DAILY PERFORMANCE REVIEW Thursday, 1st November, 2012

SENIOR OPERATING OFFICER’S SUMMARY Thursday was a satisfactory day with customers being offered a good service by the majority of lines across the day. There were, however, a number of challenges that were overcome to achieve this including a TSR being imposed at North Greenwich, points failures on the Bakerloo and Northern lines as well as a person being prevented from jumping on the tracks at Stockwell by fellow commuters.

There were two Category One incidents during the traffic day.

NORTHERN 17.12 – Hampstead – defective lift – station closure Owner – TLL Summary Lift No.1 failed in its shaft whilst ascending. Immediately brought to top landing with manual control, and then failed to respond. No cross-transfer ability for Lift No.2. Lifts Nos. 3 and 4 under refurbishment. Impact Station closed. I-NACH 3 Stalled trains N/A Response Project Engineers already on site (lifts 3 and 4).

17.38 – lift returned to service after door-drive reset. Recovery 17.40 – station re-opened. Immediate Defective lift – door-drive found tripped and reset by engineer. (CA) cause & resolution

19.23 – Edgware – points failure – part suspension Owner - TLL Summary Loss of signalling control – unidentified point problem. Impact Northern Line suspended between and Edgware. Minor delays on remainder of Edgware branch. I-NACH 100 Stalled trains Northbound train 004 stalled at signal AE1 (92 customer) and train 052 stalled behind (200 customer). Response DRM despatched ex Golders Green, DSIM from Euston and NIRM from Moorgate.

19.32 – Category one incident, with Service Manager as Silver Control 19.33 – Train 052 commenced wrong directional move, toward 19.38 – Train 052 berthed at Burnt Oak – no customer issues. 19.56 – Train 004 berthed at Burnt Oak after wrong directional move and train 052 moved south – no customer issues. 20.42 – ERU despatched ex Camden Town to site, to assist Technical staff. 20.47 – DSIM on site. 21.38 – ERU called off. Points being secured with platform 3 out of commission. 21.57 – Routes secured and service resumes. 23.08 – No. 20 points rectified and can now access the Depot. Recovery 21.57 – Minor delays advertised between Camden Town and Edgware. 22.09 – Good service advertised. Immediate Switch fixed and normal operation restored overnight. BAKERLOO 18.58 – Elephant & Castle – points failure – part suspension Owner – APD Summary No.26 points failed to ‘normalise’ - train 225 worked into platform three, then service suspended. Impact Bakerloo Line suspended between Lambeth North and Elephant & Castle. Minor delays remainder of Line. I-NACH 36 Stalled trains None. Response Technical Officer in Waterloo IMR. Further Technical officer despatched to Elephant & Castle from Victoria. NIRM tasked to attend.

19.28 – Category one incident, with Service manager as silver Control. 19.30 – Technical Officer arrived on site at Elephant & Castle. 19.54 – Normal point operation restored after broken chog-bolt removed from the right-hand switch of No.26B points. Recovery 19.57 – Through services restored, with severe delays whole line. 21.06 – Minor delays advertised. 21.52 – Good service.
