This form is an application only for employment. It serves as an instrument for Transit Systems (NSW) to effectively recruit staff. If looking to upgrade your licence, you should provide a copy of the inside cover of the MR Log Book (receipted) showing successful completion of the MR Knowledge Test and two good quality colour copies of both sides of your current NSW ‘C’ class drivers licence, a Current NSW Bus Driver Authority (clear colour copy of both sides) or a completed Driver Authority Application Form and NSW Driver Authority Medical. o Current National Criminal Check (only required if you do not have a current NSW Drivers Authority) o Current Working With Children’s approval number or receipt if you are waiting for approval o Have a Unique Student Identifier USI
