Croham Place Croham Place is a specialist care home located in , Surrey which supports up to 35 clients over two units, The Manor and The Beeches. We focus on improving Service User’s quality of life through promoting independence and empowerment, while supporting our Service Users to reach their maximum rehabilitation potential.

Service Offering Units:

THE BEECHES - 12 BEDS, ALL MALE UNIT THE MANOR is a 14 bedded unit that supports males and females with a diagnosis of acquired brain injury (ABI) and/or Mental Health issues who also have THE MANOR - 14 BEDS, MALE & FEMALE UNIT complex nursing needs and physical disabilities.

The majority of people we support on an ongoing NIGHTINGALES - 9 BEDS, MALE UNIT basis suffer from limb paralysis and are wheelchair bound. We have hoists in every room, large spacious areas and widened doorways for accessibility. Croham Place provides an in house Physiotherapy service Inclusion Criteria: led by a Physiotherapist registered with both the Chartered Society of Physiotherapy (CSP) and the We are able to support Service Users who: Health & Care Professions Council.

THE BEECHES is a 12 bedded unit that supports HAVE A DIAGNOSIS OF ABI/NEUROCOGNITIVE DISORDERS males only with an ABI and/or Mental Health diagnosis. Individuals in our care may present with MAY HAVE A MENTAL HEALTH DIAGNOSIS a variety of neurological (Dementia, Huntington’s, MAY HAVE PHYSICAL DISABILITIES AND/OR COMPLEX NEEDS Korsakoff’s and Alzheimer’s disease) and Mental Health (Schizophrenia, Schizoaffective Disorder) MAY HAVE A RANGE OF COMPLEX CARE NEEDS diagnoses. MAY HAVE BEHAVIOURS THAT CHALLENGE NIGHTINGALES is a care home with twenty four hours nursing located in Croydon, Greater , MAY HAVE RISKS REQUIRING A MORE supporting up to nine working age gentlemen who ROBUST COMMUNITY PLACEMENT have a Neurocognitive/Acquired Brain Injury condition and Mental Health presentation which affects their MAY HAVE A FORENSIC OR SIGNIFICANT RISK HISTORY behaviour, ability to process information and ability to function independently. Individuals in our care MAY BE INFORMAL, SUBJECT TO DoLS, may also present with a variety of physical and or CONDITIONAL DISCHARGES, CTO’S OR ON LICENCE neurological diagnoses such as any type of Dementia, AS DIRECTED BY THE PAROLE BOARD Huntington’s, Korsakoff’s and Alzheimer’s disease. Assessment Process Once a referral is made to our service we will review the information and if appropriate we will aim to assess the Service User within two working days. We will endeavour to complete and send the assessment report, together with a detailed pre-admission care plan and cost breakdown within three working days following the face to face assessment. We work closely with our Service Users, customers, current placement teams and families to facilitate a smooth transition into the service.

Our Aims & Approach:

Provide one-to-one focused rehabilitation and development of independent living skills where possible and support the use of daily activities of living, as required. Provide ongoing assessment and review behavioural risks, mental health related risks, physical health risks and where indicated forensic risk assessments.
