Families that attend the clinics are provided with: • Information, education, and reassurance • Diagnostic evaluation and/or treatment • Coordinated, comprehensive care • Follow-up and monitoring • Access to specialty medical care Please note that some of the clinics listed in this booklet are self-identified multidisciplinary clinics and may not be run in the same manner as those supported or sponsored by SHS. The listing of pediatric specialists in this booklet is obtained from other specialty agencies and does not guarantee acceptance of SHS enrolled clients. 1 Division of Special Health Services Mission The mission of the Division of Special Health Services (SHS) is to promote a system of care and services that improves the health and well-being of individuals with special health care needs and their families. Cross-cutting implementation strategies at the heart of all SHS programs include: • Care coordination • Collaboration • Data-informed decisions • Information and education Programs and Services COORDINATED SERVICES PROGRAM Provides services and resources that support coordinated management of specific chronic health conditions. Services include: • Metabolic Food – Provides formula and low-protein modified food products for individuals with Phenylketonuria (PKU) and Maple Syrup Urine Disease (MSUD). • Multidisciplinary Clinics – Services that provide access to pediatric specialty care and enable families to see many different medical providers and health care professionals in one place at one time. The seven types of clinics available through SHS include: o Asthma o Autism o Cleft Lip/Palate o Developmental Evaluation o Metabolic Disorders o Myelodysplasia o Neurodevelopmental Coordinated Care • Cardiac Care for Children – Provides an initial examination by a participating pediatric cardiologist, routine tests for children with cardiac conditions, and linkage and referral services. • School Nurse Consultation – Supports school nursing initiatives and promotes the connection between health and academic achievement by ensuring nursing care is safely and effectively delivered to schools, particularly in rural communities. 2 FINANCIAL SERVICES PROGRAM Helps families pay for medical services for eligible children, youth, and young adults who require specialty care to diagnose and treat their chronic health conditions. Services include: • Diagnostic Testing and Evaluation – Provides coverage for services that promote early diagnosis of SHS-eligible medical conditions. • Treatment – Provides coverage for services to children with chronic health conditions who meet medical and financial eligibility criteria. • Russell Silver Syndrome – Provides coverage for growth hormone treatment and medical food. NEWBORN SCREENING AND FOLLOW-UP PROGRAM The goal of this program is to identify conditions at an early age when treatment and intervention can prevent health problems, support early development, and save lives. Newborn screening consists of three simple tests: • Blood Spot – Includes laboratory screening tests and follow-up services for nearly 50 genetic/metabolic conditions to identify infants that may be at risk for disorders included in the testing panel. • Hearing – Screening tests using specialized equipment to identify newborn infants who are deaf or hard of hearing so appropriate intervention services can be started as soon as possible. • Critical Congenital Heart Disease – Screening that checks newborns oxygen levels to help identify serious heart defects. CHILDREN WITH SPECIAL HEALTH CARE NEEDS SYSTEM ENHANCEMENT PROGRAM Improves health outcomes of children with special health care needs (CSHCN) by advancing a quality, comprehensive system of care that promotes the healthy development and well-being of children and their families. Six core outcomes that describe the system of services for CSHCN include: • Family Professional Partnership • Medical Home • Adequate Health Insurance • Early and Continuous Screening and Surveillance • Easy to Use Services and Supports • Transition to Adult Health Care 3 Programs for Children with Cardiac Conditions Cardiac Care for Children Program Provides an initial examination by a board-certified pediatric cardiologist and routine tests for children with cardiac conditions. Clinic locations include Bismarck, Fargo, Grand Forks, and Minot. The program ensures comprehensive and coordinated care through linkage and referral to other services and programs, and through care coordination as needed. Any child, birth to twenty-one years of age, who lives in North Dakota, may participate in this program. Third party payers, such as private insurance and Medicaid, are billed when available. The Cardiac Care for Children program covers an initial examination by a board-certified pediatric cardiologist and routine tests to support the cardiac assessment, such as: • Chest X-Ray • Electrocardiogram (EKG) • Echocardiogram (echo) • Select Laboratory Tests (CBC, Liver Function, Lipid Profile, Electrolytes, Iron Testing) If an echo or EKG is completed within 45 days of the initial cardiac office visit, and was ordered by the pediatric cardiologist, this can be billed under the Cardiac Care for Children program as part of the initial visit. If more extensive diagnostic services or treatments are needed, contact SHS to discuss the Financial