Kidneys play a vital role in the body by filtering blood, removing waste and extra water, balancing electrolytes—sodium, potassium, chloride, and biacarbonate, and regulating blood pressure. As kidneys filter blood they create urine/wastes which collect in the bladder until you go to the bath- room. If your body doesn’t remove these wastes, they build up in the blood and damage the body. For most people this damage occurs slowly over many years causing Chronic Kidney Disease (CKD). CKD eventually results in End Stage Renal Disease (ESRD) which is when dialysis or a transplant becomes necessary. 2 TREATMENT OPTIONS Treatment Options This guide will cover four treatment options for your condition: l Hemodialysis (in-center & home) l Peritoneal Dialysis l Kidney Transplantation l Comfort Care Introduction When diagnosed with kidney disease, the key to feeling better is to follow the treatment plan developed by your doctor and other members of your care team including nurses, dietitians, and social workers. Following the advice of your medical professionals will help you lead the healthiest lifestyle possible. When your kidneys are unable to remove extra fluid and waste from the body, dialysis is generally recommended. Dialysis helps alleviate many of the health problems people with kidney failure experience. Dialysis can be done in a hospital, a dialysis clinic, or at home. Once you know all of your treatment options, you will make the decision of where you choose to have dialysis with the help of your doctor, other health care professionals, and your family. -

Fey's reputation as a feminist—indeed, as Hollywood's Token Feminist, as some journalists have wryly pointed out—heavily inflects the character she plays, the "bossy" Liz Lemon, whose idealistic feminism is a mainstay of her characterization and of the show's comedy. Fey's comedy has always focused on gender, beginning with her work on Saturday Night Live (SNL) where she became that show's first female head writer in 1999. A year later she moved from behind the scenes to appear in the "Weekend Update" sketches, attracting national attention as a gifted comic with a penchant for zeroing in on women's issues. Fey's connection to feminist politics escalated when she returned to SNL for guest appearances during the presidential campaign of 2008, first in a sketch protesting the sexist media treatment of Hillary Clinton, and more forcefully, in her stunning imitations of vice-presidential candidate Sarah Palin, which launched Fey into national politics and prominence. Tubule dilation is associated with chronic progressive nephropathy. Comment: Renal tubule dilation may occur anywhere along the nephron or collecting duct system. It may occur in focal areas or as tracts running along the entire length of kidney sections (Figure 1). 1 Kidney, Renal Tubule – Dilation Renal tubule dilation may occur from xenobiotic administration, secondary mechanisms, or an unknown pathogenesis (see Kidney – Nephropathy, Obstructive (Figure 2). Dilation may result from direct toxic injury to the tubule epithelium interfering with absorption and secretion (Figure 3). It may also occur secondary to renal ischemia or from prolonged diuresis related to drug administration. Secondary mechanisms of tubule dilation may result from lower urinary tract obstruction, the deposition of tubule crystals, interstitial inflammation and/or fibrosis, and chronic progressive nephropathy (Figure 4). A few dilated tubules may be regarded as normal histologic variation. Recommendation: Renal tubule dilation should be diagnosed and given a severity grade. The location of tubule dilation should be included in the diagnosis as a site modifier. -

The diagnosis is made by aspiration of blood from the abdomen by abdominocentesis. Spleen and liver are the most likely sources of traumatic bleeding. Patients are stabilized with appropriate fl uid therapy, oxygen supplementation and analgesia. With ongoing haemorrhage, serial measurement of abdominal and venous haematocrit can be helpful in making the decision between surgical and medical therapy. Most patients with traumatic haemoabdomen can be treated medically. Surgical therapy should be reserved for patients that cannot be stabilized despite medical intervention. The surgical approach should be thoroughly planned in order to minimize further abdominal blood loss and blood transfusions should be readily available. 30 Rock: Complexity, Metareferentiality and the Contemporary Quality Sitcom Katrin Horn When the sitcom 30 Rock first aired in 2006 on NBC, the odds were against a renewal for a second season. Not only was it pitched against another new show with the same “behind the scenes”-idea, namely the drama series Studio 60 on the Sunset Strip. 30 Rock’s often absurd storylines, obscure references, quick- witted dialogues, and fast-paced punch lines furthermore did not make for easy consumption, and thus the show failed to attract a sizeable amount of viewers. While Studio 60 on the Sunset Strip did not become an instant success either, it still did comparatively well in the Nielson ratings and had the additional advantage of being a drama series produced by a household name, Aaron Sorkin1 of The West Wing (NBC, 1999-2006) fame, at a time when high-quality prime-time drama shows were dominating fan and critical debates about TV. ENGLEBERT DUNmHY, M.D.* THE successful transplantation of a heart from one human being to another, by Dr. Christian Barnard of South Africa, hias occasioned an intense renewal of public interest in organ transplantation. The back- ground of transplantation, and its present status, with a note on certain ethical aspects are reviewed here with the interest of the lay reader in mind. History of Transplants Transplantation of tissues was performed over 5000 years ago. Both the Egyptians and Hindus transplanted skin to replace noses destroyed by syphilis. Between 53 B.C. and 210 A.D., both Celsus and Galen carried out successful transplantation of tissues from one part of the body to another. While reports of transplantation of tissues from one person to another were also recorded, accurate documentation of success was not established. John Hunter, the father of scientific surgery, practiced transplan- tation experimentally and clinically in the 1760's. Hunter, assisted by a dentist, transplanted teeth for distinguished ladies, usually taking them from their unfortunate maidservants.
