2/8/2018 FastFacts January 22 Subscribe Past Issues Translate Quick links to content What’s happening in the VDoFP Resources for my practice Contributing a GP perspective Events URGENT CALL OUT GPs Needed for Community Palliative Patients Who Require Urgent Primary Care The Vancouver Division and VCH Community Palliative Program are collaborating to provide urgently needed end-of-life primary care for patients. We are looking to find physicians for two patient categories: Patients who are imminently (1 month) or actively (within days-few weeks) dying. Physicians will be needed within 24-48 hours, with a home visit likely required. Demand is low; however these unattached patients require a physician to assist in undertaking the following work for a short period of time: be placed on a list provided to the Palliative Care Resource Nurse and may be called Monday-Friday, 8:30am - 4:30pm need to provide symptom management, EOL care, possible GOC conversation, medication and prescription review, and care plan Patients with life expectancy of 1 month or greater.
