Dear Friends,

Another exciting year for ACHIEVE – 2015 – as our agency was accredited by Council on Quality Leadership, an honor bestowed upon us as a result of the quality work our staff do to promote person centered planning and positive outcomes for the individuals we serve. Grounded in over 40 years of leadership and peer-reviewed research, CQL Accreditation promotes excellence in person-centered services and supports that lead to increased qualities of life for the individuals served. This prestigious 3 year accreditation ranks ACHIEVE among the premier providers of services to individuals with intellectual and developmental disabilities in the region. We are thankful, appreciative and proud of the work that was done to support this accreditation at our agency. A job well done!

This year, ACHIEVE was privileged to have had both Laura Kennedy, President, NYSARC Board of Governors, and Steve Kroll, Executive Director, NYSARC, Inc., visit and tour our facilities. Having our state leadership team learn of the specific concerns of our chapter helps us in our advocacy efforts in working with legislators and other state representatives on important initiatives such as work center transformation, increased wages for our direct support professionals, and budget issues that have significant impact on services.

We continue to work in partnership with OPWDD regarding their Transformation Agenda. Recently, ACHIEVE submitted a proposal to OPWDD with regard to a new business model for our employment centers to place us in compliance with new requirements for integration and competitive work opportunities for the individuals we support.

Two quotes on change struck me as being particularly relevant during this period of time working in our system. In the words of Deepak Chopra – “All great changes are preceded by chaos”, and in the words of David Brinkley – “Lay a firm foundation with the bricks that others throw at you”. ACHIEVE will continue to use those bricks to build on the foundation that our founding families created 60+ years ago, and it is our intent to use them to make us stronger and capable of seizing opportunities presented to us to further promote our mission to enrich the lives of those we support.

Community Foundation (August 2015) ACHIEVE received a $1,200 grant from the Women’s Fund of the Community Foundation for South Central New York. These funds supported a healthy relationship curriculum for women in ACHIEVE’s residential program. Joslin Award- Trisha Peters (September 2015) Trisha Peters was announced as the 2015 William B. Joslin Outstanding Performance Award. This award recognizes the personal and professional growth achievements of New Yorkers with disabilities. 2 ACHIEVE find yourself here a success story Daniel “Danny” Hine A Supported Employment Program Success Story Danny entered ACHIEVE’s Supported Employment Program in May of 2012. At that time, Danny did not have a lot of employment experience but had volunteered in the kitchen of a local nursing home and worked in a local hospital as part of his work study experience. Daniel enjoyed the work. After graduation, Danny set a goal of working in an integrated setting in the community. He succeeded.

In September of 2012 Danny began his first job working in a local restaurant not far from home. With the support of his father and the use of BC Transit, Danny was on time for his first day of work. His manager happily reported that Danny told her, “I need my apron and hat,” a good indication that Danny was ready to go to work. While Danny had good training and support from his job coach, he was good at following directions and demonstrated the desire to be successful. Early on in this job, Danny’s coworkers, managers, and job coaches noted one important character trait; they admired the fact that Danny likes to stay busy and find other chores to do when his work is caught up. Six months into his new job, Danny experienced a significant personal loss, the passing of his mother. A very supportive group of coworkers and managers provided natural supports during this difficult time. Danny thanked them and announced that he “needed to return to work.” Before Danny could celebrate his first year anniversary at the restaurant, a fire had closed the restaurant and they determined that it would not reopen. Danny returned to job development and said he wanted a job “real soon.” Within two months, Danny interviewed at Cracker Barrel and was hired. He trained on all five stations in the dish room and supported the kitchen staff by restocking clean dishes and cookware in the fast paced work environment. His manager stated that all of Danny’s coworkers enjoy working with him. Danny successfully completed his training during his first year and earned raises from the company. Danny has maintained his job at Cracker Barrel for two and a half years. There are many reasons for Danny’s success; he has a great work ethic, he is willing to work extra shifts to help out, he is focused on his tasks, and finds other work to do when he gets the dish room caught up. Another important reason for Danny’s success is the great natural supports he has at work, from the top level of management, to the wait staff, to the kitchen staff, and his coworkers in the dish room, people like working with Danny. He brings his positive spirit and caring attitude to the people he works with. Danny Hine is the “Spirit of Success.”

ANNUAL REPORT 2015 3 a success story John Spisak A NYSEG Investment Recovery Center through Country Valley Industries Story In May of 1983, following his graduation from BOCES, John Spisak began working at CVI at the Cutler Pond site (then known as the main CVI workshop on Front St.). In June of 1990 an enclave was initiated at NYSEG’s Investment Recovery Center through CVI; John was chosen as one of 16 individuals, along with three support staff to be employed there. Their main tasks were salvaging line hardware, street lights and sorting scrap steel. Using a vise, John learned how to operate a wide variety of tools to take apart those materials, some of which were quite challenging and complicated. Due to his good counting skills, John was also frequently considered for assisting the area manager with inventories. Gradually the need for our individuals labor at NYSEG decreased and eventually sometime during the mid 90’s, only 4 of the original workers remained. Very soon after, three more of the crew members left and moved out of the area, leaving John as the only original one. Twenty six years later, John still remains working at NYSEG with three others, recruited ‘96-’98. John is a very valuable employee and is familiar with each and every task performed at the site, stepping in when needed in many different capacities. He is very well known throughout the NYSEG plant due to a very exuberant personality and is often called upon to help their employees with a variety of tasks whenever, or wherever needed. Due to John’s experience and expertise at the jobs performed at the NYSEG/IRC, he was also able to help his co-workers learn the different components of the operations at that site by working alongside him. John’s attendance at work is also admirable; he is rarely ill and generally only takes time off for his vacations in Florida with his sisters and family. John is one of the many reasons CVI has successfully participated in the Investment Recovery Program for so many years.

