1 Subject of specification

This specification and other documents referenced herein establish the minimum requirements for the design, fabrication, erection, inspection and tests for equipment and piping subject to UDT/TDT/ZDT inspection.

2 Scope of validity

This specification applies to the preparation of tender documents, inquiries and contracts for newly planned investment or modernization projects of Grupa LOTOS S.A. It shall also be a basis for preparing both detailed specifications and inquiries for individual or package supplies.

2.1 Deviations

Any deviation from this specification and the relevant specifications, standards and related documents including proposals, which the bidder claims to be economically and / or technically superior solutions must be submitted in writing for review and approval by Grupa LOTOS S.A. Non-approved exceptions are not acceptable by Grupa LOTOS S.A.

The supplier must report in writing any ambiguity in this or any other specifications of Grupa LOTOS S.A. or non-compliance with the referred documents / standards and obtain a written explanation from Grupa LOTOS S.A.

2.2 Order of Precedence

In case of conflicts existing between this specification and other documents, the following order of precedence shall govern: . The current Polish legislation . 4 Types of technical inspection

4.1 Conformity assessment by NB

Notified Body – assesses the conformity of all new and rebuilt equipment subject to the relevant European Directives.

4.2 UDT inspection

Office of Technical Inspection (UDT) – performs inspection of new, modernized, repaired and operated equipment listed under paragraph 5..

4.3 TDT inspection

Transport Technical Inspection (TDT) – performs inspection of new, modernized, repaired and operated equipment listed under paragraph 5.

4.4 ZDT inspection

Factory Technical Inspection of Grupa LOTOS S.A. (ZDT) – performs inspection of new, modernised, repaired and operated equipment as listed under paragraph 6 of this specification..

4.5 Requirements

All new equipment shall meet the requirements of all applicable European Union Directives.

5 Equipment subject to UDT/TDT inspection

5.1 Types of technical equipment subject to technical inspection

Types of equipment being subject to technical inspection either UDT or TDT shall be in accordance with Rozporządzenie Rady Ministrów z dnia 7 grudnia 2012 r. w sprawie rodzajów urządzeń technicznych podlegających dozorowi technicznemu Dz.U 2012 nr 0 poz. 1468 (Regulation of the Council of Ministers of 7 December 2012 on types of equipment being subject to technical inspection Journal of Laws 2012 No. 0 item 1468)

5.2 European Union directives conformity assessment

It is assumed that UDT-Cert No. 1433 will be the involved Notified Body for all pressure equipment manufactured in Republic of Poland for Grupa LOTOS S.A. The selection of the conformity assessment module shall be agreed with Grupa LOTOS S.A. Grupa LOTOS S.A. reserves the right to control the manufacturing of equipment at each stage of purchase order execution.

5.3 Manufacturers' licenses

5.3.1 The manufacturers of equipment that falls within the scope of any of EU directives shall carry out a conformity assessment and shall affix CE marking, if applicable. Notified Body shall be involved in the conformity assessment procedure, if stipulated in the directive(s). 5.3.2 Manufacturers of equipment that does not fall within any EU directives shall have a license and shall be authorized by UDT to manufacture such equipment in accordance with article 9.1 of attachment (Act of Law of 21December 2000 on Technical Inspection Office) to Journal of Laws 2013 item 963. This applies also to materials and elements used to fabricate such equipment All costs connected with UDT/TDT authorization are covered by the Manufacturer. Modernization or repair of equipment shall be carried out by manufacturer having UDT/TDT authorization for such work. Moreover any repair or modernization to equipment shall be subject to UDT/TDT agreement and approval.

5.4 Import of equipment

Imported equipment subject to technical inspection and outside of scope of any of EU directives may be imported to Poland on the basis of an import application approved by UDT/TDT in accordance with requirements of article 20.1 of attachment (Act of Law of 21December 2000 on Technical Inspection Office) to Journal of Laws 2013 item 963.

6 Equipment subject to ZDT inspection

6.1 Pressure equipment

Pressure equipment containing hazardous (flammable, toxic and caustic) fluids outside the scope of UDT inspection.

6.2 Atmospheric and low-pressure tanks

Atmospheric and low-pressure tanks (including storage tanks) containing hazardous (flammable, toxic and caustic) fluids outside the scope of y UDT/TDT inspection.

6.3 Piping

Piping outside the scope of UDT/TDT inspection, of a nominal diameter DN  50 mm, used for transferring: . hazardous liquids or gases of toxic, caustic and flammable properties under operating pressure higher than 6 bar . steam at operating pressure higher than 32 bar . hot water at operating temperature above 100 oC and operating pressure in excess of 32 bar. All the above piping shall be subject to assessment of conformity with EU directive No. 97/23/EC.

6.4 Electric equipment and installations

Electric equipment working in explosive conditions: . electric motors . intermediate boxes and control cassettes . switchgear and relays which protect „Ex” motors . electronic control and measuring instruments . tracing cables with accessories . elements of lighting systems . electric power system sockets.

6.5 Cableway equipment

. Equipment outside the scope of UDT inspection, i.e. one marked with a permanent sign of technical inspection. . Equipment covered by basic (simplified) inspection i.e. hoists and hoisting winches with manual drives and a lifting capacity of up to 2000 kg and derricks with manual drive and a lifting capacity of up to 2000 kg.

1 Subject of specification

This specification and other documents referenced herein establish the minimum requirements for the design, fabrication, inspection and tests for pressure vessels tanks subject to UDT/ZDT.

2 Scope of validity

This specification applies to the preparation of tender documents, inquiries and contracts for newly planned investment or modernization projects of Grupa LOTOS S.A. It shall also be a basis for preparing both detailed specifications and inquiries for individual or package unit supplies.

2.1 Deviations

4 Designing and manufacturing of pressure equipment

4.1 General requirements

All equipment that handle fluids (liquids or gases) under pressure that differs from atmospheric pressure shall be subject to UDT inspection in accordance with requirements of Rozporządzenie Rady Ministrów z dnia 7 grudnia 2012 r. w sprawie rodzajów urządzeń technicznych podlegających dozorowi technicznemu Dz.U 2012 nr 0 poz. 1468 (Regulation of the Council of Ministers of 7 December 2012 on types of equipment being subject to technical inspection Journal of Laws 2012 No. 0 item 1468). All pressure equipment (pressure vessels) shall be designed, fabricated and furnished in accordance with Rozporządzenia Ministra Gospodarki z dnia 21 grudnia 2005 w sprawie zasadniczych wymagań dla urządzeń ciśnieniowych i zespołów urządzeń ciśnieniowych wdrażającym postanowienia dyrektywy Parlamentu Europejskiego 97/23/WE w sprawie urządzeń ciśnieniowych Dz.U. 2005 nr 263 poz. 2200 (Regulation of Minister of Economy of 21 December 205 concerning pressure equipment, implementing provision of Directive of the European Parliament and of the Council 97/23/EC on the approximation of the law of the Member States concerning pressure equipment (PED) Journal of Laws 2005 No. 263 item 2200, year 2005) or Rozporządzenie Ministra Gospodarki z dnia 23 grudnia 2005 w sprawie zasadniczych wymagań dla prostych zbiorników ciśnieniowych wdrażającym postanowienia dyrektywy Rady Europy nr 87/404/EWG odnoszącej się do prostych zbiorników ciśnieniowych Dz.U. 2005 nr 259 poz. 2171 (Regulation of Minister of Economy of 23 December 2005 concerning simple pressure equipment, implementing provision of Directive of the European Parliament and of the Council 87/404/EEC on simple pressure vessels Journal of Laws 2005 No. 263, item 2200) ,whichever applies. It shall be note that 87/404/EEC directive has been replaced with 2009/105/EC, however the new directive does not affect the content of Regulation of Minister of Economy of 23 December 2005. In terms of design codes and other Grupa LOTOS S.A. requirements that shall be used for pressure vessels, reference shall be made to Grupa LOTOS S.A. specification no 2200 for detailed requirements. In case of modernization and repair of existing pressure vessels, UDT requirements must be met. Modernization or repair of equipment shall be carried out by manufacturer having UDT authorization for such work. This specification shall be strictly used with Grupa LOTOS S.A. specification no. 8100.

