residents utilized this program.

Alliance for Immigrant Neighbors 2019 Recommendation: $ 3,500 Funding Request: To expand workshops/services to the residents of Des Plaines to help complete the paperwork to apply for citizenship. Assist qualified residents of Des Plaines with completing the U.S. citizenship application by providing immigration legal services to those who qualify by moving legal consultations forward with an immigration benefit application.

Avenues to Independence 2019 Recommendation: $ 2,500 Funding Request: To provide nursing and other staff support at the Des Plaines group homes. Support adults with intellectual, physical, and developmental disabilities so that they may live, learn, and work in the most independent settings possible. Avenues runs two group homes in Des Plaines.

Bessie’s Table 2019 Recommendation: $ 2,000 Funding Request: To provide food and supplies needed for the weekly community dinners, which items are not donated. Provides a meal to anyone in need of assistance each Monday night. Last year, approximately 2,998 guests attended this program, except during the month of August.

Catholic Charities 2019 Recommendation: $ 6,000 Funding Request: To provide financial support for their “The Nourish Your Neighbor” program that consists of their Food Pantry, the twice-weekly Supper Program, and special holiday meal distributions at Thanksgiving and Christmas. Clients can visit the Food Pantry once a month and get a 4-day supply of food, which can include canned/dry goods, meat, produce, and dairy products. The Supper Program provides a hot, nutritious dinner on Tuesday and Thursday nights.

Center of Concern 2019 Recommendation: $12,500 Funding Request: To provide housing programs, senior support service, counseling and emergency assistance, support services primarily for the elderly, disabled, and needy in the Des Plaines community. Funding will support the services they provide to Des Plaines residents. Such services include: case management, CHORE housekeeping services, home visits, counseling, family support, professional volunteer services, financial assistance to prevent homelessness, home sharing, transitional housing, transportation/shopping for seniors, and information and referral.

Children’s Advocacy Center of Northwest Cook County 2019 Recommendation: $ 2,748 Funding Request: To provide partial salary support for a Licensed Counselor, who provides services to Des Plaines residents in the both the CASI and Family Support Services Programs. They provide direct services for child victims of sexual abuse, severe physical abuse and other

Attachment 2 Page 6 of 13 violent crimes, and their non-offending parents and family members. The CASI Program provides 24-hour emergency response, coordination of the child abuse investigation, expert child interviews, crisis intervention, court advocacy, on-going support, and referral services. The FSS Program offers intensive, trauma-focused individual and family counseling, child abuse assessments, and parent/child support groups. All services are offered in English and/or Spanish and are provided free of charge.

Clean Up - Give Back 2019 Recommendation: $ 1,500 Funding Request: To assist with the general operating costs of the program. Provides a resource to individuals required to complete community service hours required through school/court. Provides individuals/organizations the opportunity to make our community better through organized clean-up campaigns.

Community Backpack Project 2019 Recommendation: $ 1,500 Funding Request: To supplement the food that is currently donated by groups and/or individuals. The Backpack Project works with School District #62 students to help bridge the gap from a child’s last meal at school, usually Friday lunch, until they return to school on Monday and receive breakfast. In the 2017-2018 school year, the Community Backpack Project supported 19 students in 4 schools. This current school year the project is supporting 69 students in 7 schools.

Connections for the Homeless 2019 Recommendation: $ 2,750 Funding Request: To support the programs and expand services providing more targeted support for shelter residents and those living on the streets. Provide housing, re-housing, shelters, employment, case management, and supportive services to those who are homeless or who are being burdened with their housing costs.

Des Plaines Community Foundation 2019 Recommendation: $ 6,000 Funding Request: To support the Neighbors Helping Neighbors Program, which supports direct assistance by providing groceries, transportation, minor home repairs, ramps, and other assistance to Des Plaines residents in need. Supports community programs of the Des Plaines Healthy Community Partnership by generating and distributing financial and community resources.

Feed My Sheep 2019 Recommendation: $ 2,200 Funding Request: To purchase additional food and supplies needed for meals that are not donated. Provides a meal to anyone in need of assistance each Wednesday night. Last year approximately 2,316 dinners where provided.

FISH of Park Ridge 2019 Recommendation: $ 1,500 Funding Request: To support their transportation program. Provide free transportation to medical appointments for residents of Des Plaines, Park Ridge and Maine Township. In 2018, Des Plaines residents received a total of 420 rides to doctor/hospital/dialysis appointments. That totaled 27% of their clients.

Attachment 2 Page 7 of 13 Five Loaves and Two Fish Ministry 2019 Recommendation: $ 1,000 Trinity Lutheran Church Funding Request: To support the monthly meal program as 75 guests are fed each month totaling approximately 900 guests a year. Provide a hot meal consisting of an entree, soup, salad, side dishes, desserts, and beverages on the second Saturday of each month. Extra meals are given to participants to take w.

