This directory should be used in conjunction with the Network’s supportive and psychosocial care pathway document, which sets out the agreed Network approach to the delivery of these aspects of care.

How to use this directory

This directory gives details of psychosocial support services that can be accessed by people affected by cancer across Kent and Medway. It includes support offered by voluntary organisations as well as the NHS. The aim of this directory is to improve awareness among professionals of sources of support that might benefit patients, and through this to improve uptake of support. The sources listed in this directory are also included in the listings on the Kent and Medway Cancer Network’s patient website (www.kentmedwaycancerguide.nhs.uk).

This directory is organised into three main parts. The first two sections (National level and Network level listings) give details of support that can be accessed from all areas of Kent and Medway. This includes national sources such as the telephone helplines run by major cancer and mental health charities. It also includes a guide to the mental health services provided by the Kent and Medway NHS and Social Care Partnership Trust, and the community care provided by the social services departments of Kent County Council and Medway Council.

The third section (Locality level listings) gives details of support that is provided specifically within each locality. There are some differences in the types and levels of support available in the three localities.

When searching for possible sources of support, it is worth checking both the National and Locality sections as, to avoid duplication. National sources are not included in the locality listings. Some local sources are, however, cross referenced from the Network level section.

The 4-level model

The NICE Guidance on Improving Supportive and Palliative Care for Adults with Cancer (2004) recommended the following model of professional psychological assessment and support:

For the purposes of this directory and ease of listing, self-help and informal support has been labelled ‘level 0’. It is expected that the majority of support needs of people affected by cancer can be met by self-help and informal support at level 0, and/or by cancer services staff operating at levels 1-3 of this model.

As a general principle, obtaining early support at levels 0, 1 and 2 may avoid escalation of needs to levels 3 and 4. It is recognised that a minority of people will require specialist support at level 4, provided by mental health service staff or on a shared basis between cancer and mental health staff. One of the goals of producing this directory is to improve knowledge among cancer services staff of sources of level 4 support.

2. Network level organisations

2.1 Clinical Lead

At the time of publication, the Cancer Network is seeking to appoint a Clinical Lead for Supportive and Psychosocial Care. Contact details for the postholder will be available via the Network’s main telephone number (01622 713146).

2.2 Cancer Partnership

The Kent and Medway Cancer Partnership is a user group for cancer patients, their relatives and carers. The Cancer Partnership involves them in the evaluation of current provision and the development of new services, listening to their views and issues. Partnership members participate in network committees to ensure that the voices of people affected by cancer are heard at all levels of the network.

2.4 Mental Health Services

Psychological Support Services

At present psychological services are organised into three tiers: • Primary Care Psychology and Counselling • Secondary Care Psychology • Specialist Psychology/Psychotherapy Services

Primary Care Psychology and Counselling This is delivered in GP surgeries and central bases. It provides brief and short-term interventions for mild to moderate mental health difficulties. It is delivered mainly within the NHS but some is independent.

Secondary Care Psychology This is aligned with both community mental health and inpatient services. It provides medium- to long-term interventions for moderate to severe mental health difficulties using a range of treatment modalities in an integrated form to best meet complex needs.

Specialist Psychology/Psychotherapy This provides medium to long-term interventions, including cognitive behavioural therapy, psychodynamic therapy and family therapy. These are available to clients with moderate to severe mental health difficulties.

Assessment and treatment

Assessment and treatment is provided as close as possible to the patient’s home through multi-disciplinary teams. Teams typically comprise community psychiatric nurses, social workers, occupational therapists, psychological therapists, technical instructors, support time recovery workers, consultant psychiatrists, other medical staff and administrative/secretarial staff. Services are backed up by inpatient facilities if necessary.

Referrals

Any professional concerned about an individual’s mental health should consult their GP in the first instance. The initial referral route at present to mental health services is through a GP, for patients who have mental health needs that cannot be addressed using primary care brief counselling/therapy or other voluntary sector counselling/therapy services.

