Southern Mallee Primary Care Partnership (Southern Mallee PCP) is an established community, health and wellbeing partnership located in North Western comprising of three local government Areas; Buloke Shire, Gannawarra Shire and Swan Hill Rural City (not including Robinvale).

Purpose: Strengthening partnerships within our rural communities to build a healthy, inclusive, resilient Southern Mallee

Aim of the resource • Increase in awareness of, and referral to a range of Mental Health Services and supports • Increase and improve communication between agencies for the benefit of the consumer • Provide agencies with a resource which clearly defines the Eligibility and Entry Criteria for Mental Health and Related Services across the Southern Mallee.