4.2 Marking of equipment – nameplate

The pressure vessel marking shall be in accordance with requirements of PED 97/23/EC. Each pressure vessel shall be furnished with durable name plate in Polish or Polish and English language in accordance with PED 97/23/EC. Nameplates shall be of corrosion resistant material installed on a nameplate bracket by solid stainless steel rivets. Inscriptions on the nameplate shall be engraved. The nameplate on vertical vessels shall be located on the shell above the lowest manhole, and on horizontal vessels it shall be located above the fixed support saddle. The nameplate shall not be covered with insulation.

5 Technical documentation

5.1 Design documentation

5.1.1 Documentation being approved by NB

Technical documentation of pressure vessels prior to fabrication commencement shall be subject to NB verification and agreement on compliance with requirements of PED. All costs related shall be covered by Manufacturer.

5.1.2 Scope of design documentation for approval

5.1.2.1 Assembly drawing that shall contain at least the following data:

. Applicable regulations and design codes . Operating pressure [bar g] . Maximum / minimum allowable pressure [bar g] . Test pressure [bar g] . Operating temperature [0C] . Maximum / minimum allowable temperature [0C] . Fluid(s) name . Capacity [L - litre] . Heat exchange area (for exchangers) [m2] . Corrosion allowance [mm] . Weight of vessel (empty, operating, testing) [kg] . Type and thickness of insulation . For columns – type and number of trays or packing quantity . Dimensions necessary for strength analyses checking . Details of welded joints . Type and scope of non-destructive examinations . PWHT requirements, if applicable . Data on materials used for pressure vessel construction, including their grades and specification . exists).

5.2 Final documentation

5.2.1 General requirements Documentation supplied with the pressure vessel shall be in number copies as specified in purchase order, including 1 copy of original documents signed by Notified Body. Moreover documentation shall be delivered in an editable (native) electronic form. Requirements for electronic form of documents are included in Grupa LOTOS S.A. specification no. 6200. All certificates, reports and conformity certificates for imported materials and pressure vessels shall be prepared in English language. All operating, assembly, maintenance, manuals, and other instructions prepared originally in English language, shall be translated into Polish language. Contractor (a part who carries out design engineering, procurement, construction or construction of the project) shall set up (work out) a database that will include basic data about pressure equipment and data about inspections during operation (e.g. location of measuring points of thickness, possible other tests with information about frequency of these tests…). This database should be connected with simplified drawings equipment in electronic form.

5.2.2 Scope of final documentation

. Design documentation (drawings and analyses/calculations) agreed with Notified Body . Safety equipment data – only if supplied together with the equipment. The scope of this documentation is defined in Grupa LOTOS S.A. specification 8400 . Accessories data – only if supplied together with the equipment . Instruments for measurement, control, signalling and automatic blocks – only if supplied together with the equipment . Feed equipment – only if supplied together with the equipment . Measurement card (protocol) of main parts of the pressure vessel . NDE reports with NDE plan and certificates of qualified NDT personnel . Mechanical test run report – if required . Heat Treatment Certificate with heat treatment plan and temperature vs. time chart– if required . Certificate of performed anticorrosion protection in accordance with EN ISO 12944-8 . Certificate of pressure test and certificate of water used for pressure test . Certificate of conformity issued by Manufacturer . Certificate of conformity issued by Notified Body . Any change must be clearly indicated. “As built“ shall be stamped on the drawing. If the changes have any influence on the results of strength calculations the “as built“ drawing shall be again approved by Notified Body.

They must be in Polish language (if original in English is available it must be included) . 5.3 Registration documentation

Registration documentation shall meet requirements of Rozporządzenie Ministra Gospodarki, pracy i polityki społecznej z dnia 9 lipca 2003 w sprawie warunków technicznych dozoru technicznego w zakresie eseksploatacji niektórych urządzeń ciśnieniowych. Dz.U 2003 nr 135 poz. 1269 rok 2003 (Regulation of Minister of Economy of 9 July 2003 on requirements for pressure vessel being in operation / service. Journal of Laws 2003 nr. 235 item 1269)

6 Costs of Notified Body / UDT inspection

Any costs of NB participation in inspection activities shall be covered by Manufacturer.

7 Pressure vessels subject to ZDT inspection

7.1 General requirements Construction of vessels and their documentation shall meet the same requirements as for vessels subject to UDT approval. However, instead of NB a ZDT inspector will be assigned.

7.2 Design and final documentation

Design documentation shall be subject to agreement with ZDT in the same scope as for vessels subject to UDT. The scope of final documentation delivered with pressure (As-built documentation) shall be the same as for vessles subject to UDT.

9 Codes and standards

Design, fabrication, installation and documentation shall be in compliance with applicable Polish laws and regulations . 1 Subject of specification

This specification and other documents referenced herein establish the minimum requirements for the design, fabrication, erection, inspection and tests for low-pressure and atmospheric storage tanks subject to UDT/ZDT inspection.

2 Scope of validity

This specification applies to the preparation of tender documents, inquiries and contracts for newly planned investment or modernization projects of Grupa LOTOS S.A. It shall also be a basis for preparing both detailed specifications and inquiries for individual or package unit supplies.

2.1 Deviations

Any deviation from this specification and the relevant specifications, standards and related documents including proposals, which the bidder claims to be economically and / or technically superior solutions must be submitted in writing for review and approval by Grupa LOTOS S.A. Non-approved exceptions are not acceptable by Grupa LOTOS S.A. The supplier must report in writing any ambiguity in this or any other specifications of Grupa LOTOS S.A. or non-compliance with the referred documents / standards and obtain a written explanation from Grupa LOTOS S.A.

2.2 Order of Precedence

In case of conflicts existing between this specification and other documents, the following order of precedence shall govern: . The current Polish legislation . 2.3 Exclusions

3 Design and Fabrication

Design, fabrication, additional accessories, marking and operate condition shall meet requirements of the following regulations (tanks manufactured both, in Poland and abroad shall to meet these requirements) : . 4 Inspection of UDT

Types of low-pressure and atmospheric storage tanks that are subject to UDT/ZDT inspection shall be according to paragraph 5 of Grupa LOTOS S.A. specification no. 8100. Manufacturers of tanks subject to UDT inspection shall have UDT authorization for fabrication. Storage tanks subject to UDT inspection shall respectively meet requirements of Regulations of Minister of Economy referenced under paragraph 3 of this specification.

4.1 Technical documentation

Technical documentation of tanks subject to UDT inspection shall be verified and agreed (approved) by UDT prior to fabrication commencement All cost associated with verification and agreement of technical documentation shall be covered by Manufacturer.

The technical documentation submitted for UDT acceptance shall include as minimum:

. Calculations according to design code being applied. Calculations for bolted flanged connections are not required provided that such flange connections are in accordance with requirements of relevant PN-EN standards. However tank technical documentation shall include data on materials of flanges, bolting, gaskets, as well as tightening bolts torques. . Accessories’ data; in case when tank is to be equipped with auxiliary systems – diagrams and parameters of such systems, . Erection procedure for tanks being site-erected. . Necessary descriptions and explanations to accessories’ drawings and diagrams.

Assembly drawing of tank shall contain:

. Dimensions necessary for verification of design calculations . Accessories data and, if tank is to be furnished with auxiliary systems, diagrams of such systems and data regarding their selection. . Name and basic properties of working medium (including vapour pressure). . Working and design parameters. . Grades of materials used for tank construction including respective material specifications . Type and data regarding anti-corrosion protection of internal and external surfaces, requirements for execution of tests and inspections, as well as foreseen operating period (lifetime) of tank. . Strength factors of welded joints and scope of non-destructive examination (NDE) . Weld map . Post welded heat treatment, if applicable . Special requirements regarding fabrication, erection and testing, including leak proof test data . Location of nameplate . Data on allowable settlement of foundation and frequency of such settlement measurement. Documentation delivered with tank shall contain number of copies as specified in purchase order, including 1 copy of original documents being signed by UDT, and shall contain at least: . Technical description of tank . Design documentation (drawings and calculations), agreed with UDT . Report of documentation agreement by UDT . Records of hydrostatic test, confirmed by UDT. . Manufacturer declaration of conformity. . Material certificates . Reports of non-destructive examination . Heat Treatment Certificate with heat treatment plan and temperature vs. time chart, if required . Certificate of performed anticorrosion protection in accordance with EN ISO 12944-8 . Register of documents (correspondences) containing additional technical arrangements. Costs of UDT participation in inspection activities shall be covered by / Manufacturer.

