Executive summary

The second regional plan for the Murraylands and Riverland region is being prepared in collaboration by the Murraylands and Riverland Local Government Association (the MRLGA) and the region’s eight constituent Councils: the Rural City of Murray Bridge, District Council of Karoonda East Murray, Southern Mallee District Council, , , , Berri Council and the District Council of Loxton Waikerie. Between 12 April and 10 May2021 the draft plan was released for public consultation. Feedback of community and stakeholders was sought via an online survey. 36 responses were received as well as two written submissions. Most survey respondents (58%) agreed that the goals and outcomes are right for the region. Around a third (32%) somewhat agreed and only 3% did not agree. Around half of respondents (54%) ‘somewhat support’ the draft plan as a whole. 42% ‘support the draft’ as a whole. Key themes of feedback were to strengthen the following in the draft plan: • Ensure plan directions address the key regional challenges (eg. mental health, non-communicable disease, socio-economic disadvantage) and priority populations (including those outside townships) • Suicide prevention, especially building the capacity of the workforce to respond to suicide distress • Reduce drug harms by engaging with Alcohol and Drug Foundation programs and resources • Child and youth engagement and wellbeing • The importance of community connectivity and access to facilities and programs that bring people together (inc. sport) • Climate change, greening neighbourhoods and establishing native vegetation • Consultation and collaboration with community, groups and services in the delivery of programs • More pointed goals and actions, and collaborative actions between councils • Political and financial commitment to delivering and reporting on the plan.

Background

In public health is guided by the Public Health Act and the South Australian State Public Health Plan 2019-2024. These acknowledge the role of local governments in public health and encourage the development of regional public health plans that support the collaboration of Councils and stakeholders to improve regional public health outcomes. The second regional plan for the Murraylands and Riverland region is being prepared in collaboration by the Murraylands and Riverland Local Government Association (the MRLGA) and the region’s eight constituent Councils: the Rural City of Murray Bridge, District Council of Karoonda East Murray, Southern Mallee District Council, Mid Murray Council, Coorong District Council, Renmark Paringa Council, and the District Council of Loxton Waikerie. In April and May2021 the draft Murraylands and Riverland Regional Public Health and Wellbeing Plan was released for the purposes of public consultation.

Engagement process

An online survey was used to gather feedback on the draft plan from the region’s stakeholders and community. The survey was accessible from the MRLGA website (www.mmlga.sa.gov.au) for a period of 4 weeks from 12 April until 10 May 2021. A copy of the survey is provided in Appendix A. The survey was promoted through: • the Facebook pages of the MRLGA and each council • a poster provided to each council to distribute through their centres (Appendix B) • emails to stakeholders and networks identified and distributed by each council. Although not a promoted method of feedback, two written submissions were received.

Survey results

Thirty-six survey responses were received. The origin of respondents across the different council areas is provided in Figure 1. Eleven of the survey responses were from organisations listed below. Feedback on the draft goals and outcomes Respondents were presented with the goals and outcomes of the draft plan (Table 1) and were asked if they agree that these are the right goals and outcomes for the region’s public health.

Most respondents (58%) agreed that the goals and outcomes are right for the region. Around a third (32%, 10 respondents) somewhat agreed, and only 3% (1 respondent) did not agree. (Figure 2). Respondents were asked if they would like to provide comments on the draft goals. 14 comments were provided. Comments provided by those who selected options other than “agree” included to: • change the wording of Goal 2 to refer to reducing drug harms and to include more tangible actions to assist in achieving this aspect of the goal including embedding ADF programs and resources (such as Good Sports, DrugInfo Services and Community Hub and ‘How to guide’ for local government) • Reword Goal 5 to reflect the value that the collaboration will provide to the community • Reflect opportunities for community to co-design and contribute to initiatives • Better reflect the needs of children and youth including nutrition, exercise, education, wellbeing, youth mental health, drug harm and positive participation, and paediatric disability services • Make the goals less broad.