Coverage program. This two-part program provides further coverage for diagnostic and treatment services. See below for further information. It is preferred that children be screened by their primary care physician (PCP) before being referred. An application through the State Office is necessary. No financial eligibility is required. Third party payers such as, private insurance and Medicaid, are billed when available. Covered tests may include: • Diagnostic Heart Catheterization • Ambulatory Electrocardiogram Monitoring (Holter Monitor) • Stress Test Treatment Program Payment for treatment of cardiac conditions for children is available for families who meet financial eligibility guidelines. An application through the State Office is necessary. Third party payers, such as private insurance and Medicaid, are billed when available. Covered services may include: • Periodic Electronic Monitoring of Cardiac Pacemaker • Surgery Cardiologist Office Visits • Hospitalization • Medication • Laboratory Tests (INR) For more information, call SHS at 701-328-2436 or toll-free at 800-755-2714. You may also visit their website at https://www.health.nd.gov/Youth/special-health-services 6 Cleft Lip and Palate Clinic For many children with cleft lip and palate, evaluation by an experienced team of professionals is necessary for an accurate diagnosis and optimal treatment planning. Families are an essential partner to the team as they are the principle caregiver and the center of strength and support for the child. Cleft Lip and Palate Clinics SHS provides cleft lip and palate clinics in Bismarck, Fargo, Grand Forks, and Minot. Cleft lip and palate clinics supported by SHS provide multidisciplinary team evaluations where families have an opportunity to see many different providers in one place at one time. Reports generated from the clinic visit include recommendations to help direct care for the coming year. All children who might benefit are eligible to attend with no direct cost to the family.     Altru & You with Medica Fact Sheet (Accountable Care System Product) Overview Altru & You with MedicaSM is an accountable care system product offered in 14 counties in northeastern North Dakota and 6 counties in northwestern Minnesota, effective as of July 1, 2016. Altru & You providers deliver care in more than 30 communities plus provide access to several specialties via telemedicine. In Grand Forks and East Grand Forks, care is provided at 13 locations including an acute care hospital, a specialty hospital, inpatient rehabilitation hospital, various clinics, Altru Cancer Center, Altru Advanced Orthopedics, Sanny & Jerry Ryan Center for Prevention & Genetics, and Yorhom Medical Essentials. In addition, local health care providers throughout the region ensure that Medica members can get the care they need close to home. Members may see any provider in the Altru & You with Medica network without a referral.     THE COMMONWEALTH FUND COMMISSION ON A HIGH PERFORMANCE HEALTH SYSTEM THE NORTH DAKOTA EXPERIENCE: ACHIEVING HIGH-PERFORMANCE HEALTH CARE THROUGH RURAL INNOVATION AND COOPERATION Douglas McCarthy, Rachel Nuzum, Stephanie Mika, Jennifer Wrenn, and Mary Wakefield May 2008 ABSTRACT: Resource constraints and the desire to preserve the local economy have made necessity the mother of invention in North Dakota, driving health care providers and policymakers to try new approaches to care and to institute better practices relatively quickly. Collaboration to support primary care and the concept of a medical home, organization of care through cooperative networks of providers, and innovative use of technology to meet patient needs and hold down costs are examples of how North Dakota is able to provide its citizens with accessible, quality, and efficient health care despite the challenges of a rural setting. Rural communities have a unique context of community trust and interdependence, a social capital that allows them to innovate in meeting patients’ needs. A strong sense of mission, vigilance to process and outcomes, and enhanced communication and collaboration among health care providers are key to improvements made in North Dakota health care. Our two organizations have a history of collaboration to improve community health. Together, we engaged multiple partners to conduct the assessment, which provides information on health issues, status, and needs and identifies areas for improvement. This report will be used by health care providers, public health officials, policy makers, area organizations, community groups and individuals who are interested in improving the health status of the community. The results of our data analysis, focus groups and surveys enable organizations to strategically establish areas of focus, develop intervention, and commit resources. Altru Health System Altru Health System is a community-owned, integrated system with an acute care hospital, a rehabilitation hospital, more than a dozen clinics in Grand Forks and the region, and large home care and outreach therapy networks. We employ more than 200 physicians and over 4,000 staff. We serve the approximately 220,000 residents of a 17-county region as shown in the map below. The passage of the Affordable Care Act in 2010 requires not-for-profit hospitals to conduct a community health assessment every three years.
