Here you will find the latest information about Callen-Lorde, New York City’s only licensed healthcare facility dedicated to meeting the needs of LGBT people and those living with HIV/AIDS. Callen-Lorde welcomes all who seek our services, regardless of any patient’s ability to pay. As you look through these pages, you’ll notice how much we’ve grown over the last year. We are proud to serve more people than ever with our primary care, dental, mental health, gynecological, case management, pharmacy, and health education services. In these difficult times, when more New Yorkers are in need of affordable, quality healthcare, Callen-Lorde is more vital than ever, and we remain committed to providing the world-class model of quality care that our patients have come to expect. Wendy Stark Gal Mayer Executive Director Medical Director Dear Friends, For over 40 years, Callen-Lorde has provided a safe, welcoming haven for literally tens of thousands of LGBT New Yorkers. When this visionary organization first opened its doors, it was a tiny volunteer-driven clinic serving mostly gay men who were experiencing discrimination and hostility in mainstream healthcare settings. Today, Callen-Lorde expands on this tradition and serves as a model for other healthcare providers across the country and around the world. As co-chairs of Callen-Lorde’s Board of Directors, we have helped guide this organization— but we cannot do this without you. Please review this report and get involved with Callen- Lorde as a supporter, as a donor, as a volunteer, or maybe even as a board member. We think you will agree with us that Callen-Lorde is an asset our community can be proud of and cannot do without. Callen-Lorde is New York City’s only community health center dedicated to meeting the healthcare needs of lesbian, gay, bisexual, and transgender (LGBT) people and those living with HIV/AIDS, regardless of any patient’s ability to pay. For over 40 years, we have welcomed all, regardless of sexual orientation or gender identity, who otherwise lack access to respectful, empowering, and informed primary healthcare services. We continuously strive to create an environment that affirms humanity and individuality and inspires people to realize their own health and wellness. Our Mission Callen-Lorde Community Health Center provides sensitive, quality health care and related services primarily to New York's lesbian, gay, bisexual, and transgender communities—in all their diversity— regardless of ability to pay. To further this mission, Callen-Lorde promotes health education and wellness and advocates for gay, lesbian, bisexual, and transgender health issues. Our Licensure Callen-Lorde is licensed by the New York State Department of Health as a Diagnostic and Treatment Center to provide comprehensive outpatient medical services. Callen-Lorde is designated by the federal Health Resources and Services Administration as a Federally Qualified Health Center (FQHC). A Federally Qualified Health Center is a licensed health facility designated to improve the specific health needs of a medically underserved area and/or population though the delivery of comprehensive medical services. 4 Our History Callen-Lorde's grassroots heritage dates back over 40 years to the volunteer-based St. Mark's Community Clinic, founded in 1969 to provide screening and treatment for sexually-transmitted infections. In 1983, St. Mark’s merged with the Gay Men's Health Project to form Community Health Project, a mostly volunteer-staffed, episodic care program housing the nation's first community-based HIV clinic . In 1998, Community Health Project changed its name to Callen-Lorde Community Health Center and moved into a modern, state-of-the-art facility in Manhattan’s Chelsea district. In 2002, Callen-Lorde became a Federally Qualified Health Center, a historic recognition by federal health agencies of the importance of improving health disparities faced by LGBT persons. Over the last decade Callen-Lorde has continued to grow and add new programs, including a dental clinic, mental health services, and an on-site pharmacy—and it remains the New York metropolitan area’s only healthcare facility Michael Callen Audre Lorde (1955-1993) (1934-1992) Michael Callen was born in Rising Sun, The daughter of Caribbean immigrants, Audre Indiana, and began his career as a successful Lorde was born in New York City and wrote composer and singer. Our providers also offer management of chronic health problems ranging from asthma to heart disease, as well as easily accessible appointments for urgent care. 20112011---- 2012 Update: Callen-Lorde received certification as a Level III (the highest level!) Patient Centered Medical Home in 2011. This certification attests to the coordination of care offered by our primary care service and to our inte- gration of electronic health record technology. Callen-Lorde provided over $4.45 million in unreimbursed services in the last year alone, and provided over 15,500 people — 40% of whom had no health insurance—with critical healthcare services. Pharmacy In November 2010, Callen-Lorde opened an onsite pharmacy in the lobby of our facility. At the end of its first year, the pharmacy was already filling over 5,000 prescriptions a month, including significantly discounted medications to people without health insurance. Prescriptions are sent electronically from our medical staff to the pharmacy - ending potential communication errors between prescribers and pharmacists. Health Outreach To Teens (HOTT) Adolescent and young adult medical and mental health program targeting homeless youth, including New York State’s largest youth-focused HIV care program. Services are provided in a dedicated clinical suite on site and through a licensed mobile medical van. All services – including prescription medications - are free of charge or low-cost to young people ages 13-24. HIV/AIDS Care Comprehensive HIV-related primary care services with experienced providers meeting New York State Department of Health standards for HIV specialists, offered in conjunction with Care Coordination services which provide assis- tance to patients in navigating concrete services such as housing, health benefits, and immigration issues in order to remove barriers to care. 20112011----20122012 Update: Callen-Lorde is proud to work in conjunction with Manhattan and South Brooklyn Le- gal Services to offer patients a health-legal partnership addressing legal obstacles ranging from health insur- ance disputes, housing issues to discrimination claims. Legal concerns can present great barriers to access- ing care. By addressing these within our health care setting with our Legal Service partners we are helping our patients stay in care. Women’s Health Gynecological services and comprehensive primary care focused on the unique needs of lesbians and bisexual women, as well as women of transgender experience. In addition to routine care, experienced medical staff pro- vide colposcopy, cryotherapy, and LEEP—procedures designed to prevent cervical cancer. Case management ser- vices are also provided to uninsured patients to obtain entitlements and access follow-up diagnostic and specialty care. Our Women’s Health programs place special emphasis on breast/chest and cervical cancer screening. To assist uninsured patients, we participate with Columbia Cancer Prevention Services, which provides coverage for all screening and diagnostic testing. For patients with abnormal screening tests, our case manager ensures that pa- tients gain access to specialty tests to determine if they have cancer and facilitates their access to treatment. 20112011----20122012 Update: In 2011, Callen-Lorde started a Women’s Community Advisory Board, in which pa- tients and community members interested in offering feedback about our women’s health programming meet monthly with staff to generate ideas and initiate projects.