Is that enough on such an important issue? Will council agree on what that role is after 'considering' and will that be outlined to the people of the District. Finally, there is nothing here about environmental health - water quality education, air quality, food safety. Does the Coorong District not having priorities in this regard over the lifespan of this plan?

'Goal 5 - Strengthen regional public health collaboration and delivery' will be delivered in partnership by the Councils and Murraylands and Riverland LGA through the Murraylands and Riverland Regional Public Health and Wellbeing Plan Committee.

Goal 5 has the following proposed actions:

1. The Committee to meet at least three times a year to drive the delivery of the Plan and build collaboration 2. The running of a forum of public health stakeholders, convened by the Committee in the first 12 months of its formation, to promote the range of public health services delivered in the Murraylands and Riverland, explore examples best practice and consider the benefits of regional collaboration 3. The development of regional collaboration teams, where practical, to further explore key regional public health issues and act or advocate for change”

Although smoking, alcohol and other drug harms were highlighted as an issue of concern, only three linkages to tangible actions were identified within 2 councils (Rural City of Murray Bridge and, Coorong District Council). It is suggested that other actions that reduce alcohol and other drug harms are incorporated so this goal can be achieved and there is clarity on how it will be achieved across all 8 councils. Suggestions: • Embed the Alcohol and Drug Foundation’s (ADF) free programs into actions to assist in achieving this goal: - Suggested example action: ‘Encourage all sporting clubs to engage in the Good Sports Program, a community health program reducing alcohol and other drug harms and building strong sporting clubs across South Australia’ (https://goodsports.com.au/) - Suggested example action: ‘Promote factual information and services to prevent harm from smoking, alcohol and drugs by utilising the Alcohol and Drug Foundation (ADF) DrugInfo Services and Community Hub’ (https://adf.org.au/ and https://community.adf.org.au/). • Access additional suggestions on strategies to address this goal within the ‘How to Guide’ developed by the ADF and VicHealth, drawing on evidence around what Local Government can do to prevent and minimise alcohol-related harm. Further information: • Good Sports is complementary to the existing STARCLUB program (mentioned in the plan) and provides free additional support for sporting clubs in the areas of smoking management, alcohol management, safe transport practices, illegal drugs, mental health and junior friendly environments (https://goodsports.com.au/program/). • Good Sports has been supporting strong community sporting clubs for over 20 years, and ADF have updated the program and have taken it online, making it even more relevant and convenient for clubs. It can be accessed on any device, anytime and anywhere. Clubs get supported at each step with tailored resources to help them move through their accreditation, generating a clear to-do list with an automated action plan. Clubs also create one public health policy that combines all the areas of the Good Sports program, so they don’t have to keep track of multiple policies. Somewhat agree Regular public communication requested on collaborative projects and outcomes, opportunities for community members to contribute (co- design of health & wellbeing projects and initiatives) Somewhat agree Goal 5 reads first and foremost as equipping Councils and talks about a milestone not an outcome or goal relevant to people or the community. ‘Councils collaborate’ as a term is an activity or milestone that leads to a goal/outcome. Is the outcome that councils collaborate, and is the outcome that councils are resourced....or is the outcome something different? For instance. What will be the outcome of the collaboration and the resourcing for the community? I would recommend these two outcomes under this goal being presented differently otherwise this goal appears to be entirely internally driven. The benefit to the people and to community should be apparent as per the language used in the other goals. Somewhat agree I do not think they completely address the concerns around mental health drug abuse and actioning and implementation of further education for youth and prevention or education on these things. Also youth recreational activities and more things for youth to do a program to promote healthy activities to prevent drug abuse, break-ins, and crime escalation. Especially for the youth not involved in sporting activities. Somewhat agree I would love to see more discussion around paediatric services for kids with disabilities, or lack there-of. My son sees three different therapists, none of which are local, as there aren’t enough services here. Great to see a focus on mental health services. Somewhat agree Goals for children are paramount. Nutrition, exercise, education, happiness. Somewhat agree They may be too broad – suggest more specific goals

