Living with dementia Admiral Nurses 01 Royal British Legion - Zoe Scowcroft [email protected] 07584 508620 Admiral Nurses are mental health nurses, specialising in dementia and in supporting families affected by the condition, including the person with dementia and any care provider(s) involved. Admiral Nurses seek to improve the quality of life for people living with dementia and their families, by using a range of interventions to help those affected by the condition to live positively, and to assist them in developing skills to improve communication and maintain relationships. Admiral Nurses also uniquely join up different parts of the health and social care system, enabling the needs of families and people with dementia to be addressed in a co-ordinated way. They provide consultancy and education to professionals to model best practice and improve dementia care in a wide range of settings. We have a comprehensive Advice and Information service, including home visits for the housebound. Our two-day care centres offer both personal care and a wide choice of activities tailored to individual needs. as well as carer respite. The majority of people attending have dementia. Our numerous Ageing Well activities also include the possibility of a volunteer “Dementia Buddy” to aid participation. Our befriending volunteers are available to visit or telephone isolated and lonely older people including many who have dementia. The day-time support Service provides specialist day care facilities for people living with dementia in both Burnley and Pendle. We provide a happy and vibrant environment where people can enjoy each other’s company while they are supported during the day. This leaves family members free to have a break knowing that their loved one is in a safe and supportive environment. We offer a wide range of flexible activities built around individual needs and focus on specific outcomes such as encouraging social interaction, restoring self-esteem, regaining basic life skills and most importantly having fun. Library items are selected by a librarian and then delivered every 3 weeks by Royal Voluntary Service volunteers. We also offer a MEMORY BOX loan scheme, in partnership with the Lancashire County Council museum service, for people with dementia . Burnley Libraries have 3 Dementia friends champions who will be training all library staff to make libraries in Burnley dementia friendly. Introducing volunteers in care home to help improve residents’ quality of life. We will be raising awareness of the project, gathering information on dementia and developing contacts as well as recruiting for volunteer. 4 Pennine Lancashire Dementia Awareness Event BwD Self Directed Support Team 08 Michael Duckett [email protected] 01254 585541 I work as a Direct Payment Officer, helping those eligible for social care funding to complete support plans and encouraging then to use their budgets flexibly to meet individual needs. I receive many referrals from people living with dementia. I have just become a Dementia Friends Champion and would like to use the event to help recruit Dementia Friends. Crossroads Care Ribble Valley 10 Christine Fish [email protected] 01200 422104 Crossroads Care Ribble Valley is a charity which provide respite care in people’s own homes. Our primary focus is the carer and we aim to relieve the pressures of caring by providing care such as personal care, help in the home, taking the cared for person out etc. We do this for people with dementia and are also involved in other dementia- related initiatives such as the dementia café and the dementia-friendly singing group. Ideal for residential care homes, for young people, day centres for elders, NHS, rehabilitation, visiting, companionship and volunteer services to encourage participation and activity for well-being. Creative resources available for young and adult carers and support workers to help people express difficulties to initiate earlier support and positive outcomes. We provide impartial help and advice to anyone who lives in the Borough. We work alongside The Council, Social Services and many other organisations, helping people to be able to live in their homes safely and securely. We are Accredited Retailers and NHS Prescription Providers for disability aids and as well as selling new products and operating a recycling centre, we offering a range of other services. E.g. We have a handy person’s service for smaller type jobs and also provide quotations for larger joinery works. If the client is uncertain about works that need carrying out to their property, our technical officer will call and provide a list of works that need attention and if finances are an issue we will look into possible grant funding that can help. Preston Care and Repair 13 Steven White [email protected] 01282 705607 To enable elderly and/or disabled people to live independently in their homes, we can carry out essential practical work and assist you with larger home repairs or adaptations. We do this through our handyman service and our technical/casework support. In some cases we are able to apply for suitable grants or charitable funding to carry out the work. Rossendale Locality – East Lancs CCG 14 Andy Laverty [email protected] 01282 644919 The Rossendale locality is keen to publicise its dementia-related activities (memory choir and dementia café) and is about to launch its local action plan/process document. It is also currently holding Dementia Friends sessions and is keen to hold more over the summer. Our handyperson service is available throughout East Lancashire (except Hyndburn) and costs only £6 per half hour (inc VAT) making it ideal for small jobs. For larger jobs we have a panel of carefully chosen private contractors covering all trades.
