After Action Report/Improvement Plan Point Beach Nuclear Plant EXECUTIVE SUMMARY On September 19 & 20, 2017, a Radiological Emergency Preparedness (REP) Full-Participation Ingestion Pathway Exercise was conducted for the 10-mile Emergency Planning Zone (EPZ) around the Point Beach Nuclear Plant (PBNP) by the U.S. Department of Homeland Security/Federal Emergency Management Agency (DHS/FEMA), Region V. The purpose of the exercise was to assess the level of State and local preparedness for response to a radiological emergency. This exercise was conducted in accordance with DHS/FEMA's policies and guidance concerning the exercise of State and local Radiological Emergency Response Plans (RERPs) and procedures. The previous exercise for this site was conducted on September 15, 2015. The qualifying emergency preparedness exercise for the PBNP was conducted on March 9, 1982. The PBNP is owned and operated by NextEra Energy, LLC. The plant consists of two-loop pressurized water reactors (Units 1 and 2) supplied by Westinghouse Electric Company, rated at 512 and 514 megawatts (MW), respectively. The operating licenses for the facility were granted in August 1973 (Unit 1) and October 1974 (Unit 2). Commercial operations began at the site during December 1973 (Unit 1) and December 1974 (Unit 2). DHS/FEMA wishes to acknowledge the efforts of the many individuals who participated in this exercise. The State of Wisconsin, the risk Counties of Kewaunee and Manitowoc, local municipalities, various non-government entities, and volunteers all contributed to the success of the exercise. Protecting the public health and safety is the full-time job of some of the exercise participants and an additional assigned responsibility for others. Still others have willingly sought this responsibility by volunteering to provide vital emergency services to their communities. Cooperation and teamwork on the part of all the participants was evident during this exercise. This Final After-Action Report/Improvement Plan contains the evaluation of the biennial exercise and the following out-of-sequence activities:

After Action Report/Improvement Plan Point Beach Nuclear Plant SECTION 1: INTRODUCTION 1.1 Exercise Basis On December 7, 1979, the President directed FEMA to assume the lead responsibility for all offsite nuclear planning and response. The DHS/FEMA activities are conducted pursuant to Title 44 of the Code of Federal Regulations (CFR) Parts 350 "Review and Approval of State and Local Radiological Emergency Plans and Preparedness," 351 "Radiological Emergency Planning and Preparedness" and 352 "Commercial Nuclear Power Plants: Emergency Preparedness Planning. These regulations are a key element in the Radiological Emergency Preparedness (REP) Program that was established following the Three Mile Island Nuclear Station accident in March 1979.

FEMA Regulation 44 CFR 350 establishes the policies and procedures for the DHS/FEMA initial and continued approval of State and local governments' radiological emergency planning and preparedness for commercial nuclear power plants. This approval is contingent, in part, on State and local governments' participation in joint exercises with licensee.

The DHS/FEMA responsibilities in radiological emergency planning for fixed nuclear facilities include the following:

• Taking the lead in offsite emergency planning and in the review and evaluation of Radiological Emergency Response Plan and procedures developed by State and local governments;

• Determining whether such plans and procedures can be implemented on the basis of observation and evaluation of exercises of the plans and procedures conducted by State and local governments;

• Responding to requests by the U.S. Nuclear Regulatory Commission pursuant to the Memorandum of Understanding between the NRC and FEMA dated October 1, 2015, and;

• Coordinating the activities of Federal agencies with responsibilities in the radiological emergency planning process:

U.S. Department of Agriculture; U.S. Department of Commerce; U.S. Department of Energy; U.S. Deprutment of Health and Human Services; U.S. Department of the Interior; U.S. Department of Transportation; U.S. Environmental Protection Agency; U.S. Food and Drug Administration; and U.S. Nuclear Regulatory Commission.

Representatives from these agencies serve on the DHS/FEMA Region V Regional Assistance Committee (RAC), which is chaired by the DHS/FEMA.

Formal submission of the RERP for Point Beach Nuclear Plant to FEMA Region V by the State of Wisconsin and involved local jurisdictions occurred on March 28, 1982. Formal approval of these RERP was granted by FEMA on M~ch 9, 1982, under 44 CPR 350.

A Radiological Emergency Preparedness (REP) Full-Participation Ingestion Pathway Exercise was conducted on September 19 & 20, 2017, and evaluated by the DHS/FEMA to assess the capabilities of State and local offsite emergency preparedness organizations in implementing their RERPs and procedures to protect the public health and safety during a radiological emergency involving the Point Beach Nuclear Plant. The purpose of this AAR/IP is to present the exercise results and findings based on the performance of the offsite response organizations during a simulated radiological emergency.

The findings presented in this Final After-Action Report/Improvement Plan are based on the evaluations of the federal evaluation team, with final determinations made by the DHS/FEMA Region V RAC Chair, and approved by the DHS/FEMA Headquarters.

The criteria utilized in the FEMA evaluation process are contained in:

• NUREG-0654/FEMA-REP-1, Rev. 1, "Criteria for Preparation and Evaluation of Radiological Emergency Response Plans and Preparedness in Support of Nuclear Power Plants," November 1980;

• FEMA Radiological Emergency Preparedness: Exercise Evaluation Methodology, as published in the FEMA Radiological Emergency Preparedness Manual, dated January 2016.

Section 2 of this report, entitled Exercise Overview, presents exercise details, exercise planning team leadership, and participation organizations.

Section 3 of this report, entitled Analysis of Capabilities, contains the exercise evaluation and results, summary results of the exercise evaluation, and criteria evaluation summaries.