4 ACHIEVE find yourself here a success story Hammond Lou A Day Habilitation Program Success Story Hammond entered ACHIEVE’s Day Habilitation Program early in 2015. Hammond is a happy person who loves Legos, food related activities, and spending time with his mother. Hammond had resided and attended program at Broome Developmental Center (BDC) for many years. With the closure of the center, Hammond and his family agreed that a transition to an ACHIEVE IRA and the ACHIEVE group Day Habilitation Program would offer supports and services that would enhance Hammond’s life. ACHIEVE Residential Staff helped Hammond with the transition from his new home to the Day Habilitation Program. Routine and structure are very important to Hammond, so the many transitions in his life also offered him some daily challenges. When Hammond first arrived to program, he wasn’t even sure he wanted to get out of the van – and that was okay! As his core room staff noted, “Hammond had made it to the building and that in itself was a valuable victory.” Slowly, Hammond became comfortable with getting out of the van, walking into the building and then walking directly into his Core Room. Although then he would immediately say he wanted to go back to his new home – and away he would go. Hammond’s first documented time at the Day Habilitation program was two minutes. He arrived at 10:00am and left at 10:02am. It was recommended by his family and his team that perhaps a step by step schedule would help. So, Hammond started by just working on his Legos, taking a walk, and then going back home. With lots of patience and hard work, Hammond began attending program every Tuesday and Thursday from 10 am to noon. Two minutes to two hours was a huge accomplishment for Hammond. Slowly, new meaningful activities were incorporated in Hammond’s day and eventually the time he spent at program was increasing every two weeks by 15 minutes. Today, Hammond is attending program Monday through Thursday for full days. Hammond independently chooses to participate in the many different activities offered at the Day Habilitation program and he has taken it upon himself to keep the core room silverware stocked at all times. He still checks out the transportation area throughout the day, just to make sure he hasn’t missed his ride home. But most of the time Hammond can be seen laughing, walking around, spending time with his friends, and giving the occasional hug throughout the day. While at first Hammond needed some encouragement and support from program staff, his recent commitment to the Day Habilitation program has been unwavering. His Core Room staff shared that “Hammond is a reminder of just how strong and astonishing people really are!” Hammond’s strength, growth, and determination are evident everyday as he inquires about new projects and asks to participate in new activities. Hammond has adapted very well to the many changes in his life, and that is something of which he can be very proud. — Linda Riggs, Day Habilitation Director

6 ACHIEVE find yourself here Residential Services Community Residences, Supervised Apartments, and Supportive Apartments Each of ACHIEVE’s 14 residential facilities are regulated by the New York State Office for People with Developmental Disabilities (OPWDD). They are designed to support individuals with intellectual and other developmental disabilities in their community with access to recreational and work opportunities. Admission requires a developmental disability diagnosis before the age of 22, with the individual requiring a supervision level other than one-on-one support. n 116 individuals served in 2013 n 117 individuals served in 2014 n 121 individuals served in 2015 Day Habilitation Services Day Habilitation Program The Day Habilitation Program provides a comprehensive spectrum of intensive rehabilitative and support services to assist individuals with moderate to severe levels of disability with developing and maintaining skills for community integration. Services include: teaching activities of daily living, cognitive skill development, recreation, communication development, socialization opportunities, volunteer activities, speech education services, and comprehensive nursing services. 8 ACHIEVE find yourself here Children’s Summer Program Services Children with intellectual and other developmental disabilities benefit from summer programs that help them maintain the skills they developed during the school year. The Children’s Summer Program exposes them to drama, music, physical activity, arts and crafts, and field trips that reinforce their academic skills, gross and fine motor coordination, and social interaction. The goal of the program is to broaden the life experiences of children with disabilities and provide them with basic learning concepts that will enhance skill levels. Students are grouped by age and led by a team leader and counselors who design activities to meet each group’s needs. 100 individuals served in 2013 101 individuals served in 2014 100 individuals served in 2015 Medicaid Service Coordination Medicaid Service Coordination (MSC) is a program that provides ongoing support to individuals with developmental disabilities and their families. Medicaid Service Coordinators assist people with obtaining available services in the community as well as services under the Home and Community Based (HCBS) Waiver. ACHIEVE’s Medicaid Service Coordination program serves adults and children of all ages both in Broome and Tioga counties. Service Coordinators work together with families to develop and maintain a personalized plan designed to meet their loved one’s dreams and desires regardless of complexity, by providing; advocacy, supports and referrals to necessary services. 301 individuals served in 2013 341 individuals served in 2014 339 individuals served in 2015 Guardianship Program The Guardianship Program is a service for individuals who require assistance in making decisions or managing their affairs when other family members are not able or willing to do so. Through NYSARC, Inc.’s Corporate Guardianship Program, ACHIEVE may serve as guardian, stand-by guardian, or alternate stand- by guardian to ensure that an individual has assistance with making critical decisions in areas related to healthcare, housing, day programs, or recreation. The Guardianship Program also provides information, referrals, and technical assistance to families who are interested in establishing legal guardianship.
