Preface Gram Seva Trust has been dedicated to providing health care and development in and around rural area of Kharel in South since its inception in 1994. Although the entry point was to provide quality health care at an affordable cost to the rural poor of the area, it was imperative to get involved with other concerns of the community well-being like malnutrition, plight of women and children, poverty and illiteracy. The inter-linkages between poverty and disease, between illiteracy and backwardness are well known and can not be ignored. It is meaningless to talk of good health care without ameliorating the adverse conditions that these people live with. While tackling malnutrition amongst pre-school children was our major initial project in the community, subsequent years saw other projects targeted towards community development focusing on mother and child health, women’s economic activities, adolescent girls and boys’ health, de-addictions and education. Projects like SNGO, Child Nutrition etc. have given us the opportunity to co-ordinate and work with the Government setup. They have also helped in developing co-ordination and understanding not only between the hospital care and the community but also between the two arms of staff, those working in the hospital and those working actively in the field. Women have emerged in the forefront of all community related projects; be it income generating schemes, health education or formal school education. GST has encouraged women in taking a leadership role. Toilet project this year was on demand by the women from Self Help Groups and has been a new learning experience for us. Mahatma Gandhi emphasized village development as central to national progress and prosperity. In the current times, Mohamed Yunus of Bangladesh revolutionized women empowerment in developing nations through Grameen Bank concept of micro-saving and micro-credit schemes. We too believe that women can lift the entire community to a different level, with improved standard of living, through education and financial emancipation and have approached rural development by similar interventions. We always believed in transparency and social accountability in voluntary sector. This year, the organisation is accredited by Credibility Alliance (C.A.) for minimum norms. This gives more credibility to the organisation and makes us proud. All through its inception we have received tremendous love and trust of our many friends from home and abroad; whatever we have achieved so far is largely due to your unstinting support and generous donations. There is a high incidence Case Study Pravinaben Mistry lived with her 4 year old daughter at the village of Kundi taluka Palsana district Surat. One night she was bitten by a snake while she was asleep. An alarmed Pravina woke her neighbours up who killed the snake. When Pravina started developing symptoms of snake poisoning she was shifted to a nearby hospital where she was put on antisnake venom and other medications. Yet her health deteriorated and she was referred to a higher centre. The scared relatives took her to a traditional healer instead. Precious time was wasted in the mumbo jumbo and Pravina’s condition worsened. One of the relatives contacted Prahari Charitable Trust that works for the conservation of nature. They advised the relatives to shift her to Kharel. Thus Pravina was brought to Gram Seva Trust hospital at 9 o’clock at night in a very serious condition. Twenty four hours of intensive care were required to revive her. After lot of efforts she got a new life and her relatives got their loved one back. Though, the story does not end here. We are forced to think, when will our society be rid of superstitions and mis-beliefs? Sonography is done every Tuesday afternoon to monitor the progress of pregnancy and timely detection of any complications in mother or baby. These women are also followed up in the village

Case Study Ritaben (name changed), 25 years of age delivered a full term stillborn at one of the Government hospital. She developed severe breathlessness after delivery and was immediately transferred to Kharel hospital. She had very fast heart rate and could not lie down because of difficulty in breathing. Her chest was full of water in the lungs (pulmonary edema) and as a result her oxygen saturation had dropped to 60%. She was diagnosed to have rheumatic heart disease (mitral stenosis) with acute left heart failure. Ritaben was put on artificial ventilatory support and given necessary medical treatment. Fortunately she responded to the timely treatment and was saved. She has been advised further investigations and treatment for heart valve disease before another pregnancy puts her life at risk again.

Case Study Manjuben (name changed), 20 years old woman was brought to Kharel hospital at night around 10 p.m. The relatives accompanying her gave history of sudden onset of cough and breathlessness at home after taking some cough syrup. Manjuben was nearly full term and had been coming regularly for antenatal check ups. On clinical examination, she was having very fast heart rate, was breathless and there was frothing from nose and mouth. Further examination of her eyes showed pin point pupils suggestive of pesticide poisoning. Her oxygen saturation was very low. Even as the treatment was instituted, she suffered a cardiac arrest (no heart beat). She was resuscitated with cardiac massage and kept on ventilator. She gradually responded to treatment and could be weaned off life support measures. Though Manjuben herself could be saved by inten- sive treatment, her critical condition resulted in loss of full tern baby. because of reduced resistance to infections. Case Study Sita Rajesh Halpati (Name changed), 20 years of age, was expecting her second child. Rajesh, her husband, was unemployed and an alcoholic. He would often fight with Sita and often beat her in an inebriated state accusing her of infidelity. Sita was fed up and came away to stay at her mother’s place where she delivered a baby boy. The baby was diagnosed to have a swelling in the kidney (Hydronephrosis) and was advised surgery at specialty hospital in . Her widowed mother was too poor to help the daughter. Rajesh on the other hand suspected that the baby was not his and took no responsibility. The GST health workers brought the matter to GST and family members were counselled to accompany her and the baby. The child was almost two and half years old by this time and disease was fairly advanced. GST decided to support Sita by paying for surgery of her child at near by Surat rather than send her for free surgery at Ahmedabad. Her child, Srujesh, was operated at Surat hospital and is doing well. Sita’s story high lights the utterly hopeless complexity of poverty, alcoholism, ignorance, difficulties in access- ing free healthcare facilities, and deteriorating family support systems.

Home visits by health workers, supervisors, interns and doctors to give advice on weaning and nutritional advice. These visits are also used to reinforce personal and social hygiene, good health practices and help in any other manner as required. Bal-Awas Bal-Awas, a shelter home for deprived children was started on 31st July 2008 in rented premises. We have audited the annexed Balance sheet of GRAM SEVA TRUST [(Reg.No E/896/ Navsari) situated at : N. H. No. 8, At & Post: Kharel, Dist: Navsari 396430] as on 31st March 2010 as well as Income and Expenditure Account for the year ended on 31st March 2010. These financial state- ments are the responsibility of the Trust. Our responsibility is to express an opinion on these financial statements based on our audit.

We have conducted our audit in accordance with auditing standards generally accepted in India. These standards require that we plan and perform the audit to obtain reasonable assurance about whether the financial statements are free of material misstatement. An audit includes examining on a test basis, evidence supporting the amounts and disclosures in the financial statements. An audit also includes assessing the accounting principles used and significant estimates made by Trust, as well as evaluating the overall financial statement presentation. We believe that our audit provides a reasonable basis for our opinion. Subject to the above and, in respect of schedules forming part of Balance Sheet and Income & Expenditure Account, we report that: 1. The accounts are maintained regularly and in accordance with the provisions of the Act and Rules 2. The receipts and disbursements are properly and correctly shown in the accounts 3. The cash balance and vouchers in the custody of the treasurer or trustee on the date of the audit were in agreement with the books of account 4. All books, deeds, accounts, vouchers or other documents & records required by us were pro- duced before us. 5. A Register of movable and immovable properties has been maintained and the changes therein have been furnished to us. 6. All the necessary information required by us has been furnished to us by the Trustees personally whenever called upon 7. No property or funds of the Trust were applied for any object or purpose other than for the object or purpose of the Trust 8. There are no amounts which are outstanding for more then one year and written off 9. For repairs, tenders are not invited but for construction involving expenditure exceeding Rs.5000/-, tenders are invited