The Mid-Murray Suicide Prevention Network would like to thank you for the opportunity to comment on the Murraylands & Riverland Regional Public Health & Wellbeing Plan during its consultation period. While acknowledging the hard work and wonderful programmes Councils have run, we also see from the data section of this plan there are considerable challenges still to be overcome. In this region the demographics are challenging for policymakers. In particular from the perspective of this group, we see the Mid-Murray has a rate of suicide 2.11 times the Regional SA rate. It is all of the other challenges — health; housing; education; isolation; financial distress and lack of access to appropriate and timely services that could drive this number up. Rural and remote communities and men are two of the groups disproportionately affected by suicide. In the Mid-Murray males significantly outnumber females in every age-band over the age of 14, except the 85+ (92,117) . In the whole of Australia in 2019 there were 2502 deaths caused by intentional self-harm compared to 1079 caused by land transport accidents. If we saw an equivalent number by both causes, but saw 2.11 times the regional SA average number of land transport deaths in the Mid- Murray, we would be outraged. From the perspective of this group, we would like to see the language clarified around Goals 1 and 2 as they pertain to each of the Councils intended programmes and how they relate back to the pages titled “What does this mean for regional public health planning?” We would also like to make a broad comment about the opportunity this plan has at this time to harness the power of the recent final advice to Governments from Christine Morgan, the Prime Minister’s National Suicide Prevention Advisor. She makes it plain that early intervention is key to bringing down suicide numbers. The support needs to be offered at the (first) point of distress. For Councils, this means strengthening the capacity and capability of the workforce. She says: “The first time a person discloses their distress or suicidality is a critical moment, so it is vital to build capability and knowledge across workforces and within communities to ensure a shared understanding about suicide distress, and the criticality of a consistent and compassionate approach. This requires strengthening capacity within all government services and community networks in touch with people at times of distress, transition or disconnection. Every interaction an individual has with a department, agency, service provider, worker or community contact is an opportunity to identify suicidal distress and to intervene to ensure they get the right supports at the right time.” Her recommendation surrounding this is for: “All governments to commit to prioritising evidence-based and compassion-focused workforce development to drive cultural change in and improve the capacity and capability of all (formal and informal) workforces involved in suicide prevention.” Specifically: “4.2 All jurisdictions implement contemporary compassion based training for frontline workers that enable them to respond to

distress – especially those providing financial, employment and relationship support to people experiencing distress.” This would mean at a minimum anyone in contact with the public, including Members of Council, but optimally the entire work-force, both paid and unpaid. Dan Cregan, the Premier’s Advocate for Suicide Prevention and Community Resilience calls this response to suicidal distress a non-delegable duty. In other words, a person can’t pretend they didn’t hear or understand and they can’t assume the duty will be picked up by the next person. Having said this, the training for workforces is crucial. In addition, this year the South Australian Government will legislate a Suicide Prevention Framework. It aims to establish a whole of government and whole of community approach to suicide prevention. Specifically, it will legislate for larger bodies in our communities to have their own Suicide Prevention Plans. It remains to be seen how this will affect Local Government. In closing we would say the National Suicide Prevention Advisor’s final advice to Governments is brand new. While there have been a couple of interim reports, this is the document that Governments now have to guide their path forward towards the Federal Government’s goal of zero suicides. You, as a group of councils, have this opportunity to look over the Murraylands and Riverland Regional Public Health and Wellbeing Plan, through the lens of this advice. It is an opportunity to interrogate the at-risk groups and to see whether the programmes you aim to put in place will actually deliver for them. It is an opportunity to look at the wellbeing of the Councils’ frontline workers. It is an opportunity to see the non-delegable duties Local Governments have under this model that the Prime Minister himself, has identified as being a Whole of Governments (plural) approach —every department across every level of Government. Social connections are the key drivers of a healthy connected district. Activities like the art events are fantastic. Football / netball clubs provide for some people but there are a lot who are not connected. Look for opportunities to build connections and many of your issues ( drugs, health, environment) will start sorting themselves out. The youth have very few opportunities to connect. They need structure and guidance. Skate parks where they hang by themselves won’t achieve anything.

