IV Drug Compatibility Chart The IV drug compatibility chart is based on information from the Handbook of Injectable Drugs, 16th Edition, by Lawrence A. Trissel, FASHP, copyright 2010, American Society of Health-System Pharmacists. Incompatibilities are based on meeting any of the following criteria: • Additive Compatibility • Drugs in Syringe Compatibility • Y-Site Injection Compatibility (1:1 Mixture) Additionally, in some cases one brand of product may be compatible but another brand of drug is not. When this occurred, “incompatible” was used. Disclaimer: This IV Drug Compatibility Chart is provided as a quick reference, however since information is subject to change it is advised that you check with a pharmacist for recommendations as well as product information to check for any changes or additional incompatibilities. Key: • = Compatible • = Incompatible • = A drug intersects with itself • = The relationship was investigated and the results were equivocal. = No data available A • • • • • • • Verapamil Verapamil •
