The Newcomer Program is for patients in our FHT: • who are newcomers • who have lived in Canada for less than 5 years • who have Ontario Health Insurance Plan/University Health Insurance Plan coverage Our program aims to address your settlement and health care needs. Who we are at the Family Health Team We are a team of family doctors, nurse practitioners, nurses, social workers, pharmacists, dietitians, health promotion coordinator, chiropodist, dietitians, physiotherapist, occupational therapist and health care trainees. Toronto Western Hospital is a teaching hospital. Our staff family doctors are also teachers at the University of Toronto. Resident doctors, senior medical students and other health care students will help with your care while being supervised by fully qualified staff. Program catchment area If you live in our service area, we would be pleased to add you to our roster: west of Yonge Street and east of Dufferin Street, and south of St. Clair West to the lake. 3 This is the region we serve: Services we provide at the Family Health Team We provide high quality primary health care to people who need regular, on-going health care and who do not receive this care elsewhere. We also offer obstetrical care and a chronic disease management program, and consults by a psychiatrist, pediatrician and internist. We can provide an interpreter if you tell us your preferred language before each appointment. We assign a new resident doctor as your primary health care provider every 2 years. 4 The Newcomer Program consists of at least 2 appointments that can be booked on the same day: • 1st appointment includes a detailed intake assessment with a nurse. It lasts 60 minutes and focuses on: • Social determinants of health • Settlement needs • Medical and psychosocial needs specific to newcomers • Referrals to community health resources as needed • 2nd appointment is a medical assessment by a doctor. It lasts 30 minutes and includes: • Review of immunization (vaccines) and communicable diseases • Mental and physical health • Dental and vision health How to use our services It is important that you make an appointment before your visit. Please bring your Ontario Health Card (OHIP) card and a list of the prescriptions and over-the-counter medications you are now taking to each appointment. If your health care provider is not available, you may need to be seen by another provider. Please give us at least 24 hours’ notice if you cannot make your appointment so that your appointment can be given to someone else. Also, tell our receptionist if your phone number or address has changed so that we can contact you when needed. Call 416-603-5888. Your call will be answered by a nurse who can give you advice and connect you with a doctor on call if necessary. Getting you prescription renewed For urgent prescription renewal, your pharmacy may fax a request to 416-603-5821 (Pod 1&2) or 416-603-5448 (Pod 3&4).
