IMPORTANT INFORMATION ABOUT YOUR COVERAGE

It is very important for you to read this information carefully. SAVE ALL information you receive from SilverScript about your retiree prescription drug coverage from for future reference.

Much of the information that SilverScript is sending you is required by . It refers to the Medicare Part D plan portion of your coverage only, not the additional coverage provided by Pfizer. Many of these documents use general language that is not specifically designed to communicate Pfizer’s benefits. If you have any questions, please call SilverScript Customer Care.

SilverScript Employer PDP sponsored by Pfizer (SilverScript) is the prescription drug plan for Medicare-eligible participants in the Pfizer Medicare Advantage Base Plan, Pfizer Medicare Advantage Buy-Up Plan and Pfizer Prescription Drug-Only Plan. This prescription drug plan is provided by SilverScript® Insurance Company which is affiliated with CVS Caremark®.

Your Welcome Kit

Enclosed is your Welcome Kit with important information about the plan:

• Evidence of Coverage: a booklet that explains the rules you must follow to be covered by this plan and your right to appeal plan decisions about payment or services.

• Abridged (List of Covered Drugs): an abridged list of drugs covered by the plan. The formulary will show some of the drugs covered under the Medicare Part D portion of your benefits. It will not include the drugs covered through the additional coverage provided by Pfizer.

• Pharmacy Directory: a list of network pharmacies in your area, including preferred and non-preferred retail pharmacies, mail-order pharmacies, long-term care pharmacies and home infusion pharmacies. Go to pfizer.silverscript.com and use the pharmacy locator to find other network pharmacies in the and U.S. territories.

You will receive your new SilverScript ID card in a separate mailing. Do not use this card until your effective date. If you do not receive your ID card by your effective date, please call SilverScript Customer Care at 1-844-774-2273, available 24 hours a day, 7 days a week. TTY users should call 711.

• SilverScript is a Medicare Part D prescription drug plan (PDP) with additional coverage provided by Pfizer. This additional coverage means that you have more coverage than the standard Medicare Part D plan and overall your coverage will be similar to the Pfizer prescription drug coverage you have today.

• You will be enrolled in SilverScript as of your Medicare eligibility date, or the date SilverScript approves your enrollment, whichever is later.

• You will continue to have no deductible for prescription drugs.

• You will continue to receive most Pfizer drugs including Greenstone generic drugs at no cost to you at network pharmacies.

• You will pay the same coinsurance percentage and per-prescription minimum or maximum for non-Pfizer prescription drugs through all the Medicare Part D coverage stages.

• You will have no coverage gap, also known as the Medicare Part D “Donut Hole.”

• You will have an annual prescription drug out-of-pocket maximum of $3,400 per Medicare-eligible participant. Once you reach this annual out-of-pocket maximum, you pay nothing for the rest of the calendar year when you use a network pharmacy. This annual out- of-pocket maximum is separate from the out-of-pocket maximum under your non-Medicare Pfizer prescription drug plan through the Retiree PPO or High Deductible PPO options. Additionally, it will not be applied to the annual out-of-pocket maximum for covered dependents who are not eligible for Medicare.

• You will be able to use most of the network pharmacies that currently are in your CVS Caremark pharmacy network.

• You will continue to get your prescriptions filled at your local network pharmacy or through the CVS Caremark Mail Service Pharmacy™.

• You will be able to get up to a 90-day supply of your at any SilverScript network retail pharmacy.

• If you have your prescription filled at a CVS Pharmacy® or other preferred network retail pharmacy or use the CVS Caremark Mail Service Pharmacy, your cost for a 90- day supply of your non-specialty non-Pfizer will not exceed the per-prescription maximum for a 60-day supply.

• You will now have access to network pharmacies at long-term care facilities and for home infusion that are not in the current CVS Caremark pharmacy network.

• If your covered spouse and/or or dependent child is eligible for Medicare, they each will receive separate mailings from SilverScript. Because Medicare is an individual benefit, they each will have their own packets of information about your benefits and their own ID cards.

• If your covered spouse and/or dependent child is not eligible for Medicare, he or she will continue to be covered on the current CVS Caremark plan and they will continue to have the current $3,500 individual and $5,500 family annual prescription drug out-of-pocket maximum for members who are not eligible for Medicare. They will continue to use their current ID card to get their prescriptions filled.

• You will receive a new ID card from SilverScript. Do not use this new ID card until your effective date.

• You can be enrolled in only one Medicare prescription drug plan at a time. If you decide to change plans and enroll in another Medicare prescription drug plan, or an individual Medicare Advantage plan with or without prescription drug coverage, you will lose your Pfizer-sponsored medical and prescription drug coverage.

You will be able to re-enroll in Pfizer coverage in the future, but you will need to certify that you have maintained creditable medical and prescription drug coverage during the time you were not enrolled in a Pfizer Retiree plan option. You may enroll within 31 days of experiencing an IRS qualified life event, such as marriage, divorce or losing coverage elsewhere.

If you are the retiree and you opt out of SilverScript, then any of your covered dependents, such as your spouse or dependent children, will also lose their Pfizer-sponsored medical and prescription drug coverage.

What you need to do

There are some things that you should do, or may need to do to make sure you have the medications you need.

• Open and read any information you receive from SilverScript. You will be receiving letters, statements about your drug costs called “Explanation of Benefits,” and other information required by Medicare. Some of the materials will be for your information, but there may be letters that require you to take an action in order to keep your coverage.

• Save all information you receive from SilverScript for future reference.

• Get your prescriptions refilled. If you will need to refill your prescription in the near future, you may want to obtain your refill before your SilverScript effective date, to make sure you have enough of your medication during the transition to SilverScript. Any eligible mail-order prescriptions with refills remaining will be transferred automatically from CVS Caremark to SilverScript. If you have a prescription with refills remaining at a retail pharmacy, you need to go to the pharmacy and show your new ID card after your effective date. New prescriptions will be required for controlled substance and compound medications.

• Check the Abridged Formulary (List of Covered Drugs) to see if your drug is covered. This booklet is included in your Welcome Kit. It is a list of the most commonly used drugs selected by SilverScript and covered under the Medicare Part D portion of the plan. Some medications that are covered by the additional coverage provided by Pfizer will not be listed on the formulary. If you do not see your drug, call SilverScript Customer Care at the phone number listed in that booklet.

• Pay an additional premium, if required by Medicare. If your annual income is over $85,000 for an individual or $170,000 for a married couple filing jointly, Medicare requires that you pay an additional premium based on your income, called the Part D Income-Related Monthly Adjustment Amount (Part D-IRMAA). You will be notified by Social Security if this affects you.

It is important that you pay this additional amount if required. If you do not pay it, Medicare will disenroll you from the plan. Pfizer will terminate both your retiree medical and prescription drug coverage based on this disenrollment.
