Maybe they have, but then the original Titanic wasn’t sunk by an iceberg. An iceberg didn’t ram the Titanic; the Titanic rammed an iceberg. Environmental scanning systems are like the spotters in the Titanic’s crow’s nest; they can only warn about the type of attack that is expected: icebergs; they cannot warn about the real but unexpected threat: excessive speed given the turning radius of such a huge ship.
