Testimony of Dr. Andre Van Mol, M.D., co chair Committee on Adolescent Sexuality American College of PediatriciansHB 33 protects already at-risk sexual minority youth from experimental and unproven gender affirming therapy (GAT). GAT permanently and prematurely medicalizes children for a condition that overwhelmingly resolves (desists) by adulthood. GAT is not proven effective, not proven safe, does not reduce suicides , and is not the international standard of care for gender dysphoric minors. Desistance is the norm for the minors with gender dysphoria (GD). GD youth have an overwhelming probability of desistance, resolving ON ITS OWN in 75-96% by adulthood. 12345. Medicalizing children for life for a psychological process that usually resolves requires strong scientific support that GAT does not have. 678 Gender Dysphoria in minors has no co morbidities. The overwhelming majority of youth with gender dysphoria have additional mental health conditions (depression, anxiety, bi polar disorder, psychoses, personality disorders, histories of self harm or suicidality, ect), neuro- developmental disabilities like autism spectrum, adverse childhood events, and family issues that pre- date or coincide with gender incongruence. 9 10 11. These issues need counseling, not GAT.Minors cannot give truly informed consent. 12 Children have developing brain, their minds change often and they don't grasp long-term consequences. 13 14 15. -The UK High Court in Bell vs Tavistock (2020) ruled that GAT in minors was experimental and could not , in most cases, be given to minors under 16 without court order, and that such was advisable for those ages 16-17: " There is no age appropriate way to explain to many of these children what losing their fertility or full sexual function may mean to them in later years. "16. -The Swedish Pediatric Society and Swedish National Council for Medical Ethics (2019) proposed a complete review of treatment of youth with gender dysphoria :"Giving children the right to independently make vital decisions whereby at that age they cannot be expected to understand the consequences of their decisions is not scientifically founded and contrary to medical practice".There is no medical proof that the long term benefits or safety of a child undergoing hormonal therapy or surgical transitioning. -World Professional Association for Transgender Health (WPATH) Standards of Care confirms, "To date, no controlled clinical trial of any feminizing/masculinizing hormone regimen have conducted to evaluate safety or efficacy in producing physical transition"-The 2017 Endocrine Society Guidelines on hormone treatment of gender dysphoria noted their recommendations for GAT were suppported by "low" or "very low" evidence. Low evidence means low science but high idealogy.-Professor Michael Biggs of Oxford (2019) found the GD children in the UK's Gender Identity Development Service's trial of puberty blockers showed greater self harm, more emotional problems and worsening of the GD.-Branstrom, et al(2019) published the first total population study of 9.7 million Swedes ultimately finding neither "gender affirming hormone treatment" nor "surgery" lead to reductions in mental health treatment.-Finlands Council for Choices in Health Care in Finlan (COHERE) in 2020 rejected routine "affirmation" pathway for minors with GD.Thy MYTH OF SUICIDE reduction from GAT-The threat of suicide is used as emotional blackmail for GAT- Pyschology professors J. Baily and R. Blanchard stated :"There is no persuasive evidence that gender transition reduces gender dysphoric children's likelihood of killing themselves.-The Lupron (the main puberty blocker in the US) package insert warns of "mood swings, depression, rare reports of suicidal indeation and attempt"-A 2011 Swedish study of all their post -sex re assignment adults showed a completed suicide rate 19 times that of the general population 10 year out with nearly 3 times the rate of psychiatric inpatient care-A 2011 long term Dutch study of cross-sex hormone therapy revealed significantly increased mortality from multiple causes including suicide. The international standard of care for youth Gender Dysphoria is watchful waiting, including psychological evaluation and support of the child and family, not gender affirming therapy (GAT). I support this bill, to protect the health of children. Reputable veterinarians will not spay/neuter animals at a young age, because it greatly increases the animal's risk of cancer. It is no different in humans. When we suppress the body's physiologically normal, healthy development we create disease, or at the very least predispose to disease. Blocking puberty carries several serious health risks that are not immediate, but develop later in life. This is a decision that should not be made by a child, whose brain is still undergoing development. We must allow our children to decide transitioning for themselves, as adults, when they can better understand the consequences and take full responsibility for such an important decision. As an educator in the Missouri public school system for over 35 years I have worked with hundreds of children and young adults. It is my firm belief that children and young adults under the age of 18 do not have the maturity to reassign their gender to the extent of surgical or medical procedures. So many of their actions and decisions are influenced by peer pressure, insecurity, family problems, etc. which can change from day to day.