Emergency

Out of hours: through the Crisis Resolution and Home Treatment (CHRT) Team/Crisis Assessment and Treatment Team (CATT), depending on location. For details see locality pages. Patients in mental health crisis presenting to A & E departments will be assessed by these teams 24 hours a day.

During working hours through the locality Mental Health Service Intake Team (for details see locality pages).

Access to selected services can also be gained via the GP ‘Out of Hours’ service or A&E department.

Non-emergency

GPs should access specialist services through the locality Mental Health Service Intake Team (for details see locality pages).

The intake teams act as a single point of access for all referrals to the secondary mental health service. The teams assess, signpost and offer short-term treatment; however services can vary between teams and there are some capacity issues. As far as possible, services will be provided in familiar surroundings and in a manner which promotes independence and choice.

The CHRT/CATT team will be able to facilitate admission to an inpatient bed, if home treatment is not appropriate or not achieving the desired therapeutic effect. Once a crisis is resolved, the Care Programme Approach (CPA) service will ensure that the client and their relatives are linked into ongoing care if required, working with the care co-ordinator, GP, existing social supports, voluntary and other involved agencies to ensure a seamless approach.

Mental Health Services for Older People

A needs-based service is provided to people aged 65 and over, with some specialist services also available to people aged under 65, such as young-onset dementia.

Locally based services are delivered through a single point of access to community mental health teams, day services and inpatient assessment. All teams comprise a range of multi-disciplinary health professionals providing person-centred assessment and treatment.

The EIP teams can be accessed directly by any professional, without the need for a GP referral.

2.5 Social Services – Community Care Community care is the help and support Social Services provides to adults and those who care for them, so that people can live independent lives. Care is provided in people's own homes – wherever possible – or in other settings such as sheltered or 'very' sheltered housing, or in a care home. People aged 18 and over who may need care services include those with needs arising from: • Old age and physical frailty • Physical disabilities • Sensory disabilities (problems with hearing or sight) • Learning disabilities • Mental health difficulties

The key objectives of care are to support people with particular needs to:

• Live as independently and fully as possible • Access advice, information and services easily • Manage their own care and support, with help where needed • Feel part of their local community.

Who is entitled to care services? A person is entitled to a community care assessment if they have a disability or any social care needs. The type of assessment carried out will depend on what the person’s needs are. In most cases the assessment can start within days and be completed within a month. Gateway

Gateway is a customer focused, large-scale cross agency service in a modern retail setting, offering convenient physical access to front line customer advisors and officers from multiple agencies/ delivery partners covering central and local government and voluntary sector.

Ashford Gateway opened in September 2005 as a pilot which proved a great success. The strength of the pilot has underpinned the strategic roll-out which may see sixteen more Kent Gateways across the county. Patient Advice and Liaison Service (PALS)

PALS aims to help patients and carers with their questions and problems concerning health and related services. The NHS can then learn from patients' experiences.

PALS people will:

• provide information for patients, their families and carers about health and local health services

• put people in touch with voluntary organisations and support groups

• help people sort out problems and concerns quickly, before they get more serious and cause anxiety

• advise on how to make a complaint if that becomes necessary

• act as an early warning system, by listening to the views of patients and their families and alerting NHS services to any gaps in services

• liaise with other NHS staff to improve local health services

• understand basic medical terminology and the internal structures of the NHS

• want to know when things are going well and you have received good service from the NHS

Healthy Living Centres

The three Healthy Living Centres within Medway include community cafés which aim to give local people and users of the centre a place to meet and relax. The cafés provide healthier alternatives to traditional café food and support people to deal with the range of issues that affect their well being and quality of life. They also train local people in catering skills, particularly those who find it diffiult to find work.

The centres also provide a range of health services under one roof including GP services, community dentistry, health visitors and school nurses, cardiology, wound leg ulcer care and family planning. Rehabilitation/allied health professional care is available including physiotherapy, speech and language therapy and podiatry. You will need to check what services are available in your particular healthy living centre.
