RESUMEN

Objetivo: Sintetizar la evidencia primaria con respecto a la eficacia y seguridad de los estilbenos en el tratamiento de la sintomatología climatérica en las mujeres de edad mediana.

Métodos: Llevamos a cabo una revisión sistemática de la literatura según las recomendaciones del Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Realizamos la búsqueda primaria en Pubmed-Medline, Embase, Scopus, Web of Science, WOS-Medline, LILACS, CINAHL, PsycINFO, SciELO, CENTRAL y en registros internacionales de ensayos clínicos, sobre la eficacia y seguridad de los estilbenos en sintomatología climatérica. Consultamos fuentes de literatura gris, hasta diciembre del 2018, sin restricción de idioma. Usamos la herramienta de la Colaboración Cochrane para la evaluación de riesgo de sesgo de los estudios incluidos.

Resultados: Obtuvimos 2752 registros en la búsqueda primaria, a partir de los cuales seleccionamos cuatro ensayos clínicos controlados aleatorizados (ECAs) que reportaron desenlaces de eficacia y seguridad. Existe alta heterogeneidad entre los estudios en cuanto a la dosis, combinaciones con otros fitoestrógenos, duración de la suplementación, escalas de medición y reporte de desenlaces. En cuanto a la eficacia, un estudio reportó disminución significativa en todos los síntomas somáticos, en el grupo con resveratrol, con mayor reducción en el ítem de molestias musculares y articulares, donde solo el 16.7% de la población reportó estas molestias al final de la intervención, comparado a un 73.3% del brazo placebo (p<0.001). Por otro lado, un segundo estudio demostró mejoría significativa en el número (p=0.0077) e intensidad (p=0.018) de los episodios vasomotores en el grupo de la intervención. En contraparte, un tercer estudio mostró diferencias no significativas en el puntaje total de la subescala somática (7.7 ± 2.2; p ITT= 0.760) para el grupo de la intervención y (8.6 ± 2.2; p ITT= 0.760) para el grupo control. En relación a la eficacia de los estilbenos en sintomatología urogenital y psicológica, los resultados fueron contradictorios. En términos de seguridad, dos de los cuatro ECAs reportaron la ausencia de eventos adversos con la suplementación de resveratrol.

Conclusión: La evidencia proveniente de los ECAs incluidos no es concluyente con respecto a la eficacia en la suplementación con estilbenos, particularmente con resveratrol, en el manejo de la sintomatología climatérica. Los ensayos clínicos reportaron la ausencia de eventos adversos con la suplementación de resveratrol, aunque este desenlace fue evaluado

III solo en la mitad de los estudios. Por ello, se requiere llevar a cabo futuros ensayos clínicos aleatorizados con un mejor diseño y mayor muestra, con el fin de alcanzar una conclusión definitiva sobre el uso de estilbenos.

Efficacy and safety of stilbenes for the management of climacteric symptoms in middle- aged women: a systematic review ABSTRACT

Objective: To synthesize the evidence regarding the efficacy and safety of stilbenes to treat climacteric symptoms in middle-aged women.

Methods: We conducted a systematic literature review according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. We searched in Pubmed-Medline, Embase, Scopus, Web of Science, WOS-Medline, LILACS, CINAHL, PsycINFO, SciELO, CENTRAL and international registries of clinical trials, about the efficacy and safety of stilbenes in climacteric symptomatology. We consulted grey literature sources, until December 2018, without language restriction. We used the Cochrane Collaboration tool for risk of bias assessment of the included studies.

Results: We obtained 2752 records in the primary search, from which we selected four randomized controlled trials (RCTs) that reported efficacy and safety outcomes. We found that the included clinical trials were highly heterogeneous in terms of dose, combinations with other phytoestrogens, duration of supplementation, measurement scales, and outcome reporting. Regarding efficacy, one trial reported a significant decrease in all somatic symptoms in the resveratrol group, with the most significant reduction in muscular and joint discomfort, where only 16.7% of the population reported these symptoms at the end of the intervention, compared to 73.3% for the placebo arm (p<0.001). On the other hand, a second RCT showed significant improvement in the number (p=0.0077) and intensity (p=0.018) of vasomotor episodes in the intervention group. In contrast, a third trial showed non- significant differences in the somatic subscale total score, (7.7 ± 2.2; p ITT= 0.760) for the intervention group and (8.6 ± 2.2; p ITT= 0.760) for the control group. In terms of safety, two of four RCTs reported the absence of adverse events with resveratrol supplementation.