We have ten years to attend to our climate change errors, re establishing native vegetation is a high priority and needs to be done in as many places as 38ossible. Addressing mental health issues for the most vulnerable in our community by improving the areas they live in can be down by concentrating council improving the streetscapes and providing services in their neighbourhoods.

Some concerning statistics for the wider region that needs to be addressed. Succinct and fairly broad areas of focus in Plan – needs to be delivered for all communities not just larger towns. All councils need to ensure their Elected Body is connected and engaged with the Plan (and

commit funding to areas of need) – keep providing feedback to the EM’s & community and/or opportunities for comment on various health & wellbeing initiatives (evaluate effectiveness)

I think Goal 5 needs some serious further elaboration. The way in which it is presented now appears as though the drafting team either ran out of ideas, ran out of contributions from the various Councils or ran out of energy. This is a very important element the plan and warrants further energy to ensure it is fully elaborated and Councils’ are required to list their planned contributions. Overall I feel that the plan would value from a strong positive critique and review of some of the language used. Particularly at goal/outcome and action level. Throughout there is often the use of very process oriented or passive language which does not provide the reader with any sense of what will e achieved for the people or the community. Council doing a review, or considering something is an internal activity but not really an ‘action’ as far as impacts of the community. Of course these things need to happen but I would really encourage extension of actions or activities listed in this way to challenge the Councils to actually determine what will happen. Otherwise the plan runs the risk of appearing very process oriented and not focussed on the people or community. I have mentioned earlier about concern for ensuring non communicable disease is well considered given the stats in this region. These diseases do not go away by establishing community gardens or having Park run, they require well considered public health response that address the root causes of the disease. Finally, there is no where in the plan that actually outlines the demographic of the region or indeed any one district. The stats and % are there. And percentages only tell a part of the story. Having a snapshot per Council of population stats, including a disaggregation of women, men, children, working, unemployed, cultural affiliation, businesses. All of which should be easily accessed through census data. Would make a big difference. This would make a difference to those developing the plan but also to the people and community that read this plan, knowing that the demographics of their community are recognised and understood. Forums for consultation with stakeholders are great but they need to move beyond assuming that everyone has equal access to those forums or has equal voice if there. Indigenous people, older adults with no mobility or impairment or no access to technology, most at risk adults and children/youth, all need consideration in these processes. Otherwise you are talking to and getting feedback from just a small proportion of the community. A great initiative very worthy of further strengthening given the importance to the people and community of the region.

You need to involve the public get them involved in promoting healthy choices in their area. Ask for consultation in programs

Funding for leadership to deliver and coordinate this plan is essential. Funding may be available from well-being SA. Community engagement is essential. The plan is positive in that it works on building protective factors in the community which is admirable. It would be good to see an emphasis on suicide prevention strategies given the data. Engaging young people is important and not really clear in the plan. Coordinating the engagement of other parties such

as Volunteering SA would be useful. Every council and community should have a well-being steward.

Mentioned earlier – maybe the goals are too broad. I would like to see something specific around supporting the mental health AND wellbeing of the community Increasing MH services, especially programs that support people after a suicide attempt. Metro has this but not Country SA.

It is evident that there was very little collaboration in the developed, which doesn’t look good considering it is a key action of the group. At the beginning of the plan it states the priority populations, however there is very little other than stating they are a priority, that outlines how anyone will actually prioritize the health or wellbeing of these communities! There are some good strategies outlined in parts across all the different Council’s – but it really needs to be more cohesive and collaborative. Most Council’s need to put more effort and resources into public health and wellbeing – there are many many flow on effects for whole of community. Regional Council’s have to be far more than roads, rates & rubbish. It’s in their best interest to invest in this!

DIT is currently working with local government, bus operators, and industry bodies to undertake a review of the regional bus network.

As part of this review DIT will develop a regional public transport framework, which will establish service level requirements for both service delivery and investment in the short and medium term.

Although the review is yet to be completed, potential recommendations may include increased service promotion leveraging from local government channels, and a more consistent approach into the way in which public transport information is published across all regions.