5 Inspection of ZDT

5.1 Tanks subject to ZDT inspection

Storage tanks of capacity exceeding 10 m3, excluded from UDT inspection scope, as well as tanks in process train designed for temporary storage of hazardous materials, which are not storage tanks, and therefore are not subject to UDT inspection, shall be subject to ZDT inspection.

5.2 Technical requirements

Tanks subject to ZDT inspection shall meet the same technical requirements, as tanks subject to UDT inspection. Manufacturers of tanks subject to ZDT inspection shall have UDT authorization for fabrication. Any deviations from this rule require ZDT approval in writing.

5.3 Technical documentation

Scope of required design documentation shall be the same as for tanks subject to UDT inspection (refer to paragraph. 4.1 for details). Technical documentation shall be verified and agreed by ZDT prior to tank construction commencing. The scope of technical documentation delivered with tank (As-built documentation) shall be the same as for tanks subject to UDT inspection. Costs of ZDT participation in inspection activities shall be covered by / Manufacturer.

1 Subject of specification This specification used in conjunction with other technical specifications of LOTOS Group S.A. establishes the minimum requirements that shall be met by safety valves and safety heads that protect equipment subject to inspection by UDT.

2 Scope of validity This specification applies to the preparation of tender documents, inquiries and contracts concerning new investments and modernisation projects planned by LOTOS Group S.A. 4 Definitions GLSA – Grupa Lotos S. A. Safety valve – a valve that prevents excessive pressure build-up by opening automatically and which closes automatically when the pressure drops. Safety head – a device protecting against excessive pressure build-up, whose functioning consists in evacuating the working medium through a free cross-section created when a replaceable pressure element is destroyed.

5 Conditions for use Only safety valves and heads which have been examined for compliance with the basic requirements of Directive 97/23/EC (PED) and which have CE marking may be used.

6 Requirements for safety valves and safety heads Only safety valves and safety heads which have certificates issued by a manufacturer that has implemented an ISO 9000 quality system shall be admitted for work in LOTOS Group installations.

6.1 General requirements Safety valves and safety heads must meet the requirements of the technical conditions stipulated in specification WUDT-UC/WO-A/01or meet the requirements of ASME standard "Pressure Vessels", part VIII and the requirements of the API standards specified in point 8 of std 8400. Safety devices shall be installed in accessible and sufficiently illuminated places. The design of safety devices shall ensure effective protection against an unintentional change of the setting of the equipment and its control elements. The designer or manufacturer of a safety valve shall specify the frequency of inspections of the valve setting on a test bench, considering the placement of the valve in the installation, the type of the medium and its corrosive power as well as other factors affecting the reliability of the valve operation. The designer shall provide for a possibility of dismantling the valve during the work of the installation. Safety valves may be ordered only from manufacturers that are on LOTOS Group's suppliers list.

6.2 Technical documents The technical documentation of the safety devices to which this specification applies shall comply with WUDT-UC-WO-D or shall comply with the relevant ASME and API standards specified in point 8 of std 8400. The designer has to provide the list of protecting devices in compliance with the enclosure No. 1 and to deliver it to the Lotos Inspection Department and to deliver the second copy of the list to the Lotos Serwis together with valves for the purpose of the examinations about which is in the point 6.3.2. Contractor is to deliver the set of the technical documentation according to point 6.2.1 or 6.2.2 for all protecting devices, mentioned in the list. - before the finish of the plant construction.

6.2.1 Scope of documentation for safety valves Documentation supplied by the valve manufacturer shall contain the following: . the declaration of the valve conformity with the basic requirements of Directive 97/23/EC (PED) . the valve calculation sheet Arch. . a cross-sectional drawing of the valve with a specification of the parts stating their materials and code numbers (for ordering spare parts) . material certificates (chemical analysis and strength properties) of the casing, flanges and spring: material Inspection Certificate 3.1B as per PN-EN 10204 . a certificate stating the value of the coefficient of discharge issued by an inspection body authorised in the manufacturer's country . a certificate from the tests of the valve according to API 527 for Polish and imported valves . the valve operating manual in Polish language according to WUDT-UC-WO- A/01 point 6.4.2. or according to the ASME and API codes. If the operating manual is edited in English language then it shall be also delivered. 6.2.2 Scope of documentation for safety heads For every safety head the manufacturer shall enclose documentation including: . CE declaration of conformity . an operating manual according to WUDT-UC-WO-A/01 for Polish-made safety heads, or according to ASME and API standards for imported safety heads (point 8 of std 8400).

6.3 Inspection 6.3.1 Inspection and testing to be done by manufacturer Safety valves and heads are subject to the following inspection and testing: . EC type testing – to define its full characteristics, if conformity assessment modules G or H1 according to Directive 97/23/EC (PED) are not used . during manufacturing – inspection and tests carried out by the manufacturer's quality department, in order to verify the manufactured equipment conformity with the type and the technical documentation, including marking . as part of inspection of the manufacturer's approved quality system The scope of type testing covers the following: . verification of the technical documentation including assessment of the stresses and safety of the structure . assessment of materials used . approval of manufacturing process procedures . verification of the personnel performing welded joints and non-destructive tests . verification of the construction conformity with the verified documentation . carrying out of the pressure test of the housing . definition of performance and flow characteristics.

6.3.2 Inspection and testing to be done by Lotos Serwis All new PSV’s are the subject of checking after dismounting from the plant. First, all the PSV’s have to be installed and connected with pipes or equipment. Scope of activity: . pressure test (checking of opening pressure and leak tightness) . establish the PSV card and the registration number . sealing the PSV after test by Lotos Serwis Quality Control Department.

6.4 Marking 6.4.1 Marking of safety valves A safety valve shall have a permanently attached data plate containing, as a minimum, the following data: . the manufacturer's trademark . type number . the smallest diameter of the flow channel in mm . the symbol of the working medium . the allowable coefficient of discharge . setting pressure in bar(g) . cold differential test pressure CDTP in bar(g) . serial number / year of manufacture . CE marking . the valve process number according to P&ID. 6.4.2 Marking of safety heads Safety head components shall have at least the following marking: . safety head housing: - the manufacturer's name and trademark - the head type - rated pressure - nominal diameter - the direction of flow of the working medium - process number according to P&ID - CE marking . pressure element - the manufacturer's name and trademark - the element type Arch. - serial number or batch number - year of manufacture - the number of the type test of the respective head - type of material - the design area of the head inlet channel - the highest and the lowest bursting pressure at ambient temperature and/or at working temperature or the rated bursting pressure and the dispersion of bursting pressures - the direction of flow of the working medium If the element dimensions do not allow placement of the above listed marking on the element, the marking shall be placed on the packaging. The packaging shall be factory sealed until the moment of installation of the elements. Marking of a pressure element and of a safety head casing shall be made to be visible after installation. This requirement does not apply to small pressure elements. For pressure elements used without a special casing, the direction of flow must be indicated on the element.

7 The design of safety valves and heads

7.1 Safety valve design 7.1.1 Safety valves shall be designed, fabricated and tested in compliance with API 526, API 527 and ASME Code Section VIII or WUDT-UC/WO-A/01 7.1.2 Calculations of valve relief capacity and setpoint pressure shall comply with ASME Code Section VIII and API 520, API 521 or WUDT-UC/WO-A/01. 7.1.3 Only flanged valves shall be used. 7.1.4 All valves shall be fitted with release levers (if can be applied) and test gags. 7.1.5 Valves shall have replaceable heads, seats and bellows. 7.1.6 Valves operating with variable back pressure at outlet or with a back pressure exceeding 10% of the setting pressure shall be fitted with an compensating bellows.