Conclusion: Evidence from included RCTs is inconclusive regarding the efficacy of stilbene supplementation, particularly resveratrol, to manage climacteric symptomatology. Clinical trials reported the absence of adverse events with resveratrol supplementation, although this outcome was assessed in only half of the studies. Thus, future randomized clinical trials with

V a better design and larger sample are required to reach a definitive conclusion on stilbenes's use.

1. INTRODUCCIÓN El climaterio es una etapa crítica y relevante en la vida de la mujer, se caracteriza por múltiples cambios hormonales con la consecuente repercusión en las áreas físicas, psicológicas y sociales (1). Además, en esta etapa, la probabilidad de desarrollar osteoporosis, enfermedades cardiovasculares, y la mortalidad se incrementan (2–4). Estas alteraciones hormonales se pueden manifestar a través de una compleja sintomatología que abarca aspectos somáticos, urogenitales y psicológicos, lo que conlleva a la afectación de la calidad de vida (5,6). En este contexto, se ha reportado que el 45% de mujeres en Latinoamérica presenta deterioro leve de la calidad de vida, un 69%, deterioro moderado, y 13.7%, deterioro severo de ésta (7). Por lo anteriormente mencionado, el manejo de la sintomatología climatérica es crucial como parte del abordaje integral de la salud de la mujer en esta etapa de la vida.

Existen diversas alternativas terapéuticas para el manejo de la sintomatología climatérica, las cuales se dividen en farmacológicas y no farmacológicas. Dentro de los tratamientos farmacológicos, el más recomendado es la terapia hormonal (TH) que presenta beneficios para la reducción de esta sintomatología; aunque podría tener efectos perjudiciales fundamentalmente relacionados al incremento del riesgo de cáncer de mama invasivo, la demencia, la enfermedad de la vesícula biliar, los accidentes cerebrovasculares, la incontinencia urinaria y el tromboembolismo venoso (8,9). En base a ello, se han planteado tratamientos alternativos, los cuales muestran distintos niveles de eficacia y usualmente con un menor riesgo (10). Es así que, se han desarrollado tratamientos no farmacológicos, con especial interés en los fitoestrógenos (11,12), los cuales surgen como una nueva opción para el manejo de la sintomatología climatérica.

Los fitoestrógenos son una opción terapéutica de origen vegetal, los cuales se agrupan según su composición química en cuatro clases: isoflavonas, lignanos, cusmetanos y estilbenos (13). Estos compuestos actúan como moduladores selectivos sobre los receptores estrogénicos de tipo alfa y beta, lo que les permite ejercer un efecto agonista en algunos tejidos y antagonista en otros (14). Dentro de esta gran familia, es necesario destacar al grupo de los estilbenos, el cual es uno de los menos investigados, pero con alto potencial de efectos benéficos (15). El resveratrol es uno de los estilbenos más estudiados, al cual se le atribuyen propiedades cardiprotectoras (4), anticarcinogénicas (15), antiinflamatorias (16), antidiabéticas (17), neuroprotectoras (18) y antivirales (19).

De este modo, los estilbenos podrían ser una interesante alternativa para el manejo climatérico. No obstante, a pesar de lo expuesto, aún no se han publicado estudios de síntesis que evalúen su eficacia y seguridad. En consecuencia, el objetivo de nuestro estudio fue sintetizar la evidencia primaria disponible con respecto a la eficacia y seguridad de los estilbenos en el manejo de la sintomatología climatérica.

2. MÉTODOS 2.1 Tipo de Estudio Llevamos a cabo una revisión sistemática en base a las recomendaciones del Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) (20). Registramos el protocolo en PROSPERO (CRD 42019117569).

