Respected Elders, dear friends and well wishers,

Greetings for the New Year.

Gram Seva Trust has completed 11 years in its efforts towards promoting health and development in the rural community with active co- operation from all of you. We have tried to strengthen the programs carried out during first decade with renewed vigor and enthusiasm through the second decade of the organization.

Complete transparency in working of the organization has been our guiding principle.

It has also been our effort to critically appraise and analyze the achievements each year and identify new areas of challenges so as to bring about the necessary changes where necessary. Our people are still living with age-old beliefs and superstitions. This requires building of a scientific temper and a modern outlook on life amongst the communities. It needs sometimes only but minor adjustments in attitude and thought-process of a few to bring about substantial and positive changes in raising the social and economic standard of living of the entire community.

There was all around havoc in Gandevi Taluka following heavy rains during last monsoon. The flood waters of swollen Ambica river caused lot of destruction in surrounding villages. All possible help was rendered with co- operation from everyone.

Timely help... Shivlal, a resident of Dharampur, made his living by leasing a pond every year for cultivating 'Singhoda' (Chestnuts). He used to live in a small hut by the side of the pond. One night during the month of June when he was fast asleep after the day's hard work, he felt a sharp sting in his foot. Thinking this to be a mosquito bite, he changed side and went back to sleep. When he woke up, he was feeling giddy with black outs and blurring of vision. He was alone but got help from an old man living nearby. This 70-year- old man thought this to be a snakebite and promptly brought him to Kharel Hospital. Shivlal was diagnosed to have systemic effects of snake bite and was administered Anti Snake Venom along with other treatment. Shivlal was cured in two three days and a life was saved. Shivlal was overwhelmed while leaving the hospital…What if he had not been brought to the hospital by the kind elderly man!

Suresh lived in Jervavra and was on his way to Surat. He suffered a snake bite one night while on his way to another village. He reached his uncle's house who took him to a 'Bhagat', the village quack who used to treat snakebite victims. This 'Bhagat' performed some rituals for half an hour or so and sent him home. Suresh's condition worsened at night, as he started vomiting and the vision got blurred. The village folk arranged for a vehicle and brought him to Kharel Hospital. Suresh received timely help and treatment in the hospital and was soon on his way to recovery due to the presence of mind and thoughtful help of the community.

Blind Faith.... A vast majority of people in remote tribal areas belong to poor social and economic status. They have had years of deprivation and the resulting poverty and illiteracy are often compounded with misbeliefs and superstitions. The stories of Coomi and Bhanu, two young women from similar backgrounds illustrate this phenomenon. Coomi belonged to an extremely backward and deprived village of Khaparia barely 2 kms from Kharel. She got married in a nearby village. She did not conceive for 9 long years after marriage. During this time she did not seek any medical help but instead she was taken to the village quack - "Bhagat" for treatment. So when she became pregnant, the family gave credit to the village quack. The latter advised her not to eat eggs, fish or any other non-vegetarian food & other nutritive food like pulses for the well being of the baby. Coomi, as such undernourished, became severely anemic. She never went for antenatal checkup, and therefore, did not receive necessary shots of Tetanus Toxoid or Iron-Folic acid supplements. As per the village custom of first delivery at parental home, she came to Khaparia when she was 7 ½ months pregnant. The Primary Health Centre (PHC) nurse found her to be very weak during her routine home visit and advised her to attend the free antenatal clinic. The Bhagat, however, advised her against any medicines. The PHC doctor and the community members persuaded her family to take her to the PHC. Her hemoglobin was only 2.8 Gms% and two units of blood were transfused. The nurse also informed Gram Seva Trust workers about her. GST workers made home visits to emphasize the need for proper antenatal checkup and hospital delivery. Coomi finally went to the hospital after several attempts of GST Health Workers, promising financial assistance. She was given four more units of blood. She and her family, however, continued with their blind faith in the village doctor and did not come for further check up. There was no adult help at home when she went into labor. GST workers came to know about the incidence and rushed to her house along with a doctor. They were shocked to see her lying on the floor covered with dirty blankets while the baby too was wrapped in dirty linen. The health workers got the mother and baby to the hospital. Baby weighed just 1.3 Kg and died next morning in the NICU. Coomi herself had lost lot of blood and her Hb was down to 3.6 Gms % needing further transfusions. She made slow recovery but had lost precious baby conceived after 9 years of marriage.

