Greetings Everyone!

This past year has been very busy at ACHIEVE! Since the partnership between ACHIEVE’s Broome, Chenango and Tioga County locations became official through unification on July 1, 2017, we have worked diligently to blend cultures, merge work practices, and articulate a path forward as one newly united organization. We find ourselves uniquely challenged in that endeavor, but with so much excitement for the opportunities that lie ahead, specifically around our ability to provide expanded services into rural Chenango County, where vocational, employment and residential supports will grow rapidly this year!

At present, ACHIEVE employs just over 500 staff and they are truly the backbone of our organization. Employees across our agency continue to amaze us with their ongoing expressions of care and concern for the individuals we serve. We are a family here at ACHIEVE and we commend the staff for not only embracing, but living the agency’s mission every day. To that end, we would like to express our deep appreciation for all who advocated in the statewide #BFair2DirectCare campaign, which is focused on paying direct care workers a living wage. You wrote letters, made phone calls, and even visited our local legislators to advocate. You were heard and together, we will continue to stand behind our amazing direct support professionals!

ACHIEVE was privileged to support more than 2,200 individuals in its wide continuum of services this year! It is an honor to wrap our supports around each person individually as they continue to pursue their definition of a meaningful day and a good life. Individuals and self-advocates are the heartbeat of ACHIEVE and it is toward their satisfaction as well as individual and collective needs that we will continue to focus our work, always.

We thank all of our community partners for their support, in whatever form it has been provided to our organization and the individuals we serve. Teamwork makes the dream work and you are a valued member of our ACHIEVE family.

ANNUAL REPORT 2017 1 2017 Awards & Accolades 2017 Grants • Empire State Development: ACHIEVE received a grant to help • On Tuesday March 28 ACHIEVE/CVI Community Contract Specialist fund renovation of 11,000 square feet at Cutler Pond to build the John Backus and Conduent Operations Manager Shahzad Azad Adult Social Program. presented ACHIEVE/Country Valley Industries participant Michael • Binghamton Noon Rotary: The Rotarians awarded ACHIEVE a Phillips a letter of recognition and certificate from the NY State grant to grow and sell petunias in the Cutler Pond Day Habilitation Assembly for being selected as one of 59 statewide 2016 Joslin green house. Outstanding Performers recognized through NYSID’s annual people- centered awards program. • City of Binghamton Community Development Block Grant: The City of Binghamton provided annual support of ACHIEVE’s Children’s Summer Program. • NYS Office for People With Developmental Disabilities: • Granted funding to support the Children’s Summer Program’s Work Study Program. • Funded a sidewalk around the Cutler Pond Day Habilitation entrance through their Voluntary Preservation Funding program. • IBM: ACHIEVE received a grant to purchase laptops and tablets for the nursing program. This grant was awarded in recognition of the volunteer hours of Ellen Feldman. • Town of Union Community Development Block Grant: The Town of Union provided annual support of the Children’s Summer Program. • The Dr. G. Clifford and Florence B. Decker Foundation awarded a grant to ACHIEVE to support the Adult Social Program. • Miller S and Adelaide S Gaffney Foundation supported the Adult Social Program project through a grant award.

The French Family Assemblyman Clifford Crouch a success story Edwin Tripp An ACHIEVE Success Story Ed enjoys life. He has a smile that can turn a bad day into a good one whether he is at the program or at his residential house. Ed also loves to cook and bake at Cutler Pond and has a cooking goal at his house. He enjoys the challenge to select food items to make that are also on his special restricted diet. Ed has made spaghetti and meatballs with homemade marinara sauce. He also made chicken and eggplant parmesan, Chinese stir fry and the best dish he ever made was the award winning ACHIEVE chili that he served to hundreds of guests at the Savor the Summer event last year. Ed was part of a team who made over 30 pounds of chili for the event. Along with the chili, Ed served a side dish of sour cream and tortilla chips topped with shredded cheese. He proudly wore his chef hat and jacket and is planning to do something different for the 9th Annual Savor the Summer, August, 2018. Ed Tripp has shown great improvement in his ability to communicate to peers and staff. He enjoys visiting many Ed Tripp and Lila Matthews following their Valentine’s Day dinner core rooms throughout the day. There’s one special room he visits because there is a special lady in core room 10. Her name is Lila and she has been Ed’s “girl” for many, many years. Ed had help from staff, but planned a romantic evening dinner and gift giving with his “girl” this past Valentine’s day. He found a ring design online that he liked and printed it out and took it to Comfort and Son’s Jewelry store on the Parkway. Adam Comfort designed the ring and graciously provided Ed with a real life experience of shopping for a ring for his girl, Lila. Ed bought the ring, bought a new suit, made the meal for the dinner and then had a nice evening with Lila. They exchanged gifts and near the end of the dinner, Ed gave Lila a Promise ring and said he would be her best friend forever.