2.2 Elegibilidad de los estudios Incluimos ensayos clínicos aleatorizados controlados con las siguientes características: (i) Realizados en mujeres con sintomatología climatérica reportada a través de cualquier cuestionario validado. (ii) Recibieron suplementación con estilbenos independientemente de la dosis administrada. (iii) Se compararon frente a placebo, otro tratamiento no farmacológico u otro fitoestrógeno (isoflavonas, lignanos, cusmetanos). (iv) Reportaron al menos alguno de los siguientes desenlaces: síntomas somáticos, síntomas urogenitales, síntomas psicológicos, eventos adversos, calidad de vida o sintomatología depresiva.

Excluimos los estudios que evaluaron (i) la terapia hormonal menopáusica y que incluían a (ii) mujeres con cáncer de mama u otra neoplasia.

2.3 Medición de los desenlaces 2.3.1 Desenlaces Primarios Los desenlaces primarios fueron eficacia y seguridad. Definimos eficacia como la disminución de la sintomatología climatérica, expresada en la severidad de los síntomas somáticos, urogenitales o psicológicos. Además, consideramos los cambios en el puntaje total de las escalas validadas después del tratamiento con estilbenos. En términos de seguridad, valoramos los eventos adversos del fármaco, las reacciones adversas al medicamento, y los eventos adversos serios, según las definiciones de la European Medicines Agengy (EMA) (21).

2.3.2 Desenlaces Secundarios Los desenlaces secundarios fueron la sintomatología depresiva y la calidad de vida.

2.4 Estrategia de búsqueda Realizamos una búsqueda primaria en Pubmed-Medline, SCOPUS, Web of Science (WOS), WOS-Medline, Excerpta Médica (EMBASE), Literatura Latinoamericana y del Caribe en Ciencias de la Salud (LILACS), Cumulative Index to Nursing and Allied Health Literature (CINALH), Scientific Electronic Library Online (SciELO), PsycINFO, Cochrane Central

Register of Controlled Trials (CENTRAL). Asimismo, buscamos en los registros de ensayos clínicos de Estados Unidos de América (clinicaltrials.gov), la Unión Europea (https://www.clinicaltrialsregister.eu) y de la Organización Mundial de la Salud (www.who.int/ictrp/en/). Adicionalmente, consultamos fuentes de literatura gris que incluyeron los congresos de The North American Menopause Society (NAMS) y de The European Menopause and Andropause Society (EMAS) de los últimos 5 años; revisamos el listado de referencias bibliografías de los estudios incluidos. Realizamos la búsqueda sin restricción de idioma ni estado de publicación, hasta diciembre del 2018, y validamos la estrategia de búsqueda por un experto en gestión del conocimiento (JPM). (Anexo 1)

2.5 Selección de artículos y extracción de datos Recopilamos todos los documentos identificados de las bases de datos en una carpeta electrónica y luego generamos un archivo Research Information System RIS®️ (Elsevier Inc, NY, USA). Posteriormente, exportamos los registros a la aplicación web Rayyan QCRI ®️ (Qatar Computer Research Institute, Doha, Qatar) (22) para la consecuente eliminación de duplicados. Dos autores (AVA y CTP), con cuentas separadas en la mencionada aplicación, revisamos individualmente el título y resumen, y seguidamente aplicamos los criterios de elegibilidad preespecificados para la selección de registros y la lectura de los artículos a texto completo. En caso de la aparición de discrepancias, las resolvimos por consenso y con participación de un tercer autor en calidad de dirimente (EMH).

Extraímos los datos que incluyeron características específicas sobre el diseño de estudio, participantes, intervención, metodología de análisis, así como los desenlaces primarios y secundarios.

2.6 Evaluación de riesgo de sesgo Realizamos la evaluación del riesgo de sesgo de cada artículo seleccionado con la clasificación de la Colaboración Cochrane, la cual aborda los siguientes componentes: generación de secuencia aleatoria, ocultación de la asignación, cegamiento de los participantes y personal, cegamiento de la evaluación, datos de resultados incompletos, notificación selectiva de resultados y otros sesgos (23). En adición, usamos el software Review Manager ® versión 5.3 (Copenhagen: The Nordic Cochrane Centre, The Cochrane Collaboration, 2014) para valorar cada dominio, los cuales fueron calificados como “alto riesgo” (círculo rojo), “bajo riesgo” (círculo verde) o “riesgo incierto” (círculo amarillo) (24).

