This fact sheet provides information to patients with eczema and their carers. About topical corticosteroids You or your child’s doctor has prescribed a topical corticosteroid for the treatment of eczema. For treating eczema, corticosteroids are usually prepared in a cream or ointment and are applied topically (directly onto the skin). Topical corticosteroids work by reducing inflammation and helping to control an over-reactive response of the immune system at the site of eczema. They also tighten blood vessels, making less blood flow to the surface of the skin. Together, these effects help to manage the symptoms of eczema. There is a range of steroids that can be used to treat eczema, each with different strengths (potencies). On the next page, the potencies of some common steroids are shown, as well as the concentration that they are usually used in cream or ointment preparations. Using a moisturiser along with a steroid cream does not reduce the effect of the steroid. There are many misconceptions about the side effects of topical corticosteroids. However these treatments are very safe and patients are encouraged to follow the treatment regimen as advised by their doctor. How to apply topical corticosteroids How often should I apply? Prednisolone Versus Dexamethasone for Croup: a Randomized Controlled Trial Colin M. Parker, MBChB, DCH, MRCPCH, FACEM,a,b Matthew N. Cooper, BCA, BSc, PhDc OBJECTIVES: The use of either prednisolone or low-dose dexamethasone in the treatment of abstract childhood croup lacks a rigorous evidence base despite widespread use. In this study, we compare dexamethasone at 0.6 mg/kg with both low-dose dexamethasone at 0.15 mg/kg and prednisolone at 1 mg/kg. METHODS: Prospective, double-blind, noninferiority randomized controlled trial based in 1 tertiary pediatric emergency department and 1 urban district emergency department in Perth, Western Australia. Inclusions were age .6 months, maximum weight 20 kg, contactable by telephone, and English-speaking caregivers. Exclusion criteria were known prednisolone or dexamethasone allergy, immunosuppressive disease or treatment, steroid therapy or enrollment in the study within the previous 14 days, and a high clinical suspicion of an alternative diagnosis. A total of 1252 participants were enrolled and randomly assigned to receive dexamethasone (0.6 mg/kg; n = 410), low-dose dexamethasone (0.15 mg/kg; n = 410), or prednisolone (1 mg/kg; n = 411). Primary outcome measures included Westley Croup Score 1-hour after treatment and unscheduled medical re-attendance during the 7 days after treatment. RESULTS: Mean Westley Croup Score at baseline was 1.4 for dexamethasone, 1.5 for low-dose dexamethasone, and 1.5 for prednisolone. Adjusted difference in scores at 1 hour, compared with dexamethasone, was 0.03 (95% confidence interval 20.09 to 0.15) for low-dose dexamethasone and 0.05 (95% confidence interval 20.07 to 0.17) for prednisolone. -

ACTION AND CLINICAL PHARMACOLOGY CLOMID (clomiphene citrate) is an orally-administered, non-steroidal agent which may induce ovulation in anovulatory women in appropriately selected cases.1-16 The ovulatory response to cyclic CLOMID therapy appears to be mediated through increased output of pituitary gonadotropins, which in turn stimulate the maturation and endocrine activity of the ovarian follicle and the subsequent development and function of the corpus luteum. The role of the pituitary is indicated by increased plasma levels of gonadotropins and by the response of the ovary, as manifested by increased plasma level of estradiol. Antagonism of competitive inhibition of endogenous estrogen may play a role in the action of CLOMID on the hypothalamus. CLOMID is a drug of considerable pharmacologic potency. Its administration should be preceded by careful evaluation and selection of the patient, and must be accompanied by close attention to the timing of the dose. With conservative selection and management of the patient, CLOMID has been demonstrated to be a useful therapy for the anovulatory patient. Based on studies with 14C-labeled clomiphene, the drug is readily absorbed orally in humans, and is excreted principally in the feces. Cumulative excretion of the 14C-label averaged 51% of the oral dose after 5 days in 6 subjects, with mean urinary excretion of 8% and mean fecal excretion of 42%; less than 1% per day was excreted in fecal and urine samples collected from 31 to 53 days after 14C-labelled clomiphene administration. -

Flonase® Sensimist™ Allergy Relief (fluticasone furoate) – Rx-to-OTC Approval • On February 8, 2017, GlaxoSmithKline Consumer Healthcare announced the launch of Flonase Sensimist Allergy Relief (fluticasone furoate) nasal spray, as an over the counter (OTC) treatment to temporarily relieve symptoms of hay fever or other upper respiratory allergies: nasal congestion; runny nose; sneezing; itchy nose; and itchy, watery eyes (in ages 12 years and older). — Flonase Sensimist Allergy Relief contains 27.5 mcg/spray of fluticasone furoate. — Flonase Sensimist Allergy Relief should not be used in children less than 2 years of age. • Previously, fluticasone furoate was only available by prescription as Veramyst®. Veramyst is no longer commercially available. • Fluticasone is also available OTC as brand (Flonase® Allergy Relief, Children’s Flonase® Allergy Relief) and generic products containing 50 mcg/spray of fluticasone propionate. — These products share the same indications as Flonase Sensimist Allergy Relief, but are not intended for children under 4 years of age. • Prescription fluticasone propionate nasal spray (50 mcg/spray) is available generically and indicated for the management of the nasal symptoms of perennial non-allergic rhinitis in adult and pediatric patients aged 4 years and older. • Warnings for Flonase Sensimist Allergy Relief state the following: — Do not use: in children under 2 years of age, to treat asthma, if there is an injury or surgery to the nose that is not fully healed, or if an allergic reaction to Flonase Sensimist Allergy Relief or any of its ingredients has occurred. — Ask a doctor prior to use if the patient has or had glaucoma or cataracts.