Section 4 of this report, entitled Conclusion, presents detailed information on the demonstration of applicable exercise criteria at each jurisdiction or functional entity evaluated in a jurisdiction­ based, issues-only format. This section also contains: (1) descriptions of all Level 1, Level 2, and Plan Issues if any, assessed during this exercise, recommended corrective actions, and the State and local governments' schedule of co1Tective actions, if applicable, for each identified exercise issue; and (2) descriptions of unresolved Level 2 assessed during previous exercise's and the status of the offsite response organization (ORO) efforts to resolve them.

1.2 Emergency Planning Zone Description

Point Beach Nuclear Plant is located about 30 miles southeast of the city of Green Bay in Two Rivers, Wisconsin. Population centers within 50 miles of the site with more than 25,000 people include: Manitowoc (2010 census population: 33,736), located 13 miles southwest of the site; Green Bay (2010 census population: 104,057), located 30 miles northwest of the site; Appleton (2010 census population: 72,623), located 43 miles southwest of the site; and Sheboygan (2010 census population: 49,288), located 36 miles southwest of the site. The town of Two Rivers, which is located 10 miles south of the site, had a 2010 census population of 11,712. The total population located within the 10-mile plume pathway EPZ was 20,954. The protective action Sub-Areas located within the 10-mile EPZ are as follows: 5, ION, lONW, lOW, lOSW and lOS.

1.3 Exercise Purpose and Design The DHS/FEMA Region V Office evaluated the Point Beach Nuclear Plant (PBNP) Radiological Emergency Preparedness (REP) Full-Participation Ingestion Pathway Exercise conducted on September 19 & 20, 2017, to assess the capabilities of local emergency preparedness organizations in implementing their Radiological Emergency Response Plans (RERP) and procedures to protect the public health and safety during an incident involving the PBNP which precipitated a radiological emergency. The purpose of this report is to present the results and findings on the performance of the ORO during a simulated radiological emergency.

1.4 Exercise Objectives, Capabilities, and Activities Exercise objectives and identified capabilities/REP criteria selected to be demonstrated are discussed in Appendix E "Extent-of-Play." The Exercise Planning Team (EPT) selected objectives that focus on evaluating emergency response procedures, identifying areas for improvement, and fostering collaboration between the various OROs and stakeholders. This exercise focused on the following objectives:

• ORO demonstration of effective Emergency Operations Management;

• ORO demonstration of effective Protective Action Decision Making; and

• ORO demonstration of effective Protective Action Implementation;

• ORO demonstration of Field Measurements and Analyses;

• ORO demonstration of effective Emergency Notification and Public Information; and

• ORO demonstration of effective Support Operations and Facilities.

1.5 Scenario Summary Appendix F, Scenario Timelines, contains a summary of the exercise scenario, and a simulated sequence of events that was used as the basis for invoking emergency response actions by OROs during the Point Beach Nuclear Plant REP Full-Participation Ingestion Pathway Exercise on September 19 & 20, 2017.

Results of a technical review of the scenario, submitted by the State of Wisconsin and NextERA Energy on July 17, 2017, indicated that the scenario was adequate to support demonstration of Department of Homeland Security's requirements, as well as criteria selected by the OROs provided in'the State's July 17, 2017, extent-of-play submission. DHS/FEMARegion V accepted this exercise scenario on August 16, 2017.

During the exercise, in addition to information and data provided through the PBNP onsite scenario, controllers from the state of Wisconsin provided inject messages containing scenario events and/or relevant data to those persons or locations who would normally receive notification of such events. The inject messages were the method used for invoking additional specific response actions by OROs.

There were approximately 350 participants during this multiple day exercise which consisted of the State of Wisconsin and the counties of Calumet, Kewaunee, and Manitowoc .

After Action Report/Improvement Plan Point Beach Nuclear Plant SECTION 3: ANALYSIS OF CAPABILITIES 3.1 Exercise Evaluation and Results Contained in this section are the results and findings of the evaluation of all jurisdictions and functional entities that participated in the September 19 & 20, 2017, Point Beach Nuclear Plant Radiological Emergency Preparedness (REP) Full-Participation Ingestion Pathway Exercise. The exercise was conducted to test the offsite emergency response capabilities of State and local governments in the 10-mile Emergency Planning Zone (EPZ) surrounding the Point Beach Nuclear Plant.

Each jurisdiction and functional entity was evaluated based on its demonstration of exercise criteria delineated in the FEMA REP Program Manual, dated January, 2016. Appendix E, Extent-of-Play, contains detailed information on the selected exercise criteria and the extent-of­ play agreements used for this exercise. Presented below are definitions of the terms used in this report relative to the criteria demonstration status.

• M - Met: The status of a REP exercise evaluation area criterion indicating that the participating OROs demonstrated all demonstration criteria for the evaluation area criterion to the level required in the extent-of-play agreement with no Level I Findings, Level 2 Findings or Plan Issues assessed in the current exercise and no unresolved prior Level 2 Findings.

• 1 - Level I Finding: An observed or identified inadequacy of organizational performance in an exercise that could cause a finding that offsite emergency preparedness is not adequate to provide reasonable assurance that appropriate protective measures can be taken in the event of a radiological emergency to protect the health and safety of the public living in the vicinity of a nuclear power plant.

• 2 - Level 2 Finding - An observed or identified inadequacy of organizational performance in an exercise that is not considered, by itself, to adversely impact public health and safety.

• P-Plan Issue -An observed or identified inadequacy in the ORO's emergency plan or implementing procedures, rather than in the ORO' s performance.

The DHS/FEMA has developed a standardized system for numbering exercise issues. This system is used to achieve consistency in numbering exercise issues among DHS/FEMA Regions and site-specific exercise reports within each Region. It also is used to expedite tracking of exercise issues on a nationwide basis.