7.2 Safety head design 7.2.1 Safety heads shall be designed, fabricated and tested in compliance with the requirements of technical specification WUDT-UC-WO-A/01or with the ASME and API standards specified in point 8 of std 8400. 7.2.2 Calculations of safety head relief capacity shall comply with ASME Code Section VIII and API 520 or WUDT-UC/WO-A/01.

1 Subject of specification

This specification and other documents referenced herein establish the minimum requirements for the design, fabrication, inspection, testing and accaptance for control and protection systems for burners of furnaces and boilers subject to UDT/ZDT.

2 Scope of validity

This specification applies to the preparation of tender documents, inquires and contracts for newly planned investment or modernization projects of Grupa LOTOS S.A. It shall also be a basis for preparing both detailed specifications and inquires for individual or package supplies.

2.1 Deviations

Any deviation from this specification and the relevant specifications, standards and related documents including proposals, which the bidder claims to be economically and / or technically superior solutions must be submitted in writing for review and approval by Grupa LOTOS S.A. Non-approved exceptions are not acceptable by Grupa LOTOS S.A. The supplier must report in writing any ambiguity in this or any other specifications of Grupa LOTOS S.A. or non-compliance with the referred documents / standards and obtain a written explanation from Grupa LOTOS S.A.

2.2 Order of Precedence

4 Description Construction of burners, control and protection system, accessories, as well as production and testing, should fulfil requirements of standards and regulations, as well as specifications Grupa LOTOS SA listed in paragraph 5 and 6 of this specification. Scope of documentation that shall be elaborated and submitted for Notified Body approval, is specified below. Particularly important part of the documentation constitutes specification of safety requirements for Safety Instrumented System (SIS) by package supplier (or supplier of licence/process). Essential requirement for safety of burner system operation is completion of hazards and risk analysis, which in turn through determination of so-called safety requirements, their addressing, implementation and verification, leads to required/necessary risk reduction, and this in turn corresponds to tolerable risk level for burner system. Schemes of proceeding are specified by standards PN–EN 61508, PN-EN 61511, PN–EN50156. For details refer to specification Grupa LOTOS S.A. no. 3650. Results of individual stages must be documented. The supplier is obliged to deliver documentation, resulting from a study determining safety requirements specification in relation to SIS. Furthermore, important element of this part of documentation required from package supplier, is reasonable and adequate for safety requirements service and testing program of SIS system during operating period. Detailed results of analysis, being the base for safety requirements specification, should be available and supplied to Grupa LOTOS S.A. on request. Pressure equipment, burners, as well as protecting equipment shall fulfil requirements of the following directives: Appliances burning gaseous fuels 90/396/EEC, Machines 2006/42/EC, Pressure Equipment 97/23/EC, Measuring instruments 2004/22/EC, and shall be marked according to them and to requirements included in Grupa LOTOS S.A. specifications no. 2110 and 3030. The supplier shall deliver documents necessary for operation, resulting from directive 2009/104/EC– Minimum safety and health requirements for the use of work equipment by workers.

4.1 Documentation

The documentation should be delivered to Notified Body in two copies, in Polish or English language. Procedure of approval lasts 1 to 3 months, and its result is approved (stamped) documentation and Design Approval Report. Any changes introduced after approval require approving once again.

4.2 Manufacture and acceptance test

4.2.1 Manufacture

Materials used for production of burners should correspond to requirements of standards and regulations listed in paragraphs 5 and 6. Materials, elements and sub-assemblies subject to inspection prior to being passed for production. Burners’ manufacturing technology shall conform to technological documentation of burner manufacturer. For unitary manufactured burners, their producer should have results of prototype tests, confirming achieving of declared parameters. A Party that installes and start-up shall issue an attest confirming completion of burner installation and its adjustment on boiler, furnace or process pressure equipment according to technical and operating documentation of boiler, furnace or process pressure equipment, as well as enclose results of measurements of harmful substances concentrations in flue gases, and for oil and gas-oil fired burners additionally results of carbon black concentration in flue gases measurements.

4.2.2 Testing and acceptance

The burners manufactured unitary for specific boiler, furnace or process pressure equipment shall subject to acceptance tests on individual basis in place of operating site; the tests should be performed within a framework of external on-duty revision by Notified Body. The tests shall be performed on burners in place of operating site on order and cost of burners’ manufacturer, or furnace/boiler vendor in agreement with burners’ manufacturer. Preparation of burners for testing, as well as furnishing boilers, furnaces or process pressure equipment with measuring and monitoring instruments necessary for completion of tests by Notified Body, falls within scope of responsibility of furnace/boiler vendor or manufacturer of burners. Scope of burners’ tests shall include: . testing of burner construction and its equipment for conformity to technical and design documentation . verification of certificates and attests presented by manufacturer for safety equipment constituting equipment of the burner (burner system) . verification of electrical safety requirements fulfilment on burner . operational testing of burners, consisting in verification of: 1. minimum and maximum burner output with simultaneous analysis of harmful substances emission in flue gases

2. effectiveness of burners’ safety instruments operation in normal operating mode and during simulated interferences, which can occur during burners’ operation on boiler, furnace or process equipment 3. co-operation of burner control system with boiler, furnace or process equipment protection system . additional testing and proofing tests recognized necessary during acceptance tests. Detailed programs of tests shall be each time individually established by Notified Body, with consideration given to design solutions of burners and their operating conditions.

5 General requirements Hoisting devices and their accessories, covered by European Union (EU) directives must comply with Polish regulations which introduced EU directives to Polish law. For equipment produced in Poland and supplied to Grupa LOTOS S.A. the Notified Body shall be UDT-CERT (ident. no. 1433). Manufacturers of hoisting devices no covered with EU directives but subject to Polish law regulation shall have UDT authorization for their production, according to art. 9 Ustawa of Dozorze Technicznym z dnia 21 grudnia 2000r. (Dziennik Ustaw nr 122, poz. 1321). (The Law on Technical Inspection of 21 Dec 2000 (Journal of Laws No 122, pos. 1321). This also applies to materials and components for manufacturing of such equipment. Manufacturer of equipment incurs costs related to authorization issuing. In case of modernization and repairs of existing equipment, UDT requirements must be met. Manufacturers performing refurbishment / modernization works must have UDT authorization. In case of importing equipment from manufacturers that do not have UDT authorization, inspection and tests in manufacturer’s workshops shall be performed on the grounds of technical requirements agreed with UDT. Manufacturer incurs costs of such agreements and potential UDT inspection. Requirements specified above are considered met, if manufacturer or the party repairing/modernizing technical equipment has a quality management system that conforms to Polish Norms and is approved and supervised by UDT.

5.1 Nameplate Hoisting devices shall be furnished with nameplates containing at least the following data: . Manufacturer name and address . Designation of equipment series or type . Year of production . Serial number, if applicable . Lifting capacity (not necessary) . CE marking (not necessary on the nameplate) In case of device that is foreseen for operation in explosion hazard zone, adequate designation shall be put on nameplate. Designation and inscriptions related to operation and safety at work shall be prepared on Polish language, in legible and durable way. Use of pictograms is allowed.

6 Technical documentation

6.1 Hoisting device documentation The hoisting device documentation shall contain: . Technical description of device and its accessories . Dimensional outline drawings . Diagrams of electric, hydraulic and pneumatic systems together with descriptions. . List of equipment and characteristic of individual components. . Kinematic schemes (the schemes should contain parameters of gear ratios) . Diagrams of cable or chain systems, giving diameters of drums, cable and chain pulleys, as well as clamping method of cables and chains ends. . Diagram of lift capacity limiter . Characteristics of lifting capacity . Operating instructions . All necessary strength calculations (steel structures, lifting capacity of cables, coefficient of stability)

6.2 Registration documentation Registration documentation (acceptance) shall comply with Rozporządzenie Ministra Gospodarki, Pracy i Polityki Socjalnej z dnia 29 października 2003 w sprawie warunków technicznych dozoru technicznego w zakresie eksploatacji niektórych urządzeń transportu bliskiego (Regulation of Minister of Economy, Labour and Social Policy of 29 Oct 2003 on technical requirements of technical inspection in scope of operation of some material handling equipment), and shall contain in particular: . Operation and maintenance instructions, which should contain: - functional description of mechanisms and electric, hydraulic and pneumatic equipment - adjustment instruction for protecting devices, brakes - installation instruction of running rails for overhead cranes - instruction of inspections and repairs, determining inspection types and time intervals, containing also a list of most frequently occurring failures along with remedies for them . lubricating instruction, determining recommended lubricants and their possible substitutes, as well as physical and chemical characteristics, first filling amount, working time until first change and frequency of replacement. . Manufacturer’s Declaration of Conformity (original and translation).