Hands-On Suburban Chicago (HOSC) 2019 Recommendation: $ 1,500 Funding Request: To support and development their agency so they can continue recruitment and also connect with volunteers to assist with over 200 nonprofits and schools in need of assistance. In 2018, a total of 2,976 volunteers were referred to 93 nonprofits. Of those, 218 volunteers were referred to 10 nonprofits located in the Des Plaines area. In the Plainfield Elementary School, there were 51 students, which all reside in incorporated Des Plaines, that were also served by HOSC.

JOURNEYS | The Road Home 2019 Recommendation: $ 3,500 Funding Request: To support the Hope Center which provides vocational counseling, mental health counseling, housing counseling, food pantry, and clothing closet. Provide services for those who are homeless or at risk of becoming homeless. Coordinates PADS a temporary shelter 7- nights a week from October through April. Area churches last year where open 5-days a week during the summer. In the last 12 months, 64 clients were served from Des Plaines.

Korean American Women in Need (KAN-WIN) 2019 Recommendation: $ 3,000 Funding Request: To help support KAN-WIN’s Crisis Intervention, via bi-lingual 24-hour hotline and case management services, for domestic violence victims in the Asian immigrant community. In the last 12-months KAN-WIN provided services to 14 Des Plaines residents.

Kenneth Young Center 2019 Recommendation: $ 4,000 Funding Request: To provide case management and counseling services to help seniors remain in their privately-owned residential homes. This senior services agency serves Des Plaines residents residing in the Elk Grove Township by providing case management, counseling, community care services, and adult protective services for seniors.

Life Span 2019 Recommendation: $ 3,141 Funding Request: To support the salaries of counselors, advocates, and attorneys who provide direct service to Des Plaines residents. Provide legal civil representation and informational resources/counseling for victims of domestic violence or sexual assault. Also provide community education, violence prevention programming in schools, and a 24/7 crisis hotline.

Maine Community Youth Assistance Foundation 2019 Recommendation: $ 1,000 Funding Request: To provide education to Maine West High School students and their parents on the dangers of vaping and drug/alcohol use. Continue education for Maine West students/parents through providing a Vaping Class subsidy for free and a reduced lunch for students along with the development of a Vaping Cessation Program for students who want to stop vaping.

Attachment 2 Page 8 of 13 Maine Niles Association of Special Recreation 2019 Recommendation: $ 1,500 Funding Request: To provide transportation to individuals participating in the Special Recreation program. MNASR assists Des Plaines individuals with special needs to recreational programming using MNASR Accessible Transportation Service. In the last 12 months, 25 Des Plaines residents received financial assistance for transportation.

Maryville Academy 2019 Recommendation: $ 8,000 The Family Behavioral Health Clinic (FBHC) Funding Request: To provide and support the range of therapeutic services, psychological testing and evaluation, substance use disorder, and recovery services to children, adolescents, and adults. In the last 12-months, 99 Des Plaines residents were provided services.

National Alliance on Mental Illness Cook County 2019 Recommendation: 3,225 North Suburban (National Alliance on Mental Illness-NAMI CCNS) Funding Request: To support dedicated programs and services for Des Plaines residents with mental health issues including their families and loved ones to lead fuller and more productive life without fear of stigma. Their mission is to improve the lives of individuals with serious mental illness and those who love and care for them through education, support, and advocacy.

North Shore Senior Center 2019 Recommendation: $ 9,500 Funding Request: To provide case management and counseling services to help seniors remain in their privately-owned homes. This senior services agency serves Maine Township by providing case management, counseling, community care services, and adult protective services for seniors and adults with disabilities. In addition, a social worker goes to the Frisbie Senior Center twice a month to provide information and assistance, including help with applying for entitlement programs. In the last 12-months 2,403 seniors received services from this agency.

Northwest Compass 2019 Recommendation: $ 1,500 Funding Request: To support with their Helping Hands Program, which works directly with the Des Plaines residents identified by HHS who are in financial need. This program provides temporary financial aid (maximum $150) to households to help alleviate their current monetary crises. Once the crisis is diverted, the staff involves the family in solution-focused case management, financial coaching, and looks for a permanent solution to avoid future crises.

North West Housing Partnership 2019 Recommendation: $ 6,500 Funding Request: To support the Handyman Program (NWHP) in coordinating and providing experienced and insured handymen to assist residents with minor, non-emergency repairs. The cost for these services can range from $10, $20, $30 per hour depending on the individual’s income. In the last 12-months NWHP served 162 Des Plaines homeowners.

Northwest Center Against Sexual Assault 2019 Recommendation: $ 2,600 Funding Request: To support the salaries and benefits of their Advocate and Bi-lingual Counselor. They offer free services to sexual-assault survivors and their loved ones within the North and Northwest Suburbs of Cook County. Provide victim-centered 24/7 crisis intervention, advocacy, counseling, and prevention. In the last 12-months 50 individuals received services from this agency.