The identifying number of Level 1 Findings, Level 2 Findings, and Plan Issues includes the following elements, with each element separated by a hyphen (-).

The exercise criteria, contained in the FEMA Radiological Emergency Preparedness (REP) Program Manual dated January 2016, represent a functional translation of the planning standards and evaluation criteria of NUREG-0654/FEMA­ REP-1, Rev 1, "Criteria for the Preparation and Evaluation of Radiological Emergency Response Plans and Preparedness in Support of Nuclear Power Plants," November 1980. Special arrangements regarding staffing are as follows: • The Wisconsin Emergency Management Regional Directors and their Program Assistants will be pre-positioned in the vicinity of the Kewaunee and Manitowoc EOCs, and will be instructed to report to the County EOC's -10 minutes after notification of activation of the state BOC. • State Joint Information Center (JIC) staff will be pre-positioned in the area of the JIC in Green Bay and will be notified by the WEM Public Information Officer and/or RA VE Alert to report to the JIC when the Alert is declared. They will wait -10 minutes after notification before reporting to the JIC. The JIC is located at 3060 Voyager Drive, in Green Bay, Wisconsin. • The public inquiry hotline [Wisconsin Disaster Information Assistance Line (WI-DIAL) is located at Department of Military Affairs, Wisconsin Emergency Management, 2400 Wright St., Madison. Staff will be notified in person or via RAVE Alert and will report to the WI-DIAL. • Forward Operating Center/Mobile Response Lab (FOC/MRL) and Field Teams will be pre-positioned at the Manitowoc Highway Department and will be activated when needed and requested. • SRC room staff will be pre-positioned in the DMA break room pending SRC notification via pager. INTENT: This Sub-element is derived from NUREG-0654/FEMAREP- Environmental Response/Health 1, which requires that OROs have the capability to assess and control the and Safety radiation exposure received by emergency workers and have a decision Situational Assessment chain in place, as specified in the ORO's plans/procedures, to authorize emergency worker exposure limits to be exceeded for specific missions.

Radiation exposure limits for emergency workers are the recommended accumulated dose limits or exposure rates that emergency workers may be permitted to incur during an emergency. These limits include any pre­ established administrative reporting limits (that take into consideration TEDE or organ-specific limits) identified in the ORO's plans/procedures.

Should an evacuation of the FOC/MRL be ordered as a protective decision, this evacuation will be simulated by interview. Operational Coordination Sub-element 2.b. - Radiological Assessment, Protective Action Recommendations, and Precautionary and/or Protective Action Intelligence & Information Decisions for the Plume Phase of the Emergency Sharing INTENT: This Sub-element is derived from NUREG-0654/FEMAREP- 1, which requires that OROs have the capability to independently project Environmental Response/Health integrated dose from projected or actual dose rates and compare these and Safety estimates to the PAGs. Operational Communications OROs must have the capability to choose among a range of protective Situational Assessment actions those most appropriate in a given emergency. Sub-element 2.d. - Radiological Assessment and Decision Making for the Ingestion Exposure Pathway [DAY 2 TABLETOP]

INTENT This Sub-element is derived from NUREG-0654/FEMAREP-1, which requires that OROs have the means to assess the radiological consequences for the ingestion exposure pathway, relate them to the appropriate PAGs, and make timely, appropriate P ADs to mitigate exposure from the pathway. During an incident at an NPP, a release of radioactive material may contaminate water supplies and agricultural products in the surrounding areas. Any such contamination would likely occur during the plume phase of the incident and, depending on the nature of the release, could impact the ingestion exposure pathway for weeks or years.

Criterion 2.d.1: Radiological consequences for the ingestion pathway are assessed and appropriate protective action decisions are made based on the ORO's planning criteria. (NUREG-0654/FEMA-REP-1, A.3; C.1, 4; D.4; J.9,11)

EXTENT OF PLAY: In the event of a Site Area Emergency classification, the Department of Agriculture, Trade and Consumer Protection (DATCP) may consult with the SRC regarding the issuance of a Livestock Advisory for the 10 mile EPZ during the plume phase of the exercise. If no other Protective Action recommendations have been issued that would conflict with an advisory, DATCP directs Public Information to issue a media release.

An ingestion sampling plan will be developed and implemented to define the consequences of the release of a radioactive plume to the environment. This sampling plan will serve to prioritize ingestion samples based on plume and meteorological information. Additionally, the SRC and radiological assessment staff will assess simulated ingestion data provided by controller inject from an SRC controller.

In consultation with all relevant local, state, and federal agencies, the SRC will make recommendations for minimizing the radiological consequences of any radiological release on the 50 mile ingestion planning zone. These recommendations will be shared with all affected off-site response organizations. The ingestion counties, participating in the respective county EOCs, may be involved in the decision making process, but will not be evaluated.

The DATCP will evaluate whether it is necessary to impose agricultural holds within the affected area. The agricultural hold decision will be shared amongst the affected counties.

Manitowoc and Kewaunee County BOC staff will participate in planning and decision-making with state and Federal counterparts via simulated conference call during the facilitated tabletop. Some recommendations may come into the BOC to the County Radiological Officer (RO) or the Emergency Management Director. Once these recommendations are received, they are discussed among the BOC members. All final decisions are made by the Chief Elected Official (Kewaunee) or County Emergency Management Director (Manitowoc).

Sub-element 2.e. - Radiological Assessment and Decision Making Concerning Post-Plume Phase Relocation, Re-entry and Return

INTENT: This Sub-element is derived from NUREG-0654/FEMA-REP- 1, which requires that OROs have the capability to make decisions on post-plume phase relocation, reentry, and return of the general public. These decisions are essential for protection of the public from direct long­ term exposure to deposited radioactive materials from a sever incident at anNPP.