6.3 Complement documentation Complement documentation for devices assembled at site shall contain: . Acceptance report of construction part along with annexes . Report stating correctness of erection along with annexes . Location sketch of equipment . Electric supply diagrams . Reports of protective measurements (electric) . Reports of tests performed after erection completion. After completion of equipment modernization, it is necessary to issue documentation in scope agreed with relevant body of technical inspection, which shall contain in particular: . Authentication of proper execution of equipment modernization, erection and testing after completion of modernization, giving scope of executed works issued by the manufacturer with proper authorisation for modernization of UTB. . Certificate for load carrying elements as well as for protective devices . Report of protective measurements (electric) . Other documents, as agreed with technical inspection body.

1 Subject of specification

This specification and other documents referenced herein establish the minimum requirements for the design, fabrication, inspection and tests for pressure vessels tanks subject to UDT/ZDT. for filling and emptying equipment (NO) for transportation of hazardous materials subject to TDT.

2 Scope of validity

This specification applies to the preparation of tender documents, inquiries and contracts for newly planned investment or modernization projects of Grupa LOTOS S.A. It shall also be a basis for preparing both detailed specifications and inquiries for individual or package unit supplies.

2.1 Deviations

Any deviation from this specification and the relevant specifications, standards and related documents including proposals, which the bidder claims to be economically and / or technically superior solutions must be submitted in writing for review and approval by Grupa LOTOS S.A. Non-approved exceptions are not acceptable by Grupa LOTOS S.A.. The supplier must report in writing any ambiguity in this or any other specifications of Grupa LOTOS S.A. or non-compliance with the referred documents / standards and obtain a written explanation from Grupa LOTOS SA.

2.2 Order of Precedence

3 General requirements

The loading and unloading equipment-NO shall meet requirements of European Machinery Directive 2006/42/EC and European Pressure Equipment Directive 97/23/EC. This specification shall be considered together with LOTOS Group S.A. specification No. 2530, for which all requirements have to be fulfilled. It is assumed that for loading and unloading equipment NO manufactured and assembled in Poland for LOTOS Group S.A., TDT should be appointed as Notified Body.

3.1 Definitions and designations . NB – Notified Body . UDT – Office of Technical Inspection . TDT – Transportation Technical Supervision . NO – Loading and Unloading Equipment In understanding of these requirements, reloading equipment should be meant as technical equipment consisting of tubular members (rigid and flexible) along with fittings and accessories used, connected by means of tight elements, enabling filling or emptying of below given tanks, intended for transporting of dangerous materials. . cisterns in railway /road traffic and in inland navigation . cistern containers . large containers for bulk transport These technical requirements include listed equipment, starting from flanges of cut-off valves connecting them with transfer or process pipelines, up to elements (inclusively) connecting reloading equipment with transporting containers. Considering type of dangerous material being reloaded, equipment for reloading of following materials can be distinguished: . gases class 2 acc. to ADR/RID . liquid inflammable class 3 acc. to ADR/RID . solid inflammable class 4.1 acc. to ADR/RID . toxic class 6.1 acc. to ADR/RID . caustic class 8 acc. to ADR/RID . various class 9 acc. to ADR/RID Temperature of material being reloaded is assumed as temperature specified in ºC, measured on inlet flange of equipment – according to direction of dangerous material flow. Working pressure of dangerous material being reloaded is assumed as pressure specified in MPa, given in technical documentation of reloading equipment.

3.2 Authorities of manufacturers

3.2.1 Manufacturers of NO included in scope of relevant European Union directives, should submit each product to procedure of conformity assessment performed by NB. 3.2.2 Importer of NO is obliged to agree technical requirements with organ of relevant TDT entity. Such agreement follows on application of importer, which should contain: for NO . designation of manufacturer and supplier . type and name . type . working parameters . number of devices for materials  type, grade and dimensions of material or elements  destination of material or elements  quantity of material or elements Authorizations for manufacturers of transport containers NO, are granted by TDT according to UDT procedure given in specification No 8100. The same applies to manufacturer performing repair / modernization works.

3.3 Design, accessories and materials Reloading equipment is composed of tubular members and tight connecting elements, as well as accessories. Tubular members can be made as: . rigid, . flexible . rigid – flexible, made of below specified materials. Connecting elements can be made as: . rotary articulated joints, supported on bearings, with suitably shaped sealing, . bolted flanges, which render rotation of tubular members impossible (bolted connections with specified tightening torque), . quick joints . other, admitted by TDT.

Connections must be accomplished by method ensuring durable and tight joint, which introduces no additional stresses. Accessories of reloading equipment should meet requirements of WUDT-UC-WO-A/01:10.2003, in scope established by TDT. Design of reloading equipment should ensure its tightness in every operating condition, and should also: . give consideration to nature of transport containers handling . give consideration to type of dangerous material being reloaded . preclude leakages of dangerous material residues after completion of reloading activities. Off-position of reloading equipment should be specified in technical documentation. Reloading equipment and pipelines should be colour coded and labelled with safety marks according to PN-92/N 01255 requirements and specification of pipelines’ marking. Design of equipment should ensure ease and reliability of connection with transport container. Equipment intended for reloading of gases, toxic, liquid inflammable and caustic materials, should be furnished with quick-acting end valve, and joint of emergency equipment disconnection from tank valve flange. Listed elements of emergency protection should be actuated automatically or remotely. It is allowed, on the grounds of agreement with TDT organ, to use other safety devices, accomplishing the same function. Task of quick-acting end valve is to prevent outflow of dangerous material from equipment, after disconnecting it from transport tank. Joint of emergency disconnecting should enable disconnecting of equipment from transport tank valve (valve flange) in emergency. Such joint should be equipped with two quick-acting valves, which after actuation should cause closing of connecting conduits on both sides of joint of emergency disconnection, ensuring tightness of disconnected elements. Joint of emergency disconnecting have to be protected against unintentional activation. Detailed information regarding control and instrumentation elements is given in 2530 standard. Equipment intended for reloading of gases, toxic, liquid inflammable and caustic materials must be equipped with system of equipment draining (residual draining system), built as system of tubular conduits of small diameter, serving for emptying of equipment from dangerous material after completion of reloading. In cases required by regulations, removal of dangerous materials residues from equipment must give consideration to conditions of environment protection, Health and Safety at Work and fire safety. Thermal insulation is required for reloading equipment of dangerous materials, which temperature exceeds 70ºC. On the grounds of TDT organ consent, other protective measures against effects of dangerous material temperature influence, are also allowed. Equipment should be furnished with earthing and electric shock protection, according to regulations and standards in force. Use of welded tubes with longitudinal or spiral seam for elements of reloading equipment, is not allowed.

Materials used for construction of reloading equipment should be resistant to influence of dangerous materials being reloaded, or effectively protected against their action. Materials used for construction of reloading equipment should comply with technical conditions of technical inspection WUDT-UC-WO-M/01:10.2003 – „Materials”. Guaranteed impact resistance of materials used for construction of equipment should conform to relevant ADR/RID requirements. Materials and subassemblies of reloading equipment should be manufactured by works possessing authorization of technical inspection organ. Materials used for construction of reloading equipment and their subassemblies should possess material certificates. UNO should have nameplate containing following data: . name of manufacturer, . serial number, . year of production, . type designation, if exists . parameters of equipment (nominal diameter, flow rate, working pressure and range of allowable temperatures of reloaded medium), . destination of equipment (type of reloaded medium), . manufacturer quality control mark . date of recent testing and stamp of TDT inspector. Each flexible reloading conduit should be furnished with nameplate durably affixed on it, made of corrosion resistant material and containing at least following data: . name or brand of manufacturer . number of standard or designation of intended use (admitted types of reloaded media), . serial number and date of production, . date of recent testing, . test pressure. Above data may be imprinted on bodies of conduits terminals.

