C. Summary of Major Research Activities

ƒ Quantitative light microscopic studies of human myocardium. ƒ Techniques and procedures were established in the laboratory for determining myocyte diameter, percent interstitial fibrous tissue, nuclear area and endocardial thickness. These techniques have been used to evaluate over 300 endomyocardial biopsies in various cardiac conditions. These techniques have been used to evaluate quantitative histologic characteristics across the left ventricular free wall, right ventricular free wall and interventricular septum in hearts obtained at autopsy and cardiac transplantation.

ƒ Ultrastructural studies of human myocardium. Transmission electron microscopy has been used to study over 900 endomyocardial biopsies from native hearts. Qualitative observations have been recorded on each biopsy. A semi-quantitative scoring system was developed and used in several studies. A computerized image analysis system, available in the Department of Electrical Engineering, was modified and computer programs were developed for morphometric studies of myocardial ultrastructure. This system has been used for 2 major projects examining quantitative ultrastructural features in different forms of myocardial disease and correlating quantitative ultrastructural features with clinical parameters of cardiac function. ƒ Valvular pathology. Studies of floppy or myxomatous mitral valves include light microscopy of the valve cusps and chordae tendineae, biochemical studies of the collagen, physical measurements of cusps and chordae tendineae, transmission electron microscopy of valve cusps and scanning electron microscopy of the cusp surface. Stenotic mitral valves have been studied by light and electron microscopy. Ongoing studies include physical measurements (weights, volume, areas and others) of floppy, stenotic and normal mitral valves. ƒ Physical-Chemical characterization of calcific, atherosclerotic arteries. In collaboration with an investigator in the College of Dentistry, calcific atherosclerotic plaques were studied to investigate elemental composition, crystallographic features, mode of crystallite aggregation and nature of the interface between pathologic calcification and tissue components. Techniques employed include microprobe analysis, x-ray energy dispersion analysis, x-ray diffraction and transmission electron microscopy. ƒ Characterization of an Animal Model of Congestive Heart Failure. The pathologic characteristics of a new strain of rats which develop spontaneous congestive heart failure are being studied. Grossly, the weights and thicknesses are determined for the right ventricular free wall, left ventricular free wall and interventricular septum. Light microscopic studies include measurement of myocyte diameters in specified zones from the endomyocardium to epimyocardium across the ventricular walls. Fractional area of fibrosis in the same zones is determined by morphometric techniques. Quantitative ultrastructural characterization of the myocardium is done by previously developed morphometric techniques. Multiple histologic sections are evaluated for pathologic changes including necrosis and inflammation. Various stages in the progression of heart failure are being studied including pre CHF, mild or early CHF and advanced CHF with appropriate age-matched controls at each stage. ƒ Arterial studies correlating histopathology with geometry and flow dynamics. In collaboration with MH Friedman, Ph.D., early atherosclerotic lesions in coronary arteries are correlated with 3-dimensional arterial geometry. The geometry is determined by postmortem angiography with radiographs taken at specific angles in hearts that have been fixed by pressure perfusion of the coronary arteries. The course is reconstructed by a computer image analysis program. Intimal and medial morphology is studied histologically using quantitative image analysis techniques. The histologic features are correlated with geometric characteristics. ƒ Assessment of injury from side impact loading. In collaboration with Margaret Hines, Ph.D., human cadaver subjects are used in motor vehicle crash simulations. The test subject is seated on a bench seat that simulates a motor vehicle interior. The seat is accelerated until striking an instrumented load- measuring wall that simulates a motor vehicle door panel. Injuries are assessed by autopsy of the chest and abdomen. A separate similar project focuses on shoulder injury. ƒ Pathology associated with ablation energy. In collaboration with A. Mehdirad, M.D. multiple ablation lesions were produced in dog hearts. The gross and microscopic pathology of the lesion sites are described. The hearts are assessed for unexpected complications related to the ablation procedure. ƒ Pathology of Familial Heart Disease. In collaboration with H Graber, M.D., H Boudoulas, M.D. and others, the clinical and pathologic features of a well-documented familial heart disease are being studied. Pathologic studies include endomyocardial biopsies and autopsies. ƒ Pathology of various forms of congenital heart disease.