Criterion 2.e. l: Timely post-plume phase relocation, reentry, and return decisions are made and coordinated as appropriate, based on assessments of the radiological conditions and criteria in the ORO's plan and/or procedures. (NUREG-0654/FEMA-REP-l, 1.10; J.9; K.2.a; M.1)

Criterion 3.a. l: The OROs issue appropriate dosimetry, KI, and procedures, and manage radiological exposure to emergency workers in accordance with the plans/procedures. Emergency workers periodically and at the end of each mission read their dosimeters and record the readings on the appropriate exposure record or chart. Operational Coordination Public Sub-element 3.b - Implementation of KI Decision for Institutionalized Individuals and the General Public Information and Warning INTENT: This Sub-element is derived from NUREG-0654/FEMAREP- Environmental Response/Health 1, which requires that OROs have the capability to provide KI for and Safety institutionalized individuals, and, if in the plans/procedures, to the general public for whom immediate evacuation may not be feasible, very Public and Private Services and difficult, or significantly delayed. While it is necessary for OROs to have Resources the capability to provide KI to institutionalized individuals, providing KI to the general public is an ORO option and must be reflected as such in ORO plans/procedures. Provisions must include the availability of adequate quantities, storage, and means of distributing KI.

The focus is on those persons with disabilities and access/ functional needs that are (or potentially will be) affected by a radiological release from an NPP.

Criterion 3 .c. l: Precautionary and/or protective action decisions are implemented for persons with disabilities and access/ functional needs other than schools within areas subject to protective actions. (NUREG- 0654/FEMA-REP-1, J.10.c, d, e, g)

EXTENT OF PLAY: Manitowoc and Kewaunee County will demonstrate the ability and describe the resources to implement appropriate protective actions for their special population groups. The County's Chief Elected Official or his/her designee will evaluate protective action recommendations from the State BOC and determine whether those recommendations are appropriate for the local situation. The County EOC staff will simulate providing evacuation assistance for these people by contacting transportation providers.

Kewaunee County will call three contacts. The County BOC staff will plan for people with special transportation needs and simulate providing evacuation assistance as driven by the scenario.

Manitowoc County BOC staff maintains a list of people with special transportation needs (as provided by the clients) as well as agencies who serve clients with special needs and will simulate providing evacuation assistance for these people by contacting transportation providers. Manitowoc County will call three of their five contacts for transportation services.

The list of people with special transportation needs is available for evaluators to review, but cannot be copied or removed from the county BOC in order to protect confidentiality. The County BOC staff will plan for people with special transportation needs and simulate providing evacuation assistance, as driven by the scenario.

Criterion 3.c.2: OROs/School officials implement precautionary and/or protective actions for schools. (NUREG-0654/FEMA-REP-1, J.10.c, d, ~ EXTENT OF PLAY: Kewaunee County has no schools within the 10- mile EPZ of the Point Beach Nuclear Plant. However, there are plans to keep children from returning home that live in the EPZ, but attend school outside of the EPZ. School Superintendent, or their designee, the principal, a teacher and a bus driver will be available to discuss the plans via interview on September 19th, 2017 at 10:30 am, at the Kewaunee County School District, 915 Second Street, Kewaunee.

Manitowoc County will simulate the evacuation of a school and demonstrate this through interviews with the Two Rivers School Superintendent, a principal, the bus manager and a bus driver from Two Rivers Busses on September 19 at 10:00 am, at 4521 Lincoln Avenue, Two Rivers, WI.

Operational Coordination Sub-element 3.d. -Implementation of Traffic and Access Control

INTENT: This Sub-element is derived from NUREG-0654/FEMAREP- Critical Transportation 1, which requires that OROs have the capability to implement protective On-scene Security and Protection action plans/procedures, including relocation and restriction of access to evacuated/sheltered areas. This Sub-element focuses on selecting, Infrastructure Systems establishing, and staffing of traffic and access control points, and removal of impediments to the flow of evacuation traffic.

Criterion 3.d. l: Appropriate traffic and access control is established. Accurate instructions are provided to traffic and access control personnel. (NUREG-0654/FEMA-REP-1, A.3; C.1,4; J.10.g,j)

EXTENT OF PLAY: The Manitowoc County Department of Public Works and the Manitowoc County Sheriffs Office will discuss and designate traffic and access control points. This will occur at the Manitowoc County EOC. EXTENT OF PLAY: One impediment to evacuation via inject message for each county and they will be demonstrated by simulation, including the decision making process to select alternative routes, inform the public of restrictions related to the impediment and develop a plan to remove the impediment. This coordinated effort will be demonstrated by both the Manitowoc County BOC, Kewaunee County BOC, as well as the nc. Sub-element 3.e - Implementation of Ingestion Exposure Pathway Decisions

INTENT: This Sub-element is derived from NUREG-0654/FEMAREP- 1, which requires that OROs have the capability to implement protective actions, based on criteria recommended by current FDA guidance, for the ingestion exposure pathway EPZ (i.e., the area within an approximate 50- mile radius of the NPP). This Sub-element focuses on those actions required for implementation of protective actions.

Criterion 3.e.l: The ORO demonstrates the availability and appropriate use of adequate information regarding water, food supplies, milk, and agricultural production within the ingestion exposure pathway emergency planning zone for implementation of protective actions. (NUREG-0654/FEMAREP-1, A.3; C.1, 4; J.11)

EXTENT OF PLAY: The State of Wisconsin will demonstrate the use of agricultural and natural resource databases maintained by the Wisconsin Department of Agriculture, Trade and Consumer Protection (DATCP). All State Radiological Coordinator (SRC) recommendations in regards to protective actions will be by using controller data based on simulated laboratory analysis of ingestion samples. Data regarding water, food supplies, milk, and agricultural products will be utilized for implementation of protective actions. Any additional information and data generated from federal play will also be used as appropriate for implementing protective action recommendations.