4 Documentation

4.1 Technical documentation

Technical documentation has to be elaborated during design stage of NO. Manufacturer or person acting in his name should submit it in at least two copies to TDT body in order to approve it. Technical documentation should contain: . list of regulations and standards used,

4.2 Registration documentation

To the application for registering NO in TDT should be submitted registration documentation in two copies, which should contain following documents: . description of NO, containing its type and destination, place of installation, year of construction, serial number, allowable working and design pressure, range of allowable working temperatures, maximum throughput, nominal diameter of tubular members, overall length and dimensions of equipment for characteristic operation states, type of drive, type of current and supply voltage, description of electric shock protection, type of limit switches, description of signalling and emergency devices, mass of main components and overall mass; . documentation delivered by manufacturer along with NO elements and accessories, specified in regulations regarding CE marking or in technical specifications agreed with TDT, in range relevant for assessment of starting safety level of equipment and execution of activities specified in point 5; . assembly drawing of NO printed in legible scale, giving main dimensions, and dimensioned emergency and working zones of equipment or set of equipment; . drawing of working zone with marked position of equipment, arrangement of control cab devices, distributing valves connecting arterial pipelines, and other devices present in working zone of NO; . diagram of hydraulic and pneumatic systems, . . diagram of heating system, if exists, . electric diagram of drive and control, if exists . operating, maintenance and repair instruction, prepared on Polish language, . certificate of proper manufacturing and testing, issued by NO manufacturer, . certificate of proper erection of NO and after-erection testing completion by erecting contractor . certificate of completed non-destructive testing (radiographic or ultrasonic) proper assembly welds (if exist) . certificate for cables according to standards PN-EN 10204, if exist . reports of electric measurements, if required . certificates of type examination for equipment made in explosion-proof version, if exist

5 Acceptance tests

NO acceptance tests include at least: . inspection of the construction, consisting in checking of NO conformity to submitted registration documentation, specified in p. 4.2. . hydraulic tests of all NO pressure elements, at pressure equal to 1,5 x maximum working pressure, lasting not less than 30 minutes; hydraulic subassemblies should be hydraulic factory tested before protecting them against corrosion or installing of thermal insulation. Hydraulic pressure test of whole equipment should be completed after full assembly is done. Hydraulic test performed with use of water may be in exceptional cases, and on the grounds of relevant technical inspection organ consent, replaced by test performed with use of another liquid or gas. . checking of protective devices (including interlocking, if any) . checking of limit and end switching devices operation (if exists) . checking of tension members condition, their fixing and suspension elements (if exist) . checking of driving machinery operation (if exist) . checking of working movements speed, . checking of state and operation (if such test will be possible) of emergency disconnection joint . operating test of quick-closing valve . checking of electric measurements reports (resistance of insulation, effectiveness of electric shock, continuity of grounding protection and resistance of grounding, resistance of insulating flange), if required . leak proof test at working pressure during reloading operations.

Prior to commencing of acceptance, periodic test, user should submit declaration of reloading equipment and its co-operating devices readiness to testing.

1 Subject and purpose of specification This specification used in conjunction with other technical specifications of LOTOS Group S.A., establishes minimal requirements regarding designing, execution and acceptance of pipelines, subject to inspection of UDT/ZDT during operation of pipelines within GRUPA LOTOS S.A. plants (according to technical specification no 6600, attachment no 8, column 26).

2 Scope of validity This specification shall be used when preparing both quotations and Purchase Orders concerning planned LOTOS Group S.A. new investments and modernizations. It shall also be a base for preparation of other detailed specifications and quotations relevant for package units or other equipment deliveries.

2.1 Deviations Any deviation from this specification and the relevant specifications, standards and related documents including proposals, which the bidder claims to be economically and / or technically superior solutions must be submitted in writing for review and approval by Grupa LOTOS SA. Non-approved exceptions are not acceptable by Grupa Lotos S.A. The supplier must report in writing any ambiguity in this or any other specifications of Grupa LOTOS SA or non-compliance with the referred documents / standards and obtain a written explanation from Grupa Lotos SA

4.1 General requirements: 4.1.1 Designing, materials, fabrication and acceptance of pipelines must fulfil the requirements of the European Pressure Equipment Directive 97/23/EC and other directives EC applicable to the given pipeline. Essential Safety Requirements (ESR) described in Annax 1 of Directive 97/23/EC establishes materials which are allowed to be use for installations approved by PED Directive, there are materials: a) compatible with harmonized standards or b) included in European Approval of Material (EAM), use for construction of pressure devices or, c) use of materials within Particular Material Appraisal (PMA) 4.1.2 Materials for elements used for construction of pipelines must have guaranteed by manufacturer resistance to brittle cracking at lowest allowable working temperature. 4.1.3 For pipelines designed for media with working temperature ≥20°C, pipeline designer will provide in pipeline maintenance instruction the way of pipeline start up when ambient temperature is below 20°C (eg during winter conditions when ambient temperature has minimal value) 4.1.4 Materials used for construction of pipelines must meet requirements of standards or specifications, confirmed in acceptance certificate 3.1. or 3.2 according to EN 10204: 2005. 4.1.5 Pressure equipment such as valves, strainers or special elements must have CE marking, if required. 4.1.6 Metallurgical products must be permanently marked to allow for unambiguous identification of the material, manufacturer and heat number. 4.1.7 Pipeline shall be equipped with pipeline identification plate (acc. Standard 2620), located and orientated in way that it shall be visible from accessible place. Accessible place shall be easy to approach and see the pipeline identification plate from the ground level or from designed (for others reasons) platforms.

4.2 Pipelines subject to conformity assessment by NB 4.2.1 Technical requirements 4.2.1.1 At the designing and manufacturing stage all new pipelines shall be subject to assessment for the fulfilment of the criteria of the EC directives, and after qualifying to hazard category I – III according to PED they are subject to conformity assessment by NB. 4.2.1.2 Method of proper qualifying of pipeline gives consideration to following criteria: - type of medium: group 1 – dangerous - type of medium: group 2 – others - type of fluid: liquid or gas - value of PS x DN (bar) product where: PS[bar] – maximum allowable pressure DN – nominal diameter

4.3 Pipelines subject to UDT inspection 4.3.1 Technical requirements 4.3.1.1 At the designing and manufacturing stage all modernised pipelines are subject to assessment for the fulfilment of the PED directive criteria, and after qualifying to hazard category I – III, according to UDT requirements, they are subject to immediate testing preceded by the agreement of the technical documentation within the scope of modernisation, for conformity with UDT or ASME technical conditions. 4.3.1.2 Materials use for modernization of pipeline shall be providing from manufacturer approved by UDT. 4.3.1.3 The following pipelines subject to UDT technical inspection: . Process pipelines of nominal diameter higher than DN25, intended for transferring dangerous fluids under pressure exceeding 0.5 bar, having toxic, caustic or flammable properties which are: 1) compressed gases, liquefied gases, gases dissolved under pressure, vapours and liquids whose vapour overpressure at the highest allowable temperature exceeds 0.5 bar; 2) liquids whose vapours at the highest allowable temperature have overpressure lower than 0.5 bar if the product of the liquid overpressure and the pipeline nominal diameter is more than 2000 bar, and classified according to rules specified in table No 4,

4.4 Pipelines subject to ZDT inspection 4.4.1 Technical requirements 4.4.1.1 To technical inspection of ZDT are subjected new pipelines and pipelines under modernization that have not been qualified under UDT inspection, of nominal diameter starting from DN50 transferring: - dangerous liquids or gases (flammable, explosive, toxic and caustic), of working pressure above 6 bar, - steam of working pressure above 32 bar, - hot water of working temperature above 100ºC and working pressure above 32 bar, - other pipelines, considered by designer or user as requiring inspection, after approval of ZDT manager. 4.4.1.2 Division of pipelines, subject to ZDT, on classes is given in table No 6. 4.4.1.3 During designing and manufacturing stages, all pipelines qualified under ZDT inspection must meet conditions of technical requirements specified in WUDT-UC-WO-O, ASME B31.3 or ASME Boiler and Pressure Vessel Code Section VIII- Pressure Vessels 4.4.1.4 Manufacturer has to ask ZDT for inspection (piping examination and pressure test) with reference to these lines, which are under ZDT inspection during operation.

