INFORMATION FOR CLUBS a. All home clubs must text or email or where necessary telephone the result of their fixture within 30 minutes of completion of the fixture with the names and times of all goal scorers in the match for both teams. On submitting your match report sheet you must include the name of the match referee, his/her marks and the name of player of the match for the opposition. It is important that the league has this information. Any club failing to adhere to this rule will be fined as per rule. b. All match report forms (seniors) are to be submitted to the Assistant General Secretary Mr. Trefor Jones within four days of conclusion of match (Sunday not included). d. All match report forms (Youth) must be submitted to the Chairman Mr Bryan Roberts within four days of the conclusion of the match. Remember that you must fill out the sheet correctly and it is important that the referees name and marks given are included also the name of your player of the match for the opposing team e. Kick off times: Seniors August, September, October (part of) 2.30pm October (part of), November, December, January, February, March & April (part of) 2.00pm April (part of), May 2.30pm Mid-week matches 6.30pm Floodlight matches only 7.30pm Youth Sundays 2pm f. Clubs are reminded that alcohol is strictly forbidden at all league and cup matches and clubs are responsible for their players/officials/supporter’s behaviour g. If there are any changes to your club information then please submit them to the General Secretary of the league immediately. h. The HOME club secretary must confirm all fixtures with the away club and the match referee at least TWO days prior to the date of the fixture either in writing or by telephone. The information required when speaking to the opposing team is kick off time, venue, team colours and most important arrangements for after match hospitality. i. Cost of match officials can be found in Appendix B of the rule book

APPENDIX C

LEAGUE WELFARE SCHEME

The function of the welfare scheme shall be to assist those who suffer financial hardship due to an accident whilst playing or involved in a properly sanctioned game of football.

GUIDANCE NOTES: 1. The scheme will be called ‘The Vale of Clwyd & Conwy Football League Welfare Scheme’. 2. The treasurer of the league shall keep all records of this account. 3. Membership of the welfare scheme shall be open to any member of a club affiliated to the ‘Vale of Clwyd & Conwy Football League’ on payment of an annual subscription fee. Clubs di- rectly affiliated to the FAW who participate in the league may also become members of the scheme. Membership subscriptions must be paid through the club and cannot be paid on an individual basis. Claims by players/club officials may only be made by persons who participate in the Vale of Clwyd & Conwy Football League sanctioned matches. 4. The league upon request will provide a claim form on which a claim may be made. It is expected that a medical certificate will be submitted with any claim. The executive committee of the league will update the claim form as and when necessary. 5. All claims are to be made via the league General Secretary and thus forwarded to members of the executive committee. 6. Membership of the scheme of the scheme will commence on receipt on appropriate membership subscription. This to be submitted before the end of September in any playing season and will continue until the last day of that season and shall include any correctly granted extension to the season plus any properly sanctioned friendly matches. 7. The executive committee of the league shall decide the membership subscription annually and the subscription fee shall be 50p per person and clubs will be expected to pay this as part of a players registration fee with the exception of those players whose clubs are affiliated to the FAW whereby membership fee of £20 will be paid by the club. 8. The league executive committee may appoint a welfare officer every season to investigate and oversee all claims normally the General Secretary. 9. A sub-committee consisting of the league Chairman, General Secretary and a nominated Ex- ecutive officer will deliberate on all claims submitted and duly decide as to the amount of grant to be paid out by the welfare fund. Payment will be made if there are sufficient funds available and will be a one off payment. 10. The decision of the committee will be final and no member may appeal the amount of grant to any other body or a court of law. 11. If a false claim is made then legal action may be taken against any person involved in the claim and the whole of the cost of any such action will be borne by the person(s) making such false claim. 12. The Vale of Clwyd & Conwy Football executive committee shall have the power to alter these guidance notes as and when appropriate and to deal with any matter not provided for in these notes.