The Kewaunee County Public Health nurse will confer with the Manitowoc County Radiological Officers (ROs); who will relay the

Criterion 3.e.2: Appropriate measures, strategies, and preprinted instructional material are developed for implementing protective action decisions for contaminated water, food products, milk, and agricultural production. (NUREG-0654/FEMA-REP-1, G.l, J.9, 11)

EXTENT OF PLAY: The SRC and radiological assessment staff, in consultation with other appropriate agencies, will use current lists of farmers, food processors, distributors, and water supplies within the IPZ to demonstrate the capability to develop appropriate measures, strategies, and pre-printed instructional materials for implementing protective actions for contaminated water, food products, milk, and agricultural products within the ingestion exposure pathway. Demonstration of this will occur during facilitated discussions at the September 20 tabletop exercise in Manitowoc. The printing and distribution of "Radiological Emergency Information for Wisconsin Farmers, Food Processors and Distributors" is performed at the Site Area Emergency classification, and will be simulated.

Manitowoc and Kewaunee County will demonstrate, through discussion, action, or document review, the capability to coordinate with the state to implement protective actions for the ingestion exposure pathway and to distribute ingestion information to members of the general public.

Sub-element 3.f - Implementation of Post-:Plume Phase Relocation, Reentry, and Return Decisions [DAY 2 TABLETOP]

INTENT: This Sub-element is derived from NUREG-0654/FEMAREP- 1, which requires that OROs have the capability to implement plans, procedures, and decisions for post-plume phase relocation, reentry, and return. Implementation of these decisions is essential for protecting the public from direct long-term exposure to deposited radioactive materials from a severe incident at a commercial NPP. Criterion 3.f.1: Decisions regarding controlled reentry, relocation, and return of individuals during the post-plume phase are coordinated with appropriate organizations and implemented. These actions will be coordinated with the appropriate county agencies.

Manitowoc and Kewaunee County will address short and long-term relocation through coordination with other OROs. Coordinated instructions to the public and local businesses will be disseminated via a press release during the facilitated tabletop discussion on September 20.

County agencies will demonstrate the capability to coordinate with the state to implement controlled re-entry of emergency workers to the evacuated area and the return and any necessary relocation of the public.

Sub-element 4.a - Plume Phase Field Measurements and Analyses Operational Coordination

INTENT This Sub-element is derived from NUREG-0654/FEMAREP-1, Situational Awareness which requires that OROs have the capability to deploy FMTs with the equipment, methods, and expertise necessary to determine the location of airborne radiation and particulate deposition on the ground from an airborne plume. In addition, NUREG-0654/FEMA-REP-1 indicates that OROs must have the capability to use FMTs within the plume exposure pathway EPZ to detect airborne radioiodine in the presence of noble gases and radioactive particulate material in the airborne plume. In an incident at an NPP, the possible release ofradioactive material may pose a risk to the nearby population and environment. Although incident assessment methods are available to project the extent and magnitude of a release, these methods are subject to large uncertainties. During an incident, it is important to collect field radiological data to help characterize any radiological release. EXTENT OF PLAY: Three field monitoring teams will be pre-staged and dispatched (two for evaluation) from the FOC/MRL under the control of the Field Team Manager and at the direction of the State Radiological Coordinator (SRC). Field monitoring team survey results will be transmitted to the FOC/MRL. At least one sample will be transported to the FOC/MRL to demonstrate sample receipt via courier or directly by a team.

Criterion 4.a.3: Ambient radiation measurements are made and recorded at appropriate locations, and radioiodine and particulate samples are collected. Teams will move to an appropriate low background location to determine whether any significant (as specified in the plan and/or procedures) amount of radioactivity has been collected on the sampling media. EXTENT OF PLAY: Field monitoring teams will be capable of obtaining airborne radioiodine and particulate samples, determine field radioiodine measurements, transmit data and transport samples to the FOC/MRL team. Personal Protective Equipment (PPB) will be simulated. Charcoal cartridges will be used as a substitute for silver zeolite. Field Monitoring Teams will demonstrate the ability to obtain ambient radiation measurements and air samples. All radiological readings for this exercise will be iniected by a controller. Sub-element 4.b-Post-Plume Phase Field Measurements and Sampling

INTENT: This Sub-element is derived from NUREG-0654/FEMAREP­ l, which requires that OROs have the capability to assess the actual or potential magnitude and locations of radiological hazards to determine the ingestion exposure pathway EPZ and to support relocation, reentry, and return decisions. This Sub-element focuses on collecting environmental samples for laboratory analyses that are essential for decisions on protecting the public from contaminated food and water and direct radiation from deposited materials.

EXTENT OF PLAY: The ingestion teams, or unrestricted area field teams (URAFT) will be dispatched from the FOC/MRL to pre-designated sites where ingestion samples will be taken. Once dispatched, the actual sampling will be- conducted out of sequence. Members of the sample teams will deliver the samples directly to the FOC/MRL sample receipt area. In the interest of time, the results of the samples will be provided to the SRC via controller inject.

Two State Ingestion Teams (one evaluated) will demonstrate approved ingestion sampling procedures for ingestion samples which may include any of the following: ground and surface water, vegetation, freshly packaged food, milk, and soil. Additional ingestion teams may be participating for training purposes only. The teams to be evaluated will be chosen by the Field Team Coordinator. Both of the ingestion teams will take at least one full set of samples. Chain of custody for samples will be demonstrated in accordance with sampling procedures.