5 Technical documentation

5.1 Technical documentation of pipelines subject to conformity assessment by NB 5.1.1 Technical documentation must be accompanied with design examination certificate, issued by Notified Body that is carrying design examination. 5.1.2 Independently from basic technical documentation, for each pipeline qualified to I, II, III hazard category according to PED, passport documentation is required according to Specification No 8820. 5.1.3 For pipelines requiring registration with UDT the passport documentation according to the Specification No 8820 should be agreed with Notified Body after review by ZDT.

5.2 Technical documentation of pipelines subject to UDT inspection 5.2.1 Independently from basic technical documentation, for each pipeline qualified to I, II, III hazard category according to UDT, passport documentation is required according to Specification No 8820. 5.2.2 Technical passport of pipeline according to Specification No 8820 is to be agreed with UDT, after review by ZDT.

5.3 Technical documentation of pipelines subject to ZDT inspection 5.3.1 Independently from basic technical documentation, for each pipeline qualified under ZDT inspection, passport documentation is required according to Specification No 8820. 5.3.2 Technical passport of pipeline according to Specification No 8820 is to be agreed with ZDT.

6 Requirements relating to manufacturing Installation of pipelines subject to acceptance may be performed by entities having an implemented quality management system.

6.1 Requirements for manufacturing of pipelines subject to conformity assessment by NB 6.1.1 The manufacturer should have a contract signed with the Notified Body UDT-CERT for providing the services of conformity assessment within the framework of B1+F or G modules. 6.1.2 Conformity assessment modules are shown in table 7.

6.2 Requirements for the manufacturing of pipelines subject to inspection by UDT 6.2.1 Pipelines should be manufactured in accordance with the UDT or ASME requirements.

6.3 Requirements for the manufacturing of pipelines subject to inspection by ZDT 6.3.1 Pipelines should be manufactured in accordance with the UDT or ASME requirements.

7 Requirements for acceptance tests

7.1 Requirements for acceptance testing of pipelines subject to conformity assessment by NB 7.1.1 Pipelines and their elements shall be subjected to tests to assess their manufacturing conformity with PED and with the design technical documentation. The acceptance of pipelines subject to final assessment according to the conformity assessment procedure app. III to Directive PED shall be done by authorised experts of the Notified Body. 7.1.2 Depending on adequate hazard category, among which the pipeline has been numbered and the application of the appropriate conformity assessment module, relevant acceptance competencies shall be used according to PED and agreements between user and Notified Body. 7.1.3 For projects executed in the LOTOS Group it shall be recommended to apply acceptance competence according to the tables Nos. 8, 9 and 10 below.

7.1.4 Non-destructive testing of pipelines The testing methods are subject to agreement with the organ of the competent NB for the scope and the application of testing techniques, the measuring and testing equipment, the possibility of fault identification and the qualifications of the testing personnel. 7.1.4.1 Unless the agreed technical documentation specifies more strict requirements, the number of welded joints subject to non-destructive examination that should be tested shall not be less than stated in table 11 below.

7.1.4.2 Welded joints that have been subjected to post-weld heat treatment shall be 100 % examined by the radiographic or ultrasonic method. 7.1.4.3 Field welds shall be examined at a proportion not less that shown in Table 11. 7.1.4.4 Fillet welds shall be non-destructive examined 100%. 7.1.4.5 In design documentation, where the scope of radiographic examinations is specified (% of RT) , the substitution of this method with the ultrasonic one shall be allowed (UT) – only for field welds of diameters not less than 3" and of wall thickness not less than 8 mm –after agreement with NB. 7.1.5 Pipeline construction examination Pipeline construction examination consists in external inspection of the pipeline over its whole length and in checking in particular the following: - conformity of the pipeline construction with the technical design documentation, particularly conformity of material and fittings use for pipeline construction and its manufacturer tests certificates with material requirements specified in standards and technical specifications as required in design. - conformity of tests results with the requirements, - completeness and correctness of the as-built documentation,. - licenses of people performing welding, heat treatment and plastic working, - conformity of person authorisations responsible for the pipeline manufacturing and quality control with the personnel lists and with the enclosed manufacturing and quality control licenses, - conformity of pipeline marking with the rules provided in the documentation. The result of pipeline construction examination shall be considered to be successful if the above- mentioned requirements have been fulfilled. 7.1.6 Pressure tests 7.1.6.1 A pressure test shall be done by means of appropriate equipment and applying the necessary safety measures, so that persons responsible for the test can safely control all parts of the pipeline under test pressure. 7.1.6.2 A pressure test can be made as one of two possible types, according to the documentation: - a water-pressure test, - a pneumatic test.

7.1.6.3 Conditions of conducting a pressure test A pressure test during technical acceptance shall be carried out: - after the heat treatment, - before painting or insulating, - after conducting non-destructive tests, - after conducting the construction examination and other required inspections. 7.1.6.4 The water-pressure test - The water-pressure test is made with water. The water quality shall be such that equipment is protected against corrosion and undesirable substances. If other liquids are used, safety measures suitable for the type of the liquid shall be taken. - water used for testing piping systems of austenitic steel shall be controlled for chlorides content which shall not exceed 30 ppm. - Test pressure should be in accordance with design documentation requirements. 7.1.6.5 The pneumatic test This test shall be allowed only in cases where a water-pressure test would be harmful to the pipelines or impossible to carry-out; this applies in particular to pipelines in processes in which even trace quantities of liquid are not allowed. A pneumatic test may be made only with the operator's consent and the consent of the competent unit of technical inspection, on the basis of an agreed instruction. Due to the threats involved in a pneumatic test, the executor of the test shall pay special attention to keeping the maximum possible safety conditions during the test, ensuring that only personnel appointed for the test can access the hazard area by enclosing and posting warning signs and no-entry signs around the area. Pneumatic tests are made using air or other gas. Pneumatic test pressure shall not be lower than 1.1 of the allowable pressure (PS). 7.1.7 Manufacturing, inspection and test reports - EC final assessment report, EC conformity certificate.

7.2 Requirements for acceptance testing of pipelines subject to inspection by UDT 7.2.1 After completion of manufacturing process, pipelines and their elements should be subjected to tests for assessment of manufacturing conformity to design technical documentation and technical requirements agreed with UDT. Authorized inspectors of UDT execute acceptance of pipelines. 7.2.2 Acceptance competence for modernised pipelines subject to technical supervision by UDT are shown in tables 12, 13 and 14 below:

7.2.3 Non-destructive testing of pipelines The testing methods are subject to agreement with the organ of the competent NB for the scope and the application of testing techniques, the measuring and testing equipment, the possibility of fault identification and the qualifications of the testing personnel. 7.2.3.1 Unless the agreed technical documentation specifies more strict requirements, the number of welded joints subject to non-destructive examination that should be tested shall not be less than stated in table 15 below.

1 Subject of specification This specification used in conjunction with other technical specifications of LOTOS Group S.A. establishes the minimum requirements for the design and preparation of the passports of pipelines which are subject to UDT/ZDT inspection and which are operated in LOTOS Group's plants.

2 Scope of validity . This specification applies to the preparation of tender documents, inquiries and contracts concerning new investments projects planned by LOTOS Group S.A. In case of modernisation of existing pipeline scope of passport documentation must be separately agreed with ZDT, based on rules described in Attachement no 9 of this specification “Wykaz dokumentacji dostarczanej przez modernizującego rurociąg” (List of documentation for modernized pipelines). Signed memo contained such agreement override requirements of this specifications. This specifications and agreement mentioned above shall be a basis for preparing both detailed specifications and inquiries for individual or package supplies. If subject of tender is engineering or performing tie-in or any existing pipeline modernisation, then offered price must cover all cost of performing a passport documentation within scope as agreed with ZDT.