Attachment 2 Page 9 of 13 Northwest Suburban Day Care Center 2019 Recommendation: $12,125 Funding Request: To help off-set their daily costs that the government programs do not cover. Low-income families are provided high quality daycare for children ages 15-months to 5-years. In the last 12-months, 33 children from low income families in Des Plaines were served by this program.

Open Communities 2019 Recommendation: $ 3,000 Funding Request: To help with Landlord/Tenant Liaison Services in assisting with covering the costs of legal assistance to Des Plaines clients. Open Communities is the only advocacy, education, and organizing resource for tenants/landlords in the Chicago Northwest suburbs. In the last 12-months, a total of 10 individuals received Landlord/Tenant Liaison Services as well as financially covering some of the cost of court filing fees.

Rainbow Hospice and Palliative Care 2019 Recommendation: $ 2,500 Good Mourning Program Funding Request: To support a bereavement program for children, teens, and families in which they participate in age-appropriate peer groups. Guardians can also attend loss-specific groups, with a weekend Family Camp in September and a Holiday Gathering in December. In the last 12- months, a total of 27 individuals received services through this program.

Self-Help Food Closet & Pantry of Des Plaines 2019 Recommendation: $12,000 Funding Request: To supplement day-to-day operations and to ensure that clients are receiving adequate food throughout the year. Provide food and clothing to Des Plaines residents in need. This program is managed by one (1) part-time employee and the rest are volunteers. They rely solely on donations and fundraising for their program.

Shelter, Inc. 2019 Recommendation: $ 2,200 Funding Request: To supplement the Foster Care Program, Healthy Families Program, Emergency Group Home for adolescents, Transitional Living Program, and the Emergency Hotline. Provides 24-hour emergency and longer term care to children who are abused, neglected or in need of supervision. Is responsible for the Healthy Families Program, which provides parenting education and support through home visits to first-time, low-income, and at-risk parents for up to 5-years. Facilitates a 24-hour emergency hotline to offer immediate help or referrals. In the last 12-months, 91 residents received services.

St. Mary’s Services 2019 Recommendation: $ 2,400 Funding Request: To offset the costs for their Teen Parenting Program, which is designed to teach pregnancy and disease prevention, domestic violence/abuse prevention, and help to teach self- sufficiency to teen parents in high school. The Teen Parenting Program is provided to School District 214 students. Provides maternity counseling, adoption services, and supportive services to women and teens facing unplanned pregnancies or experiencing anxiety, depression, or other mental health issues as a result of their pregnancy.

Attachment 2 Page 10 of 13 Suburban Primary Health Care Council 2019 Recommendation: $ 2,400 Access to Care Funding Request: To cover the cost to enroll three (3) residents of Des Plaines into the Access to Care program for one year. A non-profit health care program for low-income uninsured and under- insured people living in suburban Cook County that connects patients with primary health care services. Currently 220 residents of Des Plaines are enrolled in this program.

The Bridge Youth & Family Services 2019 Recommendation: $ 3,141 Funding Request: To provide a 24/7 on-call coverage for the Bridge Crisis Intervention Program. This program provides services to Des Plaines youth who are runaways, locked-outs, suicidal or homeless and need immediate intervention. The program successfully reunifies 95% of youth with their families. Without services to aid reunification many of these youth are at risk of entering the juvenile justice system or child welfare system.

The Harbour, Inc. 2019 Recommendation: $ 1,570 Funding Request: To help offset the costs of providing essential services i.e., housing, food, and case management for their housing programs. Provide shelter and crisis intervention for girls ages 12-21, transitional living for girls ages 16-21, an independent living program for girls ages 16-21, and a teen mothers program in the Northwest Suburbs. In the last 12 months, 7 youths were housed in shelters/transitional group homes in Des Plaines.

The Josselyn Center 2019 Recommendation: $ 4,000 Funding Request: To support the delivery of comprehensive mental health services to Des Plaines residents. This center is the only outpatient psychiatric facility for Medicaid recipients in 375 square miles that is a licensed Medicaid and Medicare provider. This center is the only area comprehensive mental health services provider, including medication monitoring, therapy/counseling, case management, and psychological testing and psychiatric evaluation for Medicaid, Medicare, Affordable Care Act insurance products, private insurance, and private pay. They now provide psychiatric services at Maine Township, filling the gap left behind by the closure of Maine Center. In the last 12-months, 107 Des Plaines residents have received services from this agency.

The Salvation Army – Des Plaines Corps: 2019 Recommendation: $ 3,500 Funding Request: To support and offset their Comprehensive Emergency Assistance Program. The goal of this program is to help individuals and families that are struggling to make ends meet by providing emergency assistance requests i.e., rent, utility bills, clothing and furniture vouchers, and additional assistance.

The Salvation Army Metro. Division - PAI Program: 2019 Recommendation: $ 2,000 (PAIP) for men arrested for domestic violence. The goal of the group is to protect the victim and challenge the attitudes and thinking of the offender. As of last year, 12 Des Plaines residents were provided with these services.