Bhanu, a young woman of 20 years, was married in a far off village. She also came to her parental home for first delivery. She was brought to Kharel hospital as an emergency with very high blood pressure and convulsions. This was a classic case of eclampsia. Doctors could save the mother and the baby with great difficulty. Child unfortunately died of respiratory infection at the age of two months. When she conceived the second time, she came for regular check-ups in the antenatal clinic and followed the medical advice since she had pre-eclampsia this time also. She was advised hospital delivery. Yet when she went into labor on a Saturday night, her grand mother insisted on home delivery by an untrained Dai (Birth attendant). Bhanu and her husband begged the grand mother that she be taken to the hospital but in vain. Next morning while Bhanu was still writhing in pain, her husband with the help of neighbors hired an ambulance but the grandmother continued with her tirade of abuse and chased away the ambulance driver. Finally the Dai also gave up and asked them to remove her to the hospital immediately. Grandmother was still unrelenting. The community members insisted on bringing her to the hospital where she underwent an emergency caesarian section. The long delay had taken its toll. The baby had died, and the mother had ruptured uterus with profuse bleeding necessitating a hysterectomy. While Bhanu's life could be saved, she had no living child and had lost the chance of having one in future !

Some Happy Endings… One day, an elderly tribal woman traveled almost 10 Kms and brought an 11-year-old child Jaydeep to Kharel Hospital. The boy studying in 6th standard had been running high fever for last 10 days with loss of appetite. While his father had died some time back, the mother had got remarried and his younger sister was living with the mother. The elderly lady was Jaydeep's grandmother who had brought him up with meager earnings as a daily laborer. Jaydeep was admitted in the hospital for necessary diagnostics and treatment. He was diagnosed to have T.B Meningitis that responded well to treatment. He was discharged from the hospital with advice for regular treatment and follow-up. Jaydeep's grandmother kept all the appointments despite long distance and completed the full course of regular treatment resulting in complete cure. Jaydeep's grandmother persevered in taking care of the boy even when his own mother had abandoned him. Despite her age and economic difficulties she saw him through serious illness requiring prolonged treatment. This also shows the immense love and wisdom in an otherwise illiterate Adivasi (tribal) woman……

Auditors' Report (Under Sub Section (2) of Section 33 and 34 and Rule 19 of the Bombay Public Trust Act 1950) We have audited the annexed Balance sheet of GRAM SEVA TRUST (Reg.No E/896/ Navsari)situated At & Post: Kharel, Dist: Navsari (396 430) as on 31st March 2006 as well as Income and Expenditure Account for the year ending on 31st March 2006. These financial statements are the responsibility of the Trust. Our responsibility is to express an opinion on these financial statements based on our audit. We have conducted our audit in accordance with auditing standards generally accepted in India. These standards require that we plan and perform the audit to obtain reasonable assurance about whether the financial statements are free of material misstatement. An audit includes examining on a test basis, evidence supporting the amounts and disclosures in the financial statements. An audit also includes assessing the accounting principles used and significant estimates made by Trust, as well as evaluating the overall financial statement presentation. We believe that our audit provides a reasonable basis for our opinion. Subject to the above and, in respect of schedules forming part of Balance Sheet and Income & Expenditure Account, we report that : 1) The accounts are maintained regularly and in accordance with the provisions of the Act and Rules 2) The receipts and disbursements are properly and correctly shown in the accounts 3) The cash balance and vouchers in the custody of the treasurer or trustee on the date of the audit were in agreement with the books of account 4) All books, deeds, accounts, vouchers or other documents & records required by us were produced before us. 5) A Register of movable and immovable properties has been maintained and the changes therein have been furnished to us. 6) All the necessary information required by us has been furnished to us by the Treasurer personally whenever called upon 7) No property or funds of the Trust were applied for any object or purpose other than for the object or purpose of the Trust 8) There are no amounts which are outstanding for more then one year and written off 9) For repairs, tenders are not invited but for construction involving expenditure exceeding Rs.5000/-, tenders are invited 10) The money of the trust have not been invested contrary to the provisions of section 35 of the Act. 11) There is no alienation of the immovable property contrary to the provisions of Section 36 12) There are no case of irregular, illegal or improper expenditure or failure or omission to recovery monies or other property belonging to the public trust or of loss or waste of money or other property thereof 13) The provision of Budget to be filed under Rule in the form under Rule 16A is not applicable to the Trust. 14) The maximum and minimum number of trustees is maintained during the year. 15) The meeting are held as provided by such instrument. 16) The Minutes Book of proceedings of the meetings are maintained. 17) None of the trustees has any interest in the investment of the trust. 18) None of the trustees is any debtor or creditor of the trust. 19) The irregularities pointed out by Auditors in the accounts for the year under audit are have been duly complies with by the trustees. 20) There is no special matter, which we think fit or necessary to bring to the notice of the Deputy or Assistant Charity Commissioner
