Basketball 3. Parents register with YOUR valid email and password. We recommend using Gmail or Yahoo email addresses for registration. You will be asked to type Wrestling in a code to verify you are human. If this step is skipped, your account will not activate. Soccer 4. Login using your email address that you registered with in step 3. Ski 5. Select “New Clearance” to start the process. Snowboard 6. Choose School Year 20-21 SPRING SEASON 7. Choose “DAVIS SR.” for school (even if your student attends DV or a Jr. High) Track & Field 8. Choose only the sport(s) your child wishes to participate in each season.

Lacrosse 9. Complete all required fields for Student Information, Educational History, Medical History, Signature Forms and upload your COMPLETED PHYSICAL Baseball (in step #1 –Student Information: ‘choose file’ and click ‘save’)

Softball 10. Once you receive the Confirmation Message, you have completed the registra- tion process and are awaiting final clearance. Boys Golf 11. If you would like to register for additional sports, you may check off those sports Swimming/Diving below the Confirmation Message. Electronic signatures will be applied to the additional sports/activities. Boys Tennis 12. All of this data will be electronically filed with the DSHS Athletics Department for Boys Volleyball review. When the student has been cleared for participation by the Athletics De- partment, an email Clearance Notification will be automatically sent. Girls Badminton 13.
