The Community Nursing Service operates 7 days a week from 8am to 8pm and offers a wide range of nursing care for housebound individuals. They also provide an out of hours, on call service from 8pm to 8am.

This service is not an emergency service, however, if you need urgent support or advice in the evenings, weekends and at Bank Holidays please call 111.

The community nursing service offers an open referral system to allow individuals; families or carers, as well as any health or social care professional to refer for advice and an individual assessment for care provision, as appropriate. Referrals are accepted for people who are • Housebound • They have a nursing need that specifically requires a home visit that cannot be undertaken elsewhere

Who are community nurses?

Community nurses have a wide range of knowledge and skills, led by District Nurses who have had additional specialist training. They provide care to patients who have very specific needs which can be met by the specialist skills of the Community Nursing team.

What do community nurses do?

• On receiving a referral we may telephone you first to discuss your needs to ensure that we are the appropriate service for you.

• A community nurse will visit you at home to complete an assessment of your nursing care needs. The timing for this visit will depend on the reason our support is required.

• Following assessment, we will devise a plan of care with you, outlining your treatment plan; this plan will include what we will actually do, when and how often we will visit.

• The community nurse team will continually review your care and progress this with you. This may result in your visit pattern changing or arrangements being made for you to attend the Practice Nurse.

• In some circumstances you, or your carer, may be taught how to manage your own care, your nurse will discuss this with you.

• Community nurses will work in partnership with other agencies who may already be involved in your care, for example Health, Social Care and Voluntary Services. With your consent, we may also refer to other services for your ongoing support at home.

• Once your treatment plan is complete, the community nurses will discharge you from their care.

A record of your care will be written at each visit and these notes are to be safely stored with you as they are a legal record of your health care. Please ensure that these are returned to your nursing team when the nurses are no longer visiting.

When the nurses are providing care in your home, we do ask that you ensure that you have a clean and safe area to work. Nurses are required to clean their hands before and after consultations using either alcohol gel or liquid soap and water. This will help reduce the risk of developing an infection. To ensure the safety of our staff please make sure that any animals are secured away and we request that patients refrain from smoking during visits.

Your community nurses support the training of students therefore there may be times when your community nurse is accompanied by a student. Please let us know in advance of the visit if you prefer not to be seen by a student

Community nurses are committed to the principles that no decision for a person’s community healthcare should be made without them. We seek for our service to be flexible and responsive, working in partnership with others to ensure they receive a first class service and experience. We encourage a transparent and open approach to our practice and are open and responsive to feedback. If you have any concerns please contact your District Nurse team leader to discuss these as soon as possible.

Valuables should not be brought into the hospital. If patients have to bring in valuable items they should ask a nurse to store them safely and request a receipt for the items. You may not be able to have the valuable items returned if the time of discharge from hospital is out of hours.
