Archdiocese of Boston 2017-2018 High School Placement Test Student Information Sheet ARCHDIOCESE OF BOSTON 2017-2018 HIGH SCHOOL PLACEMENT TEST STUDENT INFORMATION SHEET To the Student: Please note: You must bring this sheet to the examination on November 18, 2017 or December 2, 2017 GENERAL INFORMATION Now that you have registered to take the HSPT® examination you must: 1. Discuss and complete this form with your parent or guardian which high school(s) you would like to attend. 2. Have one of your parent(s) or guardian(s) sign this form. 3. Plan to report for the examination at the high school where you registered at 8:15 A.M. (the examination begins at 8:30 A.M.) on November 18, 2017 or December 2, 2017. Bring with you: a. Two black lead pencils (No. 2). b. This Student Information Sheet. (You cannot be admitted to the examination unless you have this application sheet with you and it has been signed by one of your parent(s) or guardian(s). Indicate the high school(s) you want to receive your test results or you may not be accepted.) After you arrive at the high school, but before you actually begin taking the examination, you will be asked to fill out a section of the answer sheet so that your name, address, and the high school(s) you want to receive your results can be read by the scoring machine. Complete the Information Sections below to help you at the examination. SPECIFIC INSTRUCTIONS 1. In “Section 1” below, fill in your complete name, address, birthday, what your age will be on November 18, 2017 or December 2, 2017 and your home phone number. 2. High School Codes: the high schools participating in this entrance program are listed on the back of this sheet. While you can only attend one high school, you may indicate up to five high schools where your results should be sent. With the help of your parent(s) or guardian(s), select the school(s) which should receive your test results. You do not have to have results sent to five high schools. After you have selected the high school(s) to which you would like results sent, enter in “Section 2” below, the corresponding high school code numbers. The two-digit code number for each high school is printed to the left of the school name on the back of this sheet. If you are not listing a second, third, fourth, or fifth high school, put the number 00 in the appropriate blanks. 3. In “Section 3” below, please have your parent or guardian sign on the line provided. Approximately two months after you have taken the examination, you will be notified of acceptance or non-acceptance by the high schools that have received your results.
