WAITAHA HAUORA Waitaha Hauora is a smokefree organisation and is committed to a smokefree Aotearoa POSITION DESCRIPTION Position: Tamariki Ora Kaimahi (Community Health Worker) Responsible to: Tamariki Ora Nurse Team Leader Early Years Services Manager Hours of Work: Permanent 32 hours per week 0.8 FTE Based at: 11 Garden Place Tauranga And Hauora Te Puke Service area Te Puke Purpose of Position: To identify tamariki (0-5yrs of age) within 2 – 3 weeks of age who are at risk of not accessing Tamariki Ora and Immunization services, in association with their whānau. This will be achieved through: 1. Working collaboratively with TMT’s Tamariki Ora Nurses, Social Workers, Health Promoter and relevant staff from Te Manu Toroa’s GP Clinics 2. Advocating on behalf of tamariki and whānau identifying and clarifying their needs with health professionals 3. Developing effective links and networks with relevant health providers, govt. agencies, groups and organizations, who deliver services to tamariki and their whānau in conjunction with the Tamariki Ora Nurses Supervisor: Peer Supervision Group and an external supervisor Training and Development: Te Manu Toroa shall engage in all aspects of developmental training with each staff member. Cultural training includes Waitaha Tikanga Conditions of Employment: Individual Employment Contract or Collective Employment Contract Tribal Territory: Mai Nga Kuri a Wharei ki te rohe o Waitaha-a-Hei Geographical Boundary: Rural, central and urban Tauranga, Papamoa, Mount Maunganui, Arataki, Matapihi, Welcome Bay and Te Puke Page 1 of 7 Updated July 2015 PRIMARY OBJECTIVE To progressively strive towards improving Maori health gains for tamariki and their whānau by providing care and support to improve their access to Tamariki Ora / WCC and Immunization Services by means of facilitation, assessing their needs and advocating for access to Primary Health Care Services in conjunction with Tamariki Ora Nurses SERVICE OBJECTIVES The Tamariki Ora Kaimahi will: focus on improving the health status of predominately Maori tamariki , by providing Kaupapa Primary Health Services as outlined in the Job Description through: . Health promotion and support of other Kaimahi with education i.e. safety in the home, car seat safety, water safety etc . Referrals to Tamariki Ora and Nurse, Social Worker and Oral Health Teams . Enrolling tamariki with GP Clinic Services . Promoting and encouraging immunizations . Assisting with transportation of tamariki to clinics, dentist, specialist appointments etc where there is no other alternative Ensure all processes consider and meet the needs of Maori Ensure all tamariki and their whānau who utilize Tamarki Ora Services are informed of the Code of Health and Disability Services Consumers’ Rights Ensure that processes are culturally appropriate and are delivered in a culturally sensitive manner Work as a part of the Te Manu Toroa Tamariki Ora team including the Early Years Services Manager, Tamariki Ora Nurse Team Leader, Tamariki Ora Nurses and Te Manu Toroa Social workers, and work collaboratively with TMT staff engaged in the care of tamariki. This will also include other community workers and organizations providing health services to tamariki and their whanau SERVICE USERS The target users of this Service are tamariki (0-5 years) and requires working co-operatively with their whanau The priority group will be predominantly Maori.
