2/8/2018 FastFacts January 22 Subscribe Past Issues Translate Quick links to content What’s happening in the VDoFP Resources for my practice Contributing a GP perspective Events URGENT CALL OUT GPs Needed for Community Palliative Patients Who Require Urgent Primary Care The Vancouver Division and VCH Community Palliative Program are collaborating to provide urgently needed end-of-life primary care for patients. We are looking to find physicians for two patient categories: Patients who are imminently (1 month) or actively (within days-few weeks) dying. Physicians will be needed within 24-48 hours, with a home visit likely required. Demand is low; however these unattached patients require a physician to assist in undertaking the following work for a short period of time: be placed on a list provided to the Palliative Care Resource Nurse and may be called Monday-Friday, 8:30am - 4:30pm need to provide symptom management, EOL care, possible GOC conversation, medication and prescription review, and care plan Patients with life expectancy of 1 month or greater. Our process is to then send you a selection of clinic opportunities that match your profile and connect you to the clinic leads for those opportunities in which you were interested. Forms Layout Has Changed on Pathways Please note that Forms have been recategorized on Pathways to make them clearer and easier to sort through. Users also have the ability to select Forms when using the search bar for more targeted searches. Is Your Clinic Looking for a Locum or Permanent Physician? The Vancouver Division Recruitment & Retention team works closely with over 85 clinics to determine their locum and permanent placement needs. We visit each and every clinic so we can provide an accurate representation of the clinic, the amenities and local community. We post clinic opportunities online at locums.ca, SGP and Health Match BC and also share individually with our internal pool of interns. This past year, we matched physicians to over 200 locum and permanent placements. Second year family residents from the Vancouver Fraser Program have created an online survey to help understand the reasons behind choosing to establish an office-based family practice vs. other types of practice. This online survey is approx. 10-15 minutes, participation is voluntary and responses are confidential. Participants may enter a draw to win a $20 Starbucks gift card. The survey is open until February 5, 2018 and can be accessed here. For more information contact Jennifer; [email protected] or Mina; [email protected] CMHA Peer Navigator Program The Peer Navigator Program increases inclusion, self-advocacy and connection with services for people trying to navigate the mental health and substance use systems. The program leverages the expertise of Peer Navigators who have struggled with mental illness and addictions, and who have a keen understanding of the formal and often complex support systems. Peer Navigators provide guidance while working with participants to build a collaborative plan to meet the participants' self-identified needs in areas of income, legal aid, housing and health- (primary and mental health) related services. Any questions can be directed to Peer Navigator Sue Harris Family Practice Research Grant The Sue Harris Family Practice Research Grant supports research that contributes to women’s health within the discipline of family practice. The funding is intended to support new researchers (family practice residents and/or practitioners) to develop and carry out a pilot research project and includes a mentoring process to help with the application and the development of the research idea. Are you interested in the challenging, diverse and impactful work of providing care for the frail elderly? If so, we can support you with Residential Care facility matching and a paid mentorship to get you started. At the end of the mentorship process you will be given a group of residents at your matched facility as a permanent panel. Contact: Jaimie; [email protected] Contributing a GP perspective Transitional Pain Clinic Working Group Who: Vancouver Coastal Health What: The Transitional Pain Clinic is an outpatient, multidisciplinary clinic at VGH. It targets post-surgical patients who are deemed at risk for new chronic pain as a result of their surgery; have worsening of their pre-existing chronic pain; or are deemed at risk of new/continued high dose opioid use. The goal is to provide short-term outpatient service for high-risk patients for whom there is a unique opportunity to prevent chronic pain. The TPC is looking for one GP Representative who is interested in serving on the Working Group and providing input on the continuity of care between the clinic and community. Meetings are bi- weekly on Tuesdays at 2:00pm, at VGH, and are funded at the sessional rate. Please contact Alice if you are interested; [email protected] Events Click for the VDoFP Event Calendar VDoFP Events Gender Affirming Healthcare Workshop Who: Trans Care BC, VCH Trans Speciality Care and Prism Services and VDoFP What: A 3-hour training session will be held for providers interested in learning more about providing Trans primary care. The objectives of this session are to: 1) provide an overview of gender affirming medical care including trans-inclusive history taking and language, options for medical transition including cross-sex hormones and surgery, and 2) discuss the implications of medical transition on the ongoing primary care needs for transgender people. At the end of the session, information on receiving preceptorship and office staff training will be provided. Potential participation in patient transition may be discussed. The first portion of this workshop will provide an overview of maternity care in family practice and a presentation on theme-specific content, building in many opportunities for questions. The second portion of the workshop will take place in small, interactive breakout groups, maximizing opportunities for discussion, questions and networking.
