As much as we loved our building in Dean Lane, we suffered from a serious shortage of space. In November 2005 we moved into our newly-built surgery adjacent to ASDA in Bedminster as part of a multi-use development on the Old Police Station site. The surgery is on the corner of New Charlotte Street and Regent Road. It provides a fantastic building for all our patients and staff which is up to modern standards of design with suitable access and toilet facilities for disabled patients. HOW TO REGISTER WITH THE PRACTICE. The map on the back page shows the area that the practice covers. We are currently accepting new patients. To register with the practice we require proof of identity, i.e. photo identification, and proof of address, e.g. a recent utility bill. (A full list of acceptable documentation can be obtained from the surgery.) We will also require your NHS number, which can be obtained from your previous surgery. If you are an overseas visitor, please contact the surgery for further details. Please call into the surgery with the above documentation to register with our practice. Change of Details Please tell us if you change any of your details so that we can update your records. You can do this online, by completing a form in the surgery or write to us. We would like to have your up to date mobile number so that we can contact you if we need to. We would also like to have your e-mail address; we can inform you of updates to our services and the practice newsletter. One of their most difficult tasks is to judge the urgency of requests. Please help them to help you. Anything you tell them will be treated confidentially. If you require more privacy when visiting the surgery, ask the Receptionist who can arrange to speak to you away from the main waiting room. TRAINING: Teaching young doctors and medical students is an important part of our work. They may sometimes sit in on consultations with your agreement. Should you feel uncomfortable about this, please do not hesitate to say so before your consultation with us. SURGERIES Our surgeries run during our normal opening times which are as follows: Monday – Friday, 8.30am – 6.30pm We have an on-call doctor service available during opening hours to deal with urgent medical problems. We also offer some early morning and evening appointments each week and are open on 2 Saturday mornings each month for routine, pre booked appointments only, as follows: Monday evening 18.30 – 19.15 Tuesday evening 18.30 – 19.15 Wednesday evening 18.30 – 19.15 Thursday morning 07.15 – 08.00 Friday morning 07.30 – 08.00 Saturday morning Twice Monthly (usually 1st and 3rd Saturday of the month) Each morning and afternoon there is a minor illness clinic for certain illnesses led by one of our senior nurses (Val and Liz). The Reception team will offer you a telephone appointment (10.15- 11.15 a.m. or 1.00 p.m. – 2.15 p.m.) with Val or Liz, followed by a visit to the surgery later in the morning or afternoon should that be necessary. We are closed on all Sundays and Bank holidays. BFP_Practice_Leaflet_v8_160712 Page 3 of 12 APPOINTMENTS: To access the surgery, you should telephone 0117-966-3149 from 8.30am. You will be offered either a face to face consultation or a telephone appointment for all routine requests for a GP appointment. Medical emergencies are dealt with on the same day by the on call Doctor. If you are unable to attend an appointment, please phone and let us know in advance so that we can offer the appointment to another patient. You can see any of the doctors within the practice. If you wish to speak to or see a specific doctor, please inform the receptionist when booking the appointment so that this may be arranged for you. There may be a longer wait for an appointment in this case. HOME VISITS We ask all patients to come to the surgery to see a doctor unless it is physically impossible for the patient to leave the house at the time of the home visit request. If you are unable to come to the surgery and may need a home visit, please contact us before 10.00am on 0117-966-3149. Please always try to get to the surgery as we can offer more comprehensive care here. OUTSIDE NORMAL SURGERY HOURS In an EMERGENCY, please phone 999 for an ambulance. If you feel you need to see an emergency doctor, telephone 0845 121 0209 and you will be put through to BriSDoc (Bristol South Doctors On Call). This is a co-operative of doctors from GP surgeries in the South of Bristol, including Bedminster Family Practice, who provide an emergency service (telephone advice and, when necessary, face-to-face consultations or home visits) from 6.30pm – 8.00am on weekdays and throughout the whole weekend. BriSDoc is based at the Walk-in Centre, Downton Road, Knowle. Please note that, if you wish to speak to or see a doctor outside normal surgery hours, you must telephone BriSDoc first. Please do not go directly to the Walk-in Centre in Knowle unless your problem is one that you think can be dealt with by a nurse. For general medical advice or if you are unsure how to proceed, please contact the NHS Direct helpline on Tel: 0845 4647. If you post your repeat prescription request to us enclosing a stamped, self-addressed envelope, we will post the prescription back to you in the timeframe detailed above. University Hospitals Bristol NHS Foundation Trust (UH Bristol) has a solid track record over the last decade of setting ambitious strategic plans and delivering them. We have invested over £200million in new or expanded hospital facilities, most noticeably in the Bristol Heart Institute, Bristol Royal Infirmary, Bristol Royal Hospital for Children and the Bristol Haematology and Oncology Centre. We have grown and enhanced our range of specialist services for the people of the South West, South Wales and further afield. We are one of only a few acute hospital Trusts nationwide to have been awarded an Outstanding rating from the Care Quality Commission and we have achieved Global Digital Exemplar status for our work to transform healthcare through better use of information technology.
