Approval of Primary Care Networks in Nottingham North and East 1. Purpose The purpose of this report is to provide the Committee with an overview of how the proposed Primary Care Networks intend to address the concerns that have been raised about how they will meet the approval requirements, in order for the Committee to consider the applications presented to it. Four applications for Nottingham North and East have been submitted. A summary of applications is included in Appendix A, the Network Contract DES registration forms are provided in Appendix B and a map showing the proposed PCNs is provided at Appendix C for reference. 2. Introduction The proposed PCN configuration for Nottingham North and East was discussed by clinical and lay members of the Primary Care Commissioning Committee on the 23rd May. Two key concerns were identified for further consideration: the ability of other service providers to align themselves with PCN 3 and PCN 4 given the overlaps between their practice populations; the ability of PCN 3 to offer services in a way that makes geographical sense to their patients, in particular the patients registered with Daybrook Medical Practice. The locality team facilitated a meeting of the practices in PCN 2, 3 and 4 on 29th May; 11 out of the 12 practices were represented. The only practice not represented had provided comments via the LMC and the relevant PCN Clinical Director. As well as the LMC, NHS England and Local Partnerships were also represented at the meeting. 3. Alignment of Other Service Providers As the main provider of community services to the PCNs in Nottingham North and East, Local Partnerships have considered the current resources available within the locality and how they could be wrapped around the proposed PCNs. The proposals for PCN 1 and PCN 2 provide no particular issues. The proposals for PCN 3 and PCN 4 can only be readily accommodated if those PCNs undertake to ensure that there is an ongoing consistent approach to the commissioning of community services that are delivered in both PCNs. PCN 3 and PCN 4 practices have unanimously agreed that they will adopt a consistent approach to the commissioning of community services that are delivered in both PCNs and that this agreement will be written into the relevant network agreement schedules for submission by the 30th June. Offer of Services in a Way That Makes Geographical Sense to Patients There was an extensive discussion around the options to amend the proposed alignment of practices to PCNs but no agreement could be reached to amend the original proposal. The position remains that Daybrook Medical Practice continues to be an integral a core member of the PCN3. the practices have established good working relationships and have similar practice populations; Daybrook Medical Practice and Plains View Surgery have commenced discussions that may impact on their partnership arrangements and their ability to be treated as two separate practices for the purposes of PCN alignment. PCN 3 and Daybrook Medical Practice recognised the concerns around the potential risk that registered patients may not to be able to access services at their nearest “hub” (provided by PCN 2 in Arnold) if PCN 3 hub services are located in Carlton. However, they feel that this risk is relatively small and can be managed: Key points for consideration are: the provision of multiple hubs has been discussed and agreed to meet all patient needs in PCN in particular hubs with parking facilities. a significant proportion of the practice’s population live within Nottingham City and already travel into the City to access public health services patients already travel to Carlton to access many services including mental health, ultrasound and podiatry services there is a direct bus route from Daybrook to all potential hub sites in Carlton for patients not readily able to travel, the GP practice provides services in house and will continue to do so In addition, PCN 3 have agreed that they would seek to enter into an agreement with PCN 2 for patients who are unable to travel to be able to access relevant hub services within Arnold and to have this written into the relevant network agreement schedules for submission by the 30th June. 5. PCN Population Numbers The population of PCN 4 is currently below the 30,000 threshold by 353 patients. Housing developments in the area mean that the PCN will reach the 30,000 threshold during 2019 / 20. The practices within PCN 4 have stated that if they are not supported to establish a PCN of their own at this point in time because of their population size, as soon as they reach a population of 30,000 they will apply to leave the PCN that they are in and resubmit an application to form their own PCN. In order to ensure good working relationships and to avoid the disruption of practices leaving a PCN soon after it has formed it would be beneficial not to reject the PCN 4 proposal on the grounds of population size. 6.