Las decisiones para cada dominio las realizamos de manera independiente por dos autores (AVA y CTP), con resolución de desacuerdos por consenso o con la intervención de un tercer revisor como dirimente (EMH). En la valoración global de los estudios clasificamos como alto riesgo de sesgo global a aquel estudio que presente un dominio clasificado como “alto riesgo” o tres dominios clasificados como “riesgo incierto”.

2.7 Análisis estadístico Inicialmente planeamos realizar un metaanálisis; sin embargo, no fue posible llevarlo a cabo debido a la alta heterogeneidad clínica y metodológica entre los estudios. Hubo alta variación en el reporte de los desenlaces principales, así como en el diseño, dosis, duración de suplementación y combinaciones con otros suplementos dentro de las intervenciones. Evaluamos la eficacia con el cambio en el puntaje o severidad de síntomas en la Menopause Rating Scale (MRS), The Hot Flash Related Daily Interference Scale (HFRDIS) y Visual Analogue Scale (VAS), la cual fue reportada en base a la media  error estándar de la media (SEM por sus siglas en inglés) o valores porcentuales. El desenlace de depresión fue evaluado con el cambio en el puntaje total del Centre of Epidemiologic Studies Depression Scale (CES-D) o Hamilton Rating Scale (HAM-D), el cual fue descrito como media  error estándar de la media. La calidad de vida fue evaluada con el cambio en el puntaje total del Short Form Questionnaire (SF-36) o del Menopause Rating Scale (MRS). Por último, el desenlace de estados de ánimo fue evaluado con el cambio en el puntaje del cuestionario Profile Mood States (POMS) y reportado como media  error estándar de la media.

2.8 Manejo de datos ausentes Intentamos contactar con los autores corresponsales para recabar datos ausentes de los estudios, a pesar de ello no se obtuvo la información solicitada.

3. RESULTADOS Identificamos 2752 registros en bases de datos electrónicas y registros de ensayos clínicos. De éstos, 969 duplicados fueron excluidos en Mendeley y de manera manual. Tras ello, 1783 estudios fueron evaluados por título y resumen. Luego, seleccionamos 14 artículos para lectura a texto completo. Finalmente, conforme a los criterios de elegibilidad, incluimos cuatro estudios en esta revisión sistemática. (Figura 1)

3.1 Características generales de los estudios Todos los estudios fueron ensayos clínicos aleatorizados controlados, doble ciego, de intervención dietética. Las características generales de los estudios las resumimos en la (Tabla 1). Tres ensayos clínicos tuvieron diseño paralelo mientras que uno tuvo diseño cruzado. La muestra total incluyó 270 participantes. El tamaño muestral de los estudios varía entre 50 y 80 mujeres. Dos ensayos clínicos fueron desarrollados en Italia y dos en Nueva Gales del Sur. El tiempo de seguimiento osciló entre las 12 y 14 semanas. Los estudios fueron publicados entre 2015 y 2017.

3.2 Desenlaces primarios 3.2.1 Eficacia en síntomas climatéricos medida por MRS Dos estudios evaluaron síntomas climatéricos (25,26) mediante el Menopause Rating Scale (MRS) . Los resultados fueron expresados como media ± SEM para Wong et al. (26) y valores porcentuales para Davinelli et al. (25). (Tabla 2)

