They are usually less than 3 cm and can be solitary or multiple in number. Asymptomatic hemangiomas of liver need no surgicel intervention and masterly inactivity is suggested. But the rare symptomatic ones are vulnernable for various dreadful sequelae and hence operative intervention is indicated. We here report a case of 16 yr old girl presenting with symptoms of intermittent small bowel obstruction and Inferior venecava compression due to the presence of a Giant Hepatic Hemangioma. The greatest number of these lesions are benign angiomas of the body surfaces, small elevated red areas which remain without symptoms throughout life and are not subjected to treatment. Larger tumors of this type which undergb active growth after birth or which are situated about the face or oral cavity, where they constitute cosmetic defects, are more often the object of surgical removal. The majority of the vascular tumors clinically or pathologically studied fall into this latter group. Benign angiomas of similar pathologic nature occur in all of the internal viscera but are most common in the liver, where they are disclosed usually at autopsy. Angiomas of the bone, muscle, and the central nervous system are of less common occurrence, but, because of the symptoms produced, a higher percentage are available for study. Malignant lesions of the blood vessels are far more rare than was formerly supposed. An occasional angioma may metastasize following trauma or after repeated recurrences, but less than 1per cent of benign angiomas subjected to treatment fall into this group. Differential diagnosis of malignant vascular tumors can be often quite challenging, either at the low end of the spectrum, distinguishing an epithelioid hemangioendothelioma from an epithelioid hemangioma, or at the high-grade end of the spectrum, between an angiosarcoma and a malignant epithelioid hemangioendothelioma. This vascular tumor is rare in the world and, according to our acknowledge, only one case has been reported in Iran. In this case report, we described the second case of targetoid hemosiderotic hemangioma. Material and Methods: The patient is an 8 years old boy with a targetoid asymptomatic red popular lesion on his lower leg from 5 years ago .The lesion was completely excised. Skin biopsy showed dilated vascular spaces with hobnail endothelial cells in the superficial dermis accompanied with narrower vessels in the deep dermis which was compatible with targetoid hemosiderotic hemangioma. In a follow up period of three years the lesion did not recur. Conclusion: Distinction between targetoid hemosiderotic hemangioma and malignant skin lesions such as melanoma, Kaposi sarcoma and angiosarcoma is necessary. Thus, knowledge of clinical picture and pathology of this lesion can prevent unnecessary aggressive procedures.

    Mesenchymal Tumors of the Liver: What’s New and Unusual (My Perspective) Linda D Ferrell, MD, University of California, San Francisco Introduction Mesenchymal tumors of the liver are relatively uncommon (except cavernous hemangioma), so many pathologists are not as familiar with variations from the more typical appearances as described in many standard texts (See General References 1-4) and/or have not had significant experience with the rare variant types of mesenchymal lesions. Recently, there has been more focus on unusual and rare forms of mesenchymal tumors, especially within the spectrum of vascular lesions, so my personal perspective of these newly-descried and unusual variants will be the focus of this presentation. Cavernous Hemangioma, with Hemangioma-like Vessels in adjacent liver Cavernous hemangioma (CH) is the most common form of mesenchymal/vascular tumor in the liver, but an under-recognized feature is the presence of multiple smaller clusters or foci of vascular structures in the liver adjacent to a large/giant CH; these have been designated as hemangioma-like vessels (HLV) (5,6). The structure of HLVs is identical to the architecture of the large vascular spaces seen in CH, with a predominantly collagenous wall with disorganized and somewhat scant smooth muscle and elastic fiber content. This wall is lined by cytologically bland/unremarkable endothelial cells with very low proliferative index (<2%). HLVs tend to be located in the periportal region, and may be single or multiple as a cluster. A well-defined plaque on the trunk with a violaceous centre and ecchymotic rim giving a “targetoid” appearance Case synopsis A 10-year-old boy presented with a 7-year history of a single, asymptomatic plaque on the right side of the trunk (Fig. 1). The lesion started as an erythematous papule and later gradually increased in size. Cutaneous examination revealed a single non-tender oval plaque of 2x3 cm, present on the right side of the trunk. The plaque had a violaceous center and a peripheral ecchymotic rim, giving a targetoid appearance. The remainder of the skin examination was unremarkable. There was no history of fluid filled lesions, trauma, or insect bite preceding the lesion.