2.1 Deviations Any deviation from this specification and the relevant specifications, standards and related documents including proposals, which the bidder claims to be economically and / or technically superior solutions must be submitted in writing for review and approval by Grupa LOTOS SA. Non- approved exceptions are not acceptable by Grupa Lotos S.A. The supplier must report in writing any ambiguity in this or any other specifications of Grupa LOTOS SA or non-compliance with the referred documents / standards and obtain a written explanation from Grupa Lotos SA

3 Terms and conditions of using this specification Besides the basic technical documentation, pipeline passport documentation (according to the appendix to this specification) is required for the following pipelines subject to technical inspection in LOTOS Group: . those qualified for hazard categories I, II, III according to PED 97/23/WE and requiring registration with UDT or ZDT [Office of Technical Inspection or Factory Technical Inspection] . those qualified for hazard categories I, II, III according to WUDT –UC-RT and requiring registration with UDT or ZDT [Office of Technical Inspection or Factory Technical Inspection] . those subject to ZDT [Factory Technical Inspection] inspection, not classified for the above mentioned groups. 3.1 Hierarchy of documents: Should conflicts existing between this specification and other documents, the following order of precedence shall govern: . 4.2 The pipeline passport is the technical documentation of the pipeline, is to be done in Polish language (with the exception of support drawings) comprising the below listed chapters: 4.2.1 Chapter I – design technical documentation (prepared by a design office) 1 Technical description of the pipeline (do not show PSV technological symbol in case when PSV is provided not only for subject piping protection). 2 Functional description of the pipeline (among other things - to give the kind of medium, the description of the beginning and the end of the pipeline, the PSV technological symbol placed on the pipeline together with the symbols of other protected equipment). 3 Data concerning the source of supply (the source for pipeline is the nearest apparatus or equipment, with detailed list of piping between - if any; should be given Ps – max allowable pressure of source, Ts – max allowable temperature of source, Qnom – rated flow). 4 Technical data concerning corrosion protection and thermal insulation 5 Compensation of stresses in the pipeline 6 Hazard analysis (for pipeline which the passport concerns) 7 Operating manual (e.g. to show the number of the plant operating manual). 8 Calculation of the minimum pipeline wall thickness for all used diameters (calculations have to be done according to proper codes refering to the used material). 9 Accessories list. 10 Data sheet for safety valve with calculation sheet (only for PSV which protect exclusively this pipeline). 11 Specification of pipeline supports. 12 Drawings of pipeline supports. 13* Single admission of materials according to ASTM standards, different from harmonised standards according to EC. 14 Design bill of materials. 15 Bill of materials as built – updated material card, done after piping construction and final acceptance (printed with included designer changes during manufacturing) done as a one card for the whole pipeline, according to the Technical Specification No 6600. 16 Isometric drawing of the pipeline as approved for construction – sheet size A3. 17 Isometric drawing “as built edition” of the pipeline, with the changes (during fabrication and after the final acceptance) incorporated by designer in to the previous isometric edition, printed in sheet size A3. 18 Data plate – attachment no 8 to Technical Specification no 8820. 19 Medium data sheet. 4.2.2 Chapter II – as-built technical documentation (a set of documents made by the manufacturer during the manufacturing of the pipeline) 1 Manufacturer's CE declaration of conformity. 2* Conformity certificate issued by a Notified Body – if required by a respective conformity assessment module. 3 CE final assessment report. 4* CE design examination certificate or CE type testing issued by Notified Body – if applicable. 5 CE design examination report. 6 Specification of materials used for construction, stating certificate and heat numbers (containing among the others material name, dimensions, materials type, heat number, certificate number, the Grupa Lotos register number, only when Grupa Lotos is a material supplier, or volume/page number of the certificates set when the supplier is other than the Grupa Lotos). Material certificates of used materials should be numbered and collected in separate file in order. 7 Welding logbook (to enable identification of materials and welds) 8 Welders list (approved by Notified Body/UDT) 9 As-built isometric drawing, done by manufacturer with indicated marked welds and with changes done during manufacturing (significant changes have to be accepted by UDT). 10 Heat treatment diagrams – if applicable. 11 Non-destructive Tests (NDT) reports. 12 List of NDT personnel. 13 Data concerning pressure accessories used for pressure testing (e.g. pressure gauges etc.) – if applicable. 14 Reports of pressure tests carried out during fabrication or other type tests – if applicable. 15 P&ID showing the subject piping. * - Technical documentation shall not be required for pipelines that are not subject to assessment by NoBo.

4.3 Electronic recording of the technical passport documentation for all plant piping on portable HDD or DVD containing: 1 The list of pipelines according to the enclosure no 8 of the specification 6600. 2 The technical documentation of project (file recorded as for example 090-P1234 – REV0 – Passport Design Part: - in the form of one file according to above point 4.2.1 recorded in the *.DOC or *.DOCX format. The exception is point 12, 13,16,17 about which is below, - documentation according to point 12, 13 recorded in format *.PDF (300 dpi/8bit quality - the grey scale), - documentation according to point 16, 17 recorded in format *.DXF (files recorded as for example 090-P1234 –REV0 – isom1, etc.). 3 The as built technical documentation (file recorded as “090-P1234 – as built passport part”) in the form of one PDF file, described in point 4.2.2 (300dpi/16 bit – color quality). 4 one PDF file named “090-P1234 – REV0 – Complete pipeline passport” done as a recording of a source documentation in format PDF. 4.4 List of connected documents: 1 List of pipelines 2 Piping classes specification catalogue 3 Supports catalogue 4 Prefabrication, construction, examination and inspection of pipelines 5 Strength analysis of piping 6 Hazard analysis (for the all system or the installation) 7 Certificates of the personnel (welders, operators) - valid in time of the pipeline construction 8 Accepted WPS, the heat-treatment and the forming procedures - if any 9 Certificates of the personnel performing NDT - valid in the time of testing. 10 Certificates of used materials (for materials supplied by the Grupa Lotos SA. to enclose only the list of certificates) 11 Plant operating manual 12 Plot plan List of connected documents should be placed in the pipeline passport (the list of the content). Under mentioned connected documents with the passport should be delivered: . in paper form in the separate files - one set for the whole installation. The description of the file has to contain an univocal information about the content and the connection with the number of the installation (the investment job), . in electronic PDF format in file named “Connected documents”.

4.5 The pipeline passport maker shall submit the first three copies to Grupa Lotos as an example for verification. Examples (samples) shall apply to pipelines built from such elements as: a safety valve, a standard and special supports, etc., isometric should consist of at least two sheets and at least two construction area (if any). 4.5.1 All passports have to be supplied in a form of stiff files with the lever mechanism: a) in the first part - the project-documentation , b) the second (after the insert between parts) part - the as-built documentation ,

On the edge the file to place the large type the full symbol of the pipeline and the kind of the inspection during operation eg. „24”-090-P1234 / ZDT”. The width of the file assorted so that his content will fill approx. 50-70% the thickness.

4.6 Contractor shall make the other passports after a written assessment and approval of the delivered three (3) first passport samples.

4.7 Contractor shall deliver (according to procedure described in Technical Standard 6610) required passports in the paper form with: - 3 sets for piping under UDT inspection (original and 2 copies), - 2 set for piping under ZDT inspection (original and copy).

And also in electronic form according to point 4.3.

4.8 After the completion of the pipeline construction and after its acceptance by a competent body the pipeline passport will be the basis for UDT or ZDT for registering the pipeline and for taking it under its operating inspection.

4.9 The complete passport must be transferred to the Lotos Inspection Department (ZDT), via Technical Documentation Archive Department (TB) which keeps one archive copy, at once after the construction and after the obtainment of all required protocols of the inspections (with the positive result), however not later than 2 weeks before the time to transfer pipeline to the stage of tests. 5 Relevant documents “Technical Specification” issued by GRUPA LOTOS S.A. listed in Contract Files.