3.2.2 Eficacia en síntomas somáticos En el dominio somático, Davinelli et al. (25), reportaron una disminución significativa en todos los síntomas somáticos. El ítem de las molestias musculares y articulares obtuvo mayor reducción, donde solo el 16.7% del grupo de la intervención reportó estas molestias al final de la intervención (p<0.001), en comparación con el brazo del placebo que obtuvo un 73.3% (p<0.001). En cambio, Wong et al. (26), mostraron diferencias no significativas en el puntaje total de la subescala somática (7.7 ± 2.2; p ITT= 0.760) para el grupo de la intervención y (8.6 ± 2.2; p ITT= 0.760) para el grupo control. Leo et al. (27), evaluaron la eficacia del resveratrol en el tratamiento de los bochornos durante la menopausia. Este estudio tuvo como desenlace primario la reducción mensual de episodios vasomotores, la cual fue valorada por la Hot Flush Related Daily Interference Scale (HFRDIS). Además, estos autores reportaron la intensidad de los bochornos como desenlace secundario, medida a través de la Visual Analogue Scale (VAS). Los resultados de este estudio demostraron una disminución

6 significativa del número de episodios vasomotores mensual (p=0.0077), y mejoría de la intensidad de los síntomas vasomotores (p= 0.018) en el grupo de la intervención, comparado al brazo placebo. (Tabla 3)

3.2.3 Eficacia en síntomas urogenitales Davinelli et al., (25) revelaron una mejoría significativa en todos los ítems de la subescala urogenital para el grupo del resveratrol. El ítem de sequedad vaginal fue reportado como el de mayor reducción, donde solo el 13.3% de la población reportó estas molestias al final de la intervención, frente al brazo placebo que obtuvo un 89.9% (p<0.001). Por otro lado, Wong et al., (26) expusieron una disminución no significativa en el puntaje total de la subescala urogenital del grupo con resveratrol (9.0 ±2.7; p ITT =0.808), en comparación con el brazo del placebo (6.6 ±2.8; p ITT =0.808).

3.2.4 Eficacia en síntomas psicológicos Con relación a los síntomas psicológicos, Wong et al., (26) reportaron una reducción no significativa en el puntaje total de la subescala psicológica para el grupo con resveratrol (4.1 ±1.8; p ITT =0.688), frente al grupo control (3.0 ±1.9; p ITT =0.688). En contraparte, Davinelli et al., (25) mostraron una mejoría significativa en todos los ítems de la subescala psicológica, particularmente en el ítem de cansancio físico y mental, en el cual el 30% de la población que recibió resveratrol presentó síntomas, comparado al 60% del grupo control al final de la intervención (p<0.01).

3.2.5 Seguridad De los cuatro estudios incluidos, dos de ellos no reportaron diferencias con respecto a los eventos adversos entre el grupo de estilbenos versus el grupo control (25,26). Ningún estudio utilizó la clasificación de eventos adversos de la EMA. Por otro lado, dos estudios no reportaron desenlaces de seguridad (27,28).

3.3 Desenlaces secundarios 3.3.1 Calidad de vida Solo un estudio publicó sus resultados del efecto del resveratrol en calidad de vida. Wong et al., (26) utilizaron Short Form Questionnaire (SF-36) para la medición de este desenlace; los resultados mostraron mejoría no significativa en calidad de vida con el uso de resveratrol (2.8± 2.3; p ITT = 0.761) frente al grupo control (2.4± 2.3; p ITT = 0.761). Adicionalmente, los autores en este estudio reportaron mejoría no significativa con el puntaje total de MRS,

7 el cual es considerado como otra forma de valoración de la calidad de vida, obtuviéndose (6.4 ± 1.6; p ITT =0.580) en el grupo de la intervención, comparado con (5.9 ± 1.7; p ITT =0.580) para el grupo control.

