Archdiocese of Boston 2010-2011 High School Placement Test Student Information Sheet ARCHDIOCESE OF BOSTON 2010-2011 HIGH SCHOOL PLACEMENT TEST STUDENT INFORMATION SHEET To the Student: Please note: You must bring this sheet to the examination on November 13, 2010 or December 11,2010 GENERAL INFORMATION Now that you have registered to take the HSPT® examination you must: I. Discuss with your parents which high school(s) you would like to attend. 2. Complete the Information Sections below. 3. Have one of your parentis) or guardian(s) sign this form. 4. Plan to report for the examination at the high school where you registered at 8: 15 A.M . (the examination begins at 8:30 A.M.) on November 13,2010 or December 11,2010. Bring with you: a. Two black lead pencils (2H or No.2). b. This Student Information Sheet. (You cannot be admitted to the examination unless you have this application sheet with you and it has been signed by one of your parentis) or guardian(s). Indicate the high school(s) you want to receive your test results or you may not be accepted.) After you arrive at the high school, but before you actually begin taking the examination, you will be asked to fill out a special section of the answer sheet so that your name, address, and the high school(s) you want to receive your results can be read by the scoring machine. Complete the Information Sections below to help you at the examination. SPECIFIC INSTRUCTIONS I . In "Section I" below, fill in your complete name, address, birthday, and what your age will be on November 13.20 I 0 or December I I , 20 10 and your home phone number. 2. High School Codes : the high schools participating in this entrance program are listed on the back of this sheet. While you can only attend one high school, you may indicate up to five high schools where your results should be sent. With the help of your parentis) or guardian(s), select the school(s) which should receive your test results. You do not have to have results sent to five schools. After you have selected the high schoolrs) to which you would like results sent, enter in "Section 2" below the corresponding high school code numbers. The two-digit code number for each high school is printed to the left of the school names on the back of this sheet. If you are not listing a second, third, fourth, or fifth high school, put the number 00 in the appropriate blanks. 3. In "Sectio n 3" below, please have your parent or guardian sign on the line provided. Approximately two months after you have taken the examination, you will be notified of acceptance or non-acceptance by the high schools that have received your results. The results of the actuarial valuation have been prepared in accordance with generally accepted actuarial principles. The contents of the report are based on specific actuarial assumptions and methods, as selected by the Trustees of the plan. Any changes in these factors will impact the results documented in this report. In our opinion, unless disclosed otherwise, the assumptions detailed in this report represent the best estimate of anticipated experience under the plan based on the related experience of the plan and the reasonable expectations of the plan. Future actuarial measurements may differ significantly from the current measurements presented in this report due to such factors as the following: 1. Plan experience differing from that anticipated by the economic and demographic assumptions, 2. Changes in economic or demographic assumptions, 3. Increases or decreases expected as part of the natural operation of the methodology used for these measurements (such as the end of an amortization period or additional cost or contribution requirements based on the plan's funded status), and 4. Changes in plan provisions or applicable law. Due to the limited scope of our assignment, we did not perform an analysis of the potential range of such future measurements.