URAFT do not carry survey meters, nor take exposure readings. The ability to take exposure readings is demonstrated during plume phase exercises. Exposure rate readings will be provided to the SRC via controller inject. Areas of interest will be identified by the SRC based upon the controller-injected data, RASCAL models, plant conditions and meteorological conditions.

Sub-element 4.c - Laboratory Operations

INTENT This Sub-element is derived from NUREG-0654/FEMAREP- 1, which requires that OROs have the capability to perform laboratory analyses ofradioactivity in air, liquid, and environmental samples to support protective action decision making.

Sub-element 5 .a - Activation of the Prompt Alert and Notification Operational Coordination Public System Information and Warning INTENT This Sub-element is derived from NUREG-0654/FEMAREP- 1, which requires that OROs have the capability to provide prompt Operational Communications instructions to the public within the plume exposure pathway EPZ. Specific provisions addressed in this Sub-element are further discussed in Section V, Part A of this Manual, Alert and Notification Systems.

Criterion 5.a.l: Activities associated with'primary alerting and notification of the public are completed in a timely manner following the initial decision by authorized offsite emergency officials to notify the public of an emergency situation. EXTENT OF PLAY: Manitowoc County Emergency Services (primary) sounds the Point Beach Emergency Planning Zone Sirens. Some of those sirens are in Kewaunee County, and are included in the notification from Manitowoc County. Manitowoc County will simulate siren sounding if applicable. Kewaunee County is the alternative location/secondary. Emergency Alert System (EAS) and Special News Broadcast simulated messages to WOMT (Manitowoc) and WQDC (Kewaunee) will be generated and distributed, but will not be broadcast. Based on PAD activity for this exercise, simulated EAS messages may be utilized to demonstrate this activity. Siren testing data is submitted to FEMA annually with the Annual Letter of Certification packet.

WQDC in Kewaunee County does operate 24\7, but it is not manned between the hours of 5:00 p.m. to 6:00 a.m. The county has a plan to contact WQDC's on-call person at an Alert, who would then go to the station.

WQDC Radio Station will have staff available for interview during the exercise.

Sub-element 5.b- Subsequent Emergency Information and Instructions Public Information and Warning for the Public and the Media Operational Communications INTENT: This Sub-element is derived from NUREG-0654/FEMAREP- 1, which requires that OROs have the capability to disseminate appropriate emergency information and instructions, including any recommended protective actions, to the public. In addition, NUREG- 0654/FEMAREP-1 requires OROs to ensure that the capability exists for providing information to the media. This includes the availability of a physical location for use by the media during an emergency. NUREG- 0654/FEMA-REP-1 also provides that a system must be available for dealing with rumors. This system will hereafter be known as the "public inquiry hotline."

Criterion 5.b.l: OROs provide accurate subsequent emergency information and instructions to the public and the news media in a timely manner. (NUREG-0654/FEMA-REP-1, E.5, 7; G.3.a, G.4.a, c)

EXTENT OF PLAY: The state will demonstrate the formulation and dissemination of accurate information and instructions, including pre­ scripted BAS and Special News Broadcast messages provided to the JIC. The State of Wisconsin PIOs in the JIC will coordinate with the State BOC and PIOs from the counties and the utility to ensure the media is briefed in a clear, accurate, and timely manner. Procedures for early notification of the media will be demonstrated in the State BOC prior to the activation of the JIC, if driven by the scenario. Public inquiry calls will be handl~d by the state in the WI-DIAL call center. The WI-DIAL will be staffed by WEM personnel.

The state pre-distributes ingestion information in the WI Radiological Emergency Information for Farmers, Food Processors, and Distributors booklet. This is sent to County Ag Extension Agents and Emergency Management Directors both in hard copy and electronically for posting

Manitowoc and Kewaunee County will have a Public Information Officers (PIOs) present at the JIC who will work with the state, utility and other county PIOs to ensure that their county's interests and concerns are represented in the media briefings. The counties use the WI-DIAL call center to assist with rumor control. All media briefings will be done at the JIC in Green Bay. The Manitowoc and Kewaunee County PIOs assigned to the JIC will work closely with the PIOs at the county BOC to make sure media releases are accurate and coordinated.

ASSESSMENT AREA 6--SUPPORT OPERATION/FACILITIES

Sub-element 6.a - Monitoring, Decontamination, and Registration of Evacuees

INTENT: This Sub-element is derived from NUREG-0654/FEMAREP- 1, which requires that OROs have the capability to implement radiological monitoring and Decontamination of evacuees, while minimizing contamination of the facility. OROs must also have the capability to identify and register evacuees at reception centers.

Criterion 6.a. l: The reception center facility has appropriate space, adequate resources, and trained personnel to provide monitoring, Decontamination, and registration of evacuees. (NUREG-0654/FEMA­ REP-1, A.3; C.4; J.10.h; J.12)

EXTENT OF PLAY: The State of Wisconsin will demonstrate the adequacy of procedures, facilities, equipment and personnel for radiological monitoring and Decontamination. Health monitoring teams will demonstrate radiological monitoring and Decontamination of evacuees at reception centers in accordance with the applicable County reception center procedures. Both counties will set up and operate three monitoring stations (i.e. two portal monitors and one hand monitor).

The heath monitoring teams will include personnel from the DHS-RPS and individuals from the counties who have received training as Auxiliary Health Monitors. The personnel from the county will work under the supervision ofDHS-RPS staff and will be evaluated as a part of the State of Wisconsin's response.

Once each reception center is operational and monitors have surveyed several evacuees to establish flow, at least six evacuees will be monitored

Main Street, Luxemburg. In Manitowoc County, the Reception Center demonstration will be held out­ of-sequence, with set-up beginning at -6 pm on Monday, September 18, 2017, at the Manitowoc County Highway Department, 3500 Highway 310, Manitowoc, WI.