3.3.2 Sintomatología depresiva Dos ensayos (26,28) evaluaron el efecto del resveratrol en síntomas depresivos con el Centre for Epidemiologic Studies -Depression Scale (CES-D) mientras que un estudio utilizó ciertos ítems del Hamilton Rating Scale (HAM-D). Mejorías no significativas en el puntaje total del CES-D fueron reportadas por ambos estudios en el grupo de la intervención, con una variación de (0.8 ± 0.9; p =0.104) para Evans et al., (28) y (1.3 ± 1.9; p ITT =0.103) para Wong et al. (26). Por el contrario, mejorías significativas en ciertos ítems del HAM-D como trabajo y actividades, agitación y ansiedad fueron reportadas en el brazo con resveratrol por Davinelli et al. (25). 3.3.3 Otros desenlaces reportados: Profile of Mood States (POMS) Dos estudios (26,28) valoraron el efecto del resveratrol en el estado de ánimo de mujeres postmenopáusicas con el cuestionario Profile of Mood States, el cual evalúa seis subescalas: ansiedad, depresión, ira, fatiga, confusión y vigor. Evans el al., (28) reportaron una reducción no significativa del puntaje total de POMS (8.5 ± 2.5; p =0.085) para el grupo de la intervención, en comparación con el grupo control (2.0 ± 2.6; p =0.085). Del mismo modo, los resultados reportados por Wong et al., (26) mostraron disminución no significativa del puntaje total de POMS (29.0 ± 7.2; p ITT =0.434) para el grupo con resveratrol, frente al grupo control (9.4 ± 7.4; p ITT =0.434). (Tabla 5)

3.4 Evaluación de riesgo de sesgo En la Figura2 y Figura 3, presentamos el análisis de riesgo de sesgo de los artículos incluidos mediante la herramienta de evaluación de riesgo de sesgo de Cochrane. Con los resultados obtenidos, evidenciamos que dos estudios (27,28), presentaron una calificación de alto riesgo de sesgo global. El estudio de Leo et al., (27) obtuvo tres dominios de “riesgo incierto” y un dominio de “alto riesgo” correspondiente al de generación de la secuencia aleatoria, dado que seleccionaron a los primeros 50 participantes que acudieron al centro de menopausia. Asimismo, el ensayo clínico de Evans et al., (28) mostró un alto riesgo de sesgo de selección en el dominio perteneciente a la generación de la secuencia aleatoria puesto que la primera participante fue asignada por fecha de cese de menstruación. Finalmente, los estudios de Wong et al, (26) y Davinelli et al. presentaron “bajo riesgo” de sesgo en todos

8 los dominios que evalúa la herramienta Cochrane. La heterogeneidad metodológica reflejada por el alto riesgo de sesgo en la mitad de los estudios incluidos, contribuyó a la no realización de la síntesis cuantitativa.

4. DISCUSIÓN Nuestros hallazgos muestran que la evidencia primaria proveniente de estudios clínicos aleatorizados es escasa y contradictoria con respecto a la eficacia de los estilbenos, particularmente del resveratrol, en el tratamiento del síndrome climatérico. En cuanto al desenlace de seguridad, los ensayos clínicos demostraron la ausencia de eventos adversos con la suplementación de resveratrol, aunque fueron reportados solo en la mitad de los estudios. Es preciso indicar que, los resultados provienen de estudios altamente heterogéneos en términos de dosis estandarizadas, tiempos de seguimiento, escalas de medición y reporte de resultados. Sumado a ello, los estudios incluidos tienen pequeño tamaño muestral y la mitad de éstos poseen alto riesgo de sesgo global. Estos problemas en el tamaño muestral y en el riesgo de sesgo podrían contribuir a un mayor error aleatorio y sistemático, respectivamente, y así contribuir a una menor precisión y validez de las estimaciones realizadas dentro de cada ensayo (29). Adicionalmente, el riesgo de sesgo de la mitad de los estudios es alta, especialmente por la falta de generación de la secuencia aleatoria. Por lo explicado, no es posible brindar una recomendación clínica para el uso de resveratrol en el síndrome climatérico. Consecuentemente, bajo esas circunstancias, se requieren nuevos estudios que valoren la eficacia y seguridad de este estilbeno con mayor rigor metodológico.

