Altru & You providers deliver care in more than 30 communities plus provide access to several specialties via telemedicine. In Grand Forks and East Grand Forks, care is provided at 13 locations including an acute care hospital, a specialty hospital, inpatient rehabilitation hospital, various clinics, Altru Cancer Center, Altru Advanced Orthopedics, Sanny & Jerry Ryan Center for Prevention & Genetics, and Yorhom Medical Essentials. In addition, local health care providers throughout the region ensure that Medica members can get the care they need close to home. Members may see any provider in the Altru & You with Medica network without a referral. Members can also access non-network providers but may have higher deductibles and out-of pocket expenses than for in-network providers.     THE COMMONWEALTH FUND COMMISSION ON A HIGH PERFORMANCE HEALTH SYSTEM THE NORTH DAKOTA EXPERIENCE: ACHIEVING HIGH-PERFORMANCE HEALTH CARE THROUGH RURAL INNOVATION AND COOPERATION Douglas McCarthy, Rachel Nuzum, Stephanie Mika, Jennifer Wrenn, and Mary Wakefield May 2008 ABSTRACT: Resource constraints and the desire to preserve the local economy have made necessity the mother of invention in North Dakota, driving health care providers and policymakers to try new approaches to care and to institute better practices relatively quickly. Collaboration to support primary care and the concept of a medical home, organization of care through cooperative networks of providers, and innovative use of technology to meet patient needs and hold down costs are examples of how North Dakota is able to provide its citizens with accessible, quality, and efficient health care despite the challenges of a rural setting. Rural communities have a unique context of community trust and interdependence, a social capital that allows them to innovate in meeting patients’ needs. A strong sense of mission, vigilance to process and outcomes, and enhanced communication and collaboration among health care providers are key to improvements made in North Dakota health care. Step 6 | Planning for Action and Monitoring Progress Attachment One: Community Health Assessment June 2016 Attachment Two: Preliminary Community Asset Inventory Introduction The 2016 Community Health Assessment was a joint effort led by Altru Health System and the Grand Forks Public Health Department. Our two organizations have a history of collaboration to improve community health. Together, we engaged multiple partners to conduct the assessment, which provides information on health issues, status, and needs and identifies areas for improvement. This report will be used by health care providers, public health officials, policy makers, area organizations, community groups and individuals who are interested in improving the health status of the community. The results of our data analysis, focus groups and surveys enable organizations to strategically establish areas of focus, develop intervention, and commit resources. Altru Health System Altru Health System is a community-owned, integrated system with an acute care hospital, a rehabilitation hospital, more than a dozen clinics in Grand Forks and the region, and large home care and outreach therapy networks. We employ more than 200 physicians and over 4,000 staff. We serve the approximately 220,000 residents of a 17-county region as shown in the map below. The passage of the Affordable Care Act in 2010 requires not-for-profit hospitals to conduct a community health assessment every three years.
