PHYSICIANS AND SURGEONS Lindsay Hedden Introduction to the Health Workforce in Canada | Physicians and Surgeons 1 Physicians and Surgeons INTRODUCTION Focused on improving the health and wellbeing of individuals, families and communities, physicians are perhaps the most readily recognizable health-care practitioners. They diagnose and treat injuries, illnesses and impairment, and also counsel patients on how to maintain and improve their health. Physicians have a long history in Canada, with regulation and licensing first occurring in Ontario in 1865. By the late 19th century, medicine had become the dominant health-care profession in Canada; however, the 1962 Saskatchewan doctor’s strike and the rise (and regulation) of other health professions have since tempered the dominance of the physician profession (Coburn, Torrence & Kaufert, 1983). Physicians are trained in stages, beginning with a generalized medical degree followed by post-graduate training in one or more specialty areas, which become the focus of their practice. Primary care physicians (also known as family medicine physicians) are usually a patient’s first point of contact with the Canadian health-care system. REFUGEE HEALTHCARE IN BRITISH COLUMBIA: HEALTH STATUS AND BARRIERS FOR GORVERNMENT ASSSISED REFUGEES IN ACCESSING HEALTHCARE by SETAREH ROUHANI B.HSc., University of Ottawa, 2009 A THESIS SUBMITTED IN PARTIAL FULFILLMENT OF THE REQUIREMENTS FOR THE DEGREE OF MASTER OF SCIENCE in THE FACULTY OF GRADUATE STUDIES (Health Care and Epidemiology) THE UNIVERSITY OF BRITISH COLUMBIA (Vancouver) October 2011 © Setareh Rouhani, 2011 Abstract Background: Government-Assisted Refugees (GARs) have greater health needs than other immigrants due to their pre-migration and Canadian resettlement experiences. There is a lack of detailed research into their health status and access to healthcare services. This thesis investigated factors associated with reported health, mental health problems, number of annual physician visits and difficulties obtaining healthcare from a sample of GARs. Methods: Secondary data analysis was conducted on data from a study of GARs in BC who attended the Bridge Refugee Clinic during the 26 month period from April 2005 to May 2007. Multivariate logistic regression was used to model the factors associated with excellent health, mental health problems, physician visits and difficulties obtaining healthcare. Results: There were 177 participants in the study. Excellent health was inversely associated with being female, having financial burden, having no English proficiency and having a diagnosed health condition. Factors associated with mental health problems were being female, west Asian, and having financial burden. Attending refugee clinics was inversely associated with reporting mental health problems. Factors associated with physician visits were unemployment, while not having English proficiency and no access to a regular doctor were inversely associated with the number of visits. -

Understanding the complexities of immigrant‟s experiences seeking healthcare is essential to improving their ability to access healthcare. This qualitative study reports on the experiences seeking healthcare for three groups of immigrants. Methods: Seventeen one-on-one interviews were conducted with Spanish-speaking women through an interpreter. Community-based participatory action research was the framework utilized for this study. Results: An individual‟s immigration status emerged as the single most important factor affecting both an individual‟s ability to seek out healthcare and what their experiences are when trying to access healthcare. Conclusion: This study brings to light two issues that are not discussed in great depth in the migrant health service literature. The immigration status of migrants is the largest factor affecting their ability to seek healthcare. Food security is also a very stressful issue for many refugees and undocumented immigrants. ii Acknowledgments My mission for this thesis was to do something tangibly meaningful for marginalized women in Toronto. I genuinely hope that this project will bring about positive change for my participants and women who face similar challenges. This thesis project has been a profound, challenging experience for me. -

The Chaoulli v. Of particular prominence in the often-heated discussion, is the question of public versus private funding of health care services. This paper begins by providing a historical and economic background to the renewed debate over the interface between public and private health care in Canada that has been sparked by the recent decision by the Supreme Court in the Chaoulli v. Quebec case. The court's decision in the Chaoulli v. Quebec case challenged the notion that public funding remain the sole means of payment for medically necessary services. The Supreme Court decision argued that, where the government has failed to provide timely access to necessary healthcare services, disallowing payment through private insurance violates the Quebec Charter of Rights and Freedoms. Among medical professionals, there is no clear position. The membership of the Canadian Medical Association (CMA) has demonstrated a nuanced reaction to the decision, and the CMA stance continues to evolve. The impact of the Chaoulli decision, both on the future of Canadian healthcare and the future of medical education in Canada, remains to be seen.