Anualmente, 1.5 millones de mujeres cursan con clínica relacionada al climaterio, la cual incluye síntomas vasomotores, sequedad vaginal, disminución de la libido, insomnio, fatiga y dolor articular (30). Con base en ello, nuestra revisión consideró como desenlace primario la eficacia en síntomas somáticos, urogenitales y psicológicos. En particular, los síntomas vasomotores constituyen la manifestación más característica del climaterio, cuya duración puede extenderse hasta 10 años posterior al cese de la menstruación (31). El resveratrol ejerce un mecanismo vasoprotector explicado por el aumento de la síntesis de óxido nítrico y la disminución del estrés oxidativo a nivel endotelial. Esta propiedad podría influir positivamente en el manejo de los síntomas vasomotores, los cuales se encuentran directamente relacionados con el incremento del estrés oxidativo (15,32). En nuestra revisión, solo dos estudios reportaron mejoría significativa a nivel de síntomas somáticos con el uso de resveratrol (25,27).

Algunas combinaciones de fitoestrógenos han demostrado tener un efecto favorable en el manejo de síntomas urogenitales como la sequedad vaginal (33,34). Debido a que existen receptores estrógenicos en tejidos del tracto genitourinario como el útero y la vagina (35), el

10 resveratrol podría actuar como agonista estrogénico en dichos tejidos, lo que explicaría la reducción significativa de la sintomatología urogenital hallada por Davinelli et al., (25) tras 12 semanas de suplementación con resveratrol. Asimismo, estudios en animales sugieren que el resveratrol constituiría un tratamiento efectivo para los cambios en el comportamiento psicológico postmenopáusico, especialmente en los desórdenes de ansiedad y depresión (36,37). Más allá de la existencia de varios mecanismos que podrían explicar un potencial efecto, dos de los cuatros ensayos incluidos en nuestra revisión (25,26) evaluaron la eficacia del resveratrol en el dominio psicológico, los cuales mostraron evidencia contradictoria.

Respecto al desenlace de seguridad, dos de los cuatro estudios reportaron la ausencia de eventos adversos con la suplementación de resveratrol, sin diferencias clínicas entre los grupos que recibieron estilbenos frente a los grupos control, por lo que se sugiere que el uso de estilbenos es seguro y bien tolerado. En contraparte, otras investigaciones han reportado síntomas gastrointestinales y dislipidemias como los eventos adversos más comunes presentados con el uso de resveratrol (38–40).

Nuestros desenlaces secundarios evaluaron el uso de estilbenos en sintomatología depresiva, calidad de vida y estado de ánimo. Los resultados de los ensayos clínicos incluidos indican mejoría no significativa en la sintomatología depresiva con el uso de resveratrol. Estudios previos han observado el efecto antidepresivo del resveratrol en modelos animales, mediante la inhibición que ejerce sobre la enzima monoaminooxidasa, con lo que el resveratrol incrementaría los niveles de serotonina y noradrenalina al inhibir su recaptación, particularmente en la corteza frontal y el hipocampo (41,42). Respecto a la calidad de vida, solo un estudio (26) reportó valores sobre este desenlace, los cuales mostraron que no había mejoría significativa en el grupo con resveratrol frente al control. En el metaanálisis publicado por Farzaei et al., en población humana, se evaluó el uso de resveratrol y su efecto en el estado de ánimo, obteniéndose una reducción significativa de las subescalas de vigor y fatiga en el grupo de la intervención (43). En contraparte, nuestros hallazgos muestran que no existe reducción significativa del estado de ánimo valorado con POMS. Es importante destacar que, si bien inicialmente este desenlace no fue predefinido, decidimos agregarlo a la presente revisión.

Nuestra revisión sistemática presenta varias limitaciones. Primero, si bien inicialmente se planteó que la investigación incluiría mujeres en un rango de edad mediana (40-59 años), los autores decidimos no prestar excepción a la edad de la población puesto que los síntomas

