Response: Yes, a large cable fire in Elkview did extensive damage to Frontier fiber and cable facilities, which caused an extended outage in the Frame area. For reference, a map of the Elkview and Frame area is attached as Exhibit R2.1. If the Company is aware of the issue, state with specificity what steps the Company is taking to resolve the issue. Response: Following the fire, Frontier had to replace and re-splice the burned cable, which included major telephone cable facilities feeding various parts of the Elkview exchange, including Frame. Frontier then had to cut over and move customers on Frame Road off of the copper-fed system that was damaged by the cable fire and on to a new fiber optic-fed remote system. The entire complicated process of repairs and related work in the area took approximately four weeks. For further details, see the response to Data Request No. 3. State with specificity the cause of the widespread outage in the Frame, West Virginia area? Response: a The telephone facilities serving the Frame area were connected to the Elkview central office (CO) by copper T- 1 cables that crossed from the CO underneath the Elk River to the hillside on the west side of the river, then ran north along US 119, and then followed Frame Road north approximately 4 miles to the Subscriber Line Carrier (SLC) facilities in the Frame Road remote. a On June 16 a tree fell thru a power line on a steep hillside along Elk River Rd (US 119) across from the Elk Shopping Plaza and Smith’s Grocery store. The poles supporting the power lines also supported cable TV and telephone cables. a The power line caught on fire and burned major telephone cable facilities feeding various parts of the Elkview exchange, including Frame, as well as exchanges adjacent to Elkview. These facilities included: o A 72 strand and a 216 strand fiber optic interoffice cable; o A 1200 pair pulp paper insulated copper cable; o A 900 pair polyethylene insulated copper (PIC) cable; o A 400 pair PIC cable; and o 600 pair and 300 pair PIC cable stubs. a As a result of this accident, telephone service to the Frame area was severed. a The damage to the telephone cable facilities was located on top of a steep cliff, west of and approximately 50 feet above US 119. All repair materials had to be winched up the cliff with a chain. a Before repairs to telephone facilities could begin, the fire department had to finish with their work extinguishing the fire. Then the power company had to complete their work restoring poles and electrical cables. Then the cable television company had to repair and replace their cables. Only then could repairs on Frontier’s telephone facilities begin. a Once repairs began, it took 25 hours to replace and restore the fiber-optic interoffice cables. However, it took Frontier construction technicians roughly 2 weeks to replace and splice in the new copper cables: o The 1200 pair pulp paper insulated copper cable and the 900 pair and 400 pair PIC cables were replaced with a 2700 pair PIC cable; o The 600 pair and 300 pair PIC cable stubs were replaced with a 600 pair PIC cable stub; and o Two new 900 pair PIC cable stubs were placed to tie back into the existing cable because of the multiple splices. AND CITIZENS TELECOMMUNICATIONS COMPANY OF WEST VIRGINIA dba FRONTIER COMMUNICATIONS OF WEST VIRGINIA CASE NO. 18-029 1-T-C o Many of the damaged copper cables did not contain color-coded pairs and had to be spliced using the laborious process known as “toning.” This requires a technician at each end of the cable being spliced to verify that the correct pair is being connected and to manually place color identification on that pair. After the main cables were replaced and spliced, technicians started restoring the T-1’s feeding the Frame Rd remote. Unfortunately, this process caused equipment in the Elkview CO to begin smoking and popping fuses. It was determined that the equipment in the CO supporting the Frame Road remote had sustained damage from a voltage surge related to the damage to the power line serving the CO. Technicians tried to restore this CO equipment, but were unsuccessful in doing so. After weighing different repair and restoral options, the decision was made to restore the service in Frame by accelerating construction of a fiber optic-fed remote system for the Frame area. Construction on this new system had already begun, but it was far from ready for service. A substantial amount of work was left to perform to get service working on this new system. o Previously existing fiber optic cable had to be connected to new TA5000 switching equipment which was placed in the Frame Road remote. o After the new switching equipment was connected, it had to go through multiple mapping and assignment processes to be integrated into Frontier’s switching landscape. o Once the new fiber trunks and switching equipment were in place and integrated, customer loops in the Frame area had to be moved to the new facilities . 0 While this repair work at the Frame Road remote was ongoing, other service issues arose in the Elkview area which caused more outages, strained available resources, and delayed the final repair to the Frame Road remote. o There was an additional outage to a major fiber trunk which affected Elkview and surrounding exchanges. o There were multiple widespread commercial power outages in the Elkview area which caused switches in other remotes to fail after the batteries in the remotes were exhausted. o The Longridge remote is located further out Frame Road, beyond the Frame remote, and also serves part of the Frame area. As a result of commercial electric power voltage surges and fluctuations, the Longridge remote became unstable throughout the repair period. COMMISSION ORDER The Commission promulgates a proposed amendment to Rule 2.6 of the Rules and Regulations for the Government of Telephone Utilities, 150 C.S.R. Series 6 (Telephone m)for comment. BACKGROUND On September 11, 2013, the Commission opened a general investigation into the continuing need for local exchange carriers (LECs) to annually distribute printed telephone directories to customers as required under Teleuhone Rule 2.6.a. The Commission opened that proceeding after a request from Frontier West Virginia Inc., and Citizens Telecommunications Company of West Virginia dba Frontier Communications of West Virginia (together Frontier) seeking Commission consent for an opt-in directory distribution program. Instead of proceeding with the Frontier request, the Commission invited interested parties to submit comments and directed the Executive Secretary to publish notice of this matter statewide. The Executive Secretary also provided notice to all incumbent and competitive carriers of this proceeding. Beginning on September 23, 2013, the Commission began receiving letters from the public regarding the Frontier proposal. The letters, often from senior citizens, stated that they continue to use printed telephone directories and argued for ongoing directory printing and distribution. One customer, however, wrote in support of ending the requirement to distribute printed directories, concluding that eliminating the requirement will reduce waste and cut costs.