If you’re ever feeling down or having a bad day, stop into core room 4 for a quick visit. You’re almost guaranteed to leave with a smile on your face, thanks to none other than the man, the myth the legend….Joe Gould. To know Joe is to love him. He goes by many names…Joe, Joey, JJG, Jose, “Trouble”, and (when he’s being fresh)…”JOSEPH JOHN!!” Joe is the class clown and the joker of ACHIEVE. He finds humor in everything, and that is one of the many things that make him so special. When he laughs you can’t help but laugh along with him, because when Joey laughs he LAUGHS. He doesn’t do anything halfway. His mischievous smile is infectious, and while he may not be able to say what he’s thinking…you can usually guess pretty easily. Joe is one of the most intelligent people you’ll ever meet. He’s smart as a whip, quick on the uptake, and doesn’t miss a thing. You’ve got to watch what you say around him because if there’s any way it can be taken as a joke he’ll find it…and he’ll DEFINITELY let you in on it, as soon as he quits cracking up himself. He’s also resilient. Joey has overcome so many obstacles and handled so many difficulties with such grace and strength, and hasn’t let anything he’s faced break him. No matter how bad of a day Joey has or how frustrated he may get he comes in the next day with a smile on his face ready to start fresh and give it his all. And when the going gets tough he knows just what to do: get on his communication device (affectionately called “Karen”) navigate to the proper page, and tell us exactly what needs to be done to turn his day around or fix the problem…call Mom. In Joey’s mind, there’s nothing that Mom can’t fix. She’s a superhero in his eyes, and understandably so. Joey has been blessed with two amazing parents who have helped shape him into the man he is today, and he isn’t too proud to admit it. He always looks forward to going home for the weekend and loves spending time at the Gould family camper, Bluegrass festival, or just watching NASCAR and having guy time with dad. Joe is always ready for an adventure. He lives his life to the fullest and is always looking for new experiences. When Bernie Sanders came to the arena Joe knew he had to be there, and be there he was. We were there early, ready to roll, and ended up on the floor in the first row. Joe was thrilled to be there, to be part of history happening, and you could tell from the smile on his face that it was an experience he would never forget. Joe even got to shake his hand and take pictures with the secret service members…It’s safe to say Joe was really “Feelin the Bern”! Then a few months later Joe made another unforgettable memory: he went to vote. This might seem like a “so what?” moment, but this wasn’t your average trip to the polls. For the first time EVER Joe was able to have a “” and communicate his choices by himself using his communication device. Joe still needed a little help marking his ballot, but you could tell he was so proud that he was able to do this. There aren’t words enough to describe the impact Joe has made on so many lives. He has a kind heart, a sweet yet sassy personality, and a fierce desire to live the best life possible. He doesn’t do anything halfway, and that kind of strength and determination is what makes him the perfect recipient for this award. Joey, we love you, we believe in you, and you truly are “Man of the Year”. Submitted by: Alyssa Keefe, Day Habilitation Specialis 4 ACHIEVE find yourself here a success story Logan Cunningham An ACHIEVE Success Story When I first met Logan Cunningham; he was sitting at his kitchen table reading a Star Wars book. When introducing myself, Logan peered above the book, long enough to say “hello” and then returned his attention back to the words on the page. At that time, “yup,” “that sounds good,” “thanks,” and “what do you think?” were Logan’s responses when asked questions. The Logan I first met was looking at others to make decisions and self discoveries for him. Logan today, has redefined and expanded his comfort zone and is not hidden behind the words of his books any longer.

“Where’s Logan?” can often be heard on local Pickleball courts. Logan was introduced to the sport through his ACHIEVE Community Habilitation Direct Service Provider and never looked back. Logan started by observing and studying what and how the game is played. Then he tried it and never stopped. Logan is a frequent flyer among Pickleball events and matches. In addition to playing, he has taken to setting up and organizing those matches throughout the area. Through his play, hard work, and sportsmanship, Logan is quite popular among his fellow players.