11 climatéricos pueden persistir por más años tras el cese de la menstruación (44). Respecto a ello, un artículo basado en el Heart and Estrogen/Progestin Replacement Study (45), en el cual se incluyó 2763 mujeres postmenopáusicas con enfermedad coronaria de 55 a 88 años, concluyó que los síntomas atribuidos a la menopausia eran comunes en adultas mayores. Por otra parte, la exclusión de estudios podría conducir a sesgo de selección, debido a que se excluirían también a mujeres dentro de ese grupo etáreo. Segundo, observamos que los estudios incluidos presentan diferentes escalas de medición para la evaluación de la eficacia. Respecto a esta última, solo dos estudios (25,26) valoraron la eficacia del resveratrol con la misma escala (MRS); sin embargo, sus resultados fueron agrupados y reportados de manera distinta. Davinelli et al.,(25) utilizaron puntajes individuales para cada ítem de los dominios somático, urogenital y psicológico; mientras que, por otro lado, Wong et al.,(26) presentaron como resultados a los puntajes totales agrupados por dominio. Asimismo, las escalas para medir el desenlace de sintomatología depresiva fueron distintas (CES-D y HAM-D). Más aún, es importante mencionar que no hubo reporte de toda la escala HAM-D por Davinelli et al. (25). Uno de nuestros desenlaces secundarios (calidad de vida) no fue evaluado por la mayoría de los ensayos clínicos, en efecto, solo fue desarrollado por uno de ellos (26), y es por ello que no fue posible realizar una síntesis cuantitativa, por lo que optamos por el reporte cualitativo de los resultados. Tercero, la no inclusión de estudios observacionales constituye otra limitación adicional en torno a la seguridad de la intervención. No obstante, por aspectos metodológicos, se ha comprobado que los ensayos clínicos aleatorizados constituyen el máximo nivel de evidencia disponible con respecto a eficacia (46–48). Cuarto, cabe precisar que mientras el riesgo de sesgo fue bajo para la mitad de los estudios, los restantes (27,28) presentaron una calificación de alto riesgo de sesgo en la generación de la secuencia aleatoria (sesgo de selección), lo cual podría conducir a una sobreestimación del efecto de la intervención sobre los síntomas somáticos y a una subestimación de éste sobre la sintomatología depresiva. Esta variabilidad en el riesgo de sesgo es descrita por la Colaboración Cochrane como heterogeneidad metodológica (23). Quinto, es relevante acotar que podría existir cierto riesgo de sesgo de publicación debido a que usualmente solo son publicados los ensayos clínicos que muestran un efecto positivo de la intervención; además, ante la ausencia de metaanálisis, no fue posible realizar un gráfico de embudo que evalúe el sesgo de publicación (49). Si bien no podemos ejercer un control directo sobre este factor limitante, se realizó una amplia estrategia de búsqueda en las principales bases de datos y fuentes de literatura gris a manera de reducir la probabilidad de sesgo de publicación.

Sexto, el mayor tiempo de seguimiento de los estudios incluidos fue de 14 semanas. Este corto intervalo de tiempo constituiría una limitación debido a que no permitiría estimar el desarrollo de eventos adversos a largo plazo.

La heterogeneidad clínica y metodológica, expresadas anteriormente, condicionaron la no realización de un metaanálisis y consecuentemente hace que sea poco probable estimar con mayor potencia estadística el efecto de los estilbenos en la sintomatología climatérica. Más allá de las limitaciones expresadas, nuestra revisión sistemática provee una de las primeras aproximaciones que sintetiza la evidencia primaria con respecto a la eficacia y seguridad de los estilbenos como parte del tratamiento del síndrome climatérico. Existe una revisión sistemática, realizada por Franco et al., en el 2016, que evaluó la eficacia de 36 fitoestrógenos en síntomas menopaúsicos (50); no obstante, los estilbenos no fueron objeto de estudio dentro de ésta. Según lo revisado, no se han publicado revisiones sistemáticas acerca de la eficacia y seguridad de los estilbenos en el manejo de la sintomatología climatérica.

5. CONCLUSIÓN La evidencia proveniente de ensayos clínicos aleatorizados no es concluyente con respecto a la eficacia en la suplementación con estilbenos, particularmente con resveratrol, en el manejo de la sintomatología climatérica. Los ensayos clínicos reportaron la ausencia de eventos adversos con la suplementación de resveratrol, aunque este desenlace fue evaluado solo en la mitad de los estudios. Por ello, se requiere llevar a cabo futuros ensayos clínicos aleatorizados con un mejor diseño y mayor muestra, con el fin de alcanzar una conclusión definitiva sobre el uso de estilbenos.
