Clinics are held monthly (by appointment only) in Wisconsin Rapids and Marshfield.

A parent or legal guardian MUST accompany children under the age of 18.

If parent or guardian is unable to accompany the child to the clinic, they should contact the Health Department prior to clinic time for arrangements to complete and sign the consent forms.
