Personalised care and support

North Yorkshire County Council and other local authorities have made changes to the provision of care and support to older people and people with learning and/or physical disabilities. This is a national requirement that sets out the way people and those who care for them should be supported. Leaving hospital

Health & Adult Services and the NHS in North Yorkshire work very closely together to ensure that hospital patients are discharged safely and are able to return to home life as quickly as possible. Our social care staff are available to help with information and advice when you are ready to leave hospital. No matter what hospital you are in, you can request an assessment of your needs to help you stay independent and confident when you return home. A social care worker will come and speak to you to arrange an assessment which can either be done whilst you are still in hospital, or once you return home, or in interim/ temporary facilities if you are not able to return home immediately on discharge from hospital. The assessment involves asking you about your needs and what is required to help you live safely and independently at home. There will also be an explanation of any charges that may be made. Staying at home

Given the choice, many people prefer to stay in their own homes surrounded by their possessions and near to their family and friends. This, of course, applies to anyone whether they are elderly and frail, convalescing, have a physical or learning disability, or are suffering from dementia. ‘Your Support, Your Way’ (explained on page 9) can help you manage independently at home with a little, or a lot of help. Telecare

Getting help in your home

Help in your home is also known as ‘domiciliary care’ and is usually arranged in a manner dependent upon needs:
