The , grooming and demeanor of every one of us convey an important message about our professionalism and dedication. Evidence based practice has proven that patient satisfaction scores have increased when patients are able to identify who is caring for them based upon their attire. Accordingly, each employee is required to wear designated and appropriate attire while at work or conducting Faith Regional Health Services business. Scope: This policy applies to all Faith Regional Health Services (FRHS) employees, all Faith Regional Services (FRPS) employees, and FRHS and FRPS invitees who may include, but is not limited to , volunteers, students, instructors, contractors and vendors. Policy:COPY A. Employees, volunteers, students and other individuals who represent Faith Regional Health Services/ Faith Regional Physician Services and St. Joseph Rehabilitation and Care Center are expected to project a professional image to our patients, residents, visitors and physicians. Impressions of our employee’s appearance are lasting and add credibility with our patients and visitors. It is the policy that an employee’s personal appearance is appropriate to the work situation. It is vital for the safety and security of our staff, patients and their families that employees follow and adhere to Faith Regional Health Services' professional and dress. B. This policy outlines the minimum requirement for professional appearance. A Manager/Director shall retain the discretion to set standards at a higher level for employees, based on patient contact or job function. Higher standards should be communicated in unit/department orientation, evaluated in performance evaluation, and reviewed annually or as appropriate. C. It is not possible to address every conceivable question of personal appearance in these guidelines. The most effective control comes from the use of good judgment to meet our obligation of professional business attire to patient care and the public. While the following guidelines provide information about specific items of and aspects of appearance which are acceptable or unacceptable (as outlined below), the ultimate consideration is whether a professional appearance is maintained. D. Enforcement: Vice Presidents, Directors and Managers should address this policy in pre-hire interviews, job descriptions, during annual performance appraisals, and on an ongoing basis as needed. Vice Presidents, Directors and Managers are responsible for employee orientation and ensuring all staff members meet the policy. Any staff member who has questions regarding these standards should consult with his/her director/manager. E. Staff members are expected to comply with the personal appearance policy. Non-compliance with this policy may result in coaching, counseling or appropriate corrective action. Employees arriving at work inappropriately dressed or without their ID badge may be directed to clock out for the day, to return home to change their clothes and/or get their ID badge. Procedure: A. Jewelry: In patient care areas and dietary services, jewelry may not be worn by employees, with the exception of wedding rings, and post , name pins and other approved types of pins. B. Pins symbolizing an employee's graduation from school are appropriate. Pins that promote a cheerful attitude among employees and for the cheer of patients are acceptable. However, pins would be considered inappropriate if they contain the following: political messages, double entendre, off color humor, the name of a labor organization, etc. Some areas involving machine operation or other hazards may further restrict the wearing of jewelry. C. Body Piercing: The wearing of eyebrow, tongue, lip and other types of body jewelry (with the exception of post earrings noted above) is prohibited. Ear gauges are also prohibited. One (1) small, discreet nose stud is acceptable. D. Tattoos: Visible tattoos and body art may offend some patients, visitors, and co-workers while at the workplace and should be covered whenever possible. Directors/Managers also have the discretion to require that an employee cover any tattoo(s) or combination of tattoos that could be considered offensive. E. Hair Styles: Hair styles must be neat, clean and well kept. Extreme styles and colors are not appropriate duringCOPY work. Areas restricted by specific health requirements or aseptic techniques may restrict hairstyles to comply with those requirements. Facial hair must be well groomed. F. Fingernails: Artificial Nails: All Healthcare workers who regularly have direct patient contact are not allowed to wear any type of artificial fingernails. Natural Nails: The tip length of the fingernail should be less than ¼ inch long to prevent injury to patient(s) or to prevent tearing gloves. Polish: Nail polish cannot be worn in the operating room or by Nutrition Services staff. Polish can be worn in all other areas, but should be kept fresh to prevent chipping. Clear polish is preferred. G. Personal Hygiene: Good personal hygiene habits are required by all individuals at all time during work hours. Because body odor may be offensive or nauseating to patient, guests and co-workers, deodorant must be worn at all times during work hours. Excessive use of perfumes of colognes is not allowed. H. The odor of smoke on an employee's breath or clothing, or appearance of smokeless tobacco use (out pouching of lower lip/tobacco remnants in teeth) is unpleasant, distinctive and can be offensive. Any employee presenting to work with an obvious odor of tobacco usage may be required to change his/her clothing, either on-site or by returning home. For safety reasons, Occupational Safety and Health Administration (OSHA) standards requires the use of protective footwear when employees are working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where there is a possibility of the employee’s feet being exposed to an electrical hazard. a. Staff providing direct patient care (hands on), Laboratory, Environmental Services, Facilities, Grounds and Nutrition Services are required to wear shoes that comply with safety regulations for their area (i.e. solid closed toe shoes that protect from body fluids, sharp instrument, hot liquids, etc.) The style and color may be the standard specified by the department director. b. Staff working in areas other than listed above must wear shoes appropriate to the image of . (Unacceptable shoes/footwear – flip flops, beach , jelly shoes, or shoes that pose a safety risk to the employee). worn with any attire must be at the knee or lower. J. Scrubs: Staff members are required to wear the designated colored scrubs/ for their areas. Each Director/Manager will go over the uniform for their department. All scrubs must be Cherokee brand and the style you choose should be a professional look. (i.e. Cargo style pants or low cut necklines are not acceptable") Employees may choose to have the designated "Faith Regional Health Services" or "Faith Regional Physician Services" name embroidered on their scrub . The cost of the uniform and embroidering of Faith Regional Health Services/Faith Regional Services name is the responsibility of the employee. White, gray or black long or short-sleeved t- or Polo shirts may be worn under scrub tops. Other colors are not acceptable. K. Business Attire: Encompasses clothing that is comfortable at work, yet appropriate for the business environment with the nature of the employee’s position type, nature of duties, work location, type of contacts made with patients, visitors, co-workers, vendors, external customers and meetings. Acceptable attire includes, , , slacks, khakis, , turtlenecks, , golf-shirts and button-down shirts with a finished collar or neckline, or if desired. L. Fit of Clothing: Clothing should fit well and allow for comfortable movement throughout all required work activities without compromising safety or professional image. Clothing Condition: An appropriate appearance requires clothes to be neat, clean, wrinkle-free and in good condition. Faded and/or frayed clothing are not considered professional in appearance. N. Inappropriate clothing that is not considered for business attire: a. Blue b. T-Shirts or tank tops c. , exercise wear, sweatshirts d. , e. Off the shoulder, strapless, or tops or dresses unless worn with a jacket, , or blazer f. unless worn with long appropriate top/dress/sweater g. Other causal attire as determined by the requirement for a professional appearance and departmental needs.

Environmental Services Hunter green scrub pants, hunter green scrub tunics/ tops with logo and hunter green warm-up jackets (if COPYdesired). Athletic or orthopedic shoes. Miscellaneous: 1. On FRIDAYS ONLY, FRHS logo polo shirts may be worn by those employees whose dress code is listed as business attire. Employees who wear scrubs may wear FRHS polo shirts (black, white or gray only) in lieu of scrub tops but only with their assigned color of lab coat over the polo.

2. Husker/Game Day Wear: **On FRIDAYS ONLY, staff will be allowed to celebrate game day (this can include Huskers, college, high school and NFL teams). Collared polo shirts and turtleneck shirts may be worn by those employees whose dress code is listed as business attire and those in Nutrition, Rehab, Facilities, EMR Specialists, Materials Management, Patient Access, Pharmacy and Radiology clerical staff. Not acceptable are t-shirts, sweatshirts, or hooded shirts/sweaters.

Fashion changes every season and our expectation is that our employees will be dressed appropriately. Listing everything as acceptable or unacceptable changes as the world changes. Faith Regional Health Services expects everyone to dress business professional and not trendy casual.