When Logan isn’t playing on the Pickleball court, he has been hard at work setting himself up for the next phases of his life. As stated earlier, Logan enjoys books and can apply what he has read. With diligent hard work, Logan has studied, completed, and continues to maintain his EMT certifications. These high-level certifications have enabled Logan to be part of a local EMS Squad and work at a local Emergency Room. Though Logan has a laid back, easy going personality, he has flourished in these fast-paced environments.

The actor Alan Alda once said, “You can’t get there by bus, only by hard work, and risk. What you’ll discover will be wonderful. What you’ll discover will be yourself.” Logan continues to discover more about himself with support from his services. Through this discovery process, Logan has self directed his services to fit his needs. Through this discovery, Logan will have an apartment of his own starting July 2018. Logan’s desire to strive has led to him being named ACHIEVE’s Community Habilitation Program Participant of the Year for 2018. 6 ACHIEVE find yourself here Residential Services Community Residences, Supervised Apartments, and Supportive Apartments Each of ACHIEVE’s 19 residential facilities are regulated by the New York State Office for People with Developmental Disabilities (OPWDD). They are designed to support individuals with intellectual and other developmental disabilities in their community with access to recreational and work opportunities. Admission requires a developmental disability diagnosis before the age of 22, with the individual requiring a supervision level other than one-on-one support. ACHIEVE also operates a number of supportive apartments, which allow individuals the opportunity to live in a more community-based integrated setting with limited staff supports. n 120 individuals served in 2015 n 124 individuals served in 2016 n 164 individuals served in 2017

Day Habilitation Services Joel Coon receives the keys to his new apartment Day Habilitation Program The Day Habilitation Program provides a comprehensive spectrum of intensive rehabilitative and support services to assist individuals with moderate to severe levels of disability with developing and maintaining skills for community integration. Services include: teaching activities of daily living, cognitive skill development, recreation, communication development, socialization opportunities, volunteer activities, speech education services, and comprehensive nursing services. Day Habilitation Services are offered to residents living in Broome and Tioga Counties who have been diagnosed with an intellectual or developmental disability prior to their 22nd birthday. Services are individualized and assist in developing, enhancing, and maintaining skills used while living, working, or socializing in the community. Individuals in the Day Habilitation Program are also given volunteer opportunities in the community to help increase socialization skills, develop individuality, and promote independence. n 152 individuals served in 2015 n 122 individuals served in 2016 n 216 individuals served in 2017

8 ACHIEVE find yourself here Children’s Summer Program Services Children with intellectual and other developmental disabilities benefit from summer programs that help them maintain the skills they developed during the school year. The Children’s Summer Program exposes them to drama, music, physical activity, arts and crafts, and field trips that reinforce their academic skills, gross and fine motor coordination, and social interaction. The goal of the program is to broaden the life experiences of children with disabilities and provide them with basic learning concepts that will enhance skill levels. Students are grouped by age and led by a team leader and counselors who design activities to meet each group’s needs. n 100 individuals served in 2015 n 108 individuals served in 2016 n 104 individuals served in 2017 Medicaid Service Coordination Medicaid Service Coordination (MSC) is a wonderful program that provides ongoing support and advocacy to individuals with developmental disabilities and their families. Medicaid Service Coordinators assist people with obtaining available services in the community as well as services under the Home and Community Based (HCBS) Waiver, and self-direction programs. ACHIEVE’s Medicaid Service Coordination program serves adults and children of all ages both in Broome/Tioga and Chenango counties. Service Coordinators work together with individuals and families to develop and maintain a person centered plan designed to meet dreams and desires regardless of complexity, by providing; advocacy, supports and referrals to necessary services. n 339 individuals served in 2015-Broome and Tioga n 357 individuals served in 2016-Broome and Tioga n 575 individuals served in 2017-Broome-Chenango-Tioga Guardianship Program The Guardianship program serves as corporate guardian for individuals who do not have family members that are able to do so and need support with decision making. The program ensures that programmatic, medical, social and other needs are cared for just like a parent would. Through our parent agency The Arc New York, ACHIEVE may serve as Primary, Stand-by or Alternate Stand-by guardian for individuals.
