SCIENTIFIC DISCUSSION This module reflects the initial scientific discussion for the approval of Ammonpas. This scientific discussion has been updated until 1 November 2001. For information on changes after this date please refer to module 8B. 1. Introduction Ammonaps, sodium phenylbutyrate (PB), is a new active substance. It is indicated as adjunctive therapy in the chronic management of urea cycle disorders, involving deficiencies of carbamylphosphate synthetase, ornithine transcarbamylase, or argininosuccinate synthetase. It is indicated in all patients with neonatal-onset presentation (complete enzyme deficiencies, presenting within the first 28 days of life). It is also indicated in patients with late-onset disease (partial enzyme deficiencies, presenting after the first month of life) who have a history of hyperammonaemic encephalopathy. The dossier submitted in support of the application comprises data generated by the applicant: all chemical/pharmaceutical data, the two mutagenicity studies for Part III, and for Part IV, the bioequivalence study and a review of the US IND/NDA programme. Additional information was available from published literature. Urea cycle disorders (UCD) are inherited deficiencies of one of the enzymes involved in the urea cycle, by which ammonium is converted to urea. Ammonium is highly toxic to nerve cells and hyperammonaemia may result in metabolic derangement, leading to anorexia, lethargy, confusion, coma, brain damage, and death. The most severe forms of UCDs occur early in life (complete enzyme deficiencies). The classic neonatal presentation of all the UCD (with the exception of arginase deficiency) is quite uniform and includes, after a short symptom-free interval of one to five days, poor feeding, vomiting, lethargy, muscular hypotonia, hyperventilation, irritability and convulsions. -

The same has also encouraged the evaluation of multiple HDACi candidates in preclinical studies in cancer and other diseases as well as the FDA-approval towards clinical use for specific agents. In this review, we have discussed how the efficacy of immunotherapy can be leveraged by combining it with HDACis. We have also included a brief overview of the classification of HDACis as well as their various roles in physiological and pathophysiological scenarios to target key cellular processes promoting the initiation, establishment, and progression of cancer. Given the critical role of the tumor microenvironment (TME) towards the outcome of anticancer therapies, we have also discussed the effect of HDACis on different components of the TME. We then have gradually progressed into examples of specific pan-HDACis, class I HDACi, and selective HDACis that either have been incorporated into clinical trials or show promising preclinical effects for future consideration. -

Therefore, there is a crucial requirement for discovering an ingenious medicinally active agent that can amend idle drug targets. Increasing pragmatic evidence implies that histone deacetylases (HDACs) are trapped during cancer progression, which increases deacetylation and triggers changes in malignancy. They provide a ground-breaking scaffold and an attainable key for investigating chemical entity pertinent to HDAC biology as a therapeutic target in the drug discovery context. Due to gene expression, an impending requirement to prudently transfer cytotoxicity to cancerous cells, HDAC inhibitors may be developed as anticancer agents. The present review focuses on the basics of HDAC enzymes, their inhibitors, and therapeutic outcomes. Clinical Policy: Sodium phenylbutyrate (Buphenyl) Reference Number: PA.CP.PHAR.208 Effective Date: 01/18 Coding Implications Last Review Date: 07/18 Revision Log Description The intent of the criteria is to ensure that patients follow selection elements established by Pennsylvania Health and Wellness® clinical policy for sodium phenylbutyrate (Buphenyl®). FDA Approved Indication(s) Buphenyl is indicated as adjunctive therapy in the chronic management of patients with urea cycle disorders (UCDs) involving deficiencies of carbamylphosphate synthetase (CPS), ornithine transcarbamylase (OTC), or argininosuccinic acid synthetase (ASS). Limitation(s) of use: Buphenyl should not be used to manage acute hyperammonemia, which is a medical emergency. Policy/Criteria It is the policy of Pennsylvania Health and Wellness that Buphenyl is medically necessary when the following criteria are met: I. Initial Approval Criteria A. Urea Cycle Disorder: CPS, OTC, AS (must meet all): 1. Prescribed by or in consultation with a physician experienced in treating metabolic disorder; 2. Diagnosis of one of the following urea cycle disorders (UCDs) confirmed by enzymatic, biochemical or genetic analysis: a. Carbamylphosphate synthetase (CPS) deficiency; b. Ornithine transcarbamylase (OTC) deficiency; c. Argininosuccinic acid synthetase (AS) deficiency; 3. Prescribed dose does not exceed 20 grams of sodium phenylbutyrate per day.