Sub-element 6.b-Monitoring and Decontamination of Emergency Environmental Response/Health Workers and their Equipment and Vehicles and Safety

INTENT: This Sub-element is derived from NUREG-0654/FEMA­ REP-1, which requires that OROs have the capability to implement radiological monitoring and Decontamination of emergency workers and their equipment, inclusive of vehicles.

Criterion 6.b.1: The facility/ORO has adequate procedures and resources to accomplish monitoring and Decontamination of emergency workers and their equipment and vehicles. (NUREG-0654/FEMA-REP- 1, K.5.a, b)

EXTENT OF PLAY: Health monitoring teams will demonstrate this criterion out of sequence, at -6:30 p.m. on Tuesday night, June 27, at the Kewaunee Reception Center (Luxemburg-Casco Intermediate School), and on Monday night, September 18, at the Manitowoc County Reception Center (County Highway Commission), 3500 Highway 310, Manitowoc, Wisconsin at approximately 4:30 p.m. (set-up will begin at 4:00 p.m.). Reception Center personnel will demonstrate the monitoring of one emergency worker and one vehi(?le, which will require Decontamination at each reception center.

Sub-element 6.c - Temporary Care of Evacuees

INTENT: This Sub-element is derived from NUREG-0654/FEMA­ REP-1, which requires OROs to have the capability to establish relocation centers in host/support jurisdictions. The American Red Cross normally provides congregate care in support of OROs under existing letters of agreement.

EXTENT OF PLAY: Health monitoring teams along with the American Red Cross will demonstrate this criterion out of sequence, at -6:30 p.m. on Tuesday night, June 27, at the Kewaunee Reception Center (Luxemburg-Casco Intermediate School), and on Monday night, September 18, at the Manitowoc County Reception Center (County Highway Commission), 3500 Highway 310, Manitowoc, Wisconsin at approximately 6:30 p.m. (set-up will begin at 6:00 p.m.). The American Red Cross will do a full set up in accordance with their procedures. At least one evacuee will be processed through the congregate care center. A Red Cross representative will be available to the federal evaluator to answer questions about the congregate care center.

Sub-element 6.d - Transportation and Treatment of Contaminated Injured Individuals

INTENT: This Sub-element is derived from NUREG-0654/FEMAREP- 1, which requires that OROs have the capability to transport contaminated injured individuals to medical facilities with the capability to provide medical services.

Criterion 6.d. l: The facility/ORO has the appropriate space, adequate resources, and trained personnel to provide transport, monitoring, Decontamination, and medical services to contaminated injured individuals. (NUREG-0654/FEMAREP-1, F.2; H.10; K.5.a, b; L.1, 4)

EXTENT OF PLAY: Two medical hospital drills will be held for this exercise. The first demonstration (Kewaunee County) will occur out-of­ sequence on June 27th at Aurora Baycare Medical Center, 2845 Greenbrier Road, Green Bay. The hospital portion of the drill will be a continuation of the Kewaunee County ambulance drill but will be demonstrated separately. Set-up for the drill will commence at -6:00 p.m. at the Kewaunee County Reception Center. The transportation portion of the drill will begin at -7:00 p.m. and should transition to the hospital portion by -7:30 p.m. Once the ambulance drill loads the contaminated patient into the ambulance that will conclude the ambulance and transportation drill. A separate contaminated/injured

The second medical hospital drill will be held on September 18th, 2017 at the Holy Family Memorial Hospital, 2300 Western Avenue, Manitowoc. The hospital portion of the drill will be a continuation of the Manitowoc County ambulance drill. Set-up for the drill will commence at -6:00 p.m. at the Manitowoc County Reception Center. Manitowoc Fire Department EMS personnel will demonstrate the packaging and preparation for transport of a potentially contaminated medical victim. Ambulance personnel will make a report to Holy Family Memorial Medical Center (2300 Western A venue, Manitowoc) and transport to that facility.

Luxemburg Rescue Service personnel will demonstrate the packaging and preparation for transport of a potentially contaminated medical victim. Ambulance personnel will make a report to Aurora Baycare Medical Center and transport to that facility.

NARRATIVE SUMMARY

At 0730, the crew assumes the watch. A short time later, the Control Room receives a phone call from security that the driver of the gasoline tanker truck, who is filling the onsite gasoline tank, is having difficulties isolating the valve on the transfer hose. The driver is requesting help from Operations to isolate the line.

An Auxiliary Operator (AO) arrives on the scene where the gasoline truck is transferring fuel to the onsite gasoline storage tank, and reports to the Control Room that when trying to isolate the valve at the coupling to the truck, the coupling broke off and the tanker is draining gasoline onto the ground. The AO confirms that several hundred gallons of gasoline is spreading from the truck into the surrounding area. The gasoline spreads close to Warehouse 1 where the Relay Technicians are located and the fumes from the gasoline vapors are very strong. The Shift Manager (SM) should declare an UNUSUAL EVENT based on HU3.l "Report or detection of toxic or flammable gases that has or could enter the site area boundary in amounts that can affect NORMAL PLANT OPERATIONS".

While cleanup of the gasoline spill continues, Unit 1 RCS activity begins to rise due to fuel cladding failure causing lRE-104, Unit 1 Charging Pump Low Range area monitor and RE- 116, Demin Valve Gallery area monitor to rise to Alert then Alarm values.

The control room crew will enter AOP-8A, High Reactor Coolant Activity and request Chemistry to confirm the high RCS activity with an RCS sample. When 1SC-966C, RCS Hot Leg Sample Containment Isolation Valve is opened to obtain the liquid sample, Sample Room Area Radiation monitors lRE-106 Low Range and IRE-136 High Range also rise to High Alarm values and stabilize at approximately 1.3 R/hr.

