Patient statement regarding the future of services at Farnsfield Surgery It is with great regret that Sherwood Medical Partnership has come to the difficult conclusion that it is time to withdraw from provision of services to the Farnsfield Surgery population. Since 2015 when Crown Medical Centre and Farnsfield Surgery merged, Sherwood Medical Partnership has worked hard to deliver a service to the Farnsfield patient population. The partnership have been made aware of issues reported by patients and has worked closely with the PPG to make changes and improvements. Despite its best efforts and endeavours, the partnership feels it is no longer able to provide services to the Farnsfield population. Please see the accompanying ‘You said, We Did’ document. In addition, recently, a statement from Farnsfield Parish Council released via a number of community Facebook groups on behalf of Farnsfield Surgery patient population has given Sherwood Medical Partnership no alternative but to consider its future in delivering services for the Farnsfield Surgery population. We have approached the CCG and NHS England and they are working to find an alternative provider for the patient population of Farnsfield Surgery. We hope the opportunities a new provider can offer with fresh eyes and a different perspective will provide the service the Farnsfield Surgery population has been seeking. Our aim is to minimise the impact of any change on the patient population of Farnsfield as much as possible. We are keen to ensure a smooth transition to a new provider when the CCG/NHS England have identified one. The health and wellbeing of our patients both at Farnsfield and Crown has always been paramount. By taking this decision our hope is that this allows Farnsfield Surgery to move forward with a service it is comfortable with whilst allowing us to focus our efforts and attentions on service delivery at our Crown Medical Centre site. We appreciate this may come as a shock to some of our Farnsfield Surgery patients and we apologise for this. It is still possible for you to register with Sherwood Medical Partnership in future should you wish to do so. It has been requested by the CCG that for now you wait to do this until a new provider has been installed. We will endeavour to keep the patient population updated as the situation progresses. We would like to thank the patients of Farnsfield for their patience and wish them all the very best of health for the future. Statement from NHS Nottingham and Nottinghamshire CCG: As a patient of Farnsfield Surgery, you do not need to take any action. Once a new provider has been identified in the New Year, we will let you know of changes at your surgery.