The Shift Manager should declare an ALERT per RA 3.2, "Any VALID radiation monitor reading GREATER THAN IR/hr in areas requiring infrequent access defined in Table H-1 ", - Primary Auxiliary Building.

Chemistry will take the RCS sample to the lab for analysis, which will later be reported as exceeding the LOSS threshold for the Fuel Clad boundary.

The ALERT classification initiates the activation of the Emergency Response Facilities (ERFs) and augmentation of the Emergency Response Organization (ERO). During this time, the sample results for U-1 RCS are provided to the facilities. They indicate that the amount of 1-131 Dose Equivalent in the RCS is greater than 300 uCi/gm 1-131; the threshold for a LOSS of Fuel Clad barrier. lfnot already begun, the crew will begin reducing load on Unit 1.

To challenge the resources of the Technical Support Center (TSC) and Operations Support Center (OSC), a starting-air hose leak on G02, Emergency Diesel Generator will be indicated by the Control Room annunciator for "Low Air Start Pressure" for that equipment.

After a stable load reduction is established, a 75 gpm RCS leak inside containment will occur on Unit 1. This leak rate exceeds the threshold for a POTENTIAL LOSS of the RCS barrier. Coupled with the previous LOSS of the Fuel Clad barrier (>300 uCi/gm 1-131), a LOSS or POTENTIAL LOSS of Two Fission Product Barriers will exist. The Shift Manager / Emergency Director (ED) should declare a SITE AREA EMERGENCY per FS 1, Loss or Potential Loss of Two Fission Product Barriers.

To challenge the resources of the Technical Support Center (TSC) and Operations Support Center (OSC), a 1B03 Ground Fault Alarm will come in and a rising U 1Turbine Bearing vibration alarm will occur, each warranting dispatch of field operators for local reports.

Eventually, due to deteriorating plant conditions the Control Room will trip the Unit 1 at which time a IA mainstream line safety will FAIL OPEN resulting in a faulted and rapidly depressurizing Steam Generator (SG).

Following Steam Generator dry-out and plant stabilization, a tube in the IA SG will rupture creating a Ruptured and Faulted SG which is bypassing the containment boundary. This represents a LOSS of the Containment Fission Product Barrier and the threshold for GENERAL EMERGENCY per FG I , Loss of Any Two Barriers and Loss or Potential Loss of the Third Barrier will be met.

Analogous indications will be a rising IA SG level, a rising SG pressure, and 1 RE-231, Steam Line Monitor Alarm indicating an Airborne Release to the environment.

PARs for the GENERAL EMERGENCY will be to evacuate a 5 mile radius and 10 mile downwind sectors due to URI dose assessment exceeding EPA P AGs at 5 miles or identification of a Rapidly Progressing Accident per EPIP 1.3, Dose Assessment and Protective Action Recommendations Attachment A, Protective Action Recommendations.

EPA PAGs are expected to be exceeded at greater than 5 miles, but less than 10 miles.

At approximately 1400 to 1500, the State and Counties will engage in a tabletop discussion regarding the termination of exercise day 1 onsite activities. The Joint Information Center (TIC) may continue to participate in the exercise as determined by nc lead controllers (utility and emergency management).

Chemistry calls the Control Room or Rad/Chem Coordinator with an RCS sample that is 310 uCi/gm 1131 Dose Equivalent.

The Control Room initiates OP-3A, Power Operation to Hot Standby, OP-3B, Reactor 0930 Shutdown, or AOP-17 A Unit 1, Rapid Power Reduction and will begin preparations to reduce power.

RCS activity >300 uCi/gm 1131 Dose Equivalent is the threshold for a LOSS of the Fuel Clad barrier. G-02 Emergency Diesel Generator is rendered 0950 Mini-scenario #3 inoperable due to starting_ air system leak. NRC ENS notification of ALERT as soon as possible after the NARS notification and within 60 minutes of the ALERT declaration.

1000 TSC should be staffed and operational with EROS established to the NRC headquarters Office within 60 minutes of an ALERT classification.

Initiating Condition for Site Area Emergency declaration.

1010 A -75 gpm RCS leak inside containment exceeds the threshold for a POTENTIAL LOSS of the RCS barrier. Coupled with the previous LOSS to the Fuel Clad barrier, this constitutes a SITE AREA EMERGENCY per PSI.

At some point after the RCS leak the operating crew will trip the Unit 1. Upon the trip a 1A SG mainsteam safety valve will FAIL OPEN faulting the IA SG on the secondary side.

There is no success path to recover/shut the safety valve. The I A SG will completely depressurize and the crew will stabilize plant conditions and control RCS temperature post 1011 Mini-scenario #4 SG dry-out.

No release to the environment yet exists until eventually, a tube in the IA SG ruptures resulting in a Ruptured-Faulted SG that is bypassing the Containment Barrier. Note: regardless of other scenario events the SGTR will occur at 1150 to ensure the desired time­ line.

The ED declares a SITE AREA 1025 (DEP Documentation) EMERGENCY within 15 minutes of indication that there is a potential loss of the RCS barrier. A real event occurs onsite that, in the opinion of the ( actual) Shift Manager, presents a threat to personnel or plant safety and/orrequires the declaration ofareal emergency.

OR

An emerging or escalation of an ongoing offsite emergency occurs which impacts the response actions ofexercise participants.

OR

The Sequence of Events have been initiated and played out to their expected conclusion/resolution AND Lead Facility Controllers and Offsite participants have determined that the objectives of the exercise: have been met have been met to the extent that they are likely to be have not been met and the reasons therewith are understood OR Exercise termination is appropriate based upon participant decisions and responses.

3.4.2 Actions a.
